ABSTRACT Green manure promotes efficient suppression of weeds, but green manure species can exhibit distinct behaviors, depending on the environmental conditions. This study aimed to evaluate the potential of soil mulching and weed suppression by spring/summer green manure species grown in the spring/summer season, at different growth stages and after management (cut), for 90 days during the cassava crop cycle. The study was carried out in the 2010/2011 season, in a system managed under agroecological principles. The treatments consisted of different green manure species and arrangements: Crotalaria juncea, Cajanus cajan, Canavalia brasiliensis, Canavalia ensiformis, Pennisetum americanum, Crotalaria juncea and Pennisetum americanum intercropped; Mucuna aterrima, Sorghum bicolor, a mixture of all the green manures in study and a control plot under fallow. The experiment was arranged in a randomized block design with four replications. The evaluations of the soil cover either by the green manures or weeds were performed at 45, 90 and 105 days after the emergence of the green manures. The cassava crop was planted under reduced tillage system at 11 days after the cut of the green manures. The percentage of soil covered by weeds and the dry matter produced were evaluated at 30, 60 and 90 days after planting. The results showed that the green manures had a suppressive effect on weeds during their life cycle, as well as during the first months after its management (cut), composing the mulch.

Faz-se alusão à interferência das inundações na vida dos planorbídeos, quer contribuindo para a destruição de seus criadouros nas águas naturais e sistemas de irrigação, quer favorecendo sua proliferação e dispersão. Foram analisados os efeitos das enchentes e uma de suas conseqüências, que é a exacerbação da esquistossomose em focos antigos e o surgimento de novos. Foi relatada a ocorrência verificada em São José dos Campos, Estado de São Paulo, onde se deu o agravamento da parasitose em dois focos antigos, após as enchentes do Rio Paraíba e seus afluentes.

The nutritional status according to anthropometric data was assessed in 756 schoolchildren from 5 low-income state schools and in one private school in the same part of Rio de Janeiro, Brazil. The prevalence of stunting and wasting (cut-off point: <90% ht/age and <80% wt/ht) ranged in the public schools from 6.2 to 15.2% and 3.3 to 24.0%, respectively, whereas the figures for the private school were 2.3 and 3.5%, respectively. Much more obesity was found in the private school (18.0%) than in the state schools (0.8 - 6.2%). Nutritional problems seem to develop more severely in accordance with the increasing age of the children. Therefore it appears advisable to assess schoolchildren within the context of a nutritional surveillance system.

A previously calculated predictive model for health risk selects infants who suffer 4-5 times more morbidity than their unselected peers. Preliminary results suggested that this risk is related to maternal neurotic symptomatology. To evaluate this hypothesis, 52 consecutive mothers whose infants had a positive predictive score (Group 1) and 52 in whom this was negative (Group 2) were evaluated by means of Goldberg's General Health Questionnaire (GHQ - 30). A total of 41.9% and 20.5% of the mothers in Groups 1 and 2, respectively, scored above 11 points in GHQ-30, established as the cut off point. It is concluded that among poor urban families in Santiago mothers of infants with high risk of persistent diarrhoea have increased frequency of detectable neurotic symptoms. New programs aimed at this type of infant should include psychological support for their mothers.

INTRODUÇÃO: Como parte do programa de investigação sobre violência familiar e desnutrição severa na infância, especificamente um estudo do tipo caso-controle foi avaliada a confiabilidade dos instrumentos utilizados no processo de obtenção de dados. Estudou-se a confiabilidade de quatro componentes do instrumento principal: (a) as Escalas sobre Táticas de Conflitos (Conflicts Tactics Scales - CTS) que medem o grau de conflito/violência familiar; (b) o instrumento CAGE (Cut-down; Annoyed; Guilty & Eye-opener) usado para indicar suspeição de alcoolismo; (c) o NSDUQ (Non-student Drugs Use Questionnaire) que visa aferir o uso de drogas ilícitas; e (d) a medida antropométrica de comprimento. MÉTODO: Para os três primeiros componentes citados foram avaliadas a estabilidade (confiabilidade intra-observador ou teste-reteste) e a equivalência (confiabilidade inter-observador), usando-se os 50 primeiros sujeitos captados no estudo caso-controle de fundo. Para a análise, usou-se o índice Kappa (k) com ajustamento (pseudo-Bayes) para lidar com problemas de estimabilidade. Em relação ao componente "d", foi estudada somente a equivalência (n=73), usando-se o Coeficiente de Correlação Intraclasse (Intra-class Correlation Coefficient - ICC) como estimador. RESULTADOS: Todos os componentes mostraram estabilidade e equivalência aceitáveis. Quanto à estabilidade das CTS, CAGE e NSDUQ, as estimações de k foram em torno de 0,70, 0,78 e 0,85, respectivamente. Em relação à equivalência, encontrou-se os valores de 1,0 para as CTS e NSDUQ e 0,75 para CAGE. A equivalência estimada através do ICC para comprimento foi de 0,99. Algumas situações desviantes foram observadas. Os resultados apontam para uma adequada padronização dos observadores e refletem a boa qualidade do processo de aferição referente ao estudo de fundo, encorajando a equipe de pesquisa a prosseguir com maior segurança.

