Concepções de enfermeiros de um hospital universitário público sobre o relatório gerencial de custos

O presente trabalho trata de estudo exploratório-descritivo de abordagem qualitativa, sobre a construção teórica no campo da fenomenologia que objetivou compreender as concepções de enfermeiros de um hospital universitário público sobre o relatório gerencial de custos. A coleta de dados deu-se no período de agosto de 2009 a março de 2010, com 59 enfermeiros, sendo composta por três questões norteadoras: Qual o significado do gerenciamento de custos? Como você utiliza o relatório gerencial de custo? Qual a contribuição dos relatórios gerenciais de custos? Como resultado, obteve-se que os enfermeiros não utilizam tal relatório em sua prática profissional, pois mantêm o foco do trabalho voltado às questões assistenciais, e justificaram que não possuem formação acadêmica em relação ao gerenciamento de custos, faltando-lhes a compreensão do relatório. Contudo, apesar de pouco explorado, esse relatório contribui para os enfermeiros que ocupam cargos de direção, no controle dos gastos e no gerenciamento de custos.

O objetivo deste estudo foi avaliar as práticas de prevenção e controle de infecção da corrente sanguínea associada ao cateter venoso central (ICS-ACVC) de curta permanência, por meio da aplicação de indicadores clínicos processuais. A amostra foi constituída por 5.877 avaliações distribuídas entre as práticas selecionadas. Obteve-se ampla variação de conformidade: 91,6% - registro de indicação e tempo de permanência do CVC; 51,5% - cuidados e manutenção do curativo da inserção do CVC e seus dispositivos; 10,7% - higienização das mãos na realização de procedimentos de cuidado e manutenção do CVC; 0,0% - inserção do cateter venoso central (CVC). Os resultados demonstram necessidade de elaboração de novas estratégias que assegurem conformidade duradoura para a maioria das práticas de prevenção e controle de ICS-ACVC avaliadas. Conclui-se pela vantagem na aplicação de avaliação processual, pela possibilidade de não somente identificar seus índices de conformidade em relação à melhor prática esperada, como também favorecer, sobremaneira, reconhecimento das situações específicas que contribuíram para os valores encontrados.

O objetivo deste estudo foi analisar a estrutura gerencial para o manejo da dor em pacientes com aids em um hospital de referência de Fortaleza, CE, Brasil. Pesquisa descritiva com enfoque qualitativo, desenvolvida no ano de 2010. Foram realizadas 20 entrevistas com profissionais de saúde (médicos e enfermeiros), analisadas segundo o referencial da análise de conteúdo. Os dados foram organizados em categorias: condições estruturais favoráveis e desfavoráveis. Constatou-se prevalência de condições desfavoráveis no discurso dos entrevistados, como ênfase no tratamento farmacológico, inexistência de atendimento específico para dor, insuficiência de profissionais experientes no manejo da dor, demanda elevada e falhas no sistema de referência e contrarreferência. Sugere-se instituir novo modelo gerencial de cuidado aos pacientes com aids, enfatizando atendimento interdisciplinar à dor, treinamento de profissionais e aprimoramento de registros em prontuários para utilização de métodos de avaliação e tratamentos mais eficazes.

Este estudo avaliou fatores individuais, relativos ao trabalho e organizacionais relacionados à adesão às precauções-padrão por profissionais de enfermagem que atuam em terapia intensiva. Estudo de corte transversal, realizado com 178 profissionais em um hospital de grande porte. Para a coleta de dados foram utilizadas escalas psicométricas do tipo Likert. Na Escala de Adesão às precauções-padrão obteve-se um escore de 4,45 (DP=0,27) classificado como intermediário. Houve correlação quando comparada com fatores individuais para Escala de Personalidade de Risco (r=-0,169; p=0,024) e fatores relativos ao trabalho com a Escala de Obstáculos para seguir as precauções-padrão (r=-0,359; p=0,000). A adesão às precauções-padrão entre profissionais de enfermagem foi intermediária. Fatores individuais e relativos ao trabalho influenciaram a adesão às precauções-padrão.

