INTRODUCTION: Viral hepatitis is a major public health concern in Brazil. There are few past studies on this issue, especially among riparian communities. This study aims at determining the seroprevalence of viral hepatitis B and C in the riparian community of Pacuí Island, within the Cametá municipality of Pará State, Brazil. Moreover, this study aims to investigate the principal risk factors that this community is exposed to. METHODS: The current study has accessed blood samples from 181 volunteers who have answered an epidemiological questionnaire. Analyses on serological markers have been tested with commercial ELISA kits for detecting HBsAg, total anti-HBc, anti-HBs, and anti-HCV. Within seroreactive patients for HCV, RT-PCR and line probe assay have been performed to identify the viral genotype. RESULTS: In the serological marker analysis for hepatitis B, no reactivity for HBsAg, rate of 1.1% for total anti-HBc, and rate of 19.3% for anti-HBs have been observed. On hepatitis C, 8.8% seroprevalence has been found, in which 62.5% have gotten viral RNA. Among the risk factors studied, the following have been highlighted: non-use of condoms, sharing of cutting instruments, use of illicit drugs, and reports of family disease with HBV or HCV. CONCLUSIONS: The vaccination coverage against HBV is low, and the high prevalence of HCV within this community has been observed.

INTRODUCTION: This study evaluated the degree of disability, pain levels, muscle strength, and electromyographic function (RMS) in individuals with leprosy. METHODS: We assessed 29 individuals with leprosy showing common peroneal nerve damage and grade 1 or 2 disability who were referred for physiotherapeutic treatment, as well as a control group of 19 healthy participants without leprosy. All subjects underwent analyses of degree of disability, electromyographic tests, voluntary muscle force, and the Visual Analog Pain Scale. RESULTS: McNemar's test found higher levels of grade 2 of disability (Δ = 75.9%; p = 0.0001) among individuals with leprosy. The Mann-Whitney test showed greater pain levels (Δ = 5.0; p = 0.0001) in patients with leprosy who had less extension strength in the right and left extensor hallucis longus muscles (Δ = 1.28, p = 0.0001; Δ = 1.55, p = 0.0001, respectively) and dorsiflexion of the right and left feet (Δ = 1.24, p = 0.0001; Δ = 1.45, p = 0.0001, respectively) than control subjects. The Kruskal-Wallis test showed that the RMS score for dorsiflexion of the right (Δ = 181.66 m·s-2, p = 0.001) and left (Δ = 102.57m·s-2, p = 0.002) feet was lower in patients with leprosy than in control subjects, but intragroup comparisons showed no difference. CONCLUSIONS: Leprosy had a negative influence on all of the study variables, indicating the need for immediate physiotherapeutic intervention in individuals with leprosy. This investigation opens perspectives for future studies that analyze leprosy treatment with physical therapeutic intervention.

INTRODUCTION: The aim of this study was to evaluate the therapeutic response of hepatitis C in patients coinfected with human immunodeficiency virus (HIV-1). METHODS: A retrospective study of 20 patients coinfected with HIV-1/HCV who were treated in the outpatient liver clinic at the Sacred House of Mercy Foundation Hospital of Pará (Fundação Santa Casa de Misericórdia do Pará - FSCMPA) from April 2004 to June 2009. Patients were treated with 180µg PEG interferon-α2a in combination with ribavirin (1,000 to 1,250mg/day) for 48 weeks. The end point was the sustained virological response (SVR) rate (HCV RNA negative 24 weeks after completing treatment). RESULTS: The mean age of the patients was 40±9.5 years, of which 89% (n=17) were male, and the HCV genotypes were genotype 1 (55%, n=11/20), genotype 2 (10%, n=2/20) and genotype 3 (35%, n=7/20). The mean CD4+ lymphocyte count was 507.8, and the liver fibrosis stages were (METAVIR) F1 (25%), F2 (55%), F3 (10%) and F4 (10%). The early virological response (EVR) was 60%, the end-of-treatment virological response (EOTVR) was 45% and the SVR was 45%. CONCLUSIONS: The median HCV viral load was high, and in 85% of cases in which highly active antiretroviral therapy (HAART) was used, none of the patients with F3-F4 fibrosis responded to treatment. Of the twenty patients treated, 45% achieved SVR and 45% achieved EOTVR. Studies that include cases from a wider region are needed to better evaluate these findings.

