INTRODUCTION: Report the incidence of nosocomial infections, causative microorganisms, risk factors associated with and antimicrobial susceptibility pattern in the NICU of the Uberlândia University Hospital. METHODS: Data were collected through the National Healthcare Safety Network surveillance from January 2006 to December 2009. The patients were followed five times/week from their birth to their discharge or death. RESULTS: The study included 1,443 patients, 209 of these developed NIs, totaling 293 NI episodes, principally bloodstream infections (203; 69.3%) and conjunctivitis (52; 17.7%). Device-associated infection rates were as follows: 17.3 primary bloodstream infections per 1,000 central line-days and 3.2 pneumonias per 1000 ventilator-days. The mortality rate in neonates with NI was 11.9%. Mechanical ventilation, total parenteral nutrition, orogastric tube, previous antibiotic therapy, use of CVC and birth weight of 751-1,000g appeared to be associated with a significantly higher risk of NI (p < 0.05). In multiple logistic regression analysis for NI, mechanical ventilation and the use of CVC were independent risk factors (p < 0.05). Coagulase- negative Staphylococcus (CoNS) (36.5%) and Staphylococcus aureus (23.6%) were the most common etiologic agents isolated from cultures. The incidences of oxacillin-resistant CoNS and S. aureus were 81.8% and 25.3%, respectively. CONCLUSIONS: Frequent surveillance was very important to evaluate the association of these well-known risk factors with NIs and causative organisms, assisting in drawing the attention of health care professionals to this potent cause of morbidity.

Introduction Methicillin-resistant Staphylococcus aureus (MRSA) is among the most important pathogens of nosocomial infections, mainly in intensive care units (ICUs), and accounts for 40-60% of all healthcare-associated S. aureus infections. We evaluated the incidence of nosocomial infection by S. aureus, identified the risk factors for MRSA infection, and evaluated the effect of resistance to methicillin on mortality in patients. Methods We conducted MRSA surveillance at a university hospital in Brazil from January 1, 2010, to December 31, 2010, and performed a retrospective case-control matched study to evaluate the frequency of subsequent MRSA bacteremia and death among patients. We evaluated and compared the risk factors between patients with MRSA and methicillin-sensitive Staphylococcus aureus (MSSA) infection. Results Sepsis was the most common cause of infection (17.7/1,000 patient-days), followed by surgical site (11.4/1,000 patient-days), pneumonia (4.1/1,000 patient-days), and urinary tract infection (2.4/1,000 patient-days). The significant risk factors were time of hospitalization, use of central vascular catheter (CVC), urinary catheter, nasogastric tube, parenteral nutrition, tracheostomy, mechanical ventilation, and previous antibiotic administration, the latter of which was the only independent risk factor for MRSA infection. Mortality was significantly higher in patients with MRSA. The number of antibiotics tested was not related to increases in the frequency of MRSA/1,000 patient-days. The incidence of mortality attributable to MRSA (bloodstream infection) BSI was 50%. Conclusions Surveillance results showed that the use of high levels of antibiotics was directly related to the development of MRSA infection, and the mortality attributable to MRSA in patients with bacteremia was significant.

Introduction Parenteral antimony-based compounds are still the standard of care for cutaneous leishmaniasis (CL) treatment in many countries, despite their high toxicity. Previous studies showed that oral azithromycin could be an option for CL treatment. The aim of this study was to evaluate efficacy and safety of oral azithromycin (AZ) for CL treatment compared with injectable meglumine antimoniate (MA). Methods This was a randomized, open-label, 2-arm, non-inferiority clinical trial. Treatment-naïve patients with localized CL were treated with MA (15mg/kg/day up to 1,215mg) or AZ (500mg/day) during 20 consecutive days. The primary efficacy end point was a CL cure 90 days after treatment completion. The analysis was performed with intention-to-treat (ITT) and per protocol (PP) analyses. After an anticipated interim analysis, the study was interrupted due to the high failure rate in the azithromycin group. Results Twenty-four volunteers were included in each group. The MA group had a higher cure rate than the AZ group with the ITT and PP analyses, which were 54.2% versus 20.8% [relative risk (RR) 1.97; 95% confidence intervals (95%CI) 1.13-3.42] and 72.2% versus 23.8% (RR 3.03; 95%CI 1.34-6.87), respectively. No unexpected adverse events were observed. Conclusions Azithromycin is ineffective for CL treatment and does not seem to have a role in the therapeutic arsenal for CL.