O objetivo da pesquisa foi identificar o padrão de aleitamento materno exclusivo nos primeiros seis meses de vida de crianças nascidas em um Hospital Amigo da Criança e os fatores que contribuíram para o desmame precoce. Estudo de coorte prospectivo com 261 mães e crianças. Os dados foram avaliados utilizando-se a análise de sobrevivência através da construção da curva de Kaplan-Meier e teste de Log-Rank para a análise univariada. Foi realizada análise multivariada utilizando-se o modelo de Regressão de Cox com riscos proporcionais. Ao longo dos seis meses, o aleitamento materno exclusivo praticado com 30, 90, 120, 150 e 180 dias foi 75%, 52%, 33%, 19% e 5,7%, respectivamente. Na análise multivariada, as variáveis que mostraram risco para o desmame precoce foram a intercorrência mamária hospitalar e, na consulta de retorno, a posição inadequada e a associação das duas anteriores. A Iniciativa Hospital Amigo da Criança favoreceu o aleitamento materno exclusivo.

The objective of this research was to analyze the organizational culture of a Brazilian public hospital. It is a descriptive study with quantitative approach of data, developed in a public hospital of São Paulo State, Brazil. The sample was composed by 52 nurses and 146 nursing technicians and auxiliaries. Data were collected from January to June 2011 using the Brazilian Instrument for Assessing Organizational Culture – IBACO. The analysis of the organizational values showed the existence of hierarchical rigidity and centralization of power within the institution, as well as individualism and competition, which hinders teamwork. The values concerning workers’ well-being, satisfaction and motivation were not highly valued. In regard to organizational practices, the promotion of interpersonal relationship, continuous education, and rewarding practices were not valued either. It becomes apparent that traditional models of work organization support work practices and determine the organizational culture of the hospital.

Objective To analyze the direct cost of reusable and disposable aprons in a public teaching hospital. Method Cross-sectional study of quantitative approach, focusing on the direct cost of reusable and disposable aprons at a teaching hospital in northern Paraná. The study population consisted of secondary data collected in reports of the cost of services, laundry, materials and supplies division of the institution for the year 2012 Results We identified a lower average cost of using disposable apron when compared to the reusable apron. The direct cost of reusable apron was R$ 3.06, and the steps of preparation and washing were mainly responsible for the high cost, and disposable apron cost was R$ 0.94. Conclusion The results presented are important for hospital managers properly allocate resources and manage costs in hospitals .

The objective of this study was to describe the Supplemental Nursing Staff´s experiences at different hospital units. A qualitative phenomenological approach was conducted; a purposeful and theoretical sampling was implemented with supplemental nursing staff at Santa Barbara Hospital of Soria (Spain), to gain a more in-depth understanding of the Supplemental Nursing Staff ´s experience. Data were collected by in-depth interviews and through a field notebook. Data were analyzed using the Giorgi proposal. Twenty-one nurses with a mean age of 46 years were included. Three main topics emerged from the data analysis: building the first contact, carving out a niche and establishing interprofessional/interpersonal relationships. We conclude that the experience of hosting the supplemental nursing staff in changing clinical environments is conditioned by various factors. It is necessary for nurses and hospital managers to establish clear objectives with regard to the supplemental nursing staff´s role in the units.

The history of this research found a suitable ethos not only by the route of the researcher, but also by the current public policies of modernization and reform that are capable of regulating and transforming the educational and health systems, as well as their professional groups. The reflection meantime developed had raised a clear perception of the organizational change processes by which they interfered with the interorganizational coordination between School of Nursing and Hospital, where internship supervision would be the main protagonist, supported by the meanings that intervening actors have assigned to them. In this context, the search for explicit epistemological and methodological choices leads to look more attentively at the problem, ascertaining it, taking into account the organizational dimensions. In this regard, the choice of a case study was related to the fact that the method allowed to answer the purpose of knowing and understanding the interorganizational coordination phenomenon between School of Nursing and Hospital, namely through the supervision of internships.

OBJECTIVE To identify the psychopedagogical training needs of the pediatric nurses in the largest public hospital of the Balearic Islands, Spain. METHOD This study was developed with a quantitative and qualitative design, where 78 nurses (97.5% of the service) answered a questionnaire, and 15 participated in interviews that were analyzed via content analysis. RESULTS The quantitative results show gaps in the knowledge and psychopedagogical skills of the staff. These aspects could facilitate the development of tasks tailored to the personality and the psychoevolutional time of children with chronic diseases, as well as to the emotional state of families. The qualitative data was organized into four categories: family support; hospital and education; psychopedagogical training and difficulties in practice. The little communication between nurses and teachers is evident. CONCLUSION The data reinforces the need to implement training strategies and interdisciplinary work among health professionals, educators and families.

