OBJETIVO: Identificar o perfil sociodemográfico dos acadêmicos de Medicina e avaliar a prevalência de sintomas de estresse nesses estudantes. MÉTODOS: A população estudada respondeu ao Inventário de Sintomas de Stress para Adultos de Lipp (ISSL), para identificação de sintomas de estresse e a um questionário sociodemográfico. RESULTADOS: Duzentos estudantes de Medicina, matriculados nos Ciclos Básico ou Clínico da Universidade Federal do Ceará (UFC), foram estudados. Houve predominância do sexo masculino (54,5%), solteiros (100%), naturais de Fortaleza (87%), com idade média de 21 (± 2,3) anos. As alunas apresentaram níveis de sintomas de estresse maiores do que os estudantes homens, representando 30,1% e 19,6%, respectivamente (p < 0,001). A prevalência de sintomas de estresse foi de 49,7%, sendo o quarto semestre o período com níveis mais altos desses sintomas (p = 0,002). CONCLUSÕES: Conclui-se que estudantes de Medicina da Universidade Federal do Ceará (UFC) têm níveis de estresse similares àqueles relatados para amostras internacionais.

OBJECTIVE: This study aims to evaluate the citotoxic activity of two commonly used anti-depressants: paroxetine and bupropion. We also evaluated the in vitro natural killer activity (NKA) after incubating the blood samples with the antidepressants. METHODS: Peripheral blood samples from 15 healthy volunteers were collected and the mononuclear cells (PBMCs) were isolated and incubated for 24h with (or without = control cells) paroxetine and bupropion, in concentrations of 30, 100 and 1000 ng/ml. After the incubation period in both groups, the amount of dead cells was calculated using trypam blue technique. NKA was evaluated using the classic51Cr release assay. CONCLUSIONS: PBMCs dead cells occurred in both groups and in proportion to all pharmacological concentrations. Nevertheless, the NKA was not affected, even with the reduction in the number of effective cells.

Objetivo: O objetivo do estudo foi estimar a prevalência de transtornos mentais comuns (TMC) entre os estudantes do curso de medicina da UFES e avaliar possíveis correlações entre TMC e fatores de risco. Métodos: Estudo transversal realizado de setembro a novembro de 2007 envolvendo 229 alunos do curso de medicina. O questionário utilizado foi autoaplicável e anônimo. Foram coletados dados socioeconômicos, informações sobre o curso, o processo de ensino-aprendizagem e a rede de apoio social. Para o rastreamento de TMC, utilizou-se o Self-Reporting Questionnaire (SRQ-20). Resultados: A prevalência total de TMC encontrada foi de 37,1% (IC 95%, 30,8%-43,4%), que esteve independentemente associada a não receber o apoio emocional necessário (OR = 7,4, IC 95%, 3,1-17,9) e relatar "dificuldade para tirar dúvidas em sala de aula por timidez" durante a infância ou adolescência (OR = 2,5, IC 95%, 1,0-6,1). Conclusão: Os dados demonstram elevada prevalência de TMC nessa população e a importância em subsidiar ações para prevenção e cuidado com a saúde mental dos estudantes, melhorando a qualidade de vida deles.

OBJETIVO: Testar a hipótese de associação entre hipertrofia miocárdica e níveis aumentados de insulina no soro de filhos de mães diabéticas, além de determinar a freqüência de regressão espontânea e o momento de sua ocorrência. MÉTODOS: Foram estudados 72 pacientes (54 filhos de mães diabéticas e 18 controles). O diagnóstico de hipertrofia miocárdica foi realizado por ecocardiografia pré-natal, uni e bidimensional. Os registros da insulina amniótica foram obtidos de um estudo pré-natal prévio sobre hipertrofia miocárdica, pois a participação das gestantes era comum aos dois projetos. RESULTADOS: Houve 10 (18,52%) casos de hipertrofia miocárdica entre os filhos de mães diabéticas. As medidas do septo foram significantemente diferentes entre os grupos (filhos de mães diabéticas e controles) na avaliação com 1 mês (p=0,04). A insulina manteve-se elevada nos filhos de diabéticas até 3 meses de idade, período em que era significativamente diferente em relação aos controles (p=0,003 e p=0,001, com 1 mês e 3 meses, respectivamente). A associação entre a regressão do septo interventricular e a regressão dos níveis de insulina ocorreu até 1 mês de idade. CONCLUSÃO: Houve regressão espontânea das medidas do septo interventricular até 6 meses de idade e a associação entre hiperinsulinismo e hipertrofia miocárdica esteve presente até 1 mês de idade.