INTRODUCTION: Malaria caused by Plasmodium vivax species has shown signs of severity, recorded with increasing frequency in the medical literature. This study aimed to characterize the signs of severe malaria by Plasmodium vivax in the State of Maranhão, Brazil. METHODS: A descriptive cohort study of patients assisted in the field and a historical and concurrent study of a series of cases among hospitalized patients were undertaken to identify the clinical and laboratory signs of severity. RESULTS: A total of 153 patients were included in the study, 13 of whom were hospitalized. Males made up the majority, numbering 103 (67.3%). The age of the patients ranged from 10 to 70 years, 92.2% were natives of the State of Maranhão, and 65% of the patients had had malaria before. The average time elapsed between symptom onset and diagnosis among outpatients was three days, while among hospitalized patients this average reached 15.5 days, a statistically significant difference (p=0.001). The parasitemia ranged from 500 to 10,000 parasites/µl in 92.8% of cases. The clinical and laboratory manifestations of severity were vomiting and diarrhea, jaundice, drowsiness, mental confusion, seizures, loss of consciousness, agitation, bleeding, pale skin, coughing and dyspnea, thrombocytopenia, anemia, elevation of nitrogenous compounds, and elevated transaminases and bilirubin. CONCLUSIONS: The monitoring of malaria patients with Plasmodium vivax showed the possibility of aggravation, the intensity of which varied in different circumstances, especially the interval time between falling ill and diagnostic confirmation.

Introduction Aedes aegypti is responsible for the transmission of the dengue and yellow fever viruses. This study evaluated the effects of extracts from Cnidosculos phyllacanthus, Ricinus communis, and Coutarea hexandra on the developmental periods of A.aegypti larvae and pupae. Crude extracts of C. phyllacanthus and C. hexandra and oil from R. communis and C. phyllacanthus were used. Methods Bioassays of the larvicidal and pupicidal effects of these products at different concentrations and times of exposure were evaluated. The lethal and sublethal effects were determined using different concentrations in larvicidal tests. Mortality data were evaluated by Probit analysis to determine the LC50 and LC90 values. Results The vegetable oils from C. phyllacanthus and R. communis demonstrated greater efficiency for larval control with an LC50=0.28µl/mL and an LC90=1.48µl/mL and LC50=0.029µl/mL and a LC90=0.26µl/mL, respectively. In pupal tests toxic effects for all insects were verified after exposure to the products at significant LC50 and LC90 values for 24 and 48h. The effects of sublethal concentrations of C. phyllacanthus (oil) were more effective on the insects. Conclusions The vegetables oils from C. phyllacanthus and R. communis demonstrated greater potential from the control of different developmental periods in the life cycle of this insect.

Introduction Herpes simplex virus (HSV) and varicella zoster virus (VZV) are responsible for a variety of human diseases, including central nervous system diseases. The use of polymerase chain reaction (PCR) techniques on cerebrospinal fluid samples has allowed the detection of viral DNA with high sensitivity and specificity. Methods Serial dilutions of quantified commercial controls of each virus were subjected to an in-house nested-PCR technique. Results The minimum detection limits for HSV and VZV were 5 and 10 copies/µL, respectively. Conclusions The detection limit of nested-PCR for HSV and VZV in this study was similar to the limits found in previous studies.

Introduction Chronic hepatitis B virus (HBV) infection and liver steatosis (LS) are the most common causes of chronic liver disease, and their coexistence is frequently observed in clinical practice. Although metabolic syndrome is the main cause of LS, it has not been associated with HBV infection. The aims of this study were to describe the lipid profile and prevalence of LS among HBV carriers and to identify the characteristics associated with LS in this group. Methods This retrospective cross-sectional study included hepatitis B surface antigen (HBsAg)-positive patients evaluated during 2011 and 2012. Results Of the 83 patients included, the mean age was 46.4±12.5 years, 53% were men, and 9.1% were hepatitis B e antigen (HBeAg) -positive. These patients exhibited the following lipid profile: total cholesterol = 175.4±38.8mg/dL, low-density lipoprotein (LDL) = 113.0±32.7mg/dL, and triglycerides = 91.1±45.2mg/dL. Their fasting glucose was 95.3±14.5g/dL, and fasting insulin was 6.1±5.9µIU/mL. Liver steatosis was observed on abdominal ultrasound in 11.3% of individuals. Factors associated with the presence of LS included higher levels of total cholesterol, prothrombin activity, fasting insulin, and body mass index (BMI) as well as lower levels of aspartate aminotransferase (AST). Conclusions These findings suggest that LS in patients with chronic HBV appears to be a consequence of metabolic alterations and insulin action rather than of viral factors.

