We assessed the frequency of serological markers of hepatitis B virus (HBV) and hepatitis C virus (HCV) infections in 365 alcoholics by determining, by ELISA, the presence of HBsAg, anti-HBc, anti-HBs and anti-HCV. Fifty patients were cirrhotics and 315 had no evidence of hepatic cirrhosis; of the latter HBsAg was assessed in all, anti-HBc and anti-HBs in 130, and anti-HCV in 210. Among the alcoholics the frequencies of HBsAg (1.9%), anti-HBc (28.3%) and anti-HCV (3.8%) were higher (p<0.001) than among the controls (N=17,059), 0.4%, 4.0% and 0.4% respectively. The frequency of positive HBsAg was higher (p<0.001) in the cirrhotic patients (8.0%) than in alcoholics without cirrhosis (0.95%) and in controls (0.4%), and similar between the latter; of anti-HBc in alcoholics without cirrhosis (28.5%) was similar in cirrhotics patients (28.0%) and higher (p<0.001) than in the controls (4.0%); of anti-HBs in alcoholics without cirrhosis (20.8%) was similar to that of the cirrhotic patients (10.0%), and the anti-HCV was similar between alcoholics with (6.0%) and without cirrhosis (3.3%) and higher (p<0.001) than in controls (0.4%). We concluded that: a) alcoholics with or without cirrhosis have similar frequencies of infection with HBV and HCV between them, and higher than in nonalcoholics; b) alcoholics without cirrhosis had a frequency of HBV active infection (HBsAg+) which was similar to the controls, whereas among those who progressed to cirrhosis this frequency was significantly higher, what suggests that HBV may be implicated in the pathogenesis of cirrhosis in a few alcoholic individuals.

As little is known about liver histology in the co-infection of hepatitis C virus (HCV) and hepatitis G virus (HGV), HGV RNA was investigated in 46 blood donors with hepatitis C, 22 of them with liver biopsy: co-infection HCV / HGV (n = 6) and HCV isolated infection (n = 16). Besides staging and grading of inflammation at portal, peri-portal and lobular areas (Brazilian Consensus), the fibrosis progression index was also calculated. All patients had no symptoms or signs of liver disease and prevalence of HGV / HCV co-infection was 15.2%. Most patients had mild liver disease and fibrosis progression index, calculated only in patients with known duration of infection, was 0.110 for co-infection and 0.130 for isolated HCV infection, characterizing these patients as "slow fibrosers". No statistical differences could be found between the groups, although a lesser degree of inflammation was always present in co-infection. In conclusion co-infection HCV / HGV does not induce a more aggressive liver disease, supporting the hypothesis that HGV is not pathogenic.

The lack of specific laboratorial diagnosis methods and precise symptoms makes the toxocariasis a neglected disease in Public Health Services. This study aims to determine the frequency of Toxocara spp. infection in children attended by the Health Public Service of Hospital Municipal de Maringá, South Brazil. To evaluate the association of epidemiological and clinical data, an observational and cross-section study was carried out. From 14,690 attended children/year aged from seven month to 12 years old, 450 serum samples were randomly collected from September/2004 to September/2005. A questionnaire was used to evaluate epidemiological, clinical and hematological data. An ELISA using Toxocara canis larval excretory-secretory products as antigen detected 130 (28.8%) positive sera, mainly between children from seven month to five years old (p = 0.0016). Significant correlation was observed between positive serology for Toxocara, and frequent playing in sandbox at school or daycare center (p = 0.011) and the presence of a cat at home (p = 0.056). From the families, 50% were dog owners which exposed soil backyards. Eosinophilia (p = 0.776), and signs and symptoms analyzed (fever p = 0.992, pneumonia p = 0.289, cold-like symptoms p = 0.277, cough p = 0.783, gastrointestinal problems p = 0.877, migraine p = 0.979, abdominal pain p = 0.965, joint pain p = 0.686 and skin rash p = 0.105) could not be related to the presence of anti-Toxocara antibodies. Therefore, two asthmatics children showed titles of 1:10,240 and accentuated eosinophilia (p = 0.0001). The authors emphasize the needs of prevention activities.

