AbstractBackground:Risk scores for cardiac surgery cannot continue to be neglected.Objective:To assess the performance of “Age, Creatinine and Ejection Fraction Score” (ACEF Score) to predict mortality in patients submitted to elective coronary artery bypass graft and/or heart valve surgery, and to compare it to other scores.Methods:A prospective cohort study was carried out with the database of a Brazilian tertiary care center. A total of 2,565 patients submitted to elective surgeries between May 2007 and July 2009 were assessed. For a more detailed analysis, the ACEF Score performance was compared to the InsCor’s and EuroSCORE’s performance through correlation, calibration and discrimination tests.Results:Patients were stratified into mild, moderate and severe for all models. Calibration was inadequate for ACEF Score (p = 0.046) and adequate for InsCor (p = 0.460) and EuroSCORE (p = 0.750). As for discrimination, the area under the ROC curve was questionable for the ACEF Score (0.625) and adequate for InsCor (0.744) and EuroSCORE (0.763).Conclusion:Although simple to use and practical, the ACEF Score, unlike InsCor and EuroSCORE, was not accurate for predicting mortality in patients submitted to elective coronary artery bypass graft and/or heart valve surgery in a Brazilian tertiary care center. O presente trabalho relata um experimento em que foram verificados os efeitos fitotóxicos do herbicida 2-cloro-2', 6'-dietil-N-(metoximetil) acetanilida (CP 50144) em plántulas de gergelim (Sesamun indicum L.) e feijão (Phaseolus vulgaris L.), cultivados em solução nutritiva 0,5-Hoagland. 0 herbicida foi aplicado às folhas, nas doses respectivas de lug/10l, 5ug/ 10l, 25yg/10l, 125ug/10l, 250yg/10l e 500ug/10l. Para o tratamento às raízes, ele foi adicionado à solução nutritiva, nas doses de 1 ppm, 5 ppm, 25 ppm, 125 ppm, 625 ppm e 3125 ppm, respectivamente. O tratamento foliar revelou penetração lenta, tanto nas fôlhas de gergelim como nas de feijão, com pequena translocação e forte ação de contato, especialmente nas concentrações mais elevadas, superiores à da saturação (148 ppm). A absorção pelas raízes e lenta, tanto para o gergelim como para o feijão. Entretanto, o gergelim mostrou-se altamente tolerante ao CP 50144, até a concentração de 125 ppm, inclusive, não tendo demonstrado nenhum sintoma de injuria. Acima dessa concentração, o gergelim foi fortemente injuriado, tendo mostrado fortes sintomas de ação de contato, nas raízes, e sintomas gerais de intoxicação, na parte aerea, que levaram as plantas a morte. O feijão mostrou-se bastante sensível à ação do CP 50144, tendo apresentado forte sintomatologia de intoxicação, tanto por contato como por translocação apoplástica. São descritos os sintomas de fitotoxicidade do herbicida, para as duas plantas. O herbicida CP 50144 e altamente seletivo para o gergelim, mostrando-se bastante promissor para essa cultura. O herbicida mostrou ação inibidora da distensão celular, nas folhas novas de feijão. É interessante que se realizem estudos fitoteratológicos e fitofisiológicos com este herbicida, cujo modo de ação ainda nao e bem conhecido.

Uma população de milho denominada ESALQ-VD-opaco foi utilizada no presente trabalho para se estimar parâmetros genéticos e fenotípicos associados a caracteres de produtividade, altura da planta e da espiga, % triptofano e % triptofano na proteína. Um total de 364 progenies de meios irmãos foram avaliadas em látices com duas repetições. As estimativas das variâncias genéticas aditivas para produção de grãos (s²A= 333,44 g/pl), altura da planta (s²A= 187,85) e altura da espiga (s²A = 122,08) indicam a existência de suficiente variabilidade genética no material. Para os caracteres de qualidade do grão, detectou-se também suficiente variabilidade aditiva, permitindo obter progressos genéticos com a seleção. Estimativas de correlações genéticas entre produção de grãos e altura de planta e da espiga, foram de baixa magnitude favorecendo a seleção para produtividade sem alterar significativamente altura da planta e da espiga. As correlações genéticas entre produção de grãos e caracteres para qualidade protéica foram positiva para proteína (rA = 0,303), negativa para triptofano (rA = -0,178) e negativa para triptofano na proteína (rA = -0,568). Verificou-se também uma correlação negativa entre % proteína e % triptofano na proteína (rA = -0,591). A população mostra que selecionando-se para produtividade haverá uma redução na qualidade da proteína, contudo existe suficiente variabilidade genética presente que permite melhorar o valor nutricional da semente sem prejuízo para a produtividade de grãos.

