São apresentados alguns dos resultados pertencentes a uma pesquisa de três anos envolvendo empresas brasileiras e inglesas, manufatureiras do ramo metal-mecânico, analisando as relações entre as variáveis incerteza, variabilidade das saídas e flexibilidade em sistemas de produção. Os resultados são um co-produto, relacionados particularmente à flexibilidade dos recursos estruturais de produção (humanos e tecnológicos). Eles representam uma abordagem alternativa àquelas normalmente encontradas na bibliografia. Uma nova forma de analisar os estoques e o seu papel no atingimento de flexibilidade nos sistemas de produção é também desenvolvida com base nesta nova abordagem. Ela pode ajudar na determinação do que alguns têm chamado "JEDI level" - Just Enough Desirable Inventory, ou níveis mínimos desejáveis de estoques, uma idéia que vem se opor à idéia absoluta de "estoque zero" a qualquer custo.

Existing studies on global sourcing strategy have implicitly adopted a cJosed-systems perspective in which sourcing activities are managed within a multinational company across national boundaries. Produd and process innovations and components procurement that are jointly managed by a consortium of cooperating firms have not been examined. In this paper, we empiricallyexamine the issues concerning sourcing partnerships in an open-systems perspective. Findings suggest that even in a sourcing partnership arrangement with a foreign supplier, the principal firm's ability to procure and control the supply of major components has a positive bearing on its market performance.

Efetuou-se um levantamento soro-epidemiológico, abrangendo 898 crianças, para a verificação da imunidade aos poliovírus 1, 2 e 3 por meio da determinação dos títulos de anticorpos neutralizantes. De 522 crianças, ou seja, de 58,1% da população estudada, foi possível a obtenção de informações referentes à situação vacinal a partir de anotações existentes nas próprias Cadernetas de Vacinação. Neste último sub-grupo foi estudado o estado imunitário em relação ao número de doses de vacina oral tipo Sabin recebidas. Os resultados mostraram alta proporção de imunes para os três tipos de poliovírus entre as crianças estudadas, porém também revelaram a existência de lacuna imunitária nos menores de um ano, especialmente no primeiro semestre de vida. Não foram observadas diferenças importantes na proporção de imunes segundo o sorotipo considerado. Salientou-se a necessidade de um alerta constante e permanente em relação à ocorrência de possíveis surtos epidêmicos em segmentos mais vulneráveis da população, particularmente entre crianças menores de 1 ano.

Como um dos requisitos para a operacionalização do Sistema de Vigilância Alimentar e Nutricional (SISVAN) no município de Curitiba, PR, Brasil, foi realizado um levantamento epidemiológico retrospectivo com dados de 1988, que buscou através do exame antropométrico conhecer a prevalência e as formas de desnutrição calórico-protéico de crianças menores de cinco anos atendidas pela Rede Municipal de Saúde de Curitiba. Numa amostra de 4.213 crianças encontrou-se pela Classificação de Gòmez 28,1% de desnutridos, sendo que 3,6% destes situam-se no grau II e III. A Classificação de Seoane e Latham modificada por Batista Filho revelou que 19,7% das crianças apresentam alguma forma de desnutrição. O perfil antropométrico revelou maior concentração de crianças no primeiro decil a partir do sexto mês de vida, sendo esta mais acentuada no grupo etário de 12 a 24 meses para os índices: peso/idade e altura/idade.

Results obtained with catches performed at several sites of the Ribeira Valley, SP (Brazil) are reported. Collections with Shannon type traps showed a concentration of An.albitarsis s.l. in the Experimental Farm of Pariquera-Açu County, associated with the development of irrigated rice cultivation. Two species of the complex, named A and B, were recognized. Species B predominated in the samples. Indoor and outdoor crepuscular human bait captures were performed a the domiciliary environment. The two species showed a generally low William's media valve of 0.2, the highest value obtained was 1.6. No differences were found between indoor and outdoor behaviours between the two species of the complex.