This study examined the susceptibility of peritoneal macrophage (PM) from the Neotropical primates: Callithrix jacchus, Callithrix penicillata, Saimiri sciureus, Aotus azarae infulatus and Callimico goeldii to ex vivo Leishmania (L.) infantum chagasi-infection, the etiological agent of American visceral leishmaniasis (AVL), as a screening assay for evaluating the potential of these non-human primates as experimental models for studying AVL. The PM-susceptibility to infection was accessed by the PM-infection index (PMI) at 24, 72 h and by the mean of these rates (FPMI), as well as by the TNF-α, IL-12 (Capture ELISA) and Nitric oxide (NO) responses (Griess method). At 24h, the PMI of A. azarae infulatus (128) was higher than those of C. penicillata (83), C. goeldii (78), S. sciureus (77) and C. jacchus (55). At 72h, there was a significant PMI decrease in four monkeys: A. azarae infulatus (128/37), C. penicillata (83/38), S. sciureus (77/38) and C. jacchus (55/12), with exception of C. goeldii (78/54). The FPMI of A. azarae infulatus (82.5) and C. goeldii (66) were higher than C. jacchus (33.5), but not higher than those of C. penicillata (60.5) and S. sciureus (57.5). The TNF-a response was more regular in those four primates which decreased their PMI at 24/72 h: C. jacchus (145/122 pg/mL), C. penicillata (154/130 pg/mL), S. sciureus (164/104 pg/mL) and A. azarae infulatus (154/104 pg/mL), with exception of C. goeldii (38/83 pg/mL). The IL-12 response was mainly prominent in A. infulatus and C. goeldii which presented the highest FPMI and, the NO response was higher in C. goeldii, mainly at 72 h. These findings strongly suggest that these New World primates have developed a resistant innate immune response mechanism capable of controlling the macrophage intracellular growth of L. (L.) i. chagasi-infection, which do not encourage their use as animal model for studying AVL.

Candida albicans is often isolated from clinical samples, thus its presumptive differentiation from other species of the same genus can be based on its ability to form the germ tube in human serum. Nevertheless, there are two other species that share this characteristic: C. dubliniensis and C. africana. The aim of this study was to compare four different substrates to perform the germ tube (GT) test. The Candida spp. isolates were identified using a manual system (135 C. albicans, 24 C. tropicalis and one C. dubliniensis). The germ tube test was performed with fresh, previously frozen serum and Mueller-Hinton (MH) broth and agar. GT was observed in 96% (130/136) of the isolates through the fresh serum technique, 94% (128/136) through previously frozen serum, 92% (125/136) in MH agar, and 90% (122/136) in MH broth. The sensitivity of each test was higher than 90%, with 100% specificity. Both the MH agar and broth were able to identify the true positives, and false positives were not found. However, some C. albicans isolates were not identified. MH agar and broth may be used in laboratory for the rapid presumptive identification of C. albicans, as an alternative method for germ tube test.

SUMMARY The aim of this work was to compare, from a parasitological ( Cryptosporidiumspp. and Giardia duodenalis), bacteriological (total and thermotolerants coliforms) and physicochemical perspective, water sources used for drinking and irrigation of vegetables intended to be sold for human consumption. From January 2010 to May 2011, samples of different water sources from vegetable producing properties were collected; 100 liters for parasitological analysis, 200 mL for bacteriological analysis, and five liters for physicochemical analysis. Water samples were filtered under vacuum with a kit containing a cellulose acetate membrane filter, 1.2 µm (Millipore(r), Barueri, SP, Brazil). The material retained on the membrane was mechanically extracted and analyzed by direct immunofluorescence (Merifluor(r)kit). From 20 rural properties investigated, 10 had artesian wells (40 samples), 10 had common wells (40 samples), and one had a mine (four samples), the latter contaminated by Cryptosporidiumspp. In samples from artesian wells, 90 to 130 meters depth, 42.5% were positive for total coliforms and 5.0% were identified to have abnormal coloration. From the samples of common wells, 14 to 37 meters depth, 87.5% were contaminated with total coliforms, 82.5% were positive for thermotolerant coliforms, and 12.5% had color abnormalities. We did not detect the presence of Giardiaspp. or Cryptosporidiumspp. in artesian and common wells. The use of artesian or common wells is an important step in the control of the spreading of zoonoses, particularly Cryptosporidiumspp. and Giardiaspp., as well as artesian wells for coliform control in local production of vegetables to be marketed.