Este trabalho teve o escopo de dar a público os resultados das análises efetuadas num total de 625 amostras de amendoim descascado, na sua maioria HPS ("hand pick selected"), durante os anos de 1988 (517) e 1989 (108). Considerando que a legislação brasileira permite um máximo de 30 /¼g/kg, somadas as aflatoxinas B1 e G1, os resultados mostraram que em 1988, 52% das amostras (correspondentes a lotes), estavam contaminadas e em 1989, 34%. Os níveis médios de aflatoxina nas amostras contaminadas foram de 13 3,8 ¼g/kg em 1988 e 414,8 ¼g/kg em 1989, que podem ser considerados elevados. O contingente de lotes utilizáveis pela indústria foi de 67% em 1988 e 76% em 1989 (correspondentes às amostras com níveis não detectáveis e abaixo de 30 ¼g/kg).

Some anuran amphibians use the bromeliads during the entire life cycle and others only as diurnal shelter. At the sandy coastal plain of Linhares, State of Espírito Santo, Southeastern Brazil, 676 bromeliads were examined, of which 303 of Aechmea blanchetiana (Baker) L. B. Smith., 1955, 287 of Aechmea nudicaulis (L.) Griseb., 1864, and 86 of Vrisea procera (Mart. Ex Schult. f.) Wittm, 1891. The morphometric and physical-chemical analysis of different bromeliads evidenced variations among plants. During the period sampled, six anuran species were found inside the plant axils. The hylid frog Phyllodytes luteolus (Wied, 1824) was the most abundant species (260 specimens). Its abundance was higher in the epiphyte bromeliad Vrisea procera. Phyllodytes luteolus had higher occurrences in bromeliads located at a transitional area between open and under the shrub vegetation. Specimens of Scinax alterus (Lutz, B., 1973) and Aparasphenodon brunoi Miranda-Ribeiro, 1920, were more frequent mainly in transitional areas; Bufo granulosus Spix, 1824 occurred in open and transitional areas, whereas Gastrotheca fissipis (Boulenger, 1888) and Leptodactylus fuscus (Schneider, 1799) were found only in bromeliads located in open areas.

Lucilia cuprina (Wiedemann, 1830) is a cosmopolite blowfly species of medical and veterinary importance because it produces myiasis, mainly in ovine. In order to evaluate the demographic characteristics of this species, survivorship curves for 327 adult males and 323 adult females, from generation F1 maintained under experimental conditions, were obtained. Entropy was utilized as the estimator of the survival pattern to quantify the mortality distribution of individuals as a function of age. The entropy values 0.216 (males) and 0.303 (females) were obtained. These results denote that, considering the survivorship interval until the death of the last individual for each sex, the males present a tendency of mortality in more advanced age intervals, in comparison with the females.

A new nematode genus and species. Neoparaseuratum travassosi n. g., n. sp., is described from the intestine of the freshwater thorny catfish, Pterodoras granulosus (Valenciennes), from the Paraná River, Brazil. This seuratoid nematode species represents a new genus of the family Quimperiidae, being characterized mainly by the presence of numerous narrow longitudinal bands of inflated cuticle extending along the cephalic region of the body, small deirids, postoesophageal position of the excretory pore, relatively short (0.159-0.303 mm), equal spicules and a gubernaculum, the absence of caudal alae and preanal sucker in the male, and by some other features.

An ELISA test was developed to detect Paragonimus-specific antibodies, including IgG subclasses, using P. mexicanus crude water-soluble antigens. The test was standardized to detect antibodies in sera of Ecuadorian patients with pulmonary paragonimiasis and negative controls from the endemic area. The detected mean levels of IgG (0.753, SEM: 0.074) and IgM (0.303, SEM: 0.033) were significantly elevated (P<0.05). Within the IgG subclasses, IgG4 showed the highest detected mean level (0.365, SEM: 0.116) and the other three subclasses showed considerably lower mean levels (IgG1, 0.186 SEM: 0.06; IgG2, 0.046 SEM: 0.01; IgG3, 0.123 SEM: 0.047). The number of P. mexicanus eggs found in sputum of infected individuals showed a positive correlation with the level of antibodies detected for IgM, IgG and its subclasses (P<0.001). The relevance of these findings in Ecuadorian patients suffering from pulmonary paragonimiasis is discussed.
Seasonality and relative abundance of Tabanidae (Diptera) captured on horses in the Pantanal, Brazil