OBJECTIVE: To assess the effect of subsequent pregnancy after peripartum cardiomyopathy (PPCM) on maternal and fetal outcome. METHODS: Prospective study of 34 patients with the diagnosis of PPCM (mean age= 26years). At the time of first diagnosis 5 were in NYHA functional class (FC) II for heart failure, one in FC III and 28 in FC IV. After clinical treatment, patients were advised to avoid new pregnancies and a follow-up was obtained. RESULTS: There were 12 (35.3%) subsequent pregnancies in patients (pt) aged 19 to 44 years (mean 32), divided into two groups: GI: 6 pts who had normalized their heart size and GII: 6 pts with persistent cardiomegaly. GI had initially mild clinical manifestations ( 3 were in FC II, 1 in FC II and 2 in FC IV) and complete recovery of cardiac function (FC I). A new pregnancy was well-tolerated in 5 (83.3%); 1 pt presented with preeclampsia, and progressed to FC II. Presently, 5 pt are in FC I and 1 in FC II. GII pts had more severe heart failure at the onset of PPCM (1 pt in FC II and 5 in FC IV); during follow-up, 4 pt were in FC I and 2 in FC II. A new pregnancy was well tolerated in all of them, but the eldest, who had had 2 pregnancies and had a progressive worsening of clinical status, dying 8 years after the last pregnancy and 13 years after the diagnosis of PPCM. The remaining 5 pt are still alive, 3 in FC I and 2 in FC II, with worsening of FC in 1. Subsequent pregnancies occurred 3-7 years after clinical treatment of PPCM and no fetal distress was observed. CONCLUSION: Subsequent pregnancies are well-tolerated after PPCM, but not devoid of risk. No fetal distress was observed. A minimum interval of 3 years after the recovery of function seems to be safe for subsequent pregnancies.

OBJECTIVE: To evaluated the clinical diagnostic, efficiency for basic death causes in patients dying of circulatory disease and de relative frequency of those diseases. METHODS: Analysis of medical record data of 82 patients, ages from 16 to 84 years old (68 over 40 years old), whose died of circulatory disease and had undergone necropsy in the period from 1988 to 1993 years in the University Hospital of Medicine Faculty of Botucatu-UNESP, Br. RESULTS: The functional class of patients were III or IV, in 78%, and 81.7% needed urgent hospitalization. By the clinical judgment the death were by ischemic heart disease in 32 (21 acute myocardial infarction), Chagas'disease in 12, valvopathy in 11, cardiomyopathy in 7, heart failure with no specification of cardiopathy in 11 and other causes in 9. At the necropsy the death cause was ischemic heart disease in 34 patients, valvopathy in 10, Chagas'disease in 10, cardiomyopathy in 5, and heart failure with no specification of cardiopathy in 2.The concordance taxes were in thhe same order: 94,6%, 90,0%, 83.3%, 71.4% and 28.5%. CONCLUSION: There was a great efficiency of clinical diagnosis for death cause in a general university hospital. The ischemic heart disease were the main causes of death.

OBJECTIVE - To assess hypertension control rates in a specialized university-affiliated medical department, the influence of sex, diabetes, and obesity on that control, and the strategies for the treatment of hypertension. METHODS - We carried out a cross-sectional study with 1,210 patients followed up for at least 6 months. Information was gathered from medical and nursing records and comprised the following data: sex, age, weight, height, abdominal and hip circumferences, blood pressure, and class and number of the antihypertensive drugs prescribed. To assess obesity, we used body mass index and waist/hip ratio. Blood pressure was considered under control when its levels were below 140/90 mmHg. RESULTS - The study consisted of 73% females and 27% males. Most females (31.7%) were 50 to 59 years of age, and most males (28.3%) were 60 to 69 years. The blood pressure control rate found was 20.9% for the 1,210 patients and 23.4% for the hypertensive diabetic patients (n=290). Despite the low control rates found, 70% of the patients used 1 or 2 antihypertensive medications. A high prevalence of obesity (38%) was observed, and females had a greater abdominal obesity index than males did (90% vs 82%, p<0.05). Patients with a greater body mass index had less control of blood pressure. CONCLUSION - The percentage of hypertensive patients with controlled blood pressure levels was low and was associated with a high prevalence of obesity. These data indicate the need for reviewing the strategies of global treatment for hypertension.