INTRODUCTION: To evaluate predictive indices for candidemia in an adult intensive care unit (ICU) and to propose a new index. METHODS: A prospective cohort study was conducted between January 2011 and December 2012. This study was performed in an ICU in a tertiary care hospital at a public university and included 114 patients staying in the adult ICU for at least 48 hours. The association of patient variables with candidemia was analyzed. RESULTS: There were 18 (15.8%) proven cases of candidemia and 96 (84.2%) cases without candidemia. Univariate analysis revealed the following risk factors: parenteral nutrition, severe sepsis, surgical procedure, dialysis, pancreatitis, acute renal failure, and an APACHE II score higher than 20. For the Candida score index, the odds ratio was 8.50 (95% CI, 2.57 to 28.09); the sensitivity, specificity, positive predictive value, and negative predictive value were 0.78, 0.71, 0.33, and 0.94, respectively. With respect to the clinical predictor index, the odds ratio was 9.45 (95%CI, 2.06 to 43.39); the sensitivity, specificity, positive predictive value, and negative predictive value were 0.89, 0.54, 0.27, and 0.96, respectively. The proposed candidemia index cutoff was 8.5; the sensitivity, specificity, positive predictive value, and negative predictive value were 0.77, 0.70, 0.33, and 0.94, respectively. CONCLUSIONS: The Candida score and clinical predictor index excluded candidemia satisfactorily. The effectiveness of the candidemia index was comparable to that of the Candida score.

Many conditions are associated with hyperglycemia in preterm neonates because they are very susceptible to changes in carbohydrate homeostasis. The purpose of this study was to evaluate the occurrence of hyperglycemia in preterm infants undergoing glucose infusion during the first week of life, and to enumerate the main variables predictive of hyperglycemia. This prospective study (during 1994) included 40 preterm neonates (gestational age <37 weeks); 511 determinations of glycemic status were made in these infants (average 12.8/infant), classified by gestational age, birth weight, glucose infusion rate and clinical status at the time of determination (based on clinical and laboratory parameters). The clinical status was classified as stable or unstable, as an indication of the stability or instability of the mechanisms governing glucose homeostasis at the time of determination of blood glucose; 59 episodes of hyperglycemia (11.5%) were identified. A case-control study was used (case = hyperglycemia; control = normoglycemia) to derive a model for predicting glycemia. The risk factors considered were gestational age (<=31 vs. >31 weeks), birth weight (<=1500 vs. >1500 g), glucose infusion rate (<=6 vs. >6 mg/kg/min) and clinical status (stable vs. unstable). Multivariate analysis by logistic regression gave the following mathematical model for predicting the probability of hyperglycemia: 1/exp{-3.1437 + 0.5819(GA) + 0.9234(GIR) + 1.0978(Clinical status)} The main predictive variables in our study, in increasing order of importance, were gestational age, glucose infusion rate and, the clinical status (stable or unstable) of the preterm newborn infant. The probability of hyperglycemia ranged from 4.1% to 36.9%.

OBJECTIVE: To analyze the correlation of risk factors to the occurrence of urinary tract infection in full-term newborn infants. PATIENTS AND METHODS: Retrospective study (1997) including full-term infants having a positive urine culture by bag specimen. Urine collection was based on: fever, weight loss > 10% of birth weight, nonspecific symptoms (feeding intolerance, failure to thrive, hypoactivity, debilitate suction, irritability), or renal and urinary tract malformations. In these cases, another urine culture by suprapubic bladder aspiration was collected to confirm the diagnosis. To compare and validate the risk factors in each group, the selected cases were divided into two groups: Group I - positive urine culture by bag specimen collection and negative urine culture by suprapubic aspiration, and Group II - positive urine culture by bag specimen collection and positive urine culture by suprapubic aspiration . RESULTS: Sixty one infants were studied, Group I, n = 42 (68.9%) and Group II, n = 19 (31.1%). The selected risk factors (associated infectious diseases, use of broad-spectrum antibiotics, renal and urinary tract malformations, mechanical ventilation, parenteral nutrition and intravascular catheter) were more frequent in Group II (p<0.05). Through relative risk analysis, risk factors were, in decreasing importance: parenteral nutrition, intravascular catheter, associated infectious diseases, use of broad-spectrum antibiotics, mechanical ventilation, and renal and urinary tract malformations. CONCLUSION: The results showed that parenteral nutrition, intravascular catheter, and associated infectious diseases contributed to increase the frequency of neonatal urinary tract infection, and in the presence of more than one risk factor, the occurrence of urinary tract infection rose up to 11 times.