Demonstramos, no depósito subungueal de 130 crianças residentes no bairro de Nova Descoberta, Natal - RN, a presença de ovos ou cistos de enteroparasitos, tais como: Ascaris lumbricoides, Trichocephalus trichiurus, Enterobius vermicularis, Ancylostomidae, Entamoeba coli, Entamoeba histolytica e Giardia lamblia. Ao lado da demonstração dos referidos parasitos, fizemos a correlação entre a incidência parasitária e as espécies encontradas no resíduo que se forma sob as unhas. Dos enteroparasitos encontrados, parece ser esta a primeira referência a ovos de ancilostomídeos no depósito subungueal. Dos 130 exames realizados 16 mostraram-se positivos, com uma percentagem de 12,30%.

Através do estudo da musculaturalisa da parede da veia central das supra-renais de chagásicos crônicos foram identificados núcleos em 18 dentre 40 células parasitadaspelo Trypanosoma cruzi. Com o auxílio de uma ocular micrométrica foram tomados dois diâmetros de cada ninho e núcleo, obtendo-se suas médias e desvios padrões (25,7 µm ± 17,2 e 12 µm ± 5,3 e 7,5 µm ± 2,82 para diâmetros dos ninhos e dos núcleos para células mononucleadas e binucleadas respectivamente). A relação diâmetro do núcleo/diâmetro do ninho foi de 0,5 ± 0,16. 83,3% dos ninhos eram mononucleados e apenas 16,7% binucleados. A média do volume nuclear para as células parasitadas foi de 1204µm³ ± 807,0 e para as células musculares lisas não parasitadasfoi de 69,6µm ± 7,2. A forma e o contorno nucleares eram variados e irregulares. Os núcleos hipercromáticos ou vesiculosos, com membrana cromatínica geralmente espessa e bem definida, freqüentemente mostrando um a dois nucléolos por secção, podendo, no entanto, serem encontrados até 8 nucléolos. Sendo as alterações nucleares descritas freqüentes nos chagásicos crônicos, épossível que sejam a expressão morfológica de importantes interações entre o T. cruzi e o leiomiócito.

Foi feita uma avaliação do programa de controle da esquistossomose (PCE/PCDEN) na região da Bacia do Rio São Francisco em Minas Gerais. A área em estudo compreende seis municípios, com 130.000 habitantes e 916 localidades em uma área de 10.722km². As atividades tiveram início em quatro municípios entre 1983 e 1985 e em dois outros em 1987. As principais medidas de controle adotadas foram tratamentos sucessivos com oxamniquine e aplicações de niclosamida em coleções hídricas. A prevalência da infecção pelo Schistosoma mansoni nos primeiros quatro municípios, que inicialmente estava entre 18 e 32%, diminuiu abruplamente após a primeira intervenção (1984/85) e permaneceu em níveis inferiores aos iniciais até a última avaliação realizada (1990/94); tendência semelhante foi observada para a proporção de caramujos infectados. Nestes municípios, a proporção de localidades sem a infecção ou com prevalências inferiores a 5% aumentou em detrimento daquelas com níveis mais altos de prevalência. Nos dois outros municípios, com prevalências iniciais inferiores a 5 %, nãoforam observadas mudanças substanciais nos indicadores endêmicos; a relação custo benefício do programa nos últimos municípios deve ser avaliada e as prioridades redirecionadas para erradicar as áreas focais e prevenir a expansão para áreas indenes. Os autores chamam a atenção para as dificuldades a longo prazo de um programa de controle fundamentado em tratamentos sucessivos. Informações sobre os fatores determinantes da infecção pelo S. mansoni em cada localidade, ou em conjunto de localidades semelhantes, permitiriam a elaboração de medidas complementares ao tratamento mais duradouras e menos dependentes da contínua utilização deste.

This work describes the epidemiological pattern of tegumentary leishmaniasis in an area north of Salta, Argentina. The prevalence and incidence were estimated by means of a cross-sectional study and two follow-up studies during two consecutive years. The Montenegro Skin Test (MST) was administered to 7336 subjects at baseline. The prevalence and incidence between 1990 and 1992 of infection (MST reactive) was 38‰ persons and 4.5‰ persons/year respectively. The prevalence and incidence of tegumentary leishmaniasis (presence of clinical signs) was 1.8‰ and 0.8‰ persons/year, respectively. A physical examination performed on 264 patients with MST reactive during three years revealed that 130 cases (49.2%) had some evident sign of infection (scar and/or lesion), with a clinical presentation compatible with leishmaniasis. Our study demonstrated that after the epidemic outbreak of 1985 the transmission in the study area returned to endemic levels in 1992, and also demonstrated the presence of the asymptomatic infection in the area.

