FUNDAMENTO: A insuficiência cardíaca descompensada (ICD) é uma condição bastante prevalente e com alta mortalidade. O levosimendan está entre as novas drogas que têm sido testadas para o seu manejo. OBJETIVO: Realizar uma revisão sistemática da literatura e uma metanálise da redução de morbimortalidade associada ao levosimendan no tratamento da ICD. MÉTODOS: Foi feita uma pesquisa bibliográfica no Medline buscando todos os ensaios clínicos randomizados (ECRs) que avaliassem o uso do levosimendan na ICD. Os desfechos de interesse foram: morte por todas as causas, tempo de internação hospitalar e reinternação hospitalar por ICD. Todos os ECRs com desfechos de interesse foram incluídos. Critérios de qualidade metodológica, como cegamento e sigilo da lista de alocação, foram avaliados em análise de sensibilidade. O cálculo principal foi feito com efeitos randômicos. RESULTADOS: Dos 179 artigos identificados, 48 eram ECRs, sendo 19 com desfechos clínicos de interesse. Na comparação com placebo (7 ensaios clínicos, 1.652 pacientes), o risco relativo (RR) para morte total foi de 0,87 (intervalo de confiança [IC] 95%: 0,65 - 1,18). Na comparação com dobutamina (10 ensaios clínicos, 2.067 pacientes), o RR foi de 0,87 (IC 95%: 0,75 - 1,02). Três estudos tinham dados sobre tempo de internação, onde o levosimendan mostrou diminuição de 2,27 e 2,30 dias em relação ao placebo e a dobutamina, respectivamente (p < 0,05 para ambos). Nenhum artigo apresentou isoladamente dados sobre reinternação. CONCLUSÃO: As evidências disponíveis até o momento não mostram benefício em termos de mortalidade associada ao levosimendan, que apresentou benefício de pequena magnitude apenas no tempo de internação.

FUNDAMENTO: O ecoDopplecardiograma (Eco) tem-se firmado como método não invasivo de excelente acurácia no rastreio de hipertensão portopulmonar (HPP) e pesquisa dos shunts intrapulmonares (SIP) na doença hepática crônica (DHC). Nesta última década, concretizou-se o Eco como de fundamental importância no diagnóstico da cardiomiopatia cirrótica (CMC). OBJETIVO: Realizar uma revisão sistemática envolvendo artigos relevantes sobre a temática: o Eco na DHC. MÉTODOS: Em novembro de 2011, realizou-se uma revisão sistemática a partir das bases de dados da Pubmed, LILACS e SciELO, sendo relatadas as características dos estudos selecionados. RESULTADOS: Foram encontrados 204 artigos a partir da busca de descritores e um termo livre, sendo 179 da Pubmed, 21 da LILACS e 01 da SciELO. Destes, foram selecionados 22 artigos para revisão sistemática, não sendo possível, pois, fazer uma meta-análise devido à heterogeneidade dos mesmos. CONCLUSÃO: O Eco deve fazer parte da estratificação da DHC para o rastreio de HPP, SIP e da CMC, já que a maioria das vezes, o diagnóstico dessas complicações ocorre nos pacientes na fila de transplante hepático.

Com o objetivo de se estudar a marcha de absorção de macronutrientes pela colza (Brassica napus) , em função da idade da planta instalou-se um experimento em condições de campo. Determinou-se a curva de crescimento da planta em função da idade, o acumulo de nutrientes e a concentração de nutrientes em duas linhagens (1504 e 1530). O experimento foi conduzido no ano agrícola de 1980, no solo argiloso Terra Roxa Estruturada, série Luiz de Queiroz. Após cada amostragem as plantas foram separadas em raíz, caule, folha, flor, vagem e semente e analisadas quimicamente para o nitrogênio, fósforo, potássio, cálcio, magnésio e enxofre. Nas condições em que o experimento foi conduzido pode-se concluir que: a. o ponto de máximo desenvolvimento da planta e produção de matéria seca é atingido aos 142 e 179 dias, após a emergência, para as linhagens 1504 e 1530, respectivamente; b. as concentrações dos macronutrientes diminuem com o aumento da quantidade de matéria seca, mostrando o efeito de diluição; c. o acúmulo dos macronutrientes atinge um máximo no fim do florescimento, entre 120 e 139 dias, para as duas linhagens; d. a ordem na absorção de nutrientes é a seguinte: K > N > Ca > S > P > Mg; e. a ordem de exportação dos nutrientes é: N > P > K = Ca > S > Mg.

