Objetivo: determinar os fatores associados ao achado de carcinoma microinvasor no cone de mulheres com biópsia colpodirigida prévia compatível com neoplasia intra-epitelial cervical (NIC) 3 e avaliar a proporção de margens comprometidas. Pacientes e Métodos: foram revisados os prontuários de 385 mulheres (média de idade: 39 anos) submetidas à conização a frio ou por cirurgia de alta freqüência (CAF) com alça no período de janeiro de 1993 a julho de 2000. Estes procedimentos foram indicados por biópsia compatível com NIC 3. Resultados: o diagnóstico do cone foi compatível com NIC 3 em 243 mulheres (63%) e com NIC 2 em 13 (3%). Apenas 10 apresentaram HPV/NIC 1 (3%) e oito não tinham doença residual no cone. Entretanto, 101 mulheres apresentaram carcinoma microinvasor no cone (26%) e 10 (3%) carcinoma invasor franco. A idade, o estado menstrual e o número de partos não estiveram relacionados com a gravidade da lesão no cone. Mulheres com alterações da colpocitologia oncológica sugestivas de invasão apresentaram um risco significativamente maior de apresentar carcinoma microinvasor ou invasor no histológico final (p<0,01), embora 52 das 243 mulheres com NIC 2 ou NIC 3 no cone também tivessem sugestão de invasão na colpocitologia. Entre as mulheres com NIC 2 ou 3, 44% apresentaram epitélio branco, 21% pontilhado e 17% mosaico. Esta proporção foi semelhante nas mulheres com carcinoma microinvasor ou invasor, sendo estas imagens encontradas, respectivamente, em 37%, 23% e 21%. O comprometimento das margens do cone foi significativamente maior nas mulheres submetidas a CAF (49%) do que naquelas submetidas à conização a frio (29%). Conclusão: a ausência de fatores independentes clínicos e colposcópicos que se associam com o achado de carcinomas microinvasivos em mulheres submetidas à conização por biópsia compatível com NIC 3 justifica a excisão cônica da junção escamo-colunar nas lesões cervicais de alto grau.

Um estudo virológico e sorológico seccional (E1) e outro longitudinal (E2) foram realizados em granjas com (G2 e G3) e sem (G1) a síndrome de refugagem multissitêmica (SRM) no Brasil. Foram coletadas amostras de sangue, soro, swabs nasal e retal de animais de cada categoria do ciclo produtivo: porcas, leitões maternidade, creche, recria e terminação. Em E1, nas granjas G1a e G2, foram amostrados 40 animais de cada categoria. Em E2, nas granjas G1b e G3, 35 leitões na maternidade foram identificados e amostrados ao longo do ciclo produtivo. O soro foi avaliado para presença de anticorpos contra circovírus suíno tipo 2 (CVS2) e sangue e swabs para presença do ácido nucléico viral. Em E1, a categoria porcas possuía altas taxas de animais virêmicos e soropositivos, com porcentagem de porcas com títulos altos superior a G2. Em G1a a queda de imunidade passiva ocorreu entre o final da fase de creche e início da recria com aumento da eliminação viral em swabs e subsequente soroconversão. Em G2 a queda ocorreu entre a fase final da maternidade e início da creche, com diminuição da eliminação viral. Em E2, a queda da imunidade materna ocorreu entre a 1ª e 2ª coleta em G1b; e em G3, entre a 2ª e 3ª coleta. Em ambas as granjas, a queda de imunidade passiva coincidiu com o aumento da viremia e eliminação viral e a soroconversão ocorreu entre a 3ª e 4ª coleta em ambas as granjas com aumento da média de título de anticorpos e declínio da viremia. Viremia e eliminação viral foram detectadas em todas as coletas realizadas; 42% dos animais amostrados em E2 foram virêmicos em todas as coletas e todas as amostras de tecido coletadas no abate foram positivas para o CVS2. Este estudo confirma a persistência da viremia mesmo em presença de altos títulos de anticorpos e que o perfil sorológico em um rebanho com e sem a presença da síndrome pode ser diferente, principalmente em relação à duração da imunidade passiva.