OBJECTIVE: To assess the usefulness of corneal esthesiometry for screening diabetic retinopathy. METHODS: A cross-sectional study was carried out comprising 575 patients attending a diabetic retinopathy-screening program in the city of São Paulo. Corneal esthesiometry was assessed with the Cochet-Bonnet esthesiometer. The presence of diabetic retinopathy was detected with indirect fundoscopy. The validity of corneal esthesiometry in identifying diabetic retinopathy was evaluated by the Receiver Operating Characteristic (ROC) curve. RESULTS: Sensitivity and specificity analyses of the corneal esthesiometry for detecting the stages of diabetic retinopathy using different cut-offs showed values less than 80%. The best indices (72.2% sensitivity and 57.4% specificity) were obtained for the identification of patients with proliferative diabetic retinopathy. CONCLUSIONS: In the study series, corneal esthesiometry was not a good indicator of diabetic retinopathy.

OBJETIVO: Avaliar as características de desempenho da versão brasileira do questionário Tolerance, Annoyed, Cut down e Eye-opener (T-ACE), para rastreamento do consumo de álcool na gestação. MÉTODOS: Estudo observacional, transversal, em amostra seqüencial de 450 mulheres no terceiro trimestre de gestação, assistidas em maternidade no município de Ribeirão Preto, estado de São Paulo, em 2001. Foram aplicados: questionário para coleta de dados sociodemográficos, T-ACE, questionário para levantamento da história de consumo de álcool ao longo da gestação e entrevista clínica para identificação de uso nocivo e dependência ao álcool, segundo critérios diagnósticos da CID-10. Foram feitos testes de concordância entre diferentes entrevistadores e de confiabilidade teste/re-teste. RESULTADOS: Do total, 100 gestantes (22,1%) foram consideradas positivas pelo T-ACE. Os índices kappa para concordância e confiabilidade foram 0,95, com 97% de respostas concordantes. Quando comparado aos parâmetros da CID-10 e ao padrão de consumo, o T-ACE com ponto de corte igual ou acima de dois pontos, apresentou coeficientes de sensibilidade e especificidade de 100% e 85% e de 97,9% e 86,6% respectivamente. CONCLUSÕES: A versão brasileira do T-ACE mostrou preencher adequadamente os critérios de desempenho que a qualificam ao papel de instrumento básico para o rastreamento do consumo de álcool durante a gravidez. Sua utilização é recomendável nas rotinas e práticas dos serviços obstétricos devido à tendência de aumento do consumo alcoólico feminino, dificuldades para identificação do abuso de álcool pela gestante e riscos de problemas de desenvolvimento nos filhos.

OBJETIVO: Avaliar a prevalência de casos suspeitos de uso inadequado de álcool durante a gestação entre mulheres atendidas na rede pública de saúde. MÉTODOS: Estudo transversal com 537 parturientes selecionadas aleatoriamente em maternidades públicas do Rio de Janeiro entre março e outubro de 2000. Entrevistadoras adequadamente treinadas identificaram os casos suspeitos de uso inadequado de álcool utilizando os instrumentos Cut-down, Annoyed, Guilty e Eye-opener (CAGE), Tolerance Cut-down, Annoyed e Eye-opener (T-ACE) e Tolerance Worry Eye-opener Annoyed Cut-down (TWEAK). Na análise segundo variáveis socioeconômicas e demográficas utilizou-se o teste qui-quadrado. RESULTADOS: Cerca de 40% das mulheres relataram fazer uso de algum tipo de bebida alcoólica durante a gestação, sendo a cerveja a bebida mais consumida (83,9%). Dependendo do instrumento de identificação, estimou-se que entre 7,3% e 26,1% das mulheres eram casos suspeitos de uso inadequado de álcool. A suspeição de utilização inadequada foi mais comum entre as mulheres de idade mais avançada; de baixa escolaridade; que não se declararam brancas; que não viviam com companheiro; que relataram tabagismo e uso de drogas ilícitas por um dos membros do casal; e com pouco apoio social. CONCLUSÕES: A alta prevalência de suspeição de uso inadequado de álcool e sua superposição com diferentes fatores de risco para desfechos deletérios na gestação indicam um importante problema de saúde pública, merecendo ser rotineiramente rastreada durante o acompanhamento pé-natal.