Once a month, from June 1992 to May 1993, collections of tabanids on horse were conducted in the Nhecolândia, Pantanal State of Mato Grosso do Sul, Brazil. Tabanid catches using hand nets were conducted from sunrise to sunset at grassland and cerradão (dense savanna) habitats. A total of 3,442 tabanids from 21 species,12 genera, and 3 subfamilies were collected. Although species abundance varied seasonally depending on habitat, no habitat specificity was observed for the most abundant species. In the grassland, 1,625 (47.2%) tabanids belonging to 19 species were collected, while 1,817 (52.8%) tabanids from 17 species were caught in the cerradão. The number of tabanid species varied from 7 during winter (July/August) to 15 in the spring (October). Tabanus importunus (56%) was the most abundant species, followed by T. occidentalis (8.2%), and T. claripennis (8.1%). The tabanid peak, in October, coincided with the beginning of the rainy season. The population peak of most species, including those with higher vector potential, suggests that the rainy season can be considered as the period of potentially higher risk of mechanical transmission of pathogens by tabanids to horses in the region.

Natural products are an inexhaustible source of compounds with promising pharmacological activities including antiviral action. Violacein, the major pigment produced by Chromobacterium violaceum, has been shown to have antibiotic, antitumoral and anti-Trypanosoma cruzi activities. The goal of the present work was to evaluate the cytotoxicity of violacein and also its potential antiviral properties.The cytotoxicity of violacein was investigated by three methods: cell morphology evaluation by inverted light microscopy and cell viability tests using the Trypan blue dye exclusion method and the MTT assay. The cytotoxic concentration values which cause destruction in 50% of the monolayer cells (CC50) were different depending on the sensitivity of the method. CC50 values were > 2.07 ± 0.08 µM for FRhK-4 cells: > 2.23 ± 0.11 µM for Vero cells; > 2.54 ± 0.18 µM for MA104 cells; and > 2.70 ± 0.20 µM for HEp-2 cells. Violacein showed no cytopathic inhibition of the following viruses: herpes simplex virus type 1 (HSV-1) strain 29-R/acyclovir resistant, hepatitis A virus (strains HM175 and HAF-203) and adenovirus type 5 nor did it show any antiviral activity in the MTT assay. However violacein did show a weak inhibition of viral replication: 1.42 ± 0.68%, 14.48 ± 5.06% and 21.47 ± 3.74% for HSV-1 (strain KOS); 5.96 ± 2.51%, 8.75 ± 3.08% and 17.75 ± 5.19% for HSV-1 (strain ATCC/VR-733); 5.13 ± 2.38 %, 8.18 ± 1.11% and 8.51 ± 1.94% for poliovirus type 2; 8.30 ± 4.24%; 13.33 ± 4.66% and 24.27 ± 2.18% for simian rotavirus SA11, at 0.312, 0.625 and 1.250 mM, respectively, when measured by the MTT assay.

The seroprevalence of toxoplasmosis in 832 pregnant women in Miracema, Rio de Janeiro, was determined and 75.1% (625) and 2.0% (17) were anti-Toxoplasma gondii IgG and IgM positive, respectively. Out of the 17 IgM positive pregnant women, only one had low avidity IgG corresponding to the acute phase of the infection. All the other women presented with high avidity IgG and also presented with residual IgM anti-T. gondii. Of this sample, 106 received home visits (this includes 11 family nuclei of pregnant women with residual IgM anti-T. gondii, 68 nuclei of only IgG positive pregnant women and 27 nuclei of pregnant women with no antibodies to anti-T. gondii), resulting in 267 individuals visited. Out of these 267 individuals, 21 were positive for IgG and IgM anti-T. gondii and were candidates for the IgG avidity test. All of them presented with high avidity IgG and residual IgM. Five of these IgM+ individuals were (5/238; 2.1%) relatives of IgM negative pregnant women. The other 16 (16/29; 55.2%) were relatives of IgM+ pregnant women who were positive for residual IgM anti-T. gondii. This association was statistically significant (p = 0.0000). The analysis presented herein raises questions regarding the presence of residual IgM anti-T. gondii such as genetic determinants or even constant antigenic stimuli for the same family cluster.

