PURPOSE: To determine the eradication rate of an ultra-short treatment schedule for Helicobacter pylori infection in a population with peptic ulcers, using omeprazole, secnidazole, and azithromycin in a once-daily dose for 3 days. METHODS: Thirty patients with peptic ulcer diagnosed by upper endoscopy and for Helicobacter pylori infection by rapid urease test and histologic examination received omeprazole 40 mg, secnidazole 1000 mg, and azithromycin 500 mg, administered once daily for 3 days. A follow-up exam was performed 12 weeks after the end of the treatment. Patients who were negative for Helicobacter pylori infection by rapid urease test and histologic examination were considered cured. RESULTS: Patients were predominantly female, and the mean age was 50 years. Duodenal peptic ulcer was found in 73% of the patients. Eradication was achieved in 9 of the 28 (32%) patients as determined from the follow-up endoscopic exam. The eradication rate by intention to treat was 30%. Side effects were present in 3% of the patients, and compliance to treatment was total. CONCLUSIONS: In spite of the low rate of side effects and good compliance, the eradication index was low. A possible drawback of this therapy is that it reduces the efficacy of macrolide and nitroimidazole compounds in subsequent treatments.

PURPOSE: To report the experience of a radiology department in the use of computed tomography guided biopsies of mediastinal lesions with fine and cutting needles, describing the differences between them. The results of adequacy of the sample and histologic diagnoses are presented according to the type of needle used. METHODS: We present a retrospective study of mediastinal biopsies guided by computed tomography performed from January 1993 to December 1999. Eighty-six patients underwent mediastinal biopsy in this period, 37 with cutting needles, 38 with fine needles, and 11 with both types (total of 97 biopsies). RESULTS: In most cases, it was possible to obtain an adequate sample (82.5%) and specific diagnosis (67.0%). Cutting-needle biopsy produced a higher percentage of adequate samples (89.6% versus 75.5%, P = 0.068) and of specific diagnosis (81.3% versus 53.1%, P = 0.003) than fine-needle biopsy. There were no complications that required intervention in either group. CONCLUSION: Because they are practical, safe, and can provide accurate diagnoses, image-guided biopsies should be considered the procedure of choice in the initial exploration of patients with mediastinal masses. In our experience, cutting needles gave higher quality samples and diagnostic rates. We recommend the use of cutting needles as the preferred procedure.

The essential oil of the leaves and fine stems of Aniba canelilla (Kunth) Mez collected in the city of Manaus, AM, Brazil, were obtained by hydrodistillation and analyzed by GC/MS. Forty-two components were identified, of which 1-nitro-2-phenylethane, as expected, was the major (71.2%-68.2%).

O presente estudo teve por objetivo diagnosticar e caracterizar as substâncias ergásticas foliares de Oenocarpus bacaba Mart., O. distichus Mart., O. mapora H. Karst. e O. minor Mart. através de microscopias óptica e eletrônica de varredura, análises histoquímicas e microanálises físicas. Secções transversais e longitudinais, assim como maceração foram realizadas em material botânico fixado. As análises histoquímicas foram empregadas em material botânico in natura, seguindo-se protocolos específicos para mucilagem, amido e sílica. Microanálises físicas foram feitas com Energy Dispersive Spectroscopy (EDS) detector. Nas espécies estudadas de Oenocarpus Mart., as substâncias ergásticas foliares correspondem a mucilagem amorfa; grãos de amido poliédricos do tipo simples e sílica opalina sob a forma de corpos elípticos e esféricos-globosos de superfície espiculada, ambos com elevado teor de dióxido de silício. As observações microscópicas, os testes histoquímicos e as microanálises físicas permitiram diagnosticar, caracterizar e elucidar a estrutura e ultra-estrutura das substâncias ergásticas ocorrentes nas folhas dos táxons analisados de Oenocarpus Mart.

Fine litter dynamics within the canopy differ from litter dynamics on the forest floor for reasons such as differences in microclimate, substrate, disturbance level, stratum influence and decomposition rates. This study is the first attempt to quantify the fine litter accumulated in the canopy of Central Amazonian forests. We compared the canopy litter accumulation to fine litter-layer on forest floor and to other forests and also investigated which were the mostly accumulated litter omponents. We found that Central Amazonian Rainforest intercepts greater fine litter in the canopy (294 g.m-2) compared to other forest formations with higher winds speed as in a Costa Rican Cloud Forest (170 g.m-2). The mean canopy fine litter accumulated at the end of the dry season was less than a half of that on soil surface (833 g.m-2) and the fine wood component dominates the canopy samplings (174 g.m-2) while leafy component predominate on soil surface litter (353 g.m-2).

The objective of this work was to assess the fine-root (≤ 2 mm diameter) production dynamics of two forest regrowths at different ages. Fine-root production was monitored by the ingrowth core method in one 18-year-old site (2 ha) and one 10-year-old site (0.5 ha), both localized in the Apeú region, Northern Pará State, Brazil. The sites were abandoned after successive shifting cultivation, beginning in 1940. Monthly production of live fine-root was similar between sites and was influenced by rainfall seasonality, with higher production during the dry season than the wet season for mass and length. However, mortality in terms of mass was higher in the 10-year-old site than in the 18-year-old site. The seasonality influenced mortality only in the 18-year old site following the pattern observed for live fine-root. The influence seasonal on mortality in terms of length was different between sites, with higher mortality during the wet season in the 10-year-old site and higher mortality during the dry season in the 18-year-old site. Specific root length was higher during the wet season and at the 10-year-old site. Fine-root production was not influenced by the chronosequence of the sites studied, probably fine-root production may have already stabilized in the sites or it depended more on climate and soil conditions. The production of fine-roots mass and length were indicators that generally showed the same pattern.

