The authors present a rare case of hepatic fascioliasis in a female patient 53-years-old, coming from the rural zone of Rio Grande do Sul, a southern State from Brazil. She has presented with biliary colic, fluctuant jaundice and eosinophilia. Abdominal ultrasound has shown a dilated biliary tree with inside heterogeneous images. At surgery we have found inside the biliary tree several Fasciola hepatica, which have been pulled out with the choledocoscope. We have proceeded with biliodigestive anastomosis using the small intestine. The patient remains asymptomatic six months after surgical procedure. Small intestine. A fasciolose é uma enfermidade causada por um trematoda que acomete o fígado principalmente de ruminantes domésticos, podendo parasitar o homem e seu diagnóstico é realizado rotineiramente por exames coproparasitológicos. O objetivo do presente estudo foi comparar kits comerciais de ELISA para anticorpos no soro e leite com um teste coproprarasitológico em bovinos naturalmente infectados por Fasciola hepatica. Foram coletadas amostras de fezes (92) sangue (92) e leite (43) de bovinos provenientes de propriedades de gado leiteiro do município de Jerônimo Monteiro, sul do Estado do Espírito Santo. As amostras de fezes coletadas foram processadas pela técnica de sedimentação fecal para ovos de F. hepatica, utilizada como padrão ouro para as análises. Amostras de sangue e de leite foram processadas segundo a orientação do fabricante dos respectivos Kits ELISA comerciais testados. Utilizou-se o c² de McNemar para comparação estatística e calcularam-se a sensibilidade e especificidade, valores preditivos e kappa. Os resultados obtidos mostraram que as frequências de positividade pelo uso dos kits ELISA comerciais de soro e de leite diferiram significativamente (p<0,0001) em relação ao exame coproparasitológico. A sensibilidade dos Kits foi de 100%, porém possuíram baixa especificidade, 42,85 e 30% para o soro e leite respectivamente. O coeficiente de kappa mostrou concordância sofrível para os testes de soro (0,33) e de leite (0,21). Os valores preditivos positivos dos kits para soro e leite foram, respectivamente, 44,61 e 38,23% e, os valores preditivos negativos de 100% para ambos os testes. Apesar da maior sensibilidade dos kits ELISA comerciais e, destes apresentarem diferença em relação ao exame coproparasitológico na detecção dos animais positivos para F. hepatica, a escolha de um teste diagnóstico deve considerar o custo benefício. Quando se trata da presença de parasitismo em rebanhos, o tratamento é aplicado em todos os animais e, assim, o exame coproparasitológico para o diagnóstico da doença tem maior eficiência, já que é menos oneroso e de fácil execução.

Adult Lymnaea acuminata (average length 20-22 mm) were collected locally from lakes and low-lying submerged fields from Gorakhpur. The chemoattraction studies were made in round glass aquaria measuring 30 cm in diameter and filled to a depth of 10 mm with 500 ml dechlorinated tap water. Each aquarium was divided into four concentric zones. At the starting time of the assay 10 snails were placed on the circumference of outermost zone 0. Snail attractant pellets (SAP) were added simultaneously in the center of central zone 3. SAP of different amino acids were prepared at concentrations of 10, 20, 50, 80 and 100 mM/2% agar solution and, subsequently, spread to a uniform thickness of 5 mm. After cooling, SAP were cut in small pieces of 5 mm in diameter. Lymnaea acuminata's attraction to amino acids was studied using different amino acid concentrations in SAP. Pellets containing amino acids with non-polar R groups (proline and tryptophan), a charged polar group (arginine) and uncharged polar R groups (serine, citrulline and asparagine) were tested. The snails were more attracted to the uncharged polar R group amino acid serine than to other groups of amino acids. The preferred amino acid concentration was 80 mM. The attraction of snails to different amino acids was concentration dependent. Snails could discriminate amongst the different amino acids at > or = 50 mM.

A eficácia de termofosfato magnesiano (fertilizante e corretivo do solo) na alimentação de planorbídeos, em laboratório, foi testada ao longo de cinco semanas, verificando-se o crescimento da concha de populações experimentais de Bimphalaria glabrata (Say, 1818) e Helisoma duryi (Wetherby, 1879). Reversamente observou-se que aquele produto mineral bloqueou a oviposição de ambas as espécies, a partir da segunda semana do experimento, quando proporcionado, isoladamente ou associado, a latoso ou a folhas de alface. Em conseqüência, supõe-se que sua ampla utilização nas novas áreas de cultivo, obtidas através de métodos modernos de irrigação, poderá, adicionalmente - ao ser carreado pelas chuvas, para ecossistemas lênticos ou lóticos - reduzir as populações pioneiras de diferentes espécies de Bimphalaria constituindo-se útil meio de controle da esquistossomose mansoni, em zonas rurais. Ademais, poderá ser eficaz no controle dos hospedeiros intermediários de Schistosoma mansoni que, usualmente, se estabelecem em lagoas de criação de peixes - cuja multiplicação, em áreas tropicais, vem sendo incentivada, quer como atividade econômica alternativa para o pequeno proprietário rural quer como exigência de necessidade nutricional das comunidades humanas mais pobres. Supõe-se, ainda, que o material testado poderá controlar populações de limneídeos, hospedeiros intermediários de Fasciola hepatica. São sugeridos testes de campo que possam efetivamente comprovar a hipótese levantada, e também estudos destinados ao conhecimento do mecanismo de ação do elemento magnésio no bloqueio da fertilidade de moluscos. Salienta-se a necessidade de pesquisas especiais sobre o efeito de diferentes concentrações de magnésio em ovos e formas juvenís de planorbídeos, bem como sobre miracídios e cercárias (intra-caramujos e livre-natantes) de trematódeos.