Introduction Thrombocytopenia is a common complication in malaria patients. The relationship between abnormal platelet profile and clinical status in malaria patients is unclear. In low and unstable endemic regions where vivax malaria predominates, the hematologic profiles of malaria patients and their clinical utility are poorly understood. The aim of this study was to characterize the thrombograms of malaria patients from Colombia, where Plasmodium vivax infection is common, and to explore the relationship between thrombograms and clinical status. Methods Eight hundred sixty-two malaria patients were enrolled, including 533 (61.8%) patients infected with Plasmodium falciparum, 311 (36.1%) patients infected with Plasmodium vivax and 18 (2.1%) patients with mixed infections. Results The most frequently observed changes were low platelet count (PC) and high platelet distribution width (PDW), which were observed in 65% of patients; thrombocytopenia with <50,000 platelets/µL was identified in 11% of patients. Patients with complications had lower PC and plateletcrit (PT) and higher PDW values. A higher risk of thrombocytopenia was identified in patients with severe anemia, neurologic complications, pulmonary complications, liver dysfunction, renal impairment and severe hypoglycemia. The presence of thrombocytopenia (<150,000 platelets/µL) was associated with a higher probability of liver dysfunction. Conclusions Young age, longer duration of illness and higher parasitemia are associated with severe thrombocytopenia. Our study showed that thrombocytopenia is related to malaria complications, especially liver dysfunction. High PDW in patients with severe malaria may explain the mechanisms of thrombocytopenia that is common in this group of patients.

INTRODUCTION: The aim of this study was to identify a rapid and simple phenotypic method for extended-spectrum β-lactamase (ESBL) detection in Enterobacter cloacae. METHODS: A total of 79 consecutive, non-repeated samples of E. cloacae were evaluated. Four phenotypic methods were applied for ESBL detection, results were compared to multiplex polymerase chain reaction (PCR) as the gold standard reference method: 1) ceftazidime and cefotaxime disks with and without clavulanate, both with boronic acid added; 2) disk approximation using cefepime and amoxicillin/clavulanate; 3) ESBL screening by minimum inhibitory concentration (MIC) ≥ 16µg/mL and 4) by MIC ≥ 2µg/mL for cefepime. RESULTS: Method 4 showed the best combination of sensitivity (100%) and specificity (94%). CONCLUSIONS: MIC ≥ 2µg/mL for cefepime would be very useful for the phenotypic detection of ESBL in samples of E. cloacae.

O presente trabalho apresenta os resultados de estudo sobre a ictiofauna (inventano, distribuição, aspectos ecológicos) da bacia do rio Trombetas, entre Oriximiná e o igarapé Caxipacoré, na área de influência da futura UHE Cachoeira Porteira. Foram realizadas seis expedições de coleta entre os anos de 1985 e 1988, sendo três no período de cheia e três na seca, em seis diferentes regiões. Os dados quantitativos foram obtidos utilizando-se uma bateria padronizada com onze malhadeiras, em pescarias de 24 horas. O esforço empregado com este aparelho por região e época foi de 1.544,97 m2/24 horas. Outros aparelhos e métodos também foram utilizados nas capturas de modo que o inventário fosse o mais completo possível. No total foram coletadas 342 espécies de peixes, pertencentes a 11 ordens e 43 famílias. Nas pescarias padronizadas foram capturados 10.806 exemplares, com 2.181.735 gramas, correspondendo a 228 espécies, de 9 ordens e 30 famílias. O rio Trombetas apresenta duas regiões distintas no seu curso, sendo a cachoeira Porteira, o ponto de divisão. A ictiofauna apresenta distribuição espacial relacionada com esta separação, cerca de um terço das espécies são exclusivas da região de jusante e outro terço ocorre apenas na região de montante. As cachoeiras e corredeiras são importantes barreiras à dispersão das espécies dentro do rio Trombetas, mas as condições ambientais gerais parecem também influenciar esta distribuição. Das espécies que tiveram a alimentaçãoanalisada a maioria dependia de alimentos de origem autóctone. As espécies piscívoras foram dominantes em biomassa na maioria das regiões e épocas.