To test the hypothesis that the low socioeconomic population living is shanty towns in Porto Alegre presents different levels of poverty which are reflected on its health status, a cross-sectional study was designed involving 477 families living in Vila Grande Cruzeiro, Porto Alegre, Brazil. The poverty level of the families was measured by using an instrument specifically designed for poor urban populations. Children from families living in extreme poverty (poorest quartile) were found to have higher infant mortality rate, lower birth weights, more hospitalizations, and higher malnutrition rates, in addition to belonging to more numerous families. Thus, the shanty town population of Porto Alegre is not homogeneous, and priority should be given to the more vulnerable subgroups.

OBJECTIVE: To evaluate the influence of sociodemographic, clinical, and epidemiological factors in AIDS patients survival in a reference hospital. METHODS: A sample of 502 adult AIDS patients out of 1,494 AIDS cases registered in a hospital in Fortaleza, Brazil, was investigated between 1986 and 1998. Sixteen cases were excluded due to death at the moment of the AIDS diagnosis and 486 were analyzed in the study. Socioeconomic and clinical epidemiological were the variables studied. Statistical analysis was conducted using the Kaplan-Meier survival analysis and the Cox proportional hazards model. RESULTS: Three hundred and sixty two out of the 486 patients studied took at least one antiretroviral drug and their survival was ten times longer than those who did not take any drug (746 and 79 days, respectively, p <0.001). Patients who took two nucleoside reverse transcriptase inhibitors (NRTI) plus protease inhibitor were found to have higher survival rates (p <0.001). The risk of dying in the first year was significantly lower for patients who took NRTI and a protease inhibitor compared to those who took only NRTI. In addition, this risk was much lower from the second year on (0.10; 95%CI: 0.42-0.23). The risk of dying in the first year was significantly higher for less educated patients (15.58; 95%CI: 6.64-36.58) and those who had two or more systemic diseases (3.03; 95%CI: 1.74-5.25). After the first year post-diagnosis, there was no risk difference for these factors. CONCLUSIONS: Higher education revealed to exert a significant influence in the first-year survival. Antiretroviral drugs had a greater impact in the survival from the second year on. A more aggressive antiretroviral therapy started earlier could benefit those patients.

OBJECTIVE: To establish reference values for hippuric acid (HA) excreted in the urine, and to evaluate the impact of age, gender, alcohol, and tobacco, on these levels in a population nonexposed to toluene. METHODS: Reference values for hippuric acid in urine were determined in 115 toluene nonexposed healthy volunteers, from Alfenas city, Southeastern Brazil. A questionnaire was applied to each volunteer and data on occupational and personal habits were collected. Biochemical and hematological analyses were used to confirm the volunteers' good health condition. Reference values were expressed in g HA/g urine creatinine, as mean ± standard deviation (x ± SD), median, 95% confidence interval (95%CI), 95th percentile, and upper reference value (URV, mean +2 SD). RESULTS: Reference values of hippuric acid in urine were: mean ± standard deviation =0.18±0.10; median =0.15; 95% confidence interval =0.16±0.20; 95th percentile = 0.36 and upper reference value (URV, mean +2 SD) =0.38. Statistically significant differences in urinary HA (Wilcoxon - Mann/Whitney, p<0.05) were observed for different genders and age groups. Alcohol ingestion and smoking habit did not significantly affect the results. CONCLUSIONS: The reference values of hippuric acid in urine can be used in biomonitoring programs of workers occupationally exposed to toluene, especially in the southern region of the state of Minas Gerais. Age and gender may affect the HA reference values.

OBJECTIVE: A cross-sectional population-based study was conducted to assess, in active smokers, the relationship of number of cigarettes smoked and other characteristics to salivary cotinine concentrations. METHODS: A random sample of active smokers aged 15 years or older was selected using a stepwise cluster sample strategy, in the year 2000 in Rio de Janeiro, Brazil. The study included 401 subjects. Salivary cotinine concentration was determined using gas chromatography with nitrogen-phosphorus detection. A standard questionnaire was used to collect demographic and smoking behavioral data. The relation between the number of cigarettes smoked in the last 24h and cotinine level was examined by means of a nonparametric fitting technique of robust locally weighted regression. RESULTS: Significantly (p<0.05) higher adjusted mean cotinine levels were found in subjects smoking their first cigarette within five minutes after waking up, and in those smoking 1-20 cigarettes in the last 24h who reported inhaling more than ½ the time. In those smoking 1-20 cigarettes, the slope was significantly higher for those subjects waiting for more than five minutes before smoking their first cigarette after waking up, and those smoking "light" cigarettes when compared with their counterparts. These heterogeneities became negligible and non-significant when subjects with cotinine >40 ng/mL per cigarette were excluded. CONCLUSIONS: There was found a positive association between self-reporting smoking five minutes after waking up, and inhaling more than ½ the time are consistent and higher cotinine levels. These can be markers of dependence and higher nicotine intake. Salivary cotinine proved to be a useful biomarker of recent smoking and can be used in epidemiological studies and smoking cessation programs.