São mostrados os resultados do levantamento dos moluscos de água doce de 5 municípios da área de influência de uma usina hidrelétrica em estudo, formada pelas redondezas da Cachoeira Couto Magalhães no Rio Araguaia. Foram examinados 624 moluscos procedentes de amostras coletadas em 20 coleções hídricas. O exame revelou a ocorrência de 8 gastrópodos pulmonados e um prosobrânquio, além de um bivalvo esferídeo. Devido ao envolvimento com a saúde pública, merece destaque o encontro de Biomphalaria straminea e Lymnaea columella, respectivamente hospedeiros intermediários de Schistosoma mansoni e Fasciola hepatica.

Hortaliças in natura, comercializadas na região metropolitana de São Paulo, SP (Brasil), foram analisadas através de metodologia própria, visando à pesquisa e à identificação de formas de transmissão de helmintos intestinais de interesse médico. As hortaliças examinadas, constituídas de 50 amostras de cada variedade, foram de: alface (Lactuca sativa), variedades lisa e crespa, escarola (Chichorium sp) e agrião (Nasturtium officinale). Os resultados evidenciaram elevados percentuais de contaminação em todas as variedades analisadas, porém, as freqüências de helmintos foram maiores no agrião. A escarola apresentou valores médios, geralmente situados entre as alfaces e o agrião. Os números médios de ovos e larvas por 100 gramas de amostra, embora elevados, não apresentaram diferenças estatisticamente significantes entre as quatro variedades de hortaliças estudadas. Uma grande variedade de helmintos, de ocorrência freqüente na população residente na Região Metropolitana de São Paulo, foi observada nas amostras. Os mais freqüentes, no entanto, foram: ancilostomídeos e Ascaris sp. Recuperaram-se também ovos de Toxocara sp, Fasciola sp e de tricostrongilídeos, comprovando a ocorrência de contaminação das hortaliças por fezes de animais domésticos. Considerando-se os resultados obtidos, ressalta-se a importância das hortaliças na transmissão de helmintíases intestinais, bem como a necessidade de medidas que propiciem uma melhoria na qualidade higiênico-sanitária destes alimentos.

Se ha determinado el effecto inhibidor sobre la actividad Glucogeno sintetasa (E.C.2.41.11) por parte de cuatro antihelminticos: Albendazol, Mebendazol, Parbendazol y Tiabendazol. Observandose que en todos los casos, es el Parbendazol quien ha demostrado un mayor poder inhibidor sobre la glucógeno sintetasa de Ascaris suum, Fasciola hepatica y Moniezia expansa. El Tiabendazol es el anti-helmintico que menor efecto inhibidor ha presentado sobre la enzima en los tres parasitos objeto de nuestro estudio. Con el presente trabajo y otros previstos en la misma linea, se pretende aportar nuevos datos acerca del aun desconocido locus de acción de estos antihelminticos.

A well documented case of hepatic fascioliasis (HF), successfully treated with triclabendazole, is reported. Predominant clinical manifestations were fever, marked eosinophilia and abdominal pain. Triclabendazole was given as two single oral doses of 10 mg/kg each. Neither side effects nor clinical or parasitological relapses were seen after three months of follow up Based on this experience and few other similar reports in the literature, triclabendazole might be a valid therapeutical alternative in the treatment of human fascioliasis.

SUMMARY A food-borne trematode infection fascioliasis is one among common public health problems worldwide. It caused a great economic loss for the human race. Control of snail population below a certain threshold level is one of the important methods in the campaign to reduce the incidence of fascioliasis. The life cycle of the parasite can be interrupted by killing the snail or Fasciola larva redia and cercaria inside of the snail Lymnaea acuminata. In vitro toxicity of different binary combinations (1:1 ratio) of plant-derived larvicidal active components such as citral, ferulic acid, umbelliferone, azadirachtin and allicin against Fasciola redia and cercaria were tested. The mortality of larvae was observed at 2h, 4h, 6h and 8h of treatment. In in vitro condition azadirachtin + allicin (1:1 ratio) was highly toxic against redia and cercaria (8h LC50 0.006 and 0.005 mg/L). Toxicity of citral + ferulic acid was lowest against redia and cercaria larvae.