Inflammatory Bowel Diseases - ulcerative colitis and Crohn's disease- are chronic gastrointestinal inflammatory diseases of unknown etiology. Decreased oral intake, malabsorption, accelerated nutrient losses, increased requirements, and drug-nutrient interactions cause nutritional and functional deficiencies that require proper correction by nutritional therapy. The goals of the different forms of nutritional therapy are to correct nutritional disturbances and to modulate inflammatory response, thus influencing disease activity. Total parenteral nutrition has been used to correct and to prevent nutritional disturbances and to promote bowel rest during active disease, mainly in cases of digestive fistulae with high output. Its use should be reserved for patients who cannot tolerate enteral nutrition. Enteral nutrition is effective in inducing clinical remission in adults and promoting growth in children. Due to its low complication rate and lower costs, enteral nutrition should be preferred over total parenteral nutrition whenever possible. Both present equal effectiveness in primary therapy for remission of active Crohn's disease. Nutritional intervention may improve outcome in certain individuals; however, because of the costs and complications of such therapy, careful selection is warranted, especially in patients presumed to need total parenteral nutrition. Recent research has focused on the use of nutrients as primary treatment agents. Immunonutrition is an important therapeutic alternative in the management of inflammatory bowel diseases, modulating the inflammation and changing the eicosanoid synthesis profile. However, beneficial reported effects have yet to be translated into the clinical practice. The real efficacy of these and other nutrients (glutamine, short-chain fatty acids, antioxidants) still need further evaluation through prospective and randomized trials.

Nutrition is essential for maintenance of physiologic homeostasis and growth. Hypermetabolic states lead to a depletion of body stores, with decreased immunocompetence and increased morbidity and mortality. The purpose of this paper is to provide an update regarding the provision of appropriate nutrition for the pediatric surgical patient, emphasizing the preoperative and postoperative periods. Modern nutritional support for the surgical patient comprises numerous stages, including assessment of nutritional status, nutritional requirements, and nutritional therapy. Nutritional assessment is performed utilizing the clinical history, clinical examination, anthropometry, and biochemical evaluation. Anthropometric parameters include body weight, height, arm and head circumference, and skinfold thickness measurements. The biochemical evaluation is conducted using determinations of plasma levels of proteins, including album, pre-albumin, transferrin, and retinol-binding protein. These parameters are subject to error and are influenced by the rapid changes in body composition in the peri-operative period. Nutritional therapy includes enteral and/or parenteral nutrition. Enteral feeding is the first choice for nutritional therapy. If enteral feeding is not indicated, parenteral nutrition must be utilized. In all cases, an individualized, adequate diet (enteral formula or parenteral solution) is obligatory to decrease the occurrence of overfeeding and its undesirable consequences.

Necrotizing enterocolitis is a disease of the newborn that may involve the small intestine and/or the colon, and the stomach. To our knowledge, massive necrosis of the small intestine with concomitant involvement of the esophagus has never been reported. A case of a 6-month-old boy with necrotizing enterocolitis and pan-necrosis of the small intestine, cecum, and the lower third of the esophagus is presented. After 70 days of treatment, intestinal transit was established by an anastomosis between the first centimeter of jejunum and the ascending colon. Finally, esophageal transit was established by a total gastric transposition with cervical esophagogastric anastomosis. The patient was maintained under total parenteral nutrition, and after 19 months he developed fulminant hepatic failure due to parenteral nutrition; he then underwent combined liver and small bowel transplantation. After 2 months, the patient died due to undefined neurologic complications, probably related to infection or immunosuppressive therapy.

Na Amazônia encontra-se um grande número de espécies florestais, muitas delas já com reconhecido valor econômico, como a Abiurana (Eremoluma williamií), Andiroba (Carapa guianensis), Cardeiro (Scleronema micranthum), Cedrorana (Cedrelinga catenaeformis), Cumarú (Dipterix odorata), Jacareúba (Calophyllum angulare), Marupá (Simaruba amara) e Piquiá (Caryocar villosum). Pouco se sabe sobre suas características nutricionais e infecções por micorrizas arbusculares (MA) quando em solos ácidos e de baixa fertilidade da região. Com o objetivo de obter informações neste sentido, foi realizada uma avaliação de campo com estas espécies em fase adulta. Foram coletadas amostras de solos (rizosfera), raízes e folhas das espécies, de plantios experimentais na Estação Experimental de Silvicultura Tropical do INPA, Manaus, AM, em dois solos podzólicos. Todas as espécies apresentaram infecções por MA, mas diferiram quanto às colonizações, bem como nas concentrações de macro e micronutrientes nas folhas. Foram encontradas algumas correlações significativas (oito de um total de 64) entre as colonizações por micorrizas nas raízes e os teores de Ca, P, Cu, Fe, Mn c Zn nas folhas das espécies. Estas correlações estatisticamente significativas indicam que as endomicorrizas estão contribuindo para a absorção de nutrientes pelas plantas nas condições de terra firme da Amazônia Central.