Callinectes danae (Smith, 1869), siri-azul, constitui um importante recurso pesqueiro nos estuários dos rios Botafogo e Carrapicho. O objetivo do presente estudo foi estimar a maturidade sexual das fêmeas de C. danae. Os espécimes foram capturados, entre outubro de 2003 a junho de 2004, com auxílio de um barco de pesca equipado com rede de arrasto do tipo "wing-trawl" e arrastados durante 5 minutos. No laboratório, os siris foram contados, numerados, sexados, pesados e mensurados. A largura da carapaça (LC) foi medida na base do espinho lateral e a largura do abdome (LA) mensurada na altura da articulação do quinto esternito abdominal. Um total de 596 fêmeas de C. danae foram analisadas: 417 (69,97%) no rio Botafogo e 179 (30,03%) no Carrapicho; as fêmeas não-ovígeras apresentaram LC de 18,38 a 101 mm (59,14 ± 13,65 mm) e 26,70 a 83,48 mm (59,16 ± 13,77 mm), nos respectivos rios; a LC de fêmeas ovígeras foi de 57,04 a 83,30 mm (67,68 ± 6,56 mm). As médias de larguras das carapaças não apresentaram diferenças estatisticamente significativas O conhecimento da maturidade sexual das fêmeas de Callinectes danae é de grande importância para o seu manejo e conservação. O L50 gonadal foi estimado em 63,58 mm (chi2 = 140,47; g.l. = 1; p < 0,01) e 61,59 mm (chi2 = 90,94; g.l. = 1; p < 0,01) e o L50 morfológico foi de 57,13 mm (chi2 = 484,51; g.l. = 1; p < 0,01) e 56,46 mm (chi2 = 257,82; g.l. = 1; p < 0,01), nos estuários dos rios Botafogo e Carrapicho, respectivamente. Em ambos os estuários, a maturidade morfológica de fêmeas de C. danae ocorreu antes da maturidade gonadal e para garantir que esta espécie seja manejada com êxito, sugere-se que a pesca seja permitida apenas em indivíduos com largura da carapaça superior a 65 mm.

Miniature light traps used to collect Phlebotominae in a focus of dermal leishmaniasis in the eastern part of the State of Minas Gerais, Brazil. Over a period of seven months, the other Diptera captured in 179 light trap samples were identified to family level. The traps were placed in eight localities which constituted three different biotopes: three woodland aresas, cultivated land, and a peridomestic site. A comparison is made between the totals of Dipeterans collected in each biotope, the total numbers of families collected in each biotope and the estimated indices of diversity. Dendograms representing the degrees of association between families of Diptera in different biotopes are presented. Some families of Diptera are uniformly distributed throughout the study area; a few families seem to have become adapted to areas where human activity has induced the greatest ecological changes. The impact between Dipterans and human well-being is discussed. The availabel evidence indicates that transmission of dermal leishmaniasis does not occur in areas where sand flies can be captured in greatest densities.

