This report documents the first two Brazilian cases of mediastinal granuloma due to histoplasmosis, presenting selected aspects on the diagnosis. Tissue samples revealing histoplasmosis were obtained from each of the patients by mediastinoscopy and thoracotomy. In the second patient, a subcarinal calcified mass eroded into the bronchial tree, leading to secondary bilateral aspiration pneumonitis one week after thoracotomy. Although rare, histoplasmosis should be included in the differential diagnosis of mediastinal granuloma, specially if there are calcifications greater than 10 mm in dimension.

The authors study a patient carrying Aids, with exuberant dermatological manifestations of cryptococcosis. They stress the therapeutic effectiveness of short-term amphotericin B. The authors reviewed cases of cutaneous infection with Cryptococcus reported in the national and international literature, verifying that the frequency has increased with the AIDS epidemic. Also, they discuss about the differential diagnosis with some cases of dermatosis, particularly with the disseminated giant molluscum contagiosum. In relation to the therapy, they affirm that the choice of drug depends on the organ involved, as well as the immune state of the patient.

SUMMARY In a previous study our group found that the probiotic Saccharomyces boulardii was capable of reducing the intensity of infection in mice with toxocariasis. In order to assess whether the mechanism involved would be a direct action of the probiotic on Toxocara canis larvae, this study was designed. Both probiotics were singly cultivated in plates containing RPMI 1640 medium and T. canis larvae. S. boulardii and B. cereus var. toyoi cultures presented 97.6% and 95.7% of larvae with positive motility, respectively, and absence of color by the dye trypan blue, not representing significant difference to the control group (p > 0.05). We conclude that none of the probiotics showed in vitro effects on T. canis larvae and that the interaction with the intestinal mucosa is necessary for the development of the protective effect of S. boulardii.

SUMMARYThe dermatophytes, keratinophilic fungi, represent important microorganisms of the soil microbiota, where there are cosmopolitan species and others with restricted geographic distribution. The aim of this study was to broaden the knowledge about the presence of dermatophytes in soils of urban (empty lots, schools, slums, squares, beaches and homes) and rural areas and about the evolution of their prevalence in soils of varying pH in cities of the four mesoregions of Paraiba State, Brazil. Soil samples were collected from 31 cities of Paraiba State. Of 212 samples, 62% showed fungal growth, particularly those from the Mata Paraibana mesoregion (43.5%), which has a tropical climate, hot and humid. Soil pH varied from 4.65 to 9.06, with 71% of the growth of dermatophytes occurring at alkaline pH (7.02 - 9.06) (ρ = 0.000). Of 131 strains isolated, 57.3% were geophilic species, particularly Trichophyton terrestre(31.3%) and Mycrosporum gypseum(21.4%). M. nanum and T. ajelloi were isolated for the first time in Paraiba State. The zoophilic species identified were T. mentagrophytes var.mentagrophytes (31.3 %) and T. verrucosum (7.6 %), and T. tonsurans was isolated as an anthropophilic species. The soils of urban areas including empty lots, schools, slums and squares of cities in the mesoregions of Paraiba State were found to be the most suitable reservoirs for almost all dermatophytes; their growth may have been influenced by environmental factors, soils with residues of human and/or animal keratin and alkaline pH.

The identification of the fungal species belonging to the healthy microflora in animals is a precondition for the recognition of pathological processes causing them. The aim of this study was to investigate the presence of potentially pathogenic fungi in the feces of wild birds collected in Screening Centers. Samples were collected from the feces of 50 cages with different species of birds. The samples were processed according to the modified method STAIB and the plates incubated at 32 °C for up to ten days with daily observation for detection of fungal growth. The isolation of the following species was observed: Malassezia pachydermatis, Candida albicans, C. famata, C. guilliermondii, C. sphaerica, C. globosa, C. catenulata, C. ciferri, C. intermedia, Cryptococcus laurentii, Trichosporon asahii, Geotrichum klebahnii, Aspergillus spp., A. niger and Penicillium spp. Knowing the character of some opportunistic fungi is important in identifying them, facilitating the adoption of preventive measures, such as proper cleaning of cages, since the accumulation of excreta may indicate a risk for both health professionals and centers for screening public health.