Aniba canelilla (H.B.K.) Mez. is a tree species from Amazon that produces essential oil. The oil extraction from its leaves and stems can be an alternative way to avoid the tree cutting for production of essential oil. The aim of this study was to analyse factors that may influence the essential oil production and the biomass of resprouts after pruning the leaves and stems of A. canelilla trees. The tree crowns were pruned in the wet season and after nine months the leaves and stems of the remaining crown and the resprouts were collected, in the dry season. The results showed that the essential oil yield and chemical composition differed among the stems, leaves and resprouts. The stems' essential oil production differed between the seasons and had a higher production in the resprouting stems than the old stems of the remaining crown. The production of essential oil and leaf biomass of resprouts were differently related to the canopy openness, indicating that light increases the production of the essential oil and decreases the biomass of resprouting leaves. This study revealed that plant organs differ in their essential oil production and that the canopy openness must be taken into account when pruning the A. canelilla tree crown in order to achieve higher oil productivity.

OBJECTIVE: The Prodromal Questionnaire (PQ) is a 92-item self-report screening tool for individuals at ultra-high risk (UHR) to develop psychosis. This study aims to present the translation to Portuguese and preliminary results in UHR and first episode (FE) psychosis in a Portuguese sample. METHODS: The PQ was translated from English to Portuguese by two bilingual researchers from the research program on early psychosis of the Instituto de Psiquiatria HCFMUSP, São Paulo, Brazil (ASAS - "Evaluation and Follow up of Adolescents and Young Adults in São Paulo") and back translated by two other researchers. The study participants (n = 11-) were evaluated through the Portuguese version of the Prodromal Questionnaire (PQ) and SIPS. RESULTS: The individuals at UHR (n = 7) presented a lower score than first episode patients (n = 4). The UHR mean scores and standard deviation on Portuguese version of the PQ were: 13.0 ± 10.0 points on positive symptoms subscale, and FE patients: 33.0 ± 10.0. CONCLUSION: The UHR and FE patients' of this study presented PQ scores similar to the ones found in the literature; what suggests that it is possible to use the PQ in Brazilian help-seeking individuals as a screening tool.

OBJECTIVE: To report a case and to discuss the use of psychodynamic psychotherapy (PD-P) to treat individuals at ultra-high risk (UHR) of psychosis. METHODS: An individual at UHR was followed up for 24 months. The baseline evaluation included a psychiatric interview, the Structured Interview for Prodromal Symptoms (SIPS), the Scale of Prodromal Symptoms (SOPS), and neuropsychological assessment. He underwent weekly sessions of PD-P for 12 months and was followed up for 12 months after the end of PD-P. The evaluations were at baseline, after 6-, 12-, and 24-month follow-up. No medication was prescribed during the 24-month follow-up. RESULTS: The prodromal symptoms remitted. The initial total score on the SIPS/SOPS was 37 points. After the first 12 months of PD-P, there was a reduction to 12 points on the SIPS/SOPS score, which stabilized in the 24-month follow-up. There was also a slight improvement in his performance on the neuropsychological evaluations. CONCLUSION: This case report suggests that PD-P can reduce prodromal symptoms; nevertheless, a better understanding of the specificity and efficacy of PD-P as an option of treatment for UHR individuals is needed.

OBJETIVO: Descrever a distribuição dos escores de cálcio coronariano numa população de homens brasileiros brancos assintomáticos submetidos à avaliação pela tomografia ultra-rápida. MÉTODOS: Foram avaliados 2.253 homens de 22-88 anos, submetidos a exame tomográfico num aparelho ImatronR C150 para detecção do cálcio coronariano. Os dados foram separados em 7 faixas etárias: < 40 anos, 40-44 anos, 45-49 anos, 50-54 anos, 55-59 anos, 60-64 anos e >65 anos. RESULTADOS: A média e o desvio padrão da idade foram de 50,0 ± 9,7 anos. Em 48,8% dos casos ocorreu incidência de escore de cálcio coronário > zero, que apresentou distribuição não Gaussiana e mostrou grande variação para a mesma faixa etária. Houve correlação direta entre a idade e o escores de cálcio coronariano (r=0,4, p<0,01). Exceto na comparação entre os indivíduos nas faixas etárias 60-64 anos e, abaixo 55-60 anos e acima de 65 anos, quanto maior a faixa etária maiores as medianas dos escores de cálcio coronariano (p<0,0001). Os escores de cálcio coronariano estão relatados de acordo com os percentis 25,50,75 e 90 para as faixas etárias. CONCLUSÃO: Este estudo, primeiro a relatar a distribuição dos escores de cálcio coronariano em uma amostra de homens brasileiros brancos, pode ser útil para estratificação do risco de eventos coronarianos.