Foureen marmosets (Callithrix penicillata) were inoculated intradermally with promastigotes and/or amastigotes of Leishmania (Viannia) brazilensis (L. (V) b.) strains MHOM/BR/83/LTB-300MHOM/BR/85/LTB-12 MHOM/BR/81/LTB-179 and MHOM/BR/82/LTB-250. The evolution of subsequent lesions was studied for 15 to 75 weeks post-inoculation (PI). All but of the L. (V) b. injected marmosets developed a cutaneous lesion at the point of inoculation after 3 to 9 weeks, characterized by the appearance of subcutaneous nodules containing parasites. parasites were isolated by culture (Difco Blood Agar) from all 11 positive animals. The maximum size of the lesions was variable and ranged between 37 mm² to 107 mm². Ulceration of primary nodules became evident after 3 to 12 weeks in all infected marmosets, but was faster and larger in 5 of the 11 animals. The active lesions persisted in 9 out of 11 Callithrix until the en of the observation period, which varied from 15-75 weeks. In 3 animals spontaneous healing of their lesions (13 to 25 weeks, PI) was observed buth with cryptic parasitism. In another 2 infected animals there was regression followed by reactivation of the cutaneous lesions. The appearance of smaller satellite lesions adjacent to primary ones, as well as metastatic lesions to the ear lobes, were documented in 2 animals. Promastigotes of L. (Leishmania) amazonensis (L.(L)a.) MHOM/BR/77/LTB-16 were inoculated in 1 marmoset. This animal remained chronically infected for 6 months and the lesions developed in a similar manner to L.(V)b. infected marmosets. No significant differences in clinical and parasitological behaviour were observed between promastigote or amastigote derived infections of the 2 species. Both produced chronic, long lasting lesions which eventually healed. The same was true for parameters of size and ulceration. Skin tests converted to parasite in 11 of 15 inected masmosets and in 10 of 12 parasite positive animnals. Moderate levels of circulating antibodies were also observed by IFAT /IgG assays. In spite of the failure to reproduce the mucosal form of the disease, an important aspect of the Callithrix model in experimental cutaneous leishmaniasis lies in the reproduction of 2 clinical events that are common in humans, namely, the chronic ulceration and spontaneous healing of the lesions.

Three species of protistan and 22 species of metazoan parasites were obtained from a sample of 179 flatfish, (Paralichthys adspersus) taken-off Antofagasta, northern Chile. Prevalence of infection of seven parasites (Protista: 1, Copepoda: 2, Digenea: 1, Acantocephala: 1, Nematoda: 2) was significantly and positively correlated with host size. Host's sex do not seem to affect prevalence of infection, except for Nybelinia surmenicola, Capillaria sp. and Anisakis sp. (prevalence of infection significantly greater in males than females) and Philometra sp. (prevalence higher in females). Mean abundance is correlated with size in nine species (Protista: 1, Copepoda: 2, Digenea: 3, Acantocephala: 1, Nematoda: 2). Host's sex do not affect mean abundance, except for Cainocreadium sp. and Philometra sp.(mean abundance higher in females) and Nybelinia surmenicola, Capillaria sp. and Anisakis sp. (mean abundance higher in males).

Data from the Chagas Disease Control Program indicate a growing domiciliary and peridomiciliary invasion of Triatoma rubrovaria in the State of Rio Grande do Sul, where it has become the most frequent triatomine species captured there since the control of T. infestans. Bionomic characteristics that could influence the vectorial capacity of T. rubrovaria as vector of Trypanosoma cruzi were evaluated: patterns of (i) feeding, (ii) defecation, and (iii) resistance to starvation, using insects fed on mice. Fifty three percent of the females showed a defecation pattern conducive to chagasic transmission, defecating either on or near the bite site. The averages of the resistance to starvation varied from 48.1 to 179 days, for the first and fifth nymphal stages, respectively. Our study shows that with respect to the patterns of feeding, defecation and resistance to fasting, T. rubrovaria presented similar rates to the ones observed for other effective vector species, such as T. infestans. Thus, based on our studies we conclude that T. rubrovaria has biological characteristics that can positively influence its capacity to become infected and transmit T. cruzi, and also to keep residual populations after chemical control interventions.