Erythrocyte membrane proteins from 44 representative mammals were studied. Protein 4.2 was not detected in guinea pigs (Cavia porcellus) (N = 14), Southern Brazilian swamp large rats (Myocastor coypus) (N = 2), cutias (Dasyprocta sp) (N = 4), and horses (Equus caballus) (N = 13). These animals also presented high ankyrin concentrations except for the horse which did not exhibit a sharp band, although minor components located between proteins 2 and 3 could account for the ankyrin family. The rodents studied did present band 6, which was not detectable in other common rodents such as white rats (Rattus norvegicus) (N = 9) and mice (Mus musculus) (N = 12). Since the absence of protein 4.2 does not disrupt the cytoskeleton membrane, we suggest that it is not an essential protein. Its absence may be compensated physiologically by the higher ankyrin concentration observed.

To determine the effects of combined therapy of gliclazide and bedtime insulin on glycemic control and C-peptide secretion, we studied 25 patients with type 2 diabetes and sulfonylurea secondary failure, aged 56.8 ± 8.3 years, with a duration of diabetes of 10.6 ± 6.6 years, fasting plasma glucose of 277.3 ± 64.6 mg/dl and a body mass index of 27.4 ± 4.8 kg/m². Patients were submitted to three therapeutic regimens lasting 2 months each: 320 mg gliclazide (phase 1), 320 mg gliclazide and bedtime NPH insulin (phase 2), and insulin (phase 3). At the end of each period, glycemic and C-peptide curves in response to a mixed meal were determined. During combined therapy, there was a decrease in all glycemic curve values (P<0.01). Twelve patients (48%) reached fasting plasma glucose <140 mg/dl with a significant weight gain of 64.8 kg (43.1-98.8) vs 66.7 kg (42.8-101.4) (P<0.05), with no increase in C-peptide secretion or decrease in HbA1. C-Peptide glucose score (C-peptide/glucose x 100) increased from 0.9 (0.2-2.1) to 1.3 (0.2-4.7) during combined therapy (P<0.01). Despite a 50% increase in insulin doses in phase 3 (12 U (9-30) vs 18 U (11-60); P<0.01) only 3 patients who responded to combined therapy maintained fasting plasma glucose <140 mg/dl (P<0.02). A tendency to a higher absolute increase in C-peptide (0.99 (0.15-2.5) vs 0.6 (0-2.15); P = 0.08) and C-peptide incremental area (2.47 (0.22-6.2) vs 1.2 (0-3.35); P = 0.07) was observed among responders. We conclude that combined therapy resulted in a better glucose response to a mixed meal than insulin alone and should be tried in type 2 diabetic patients before starting insulin monotherapy, despite difficulties in predicting the response.

Pueraria mirifica is a Thai phytoestrogen-rich herb traditionally used for the treatment of menopausal symptoms. Pueraria lobata is also a phytoestrogen-rich herb traditionally used in Japan, Korea and China for the treatment of hypertension and alcoholism. We evaluated the mutagenic and antimutagenic activity of the two plant extracts using the Ames test preincubation method plus or minus the rat liver mixture S9 for metabolic activation using Salmonella typhimurium strains TA98 and TA100 as indicator strains. The cytotoxicity of the two extracts to the two S. typhimurium indicators was evaluated before the mutagenic and antimutagenic tests. Both extracts at a final concentration of 2.5, 5, 10, or 20 mg/plate exhibited only mild cytotoxic effects. The plant extracts at the concentrations of 2.5, 5 and 10 mg/plate in the presence and absence of the S9 mixture were negative in the mutagenic Ames test. In contrast, both extracts were positive in the antimutagenic Ames test towards either one or both of the tested mutagens 2-(2-furyl)-3-(5-nitro-2-furyl)-acrylamide and benzo(a)pyrene. The absence of mutagenic and the presence of anti-mutagenic activities of the two plant extracts were confirmed in rec-assays and further supported by a micronucleus test where both plant extracts at doses up to 300 mg/kg body weight (equivalent to 16 g/kg body weight plant tuberous powder) failed to exhibit significant micronucleus formation in rats. The tests confirmed the non-mutagenic but reasonably antimutagenic activities of the two plant extracts, supporting their current use as safe dietary supplements and cosmetics.