OBJECTIVE: To assess a new impunity index and variables that have been found to predict variation in homicide rates in other geographical levels as predictive of state-level homicide rates in Brazil. METHODS: This was a cross-sectional ecological study. Data from the mortality information system relating to the 27 Brazilian states for the years 1996 to 2005 were analyzed. The outcome variables were taken to be homicide victim rates in 2005, for the entire population and for men aged 20-29 years. Measurements of economic and social development, economic inequality, demographic structure and life expectancy were analyzed as predictors. An "impunity index", calculated as the total number of homicides between 1996 and 2005 divided by the number of individuals in prison in 2007, was constructed. The data were analyzed by means of simple linear regression and negative binomial regression. RESULTS: In 2005, state-level crude total homicide rates ranged from 11 to 51 per 100,000; for young men, they ranged from 39 to 241. The impunity index ranged from 0.4 to 3.5 and was the most important predictor of this variability. From negative binomial regression, it was estimated that the homicide victim rate among young males increased by 50% for every increase of one point in this ratio. CONCLUSIONS: Classic predictive factors were not associated with homicides in this analysis of state-level variation in Brazil. However, the impunity index indicated that the greater the impunity, the higher the homicide rate.

OBJECTIVE: To estimate the prevalence of lead poisoning in children and to identify associated factors, as well as possible local sources of contamination. METHODS: A cross-sectional prevalence study conducted in 2006 with a random sample of 97 children age zero to five years from a neighborhood in Porto Alegre, Southern Brazil. Blood lead levels were measured and a questionnaire administered to collect information on sociodemographics, recycling and dwelling. A preliminary environmental evaluation was carried out with direct analysis of soil and indirect analysis of air pollution with bioindicators to identify possible sources of contamination. To analyze lead concentrations from the different collection sites, for each type of material studied, ANOVA was performed with a Brown-Forsythe adjustment for heteroscedasticity and with Dunnett's T3 procedure for multiple comparisons of unequal variances. RESULTS: Blood lead levels > 10.0 µg/dL was found in 16.5% of children. Recycling of waste at home, low father's education level, and increased age of children were associated with increase blood lead levels. High lead levels were found in soil, and there was little indication of lead air pollution. CONCLUSIONS: A high prevalence of lead poisoning was identified, and the potential sources of contamination in this community appear related to waste recylcing activities. Studies should be conducted with other populations of Brazilian children and evaluate potential sources of local and general contamination, to accurately characterize this issue in Brazil.

OBJECTIVE To analyze spatial changes in the risk of AIDS and the relationship between AIDS incidence and socioeconomic variables in the state of Rondonia, Amazon region. METHODS A spatial, population case-control study in Rondonia, Brazil, based on 1,780 cases reported to the Epidemiological Surveillance System and controls based on demographic data from 1987 to 2006. The cases were grouped into five consecutive four-year periods. A generalized additive model was adjusted to the data; the dependent variable was the status of the individuals (case or control), and the independent variables were a bi-dimensional spline of the geographic coordinates and some municipality-level socioeconomic variables. The observed values of the Moran’s I test were compared to a reference distribution of values generated under conditions of spatial randomness. RESULTS AIDS risk shows a marked spatial and temporal pattern. The disease incidence is related to socioeconomic variables at the municipal level in Rondônia, such as urbanization and human capital. The highest incidence rates of AIDS are in municipalities along the BR-364 highway and calculations of the Moran’s I test show positive spatial correlation associated with proximity of the municipality to the highway in the third and fourth periods (p = 0.05). CONCLUSIONS Incidence of the disease is higher in municipalities of greater economic wealth and urbanization, and in those municipalities bisected by Rondônia’s main roads. The rapid development associated with the opening up of once remote regions may be accompanied by an increase in these risks to health.