Developing a fast, inexpensive, and specific test that reflects the mutations present in Mycobacterium tuberculosis isolates according to geographic region is the main challenge for drug-resistant tuberculosis (TB) control. The objective of this study was to develop a molecular platform to make a rapid diagnosis of multidrug-resistant (MDR) and extensively drug-resistant TB based on single nucleotide polymorphism (SNP) mutations present in therpoB, katG, inhA,ahpC, and gyrA genes from Colombian M. tuberculosis isolates. The amplification and sequencing of each target gene was performed. Capture oligonucleotides, which were tested before being used with isolates to assess the performance, were designed for wild type and mutated codons, and the platform was standardised based on the reverse hybridisation principle. This method was tested on DNA samples extracted from clinical isolates from 160 Colombian patients who were previously phenotypically and genotypically characterised as having susceptible or MDR M. tuberculosis. For our method, the kappa index of the sequencing results was 0,966, 0,825, 0,766, 0,740, and 0,625 forrpoB, katG, inhA,ahpC, and gyrA, respectively. Sensitivity and specificity were ranked between 90-100% compared with those of phenotypic drug susceptibility testing. Our assay helps to pave the way for implementation locally and for specifically adapted methods that can simultaneously detect drug resistance mutations to first and second-line drugs within a few hours.

O presente estudo objetivou verificar a relevância e a utilização de estratégias de comunicação em cuidados paliativos. Estudo quantitativo multicêntrico, realizado entre agosto/2008 e julho/2009, junto a 303 profissionais de saúde que trabalhavam com pacientes sob cuidados paliativos, por meio da aplicação de questionário. Os dados foram submetidos a tratamento estatístico descritivo. A maioria (57,7%) não foi capaz de citar ao menos uma estratégia de comunicação verbal e apenas 15,2% mencionaram cinco sinais ou estratégias não verbais. As estratégias verbais mais citadas foram as de cunho interrogativo sobre a doença/tratamento e, dentre as não verbais, destacaram-se o toque afetivo, olhar, sorriso, proximidade física e escuta ativa. Embora os profissionais tenham atribuído alto grau de relevância para a comunicação em cuidados paliativos, evidenciaram escasso conhecimento de estratégias de comunicação. Faz-se necessária a capacitação dos profissionais no que tange à comunicação em cuidados paliativos.

The aim of the study was to determine the prevalence and variables associated with the pattern of risky health behavior (PRHB) among adolescent students in Cartagena, Colombia. A cross-sectional study was designed to investigate PRHB in a random cluster sample of students from middle and high schools. The associations were adjusted by logistic regression. A total of 2,625 students participated in this research, with ages from 10 to 20 years, mean=13.8 years (SD=2.0), and 54.3% were women. A total of 332 students reported PRHB (12.7%, 95%CI 11.4–14.0). Age over 15 years (OR=2.19, 95%CI 1.72–2.79), not being heterosexual (OR=1.98, 95%CI 1.36-2.87), poor/mediocre academic performance (OR=1.87, 95%CI 1.47–2.38), family dysfunction (OR=1.78, 95%CI 1.40–2.28) and male gender (OR=1.58, 95%CI 1.24–2.01) were associated with PRHB. One in every eight students presented a PRHB. It is important to pay greater attention to students who are over 15 years of age, male, not heterosexual, with a poor/mediocre academic performance and a dysfunctional family.

OBJECTIVETo evaluate the level of knowledge and the availability of the Portuguese population to attend training in Basic Life Support (BLS) and identify factors related to their level of knowledge about BLS.METHODObservational study including 1,700 people who responded to a questionnaire containing data on demography, profession, training, interest in training and knowledge about BLS.RESULTSAmong 754 men and 943 women, only 17.8% (303) attended a course on BLS, but 95.6% expressed willingness to carry out the training. On average, they did not show good levels of knowledge on basic life support (correct answers in 25.9 ± 11.5 of the 64 indicators). Male, older respondents who had the training and those who performed BLS gave more correct answers, on average (p<0.01).CONCLUSIONThe skill levels of the Portuguese population are low, but people are available for training, hence it is important to develop training courses and practice to improve their knowledge.