Este trabalho analisou dados de três inventários florestais realizados na área da Floresta Experimental, pertencente à Embrapa Amazônia Ocidental e localizada no km 54 da BR-174, em Manaus/AM. O objetivo foi estudar a dinâmica da floresta - taxas de incremento, recrutamento e mortalidade - além do estoque de carbono, em uma área de floresta primária, sem qualquer tipo de intervenção. Os inventários foram realizados nos anos de 2005, 2007 e 2010, em 15 parcelas permanentes de 1 hectare cada, onde foram mensuradas todas as árvores com diâmetro a 1,3 m de altura do solo (DAP) superior a 10 cm. Foram calculadas as taxas de recrutamento e mortalidade, os incrementos periódicos anuais (IPAs) em termos de DAP, área basal e volume, as biomassas fresca e seca acima do nível do solo e total e o estoque de carbono da vegetação. As taxas de recrutamento foram de 2% e 1,8% e as de mortalidade foram de 0,95% e 1,3% para os períodos entre 2005 e 2007 e 2007 e 2010, respectivamente. O volume foi de 345,62 m3 ha-1 para o ano de 2005, aumentando para 360,67 m3 ha-1 em 2010, com IPAs de 4,32 m3 ha-1 ano-1 entre 2005 e 2007 e 1,31 m3 ha-1 ano-1 entre 2007 e 2010. O estoque de carbono total teve um acréscimo de 173,63 t C ha-1 em 2005 para 181,01 t C ha-1 em 2010, confirmando que a floresta acumulou carbono no período, atuando assim como sumidouro.

A participação dos metais pesados nas reações químicas do metabolismo dos peixes tornou esses elementos um dos principais focos nos estudos de contaminação. O objetivo deste estudo foi determinar as concentrações dos metais Cd, Cr, Cu, Pb, Zn e Hg no tecido muscular de peixes da bacia do rio Cassiporé (Estado do Amapá) e ambiente, visando avaliar os riscos de contaminação. As concentrações de Cd em Plagioscion squamosissimus (1,061 µg g-1) e Pb em Poptella compressa (4,280 µg g-1) apresentaram valores que ultrapassam os limites estabelecidos pela legislação brasileira. As concentrações de Cr em P. compressa (0,937 ± 0,5 µg g-1), Pimelodella cristata (0,463 ± 0,03 µg g-1) e Cyphocharax gouldingi (0,364 ± 0,12 µg g-1), bem como de Hg em P. squamosissimus (0,670 µg g-1), Pseudoplatystoma fasciatum (0,630 µg g-1), Hoplias malabaricus (0,570 µg g-1 e Serrasalmus rhombeus (0,548 µg g-1) foram acima do limite legal. A coluna d'água apresentou altas concentrações de metais pesados em todos os pontos analisados, mostrando uma contaminação dessa bacia. Portanto, essas elevadas concentrações de metais pesados no ambiente e tecido muscular dos peixes indicam um elevado grau de contaminação na bacia do rio Cassiporé e risco a saúde do homem.

OBJECTIVE: To assess the clinical and demographic characteristics of a population with ischemic heart disease admitted in the final decades of the 20th century. METHODS: This study retrospectively assessed patients hospitalized with ischemic heart disease divided into the following 2 groups: acute group - 11.181 patients with acute myocardial infarction admitted from 1/1/82 to 12/31/94; and chronic group - 4.166 patients undergoing coronary artery bypass graft surgery from 1/1/84 to 12/31/94. RESULTS: In the acute group, an increase in the percentage of females (from 22.7% to 27.7%, P<0.001) and diabetic individuals (from 12.4% to 17.5%, P<0.001) was observed, as was an increase in age (from 57.4±11.5 to 59.9±12.1 years, P<0.05). In-hospital mortality was greater among females (27.8% and 15.7%, P=0.001), among diabetic individuals (24.2% and 17.8%, P=0.001), and among the elderly (60.9±15.2 and 57.7±11.8 years, P=0.0001). In the chronic group, an increase in the percentage of females (from 17.5% to 27.2%, P=0.001) was observed, as was an increase in age (from 56.3±8.6 to 60.5±9.6 years, P=0.0001). In-hospital mortality was greater among females (8.3% and 5.8%, P<0.05) and among the elderly (58.1±9.1 and 62.1±7.9 years, P=0.0001). CONCLUSION: The characteristics of the population studied with ischemic heart disease point towards a worse prognosis, due to the greater percentages of females, older patients, and diabetic patients, groups known to have greater in-hospital mortality.