OBJETIVO: Estimar a freqüência de ingestão alcoólica em vítimas de causas externas atendidas em hospital. MÉTODOS: Estudo realizado com vítimas atendidas em um hospital geral universitário em Uberlândia (MG), de fevereiro a agosto de 2004. A alcoolemia foi determinada em 85 pacientes no pronto-socorro e entrevistaram-se outros 301 internados nas enfermarias sobre possível ingestão alcoólica previamente ao trauma; em ambos os grupos foi aplicado o questionário Cut-down, Annoyed by criticism, Guilty and Eye-opener (CAGE). Para as comparações das freqüências foi utilizado o teste exato de Fisher. RESULTADOS: A alcoolemia foi positiva em 31,8% dos pacientes testados, os quais mais freqüentemente necessitaram de internação (70,4% versus 37,9%; p<0,05). Proporcionalmente, alcoolemia positiva foi mais freqüente (p<0,05) entre as vítimas de agressão física (57,1%) do que as de queda (18,2%) ou de acidente de trânsito (29,3%). Nas enfermarias, 29,9% dos pacientes referiram ingestão alcoólica, proporcionalmente mais freqüente (p<0,01) entre as vítimas de agressão física (67,4%) do que entre as de acidente de trânsito (27,8%) ou queda (19,3%). Entre aqueles que ingeriram álcool, abordados no pronto-socorro e nas enfermarias, observou-se, respectivamente: que a maioria era homens (85,2% e 80,4%), a ocorrência de trauma foi maior (p<0,05) nos finais de semana (63% e 57,8%) e no período noturno (59,3% e 57,8%), e o questionário CAGE foi positivo em 81,5% e 82,2%. CONCLUSÕES: Cerca de um terço dos pacientes ingeriu bebidas alcoólicas previamente ao trauma e, entre eles, a maioria era homens. Proporcionalmente, a ingestão prévia de bebidas alcoólicas foi mais freqüente entre os pacientes vítimas de violência. Os resultados da aplicação do CAGE mostra que a maioria dos pacientes vítimas de causas externas após ingestão etílica não era alcoolista ocasional, e sim provável usuário crônico ou dependente de álcool.

OBJECTIVE: To determine the best cut-offs of body mass index for identifying alterations of blood lipids and glucose in adolescents. METHODS: A probabilistic sample including 577 adolescent students aged 12-19 years in 2003 (210 males and 367 females) from state public schools in the city of Niterói, Southeastern Brazil, was studied. The Receiver Operating Characteristic curve was used to identify the best age-adjusted BMI cut-off for predicting high levels of serum total cholesterol (>150mg/dL), LDL-C (>100mg/dL), serum triglycerides (>100mg/dL), plasma glucose (>100mg/dL) and low levels of HDL-C (< 45mg/dL). Four references were used to calculate sensitivity and specificity of BMI cut-offs: one Brazilian, one international and two American. RESULTS: The most prevalent metabolic alterations (>50%) were: high total cholesterol and low HDL-C. BMI predicted high levels of triglycerides in males, high LDL-C in females, and high total cholesterol and the occurrence of three or more metabolic alterations in both males and females (areas under the curve range: 0.59 to 0.67), with low sensitivity (57%-66%) and low specificity (58%-66%). The best BMI cut-offs for this sample (20.3 kg/m² to 21.0 kg/m²) were lower than those proposed in the references studied. CONCLUSIONS: Although BMI values lower than the International cut-offs were better predictor of some metabolic abnormalities in Brazilian adolescents, overall BMI is not a good predictor of these abnormalities in this population.

OBJETIVO: Analisar a associação entre abuso de álcool durante a gestação e baixo peso ao nascer. MÉTODOS: Estudo transversal de base populacional aninhado a uma coorte de 957 gestantes que realizaram acompanhamento pré-natal no Sistema Único de Saúde da cidade de Pelotas, RS, e deram à luz entre setembro de 2007 e setembro de 2008. As mães foram entrevistadas em dois momentos distintos: no período de pré-parto e após o parto. Para verificar o abuso do álcool foi utilizada a escala Cut down, Annoyed by criticism, Guilty and Eye-opener. Foram realizadas análises bivariadas e regressão logística múltipla, ajustada pelas variáveis prematuridade e abuso de álcool. O nível de significância adotado foi de 95%. RESULTADOS: Das mulheres que participaram do estudo, 2,1% abusaram de álcool na gestação e, entre essas, 26,3% tiveram filhos com baixo peso. Houve associação entre o abuso de álcool e baixo peso ao nascer (p < 0,038). CONCLUSÕES: Os achados indicam que o abuso de álcool durante a gestação está associado ao baixo peso ao nascer.