Determinou-se os constituintes nutricionais da alimentação de pré-escolares de uma creche de Manaus, AM, seguida da adequação da mesma. Foi realizada a coleta da alimentação (desjejum, almoço e lanches), por meio da porção em duplicata, e analisada quimicamente. Os alimentos frequentemente consumidos foram feijão (25,1%), leite com café (19.9%), chá/sucos/refrigerantes (13,9%) e arroz (10,3%). A adequação protéica e energética foram de 126,1% e 32,42%, considerando as necessidades para as faixas etárias de 1 a 3 anos e 84% e 23,4% para o intervalo de 4 a 6. A quantidade de fibra total encontrada na dieta analisada foi baixa (5,6 g/dia). A adequação de consumo de Cu (431.8% c 293.6%) c Na (512% e 384%), considerando as faixas de 1 a 3 anos e 4 a 6 anos, respectivamente. Ca, Zn e Fe foram limitantes, com adequação inferior a 50%.

Determinou-se os constituintes nutricionais das dietas dos pré-escolares do Município de Nhamundá-AM, seguido da adequação dos mesmos. Utilizou-se a técnica de amostragem da porção em duplicata, que consistiu em coletar todos os alimentos e bebidas consumidos durante um dia para a avaliação dietética. Os resultados mostraram uma baixa adequação calórica, além das deficiências para os elementos minerais ferro, cálcio e zinco.

Analisou-se o teor de vitamina C em três acessos de camu-camu coletados na região leste do Estado de Roraima, sendo um no rio Maú, e outros dois (uma planta isolada e outra população) no rio Urubu, ambos afluentes do rio Tacutu. Para a determinação de ácido ascórbico utilizou-se HPLC. De acordo com os resultados obtidos, verificou-se que os frutos coletados de uma planta individual situada no rio Urubu apresentaram as maiores concentrações de ácido ascórbico 6112±137,5 mg em 100g (polpa). As populações de camu-camu oriundas dos rios Urubu e Maú apresentaram concentrações de ácido ascórbico na ordem de 5737±236,1 mg (polpa+casca) e 3571±12,0 mg (polpa) respectivamente. Esse achado inédito demonstra a necessidade de mais estudos, considerando a variabilidade genética do camu-camu e o potencial nutricional como fonte de vitamina C.

Considerando a relevância da fibra alimentar na proteção, manutenção e recuperação da saúde e a escassez de tabelas de composição química dos alimentos no que se refere a frutos amazônicos, determinou-se o teor de fibra alimentar em algumas populações de cubiu (Solanum sessiliflonun Dunal), camu-camu (Myrciaria dubia (H.B.K) Mc Vaugh) e açaí (Euterpe oleracea Mart) pelo método enzímico-gravimétrico. Os frutos tiveram diversas procedências: cubiu, da Estação Experimental do INPA/CPCA, camu-camu do município de Barcelos e ao longo da bacia do Rio Amazonas e açaí da bacia do Rio Solimões. Verificou-se que a concentração de fibra alimentar total e insolúvel nas amostras de açaí provenientes de Benjamin Constant (7,66±0,33 g e 6,60±0.IX g) e Anamã (7,45±0,25 g e 6,51±0,04 g), foi significativamente maior (p<0,05), quando comparada com os de outras procedências. Com relação ao cubiu, as maiores concentrações de fibra alimentar total foram detectadas nas amostras Alejo (4,38±0,0 g) e ACSI (3,88±0,18 g), salientando-se que fração analisada inclui a casca, cuja contribuição foi significativamente maior (p<0,05) nas frações insolúvel, solúvel e total. Da mesma forma, os teores de fibra alimentar total (6,18±0,25 g) e insolúvel (5,08±0,14 g) da casca do camu-camu, foram superiores aos encontrados nas demais amostras. Portanto, os frutos analisados apresentaram um potencial relevante como fonte de fibra alimentar, sendo mais uma opção para contribuir na adequação nutricional da população Amazônica e do país.

Procedeu-se uma revisão de toda produção científica do Amazonas, da área da nutrição nas últimas décadas. A análise deste material, consubstanciada com as últimas pesquisas realizadas pelo grupo da Nutrição do INPA viabilizou a composição do diagnóstico da hipovitaminose A na região Amazônica. A investigação clínico-oftalmológica, envolveu 487 crianças de 2 a 5 anos, de ambos os sexos, oriundas das diferentes localidades amazônicas: Nhamundá, Alvarães, Anamã, Beruri e Benjamin Constant. Os níveis de retinol sérico foram determinados, exclusivamente, em 48 crianças do município de Nhamundá. A investigação clínica não evidenciou nenhum sinal e/ou sintoma pertinentes a carência de vitamina A. Níveis de ret-inol sérico, considerados normais e altos, foram registrados em 75,0% e 25,0%, respectivamente do universo estudado. O somatório destes resultados com as informações retrospectivas procedentes da literatura regional não permitem afirmar categoricamente a existência de hipovitaminose A como problema de Saúde Pública na região Amazônica.