OBJECTIVE to describe hospital admissions for ambulatory care sensitive conditions in children under five years of age in the State of Paraná, Brazil by condition type, age group and health region. METHOD a temporal ecological study was conducted using data from the Unified Health System Hospital Information System for the period 2000 to 2011. Conditions were grouped in accordance with the list of ambulatory care sensitive conditions in Brazil. RESULTS there was an increase in the rate of admissions for ambulatory care sensitive conditions in all age groups in 50% of the health regions, with a marked increase in children under the age of one. Pneumonia, gastroenteritis and asthma were the main causes of admissions. There was an increase in the proportion of overall admissions accounted for by pneumonia and gastroenteritis. CONCLUSION the increase in admissions reveals the need for actions to improve access to primary healthcare and provide effective treatment of the main ambulatory care sensitive conditions in order to prevent hospital admissions among children.

OBJECTIVE To evaluate intervening factors in patient safety, focusing on hospital nursing staff. METHOD The study is descriptive, with qualitative approach, excerpt from a larger study with analytical nature. It was undertaken in a public hospital in Fortaleza, CE, Brazil, between January and June 2013, with semi-structured interviews to 70 nurses, using Thematic Content Analysis. RESULTS The principal intervening factors in patient safety related to hospital nursing staff were staff dimensioning and workload, professional qualification and training, team work, being contracted to the institution, turnover and lack of job security, and bad practice/disruptive behaviors. These aspects severely interfere with the establishment of a safety culture in the hospital analyzed. CONCLUSION It is necessary for managers to invest in nursing staff, so that these workers may be valued as fundamental in the promotion of patient safety, making it possible to develop competences for taking decisions with focus on the improvement of quality care.

OBJECTIVE To identify the prevalence of Burnout Syndrome in medical professionals, nurses and nursing technicians working in an Onco-Hematological Pediatric Hospital in São Paulo. METHOD An exploratory, descriptive study with cross-sectional design and quantitative approach, with a sample of 188 health professionals. Data were collected using two self-report instruments: the Maslach Burnout Inventory (MBI-HSS) which is a biosocial data form, and a non-participant observation guide. RESULTS High depersonalization for nurses (29.8%), low job performance for physicians (27.8%), and of nursing technicians (25.5%). High scores were identified in at least two domains of Burnout in 19.2% of nurses, 16.8% of nursing technicians, and 16.6% of doctors. CONCLUSION Health professionals are highly vulnerable to each of the dimensions of Burnout syndrome - namely emotional exhaustion, alienation, and low job performance/satisfaction- in the hospital work.

OBJECTIVES To validate an instrument to assess quality of mobile emergency pre-hospital care. METHOD A methodological study where 20 professionals gave their opinions on the items of the proposed instrument. The analysis was performed using Kappa test (K) and Content Validity Index (CVI), considering K> 0.80 and CVI ≥ 0.80. RESULTS Three items were excluded from the instrument: Professional Compensation; Job Satisfaction and Services Performed. Items that obtained adequate K and CVI indexes and remained in the instrument were: ambulance conservation status; physical structure; comfort in the ambulance; availability of material resources; user/staff safety; continuous learning; safety demonstrated by the team; access; welcoming; humanization; response time; costumer privacy; guidelines on care; relationship between professionals and costumers; opportunity for costumers to make complaints and multiprofessional conjunction/actuation. CONCLUSION The instrument to assess quality of care has been validated and may contribute to the evaluation of pre-hospital care in mobile emergency services.

OBJECTIVE Identifying the violence suffered by the health team workers and their association with Burnout and minor psychiatric disorders. METHODS Cross-sectional study with 269 health team professionals of a public hospital in southern Brazil. Data were collected through the use of the Survey Questionnaire: Workplace Violence in the Health Sector, Maslach Inventory Burnout and Self-Report Questionnaire. RESULTS Workplace violence struck 63.2% of workers, prevailing mostly in women (p = 0.001), among nursing auxiliaries/technicians (p=0.014) and was associated with minor psychiatric disorders (p<0.05), as exposure to different forms of violence increased the chances of these disorders by 60% (CI 95%: 1.2-2.1). The three Burnout dimensions were also associated to violence at work (p<0.05). CONCLUSION Health workers experience violence in the workplace and this exposure is associated with Burnout symptoms and minor psychiatric disorders.