Foram colhidas amostras de sangue de 178 recém-nascidos (RN) em berçários de hospital localizado no Município de São Paulo. Noventa crianças foram puncionadas antes do primeiro "Dia Nacional de Vacinação Contra a Poliomielite" e as outras 88, após o segundo "Dia Nacional de Vacinação Contra a Poliomielite", realizados em 1980. Nessas campanhas foram imunizadas as crianças com idade de zero a cinco anos, em todo o Brasil. No presente trabalho pesquisou-se os títulos de anticorpos neutralizantes contra poliovírus nos dois grupos de recém-nascidos. Após a imunização em massa verificou-se que a taxa de recém-nascidos triplo suscetíveis decresceu de 8,9% para 4,5%, enquanto que o aumento observado do triplo imunes foi de 38,9% para 52,3%; essas diferenças mostraram-se estatisticamente significantes ao nível de 5,0%. A proporção de recém-nascidos, com títulos de anticorpos neutralizantes contra poliovírus iguais ou maiores do que 8, aumentou após as campanhas de imunização, quando passaram de 68,9% para 81,8%, de 73,3% para 83,0% e de 57,8% para 70,5%, respectivamente, para os sorotipos 1, 2 e 3. Essas diferenças mostraram se estatisticamente significantes, ao nível de 5,0%, em relação aos poliovírus 1 e 3.

The present investigation was carried out on a sample of 840 children (5 to 16 years old) from ten small towns of the State of Bahia in northeastern Brazil. The objetive was to study, by using a cross sectional methodology, the evolution of schistosomiasis morbidity (hepatic and splenic enlargement) in children, and the role of the intensity of S. mansoni infection in this process. The children were analised in three age groups (5 to 8, to 12 and 13 to 16 years old) and classified as uninfected, mildly infected, moderately infected and heavily infected according to the number of eggs in the stool. In children aged 5 to 8 years, increasing egg counts were not associated with increasing frequencies of hepatic or splenic enlargement. In the 9 to 12 years old group and association was observed with the prevalence of hepatic enlargement, but not with the prevalence of spleen enlargement. In the oldest group, 13 to 16 years old, an association was observed with the prevalence of enlargement of both organs. It was evident that in this population schistosomiasis morbidity develops in the early period of life as a gradual process starting with liver enlargement and followed by spleen enlargement some years later. It was found that the intensity of infection has a fundamental role in this process, although there is a latent period of some years before clinical splenomegaly appears in moderate-heavily infected children. The Authors suggest that the prevalence of splenomegaly in the 13 to 16 years old group is a good measure of the community level of schistosomiasis morbidity and could be used to measure the impact of control programs.

Desde 1977, o Instituto Adolfo Lutz (IAL) vem promovendo a sorotipagem do S. pneumoniae ou pneumococo de infecções causadas por esta bacteria. As cepas isoladas têm sido encaminhadas ao WHO Pneumococcal Reference Center, Pensilvania, E.U.A.. De 1977 a 1988, 1.000 cepas de pneumococo isoladas de LCR foram sorotipadas, de acordo com a nomenclatura dinamarquesa, e 60 sorotipos foram identificados. A maior freqüência foi do sorotipo 1, secundado por 6B, 18C, 14, 5, 3, 6A, 23F, 19F e 38. Estes sorotipos distribuídos segundo faixas etárias demonstraram incidência variável, notando-se uma certa peculiaridade, ou seja, a predominância do sorotipo 6B na faixa de zero a menos de dois anos; do sorotipo 1 na faixa de 2 até 50 anos e do sorotipo 3 no grupo acima de 50 anos. Nos 12 anos considerados, 25 sorotipos apresentaram uma certa uniformidade na freqüência e o mesmo foi observado com relação às estações climáticas, apenas com um número maior de infecções meníngeas nos meses mais frios. Considerando a gravidade das infecções pneumocócicas notadamente as meningites, e a pouca informação relativa aos sorotipos pneumocócicos que ocorrem na região, julgamos importante essa informação relativa aos sorotipos, uma vez que tem sido usadas, com sucesso, vacinas polissacarídicas na prevenção dessas infecções.