OBJETIVO: Analisar os parâmetros do eletrocardiograma de alta resolução da onda P no domínio do tempo (ECGAR-P) e compará-los com: a duração da onda P no eletrocardiograma clássico (P no ECG), o diâmetro atrial esquerdo (AE) e a fração de ejeção do ventrículo esquerdo (FE) obtidos no ecocardiograma, para avaliar pacientes com fibrilação atrial paroxística (FAP). MÉTODOS: Foram estudados 181 pacientes: 117 com FAP comprovada e 64 sem FAP. Os parâmetros do ECGAR-P foram: a duração da onda P filtrada (DPF), as voltagens da raiz quadrada média dos últimos 40, 30 e 20 ms da onda P filtrada (RMS 40, RMS 30 e RMS 20), a voltagem da raiz quadrada média dos potenciais da onda P filtrada (RMS P), a integral dos potenciais da onda P filtrada (Integral P) e a duração dos potenciais tardios da onda P filtrada abaixo de 3 µV (PT<3). RESULTADOS: Os parâmetros que apresentaram diferenças estatisticamente significantes entre os grupos foram: DPF, RMS 40, 30 e 20, PT<3, P no ECG e AE. Os cálculos pela curva ROC mostraram, para cada parâmetro, o melhor valor de corte e os estimadores de desempenho: sensibilidade, especificidade, área sob a curva e p-value (p) ou nível descritivo. CONCLUSÃO: O ECGAR-P no domínio do tempo mostrou-se melhor que o eletrocardiograma clássico e o ecocardiograma para identificar pacientes com fibrilação atrial paroxística.

OBJETIVOS: Avaliar a ativação elétrica cardíaca usando Mapeamento Eletrocardiográfico de Superfície (MES), em pacientes com ICC e bloqueio de ramo esquerdo [BRE] submetidos a terapia de ressincronização cardíaca (CRT) com implante de marca-passo átrio-biventricular (MP-BIV). MÉTODOS: Foram analisados os tempos médios de ativação elétrica cardíaca no ventrículo direito (tempo médio de ativação do VD [mVD]), área ântero-septal (mAS), e ventrículo esquerdo (mVE), de 28 pacientes (idade média 61,2±9,5 anos, ICC classe III-IV NYHA, fração de ejeção <40%, BRE com QRS médio 181,2±19,4ms, SÂQRS= -8,5º±68,6º), mostrados nos mapas de linhas isócronas do MES, antes e após implante de marca-passo átrio-biventricular, e comparados a valores obtidos em um grupo controle composto de indivíduos normais [GNL], em três situações: (1) BRE nativo, (2) estimulação do VD; e (3) estimulação átrio-biventricular. RESULTADOS: situação (1): mVD e mAS foram semelhantes (41,0±11,8ms x 43,6±13,4ms), com mVE tardio (81,0±12,5ms, p<0,01) perdendo o sincronismo com o VD e a área ântero-septal; situação (2): mVD foi maior que no GNL (86,8±22,9ms, p<0,001), com maior diferença entre mAS e mVE (63,4±20,7ms x 102,7±20,3ms; p<0,001); situação (3): mVE e mVD foram semelhantes (72,0±32,0ms x 71,6±32,3ms), mVD foi maior que no GNL e BRE nativo (71,6±32,3ms x 35,1±10,9ms e 41,0±11,8ms; p<0,001), mAS se aproximou do GNL e BRE nativo (51,3±32,8ms x 50,1±11,4ms e 43,6±13,4ms). CONCLUSÃO: Pelo mapeamento eletrocardiográfico de superfície, tempos de ativação semelhantes no VD e VE e próximos daqueles da região ântero-septal indicam padrões de ativação ventricular sincronizada em portadores de ICC e BRE durante estimulação átrio-biventricular.