This work had the objective of assessing the present epidemiological situation regarding schistosomiasis through performing Kato-Katz coproscopic tests on representative samples of schoolchildren from each of the 43 municipality of endemic area of the state of Pernambuco, Brazil. The methodology is recommended by the World Health Organization to conduct sampled surveys among children at elementary school levels, ideal target group for baseline surveys: (i) schools are accessible; (ii) the greatest prevalence of schistosomiasis is found within this group; (iii) the data gathered from this age group can be used for intervention within the community as a whole. The following infection indicators were utilized: positivity (percentage of individuals examined with eggs of Schistosoma mansoni in the feces) and severity (geometric mean number of eggs per gram of feces, epg). These indicators allowed the area in general and the municipalities in particular to be categorized into prevalence and severity classes for S. mansoni. The prevalence classes were: low (<10%), medium (> 10 and < 50%), and high (> 50%); the severity classes were: low (1-99 epg), moderate (100-399 epg), and severe (> 400 epg). For the geohelminthic diseases, the following indicators were used: positivity for each geohelminth (percentage of individuals examined with eggs of geohelminths), and cumulative positivity (percentage of individuals examined with eggs of at least one geohelminth). The municipalities were categorized by means of their cumulative positivity into the following geohelminth prevalence classes (WHO 2002): low (< 50%), medium (> 50 and < 70%), and high (> 70%). The study covered 271 schools in 179 different localities, thus giving a total of 11,234 examinations performed. The overall positivity for S. mansoni was 14.4% and the egg count for this parasite in the feces gave a geometric mean of 67.9 epg which suggests a low general state of infection. These results allow this mesoregion to be categorized as presenting medium prevalence and low severity of schistosomiasis. The overall positivity rates for the geohelminths, Ascaris lumbricoides, Ancylostomidae, and Trichuris trichiura were, respectively, 30.4, 10.1, and 27.8%; the cumulative positivity was 45.4%. These results allow this mesoregion to be categorized as presenting low prevalence of geohelminthic diseases. The data show some municipalities in Pernambuco with prevalence greater than 20%, while others presented parasite loads greater than 100 epg. These indicators attest to the significant morbidity due to schistosomiasis regarding to the severity of infections established in young populations.

An in-house, low-cost method was developed to determine the genotypic resistance of immunodeficiency virus type 1 (HIV-1) isolates. All 179 Venezuelan isolates analysed belonged to subtype B. Primary drug resistance mutations were found in 11% of 63 treatment-naïve patients. The prevalence of resistance in isolates from 116 HIV-positive patients under antiretroviral treatment was 47% to protease inhibitors, 65% to nucleoside inhibitors and 38% to non-nucleoside inhibitors, respectively. Around 50% of patients in the study harboured viruses with highly reduced susceptibility to the three classical types of drugs after only five years from their initial diagnoses.

The host immune response plays an important role in viral clearance in patients who are chronically infected with hepatitis C virus (HCV) and are treated with interferon and ribavirin. Activation of the immune system involves the release of pro and anti-inflammatory molecules that can be measured in plasma samples. The present study aimed to evaluate the association between pretreatment plasma levels of chemokines and soluble tumor necrosis factor receptors (sTNF-R) and the virological response in treated patients with chronic hepatitis C infection. Forty-one chronically-infected HCV patients that were being treated with interferon-α (IFN-α) plus ribavirin were included in the study. Socio-demographic, clinical and laboratory data were collected and pretreatment plasma levels of chemokine CCL2, CCL3, CCL11, CCL24, chemokine CXCL9, CXCL10, sTNF-R1 and sTNF-R2 were measured. The virological response was assessed at treatment week 12, at the end of treatment and 24 weeks after treatment. Pretreatment CXCL10 levels were significantly higher in patients without an early virological response (EVR) or sustained virological response (SVR) compared to responders [512.9 pg/mL vs. 179.1 pg/mL (p = 0.011) and 289.9 pg/mL vs. 142.7 pg/mL (p = 0.045), respectively]. The accuracy of CXCL10 as a predictor of the absence of EVR and SVR was 0.79 [confidence interval (CI) 95%: 0.59-0.99] and 0.69 (CI 95%: 0.51-0.87), respectively. Pretreatment plasma levels of the other soluble inflammatory markers evaluated were not associated with a treatment response. Pretreatment CXCL10 levels were predictive of both EVR and SVR to IFN-α and ribavirin and may be useful in the evaluation of candidates for therapy.