Em amostra da população da cidade de Rio Branco (Acre), foi pesquisada a freqüência de portadores de anticorpos contra os arbovírus, mais prevalentes na região amazônica, e o vírus vacinal da febre amarela, antes e após a imunização com a vacina 17D. Das 390 pessoas incluídas na primeira fase do estudo (agosto de 1999), somente 190 compareceram em janeiro de 2000, três meses após a aplicação da vacina 17D (outubro de 1999). Nas amostras da primeira fase, as freqüências de soropositivos (IH) para os vírus estudados foram: 17D (27,2%); Dengue-1 (0,3%); Dengue-2 (4,1%); Dengue-3 (0%); Dengue-4 (0%), entre outros 8 vírus. Nas amostras séricas de janeiro (2000), a soroconversão para o 17D foi de 89,7% (130/145) e 3,2% (6/190) passaram a ter anticorpos contra o sorotipo 3 (DEN-3). Em conclusão, por conta da elevada taxa de cobertura vacinal e de soroconversão há redução significativa do risco de urbanização do vírus da febre amarela na cidade de Rio Branco, apesar de não ser desprezível a possibilidade de uma nova epidemia de dengue, pelo DEN-3, a semelhança da registrada em 2000 e 2001 pelos sorotipos 1 e 2.

A proposta deste estudo foi a de verificar o valor da citologia do escovado gástrico no diagnóstico da infecção pelo Helicobacter pylori em pacientes submetidos à endoscopia digestiva, comparando-o a outro método endoscópico - a histologia. As endoscopias foram realizadas em 157 pacientes dispépticos, divididos em dois grupos: grupo A (n = 27) com úlcera duodenal, B (n = 130), sem úlcera. No grupo A, a porcentagem de pacientes positivos na citologia (77,8%) foi similar à histologia (74,1%; p = 0,3). No grupo B, a citologia (71,5%) foi superior à histologia (63,1%; p = 0,00002). A citologia do escovado gástrico é um método simples e prático. Foi eficiente para identificar a infecção pelo Helicobacter pylori em todos grupos de estudo.

Investigou-se casos subclínicos de leishmaniose tegumentar em um bairro de Maringá-Paraná, pelas reações de imunofluorescência indireta e enzimaimunoensaio. Das 130 amostras analisadas, uma amostra apresentou título 40 e três apresentaram título 20 pela imunofluorescência indireta, enquanto 11 amostras apresentaram-se positivas pela reação de enzimaimunoensaio. Estes dados sugerem a existência de casos subclínicos nesta localidade.

An HIV seroprevalence and molecular study was conducted among 935 subjects: 723 female commercial sex workers, 92 men who have sex with men and 120 HIV-positive volunteers. The reported injection drug use rates were 0.7% in female commercial sex workers and 3% in men who have sex with men. Sexually transmitted infections were reported in 265 (37%) of the female commercial sex workers and 38 (41%) of the men who have sex with men. A total of 20 (2.8%) female commercial sex workers and 12 (13%) men who have sex with men became HIV infected during the study period. A history of sexually transmitted infection increased the risk of subsequent HIV infection twofold (adjusted odds ratio of 2.5) among the female commercial sex workers, while cocaine use had an adjusted odds ratios of 6.61 among men who have sex with men. From 130 samples, and based on heteroduplex mobility assaying for the env gene, with sequencing of part of pol and/or full genomes, subtype B was the predominant subtype identified (66%); followed by subtype F (22%) and subtype C (4%). Recombinant CRF12-BF strains were identified in 6% and CRF17_BF was identified in 2%.

This study aimed to evaluate the Chagas Disease Control Program which has operated since 1982 in the municipality of Berilo in the Jequitinhonha Valley, Minas Gerais, Brazil, based on evaluation of 5,242 domiciliary units (DUs) and 7,807 outbuildings over an eight-year period of epidemiological surveillance implanted in 1997. A total of 391 triatomines (280 Panstrongylus megistus and 111 Triatoma pseudomaculata) were captured, indicating the continued predominance of the former species. However, Triatoma pseudomaculata is clearly becoming more important in this region, with intradomiciliary colonies being detected in recent years. Entomological parameters, such as dispersion (17%) and intradomiciliary infestation (0.15%) indices, are compatible with the results of the epidemiological surveillance. The majority of DUs were of construction type A (plaster over bricks) or C (plaster over adobe). Twenty-five percent of the inhabitants of the DUs infested by triatomines were reactive in ELISA, IHA and IIF tests for Trypanosoma cruzi antigens.