A presença de hidrocarbonetos aromáticos polinucleares (HPAs) em aguardentes foi investigada por cromatografia líquida (CLAE) após sua prévia extração em fase sólida (SPE). A separação foi realizada em uma coluna Supelco, LCPAH-octadecil silano (25cm x 4,6mm x 5mm) com gradiente acetonitrila/água e a quantificação utilizando detector de fluorescência. Os HPAs (naftaleno; acenaftaleno; fluoreno; fenantreno; antraceno; fluoranteno; pireno; 1,2- benzo(e)pireno; criseno; benzo(e)pireno; 2,3-benzo(a)antraceno; 1,2-benzo(b)fluoranteno; benzo(k)fluoranteno; dibenzo(a,h)antraceno; benzo(a)pireno; benzo(ghi)pirileno foram identificados e quantificados em vinte e oito amostras de aguardentes de cana. Os resultados experimentais para as amostras de aguardentes (cachaças) são analisados em termos de análises de componentes principais (PCA) objetivando a diferenciação entre o perfil das aguardentes produzidas a partir de cana-de-açúcar queimada e não-queimada.

The treatment of Diabetes should not only be sought through drug administration; diet is also a part of its treatment. The aim of this study was to determine the effect of a diet with six meals having equal calories on the body weight and blood glucose on diabetes type 2 patients. This research is an Experimental study conducted in 2009 on 181 patients with diabetes. The patients visited the IDSF (Iranian Diabetes Society of Fars) weekly and the patients to be studied were randomly divided into two groups of 85 and 96 patients, respectively. The participants were repeatedly requested to consume their calculated calorie in six equal parts. The average age in the Experimental and Control groups were 51.2 ± 13.3 and 53.1 ± 9.4, respectively. The mean body weight and fasting blood glucose at the beginning of the study in Experimental and Control groups were 66.3 ± 9.4 and 69.1 ± 11.1 kg, 198.9 ± 35.1, and 199.8 ± 39.1 mg.dL-1, respectively. At the end of the study, however, the values were 63.5 ± 7.5 and 66.98 ± 9 kg, 139.5 ± 34.6 and 164.2 ± 22.1 mg.dL-1, respectively. Only the mean fasting blood glucose at the end of the study revealed a significant difference (p-value = 0.001). The results show that educating those afflicted with Diabetes Type 2 aiming at changing their diet can greatly help them manage their blood glucose.

Chloropropanols, including 3-monochloropropane-1,2-diol (3-MCPD) and 1,3-dichloropropan-2-ol (1,3-DCP), comprise a group of chemical contaminants with carcinogenic and genotoxic properties. They have been found in a variety of processed foods and food ingredients, such as hydrolyzed vegetable protein, soy sauce, cereal-based products, malt-derived ingredients, and smoked foods. This study aimed to assess the dietary exposure to 3-MCPD and 1,3-DCP in Brazil and verify whether the presence of these substances in foods could represent health risks. The intake was calculated by combining data on food consumption, provided by the Consumer Expenditure Survey 2008-2009, with the levels of contaminant occurrence determined by gas chromatography-mass spectrometry. The exposure to 3-MCPD ranged from 0.06 to 0.51 µg.kg bw-1.day-1 considering average and high consumers, while the intake of 1,3-DCP was estimated to be 0.0036 µg.kg bw-1.day-1 in the worst case scenario evaluated. Based on these results, it was verified that the Brazilians' exposure to chloropropanols does not present a significant health risk. However, the consumption of specific foods containing high levels of 3-MCPD could exceed the provisional maximum tolerable daily intake of 2 µg.kg bw-1 established for this compound and, therefore, represent a potential concern.