SUMMARY Introduction: We present a fatal case of disseminated cryptococcosis in a young man whose diagnosis of HIV infection was made at the time of admission to the emergency room. Case report: The patient was a twenty-three-year-old man, with a history of daily fever during one month associated with diarrhea, weight loss, headache, vomiting and generalized seizures. He also had a history of diabetes mellitus, alcoholism and drug addiction. Upon physical examination the patient was pale, disoriented and had periods of agitation. White blood cells count was 3,440/mm3 (5% lymphocytes), hemoglobin was 10g/dL, platelets were 83,000/ mm3. Creatinine was 0.7 mg/dL; urea 19 mg/dL; Na, K, and liver enzymes were within normal limits. Lactic dehydrogenase was 494 IU/L. Cerebrospinal fluid (CSF) analysis revealed 10 white blood cells/mm3 (58% neutrophils, 31% lymphocytes, 11% monocytes) and 2 red blood cells/mm3. India ink test revealed six Cryptococcus yeasts/mm3. CSF glucose was 122 mg/dL and protein was 36 mg/ dL. VDRL test was negative and anti-HIV test was positive. Intravenous hydration, insulin, phenytoin, fluconazole, pyrimethamine, sulfadiazine, folinic acid, and amphotericin B were started. The patient did not improve and became obtunded and hypotensive. He was intubated and put on mechanical respiration. He received vasoactive drugs and died less than 24 hours after admission. A postmortem examination was performed and revealed disseminated cryptococcosis, with severe involvement of the kidneys. Conclusion: Cryptococcosis, as a rule, is a systemic disease that affects mostly immunocompromised individuals, especially patients with AIDS. When diagnosed late in its course it has a very high mortality.

Com a finalidade de caracterizar efetivamente a septicemia na salmonelose prolongada, em 6 pacientes, dos quais 2 com resistência antibiótica e quimioterápica, procedeu-se a hemoculturas seriadas, consecutivas ou não, durante um período que oscilou entre 25 e 113 dias. Em cada paciente, numa média de 7 hemoculturas, foram isoladas, respectivamente, as seguintes salmonelas: S. typhi, S. paratyphi C, S. panamá, S. cholerae-suis var. Kunzendorf e S. paratyphi A. No caso em que se isolou a S. paratyphi A, em uma das hemoculturas isolou-se, também, a S. montevideo. A duração média da septicemia nos 6 pacientes foi de 73 dias. A se considerarem procedentes os informes dos pacientes quanto ao início da doença, a septicemia, com tôda a probabilidade, deve ter tido a duração média de 4 meses. Por outro lado, tomando por base a identidade do quadro clínico e a coincidência fisiopatológica dos casos estudados, é possível admitir-se a ocorrência de septicemias de longa duração nos demais 31 casos pelas seguintes salmonelas: S. newport, S. dublin, S. cholerae-suis, S. typhimurium, S. anatum, S. derby, e Salmonellas sp (não sorològicamente identificadas). Com base nestes achados, o A. acredita não ser possível admitir-se, na salmonelose em estudo, a diferença fundamental entre salmonelas adaptadas ou não à espécie humana, conforme preceitua a Doutrina de Kiel; considera, também, limitadas as restrições preconizadas pela Doutrina de Montevidéu aos conceitos fisiopatogenéticos da escola alemã. De fato, salmonelas do grupo tifiparatifi e outras consideradas adaptadas aos animais têm produzido, indistintamente em crianças e em adultos, quadros clínicos indistinguíveis, nos quais transcende a septicemia de longa duração e tornam-se manifestas a disproteinemia marcante e a hiperplasia do S.R.E. Finalmente, discute-se no trabalho o papel representado por uma série de fatores que podem atuar, acarretando alterações no organismo parasitado e criando condições à multiplicação das salmonelas e à manutenção da infecção. Dentre êles foram ressaltados: a associação parasitária, especialmente a esquistossomose mansoni; a desnutrição crônica; a disproteinemia marcante, seja devida à deficiência de prótides, seja em decorrência ao estado reacional do S.R.E., seletivamente agredido pelas salmonelas; modificações do estado imunitário, quer por inibição da anticorpogênese e da fagocitose, quer devida à inadequada propriedade imunológica das globulinas, extraordinariamente aumentadas na doença.