OBJECTIVE To analyze the evolution of catastrophic health expenditure and the inequalities in such expenses, according to the socioeconomic characteristics of Brazilian families.METHODS Data from the National Household Budget 2002-2003 (48,470 households) and 2008-2009 (55,970 households) were analyzed. Catastrophic health expenditure was defined as excess expenditure, considering different methods of calculation: 10.0% and 20.0% of total consumption and 40.0% of the family’s capacity to pay. The National Economic Indicator and schooling were considered as socioeconomic characteristics. Inequality measures utilized were the relative difference between rates, the rates ratio, and concentration index.RESULTS The catastrophic health expenditure varied between 0.7% and 21.0%, depending on the calculation method. The lowest prevalences were noted in relation to the capacity to pay, while the highest, in relation to total consumption. The prevalence of catastrophic health expenditure increased by 25.0% from 2002-2003 to 2008-2009 when the cutoff point of 20.0% relating to the total consumption was considered and by 100% when 40.0% or more of the capacity to pay was applied as the cut-off point. Socioeconomic inequalities in the catastrophic health expenditure in Brazil between 2002-2003 and 2008-2009 increased significantly, becoming 5.20 times higher among the poorest and 4.17 times higher among the least educated.CONCLUSIONS There was an increase in catastrophic health expenditure among Brazilian families, principally among the poorest and those headed by the least-educated individuals, contributing to an increase in social inequality.

OBJECTIVE To analyze physical activity during adolescence in participants of the 1993 Pelotas Birth Cohort Study, Brazil. METHODS Data on leisure time physical activity at 11, 15, and 18 years of age were analyzed. At each visit, a cut-off point of 300 min/week was used to classify adolescents as active or inactive. A total of 3,736 participants provided data on physical activity at each of the three age points. RESULTS A significant decline in the proportion of active adolescents was observed from 11 to 18 years of age, particularly among girls (from 32.9% to 21.7%). The proportions of girls and boys who were active at all three age points were 28.0% and 55.1%, respectively. After adjustment for sex, economic status, and skin color, participants who were active at 11 and 15 years of age were 58.0% more likely to be active at 18 years of age compared with those who were inactive at 11 and 15 years of age. CONCLUSIONS Physical activity declined during adolescence and inactivity tended to track over time. Our findings reinforce the need to promote physical activity at early stages of life, because active behavior established early tends to be maintained over time.

ABSTRACT OBJECTIVE To describe the methodological characteristics of the studies selected and assess variables associated with sedentary behavior in Brazilian children and adolescents. METHODS For this systematic review, we searched four electronic databases: PubMed, Web of Knowledge, LILACS, SciELO. Also, electronic searches were applied in Google Scholar. A supplementary search was conducted in the references lists of the included articles and in non-indexed journals. We included observational studies with children and adolescents aged from three to 19 years developed in Brazil, presenting analyses of associations based on regression methods and published until September 30, 2014. RESULTS Of the 255 potential references retrieved by the searches, 49 met the inclusion criteria and composed the descriptive synthesis. In this set, we identified a great number of cross-sectional studies (n = 43; 88.0%) and high methodological variability on the types of sedentary behavior assessed, measurement tools and cut-off points used. The variables most often associated with sedentary behavior were “high levels of body weight” (in 15 out of 27 studies; 55.0%) and “lower level of physical activity” (in eight out of 16 studies; 50.0%). CONCLUSIONS The findings of this review raise the following demands to the Brazilian agenda of sedentary behavior research geared to children and adolescents: development of longitudinal studies, validation of measuring tools, establishment of risk cut-offs, measurement of sedentary behavior beyond screen time and use of objective measures in addition to questionnaires. In the articles available, the associations between sedentary behavior with “high levels of body weight” and “low levels of physical activity” were observed in different regions of Brazil.

Schedule for vaccination against HBV infection has usually been based on three separate injections of 20 meg of the vaccine by intramuscular route. One of the main shortcomings to its use in large scale programs has been its high cost. Ninety out of 300 health workers were submitted to three injections of 2 meg of plasma-derived vaccine (PDV) by intradermal (ID) route on days 0, 30, and 180. Anti-HBs was detected in 74 (82.2%) after the second dose and in 80 (88.9%) after the third dose, a non-significant difference. However, levels above 10 times the cut-off were observed in 29 (32.2%) and 77 (85.5%), respectively (p < 0.001). The results showed that a low-dose schedule is effective when used in health workers and should be tried with other risk groups.