Two snapshot surveys to establish the diversity and ecological preferences of mosquitoes (Diptera: Culicidae) in the terra firme primary rain forest surrounding the Tiputini Biodiversity Station in the UNESCO Yasuní Biosphere Reserve of eastern Amazonian Ecuador were carried out in November 1998 and May 1999. The mosquito fauna of this region is poorly known; the focus of this study was to obtain high quality link-reared specimens that could be used to unequivocally confirm species level diversity through integrated systematic study of all life stages and DNA sequences. A total of 2,284 specimens were preserved; 1,671 specimens were link-reared with associated immature exuviae, all but 108 of which are slide mounted. This study identified 68 unique taxa belonging to 17 genera and 27 subgenera. Of these, 12 are new to science and 37 comprise new country records. DNA barcodes [658-bp of the mtDNA cytochrome c oxidase ( COI ) I gene] are presented for 58 individuals representing 20 species and nine genera. DNA barcoding proved useful in uncovering and confirming new species and we advocate an integrated systematics approach to biodiversity studies in future. Associated bionomics of all species collected are discussed. An updated systematic checklist of the mosquitoes of Ecuador (n = 179) is presented for the first time in 60 years.

The aim of the present study was to assess the prevalence of Haemophilus influenzaetype b (Hib) nasopharyngeal (NP) colonisation among healthy children where Hib vaccination using a 3p+0 dosing schedule has been routinely administered for 10 years with sustained coverage (> 90%). NP swabs were collected from 2,558 children who had received the Hib vaccine, of whom 1,379 were 12-< 24 months (m) old and 1,179 were 48-< 60 m old. Hi strains were identified by molecular methods. Hi carriage prevalence was 45.1% (1,153/2,558) and the prevalence in the 12-< 24 m and 48-< 60 m age groups were 37.5% (517/1,379) and 53.9% (636/1,179), respectively. Hib was identified in 0.6% (16/2,558) of all children in the study, being 0.8% (11/1,379) and 0.4% (5/1,179) among the 12-< 24 m and 48-< 60 m age groups, respectively. The nonencapsulate Hi colonisation was 43% (n = 1,099) and was significantly more frequent at 48-< 60 m of age (51.6%, n = 608) compared with that at 12-< 24 m of age (35.6%, n = 491). The overall resistance rates to ampicillin and chloramphenicol were 16.5% and 3.7%, respectively; the co-resistance was detected in 2.6%. Our findings showed that the Hib carrier rate in healthy children under five years was very low after 10 years of the introduction of the Hib vaccine.

Este estudo objetivou adaptar culturalmente e analisar as propriedades psicométricas da versão brasileira da Underwood's Daily Spiritual Experience Scale (DSES). A adaptação seguiu as etapas internacionalmente recomendadas e a versão adaptada manteve equivalência com a original, após ajustes na redação de cinco itens. Na aplicação a 179 pacientes médico-cirúrgicos mostrou evidências de consistência interna (alfa de Cronbach=0,91), estabilidade temporal (ICC=0,94 no teste e reteste) e validade de construto convergente, na correlação com a subescala Religiosidade Intrínseca do instrumento DUREL (r=0,56; p<0,001). A análise fatorial exploratória extraiu três componentes, explicando 60,5% da variância do total. A versão brasileira da DSES apresenta evidências de confiabilidade e validade junto a pacientes hospitalizados. São necessários mais estudos para confirmar a sua composição fatorial e testar a sua aplicabilidade em diferentes populações.