Abstract:INTRODUCTION:The therapeutic scheme of triclabendazole (TCBZ), the recommended anthelmintic against Fasciola hepatica , involves 10mg/kg of body weight administered in a single dose; however, clinical trials in children are scarce. We evaluated the efficacy and tolerability of 2 schemes of TCBZ.METHODS: Eighty-four Peruvian children with F. hepatica eggs in their stools were allocated into 2 groups: 44 received 2 dosages of 7.5mg/kg each with a 12-h interval (Group I), and 40 received a single 10-mg/kg dose (Group II). Evaluation of efficacy was based on the presence of eggs in stools, and tolerability was based on the presence of symptoms and signs post-treatment.RESULTS: A parasitological cure was obtained in 100% of individuals from Group I and 95% of individuals from Group II. The most common adverse event was biliary colic.CONCLUSIONS: The tested scheme was efficacious and tolerable, and it might be an optimal scheme in the region. To the best of our knowledge, this represents the largest series of children treated with TCBZ in a non-hospital setting.

Molecular cloning of components of protective antigenic preparations have suggested that related parasite fatty acid binding proteins could form the basis of the well documented protective, immune cross reactivity between the parasitic trematode worms Fasciola hepatica and Schistosoma mansoni. We have now confirmed the cross protective potential of parasite fatty acid binding proteins and suggest that it may be possible to produce a single vaccine that would be effective against at least two parasites, F. hepatica and S. mansoni of veterinary and human importance respectively.

In many helminth infected hosts the number of eosinophils increases dramatically, often without any concurrent increases in the number of other leukocytes, so that eosinophils become the dominant cell type. Many experimental investigations have shown that the eosinophilia is induced by interleukin-5 (IL-5) but its functional significance remains unclear. Mice genetically deficient in IL-5 (IL-5-/-) have been used to evaluate the functional consequences of the IL-5 dependent eosinophilia in helminth infected hosts. Host pathology and level of infection were determined in IL-5-/- and wild type mice infected with a range of species representative of each major group of helminths. The effects of IL-5 deficiency were very heterogeneous. Of the six species of helminth examined, IL-5 dependent immune responses had no detectable effect in infections with three species, namely the cestodes Mesocestoides corti and Hymenolepis diminuta and the trematode Fasciola hepatica. In contrast, IL-5 dependent immune responses were functionally important in mice infected with three species, notably all nematodes. Damage to the lungs caused by migrating larvae of Toxocara canis was reduced in IL-5-/- mice. Infections of the intestine by adult stages of either Strongyloides ratti or Heligmosomoides polygyrus were more severe in IL-5-/- mice. Adult intestinal nematodes were clearly deleteriously affected by IL-5 dependent processes since in its presence there were fewer worms which had reduced fecundity and longevity. The implications of these results for the viability of using inhibitors of IL-5 as a therapy for asthma are considered.

In previous studies it was shown that the recombinant molecule, r-Sm14, induces high levels of protection against Schistosoma mansoni infection in two outbred animal models and immune crossprotection against infection by Fasciola hepatica in Swiss outbred mice. r-Sm14 was derived from a living worm extract, called SE, and is being developed as the molecular basis of an anti-helminth bivalent vaccine against the two parasites, for medical and veterinary application. Present data refer to SDS-PAGE and Western Blotting analysis of four different preparations of S. mansoni adult worms focusing Sm14 identification. The extracts correspond to the initial fraction of the SE extraction process, containing products released by living worms (SEi); SE2, reextraction of adult worms in PBS; and SE of separated male and female adult worms. In all extracts it was possible to detect the component of 14 kDa, that was recognized by specific anti-rSm14 antibody raised in rabbits.

Previous studies carried out with Sm14 in experimental vaccination against Schistosoma mansoni or Fasciola hepatica infections were performed with recombinant Sm14 (rSm14) produced in Escherichia coli by the pGEMEX system (Promega). The rSm14 was expressed as a 40 kDa fusion protein with the major bacteriophage T7 capsid protein. Vaccination experiments with this rSm14 in animal models resulted in consistent high protective activity against S. mansoni cercariae challenge and enabled rSm14 to be included among the vaccine antigens endorsed by the World Health Organization for phase I/II clinical trials. Since the preparation of pGEMEX based rSm14 is time consuming and results in low yield for large scale production, we have tested other E. coli expression systems which would be more suitable for scale up and downstream processing. We expressed two different 6XHis-tagged Sm14 fusion proteins in a T7 promoter based plasmids. The 6XHis-tag fusions allowed rapid purification of the recombinant proteins through a Ni+2-charged resin. The resulted recombinant 18 and 16 kDa proteins were recognized by anti-Sm14 antibodies and also by antiserum against adult S. mansoni soluble secreted/excreted proteins in Western-Blot. Both proteins were also protective against S. mansoni cercariae infection to the same extent as the rSm14 expressed by the pGEMEX system.
