INTRODUCTION: Accidents caused by fish are common in inland fishing communities in Brazil, being work-related injuries in the majority of cases. These populations have no information on the mechanisms of trauma or envenoming. METHODS: Through a questionnaire administered to fishermen, we obtained clinical and epidemiological data on accidents in Rosana, Pontal do Paranapanema, State of São Paulo, Brazil. These data were analyzed and converted into an easily understood prevention and treatment program for the colony. RESULTS: Thirty-nine fishermen replied to the survey. All of the patients had been hurt by fish. Of those mentioned, the yellow catfish (Pimelodus maculatus) was the main fish species associated with injuries, but others also caused trauma to the fishermen. Six fishermen had been envenomed by stingrays. Pain and ulcers were the main symptoms and were described as intolerable. Approximately half of those injured were treated using traditional folk remedies. CONCLUSIONS: The fishermen suffered multiple accidents with catfish, which are venomous and cause intense pain, as well as trauma due to other fish, such as surubins, traíras, freshwater croakers, and piranhas. Approximately 16% of those interviewed presented with envenomation from stingrays. Our data and previous experience in the area led to the creation of a pamphlet with clear language that can effectively help fishermen in the region, an area in need of health services and disease prevention. This initiative also applies to the whole La Plata River basin, which has similar fauna.

Worldwide aging of the human population has promoted an increase in the incidence of neoplasia, including hematological cancers, which render patients particularly vulnerable to invasive fungal infections. For this reason, air filtration in hemato-oncology units has been recommended. However, scarce literature has assessed the impact of microbiological air quality on the occurrence of fungal infections in this population. We performed an integrative review of studies in the MEDLINE database that were published between January 1980 and October 2012, using the following combinations of keywords: air × quality × HEPA, air × quality × hematology, and airborne fungal infections. The search yielded only 13 articles, suggesting that high-efficiency filtering of the ambient air in hemato-oncology units can prevent the incidence of invasive fungal infections. However, no randomized clinical trial was found to confirm this suggestion. Currently, there is no consensus about the maximum allowable count of fungi in the air, which complicates filtration monitoring, including filter maintenance and replacement, and needs to be addressed in future studies.

Abstract: INTRODUCTION: Few studies have addressed the primary characteristics of patients infected with hepatitis B virus (HBV) in the general population, especially those living in small- and medium-sized cities in Brazil. We aimed to determine the clinical, demographic, and epidemiologic characteristics of patients diagnosed with HBV who were followed up at an infectious diseases clinic of a public hospital in State of São Paulo, Brazil. METHODS: Medical records of patients aged >18 years and diagnosed with HBV infection between January 2000 and December 2013 were reviewed. RESULTS: Seventy-five patients were enrolled with male-female main infection-associated risk factors; 9 (12%) were co-infected with human immunodeficiency virus (HIV), 5 (6.7%) with hepatitis C virus (HCV), and 3 (4%) were co-infected with both HIV and HCV. Antiviral HBV therapy was applied in 21 (28%) patients and tenofovir monotherapy was the most prescribed medication. After approximately 2 years of antiviral treatment, the HBV-DNA viral load was undetectable in 12 (92.3%) patients and lower levels of alanine aminotransferase were found in these patients. CONCLUSIONS: Over a 13-year interval, very few individuals infected with HBV were identified, highlighting the barriers for caring for patients with HBV in developing countries. New measures need to be implemented to complement curative practices.

PURPOSE: To determine the causes of low vision in an elderly population attended by a university visual rehabilitation service and to check for the use of prescribed optical aids. METHOD: A cross-sectional study was carried out on patients aged 60 years or over attending for the first time a university low vision service in 2001. Ophthalmic reevaluation and interview were performed by means of a structured questionnaire in 2002. RESULTS: The sample comprised 50 subjects aged between 60 and 90 years. Severe low vision (<20/200) was present in 68.0% of patients. The main cause of low vision was age-related macular degeneration (44.0%). Regarding literacy, 16.0% were illiterate and 72.0% had completed fundamental schooling. Thirty-one patients (62.0%) had been prescribed optical aids; 54.8% of these patients stated that they use them. A majority (70.6%) held a favorable opinion of these aids. CONCLUSIONS: The main cause of low vision was age-related macular degeneration. Approximately half of those receiving prescriptions reported actually using the aids in their daily activities. Making best use of residual vision in the elderly population with visual impairment is a priority, given the social context, if the independence necessary for enhanced quality of life is to be achieved.

Este trabalho foi desenvolvido com a finalidade de apresentar algumas considerações sobre a biologia reprodutiva de Clidemia Iurta, Conclui-se que: I ) A ântese tem início em torno das 18:00 h e termina por volta das 09:00 h do dia seguinte; 2) Esta é uma espécie preferencialmente alógama, porém não apresenta auto-incompatibilidade genética; 3) A polinização parece depender de agentes polinizadores, principalmente abelhas das famílias Apidae (Bambus, Melipona, Euglossa, Trigona) e Halictidae.

Dados sobre esforço e captura por espécie correspondentes à produção pesqueira desembarcada no porto de Santarém, no ano de 1993, foram submetidos a duas técnicas de análise multivariada: uma análise de fatores segundo o método de componentes principais e outra de co-variância conforme o modelo linear geral (GLM). Os resultados indicam que a atividade pesqueira na região está influenciada pelas características dos ciclos de vida das espécies-alvo, pelo ciclo hidrológico e condição climáticas de sistema, e ainda pelas preferências culturais e interesses econômicos do mercado consumidor. Os resultados indicam o direcionamento da atividade pesqueira para determinados grupos de espécies, e as diferentes variáveis incluídas no modelo permitiram uma explicação aproximada dos padrões desse direcionamento. Esses padrões são: (a) pesca de grandes bagres, alvo de pesca para a exportação (FATOR 1); (b) pesca de entre-safra do mapará (Hypophthalmusspp.) e da pescada (Plagioscionspp.), realizada nos lagos com um componente para exportação e outro para o consumo local (FATOR4); (c) pesca de peixes de escamas de hábitos sedentários e/ou migratórios, alvo da pesca comercial de pequena escala e de importância no mercado local (FATOR2, FATOR3 e FATOR5), que inclui pescarias importantes como a do tambaqui (Colossoma macropomum)e do pirarucu(Arapaima gigas)nos lagos, ou a do jaraqui (Semaprochilodusspp.) e pacu (Mylossomaspp. e Metynnisspp.) nos rios.

O presente artigo apresenta resultados sobre o consumo de pescado e outros alimentos pela população ribeirinha do Lago Grande de Monte Alegre, no Estado do Pará, Brasil. Os dados foram coletados mensalmente, por um período de dois anos juntos a 35 famílias de 17 comunidades, que praticam a pesca com fins comerciais e de subsistência, bem como as famílias que nào pescam. O consumo médio de pescado foi de 369 g/capita/dia, complementado com 6,lg/capita/dia de farinha de peixe (piracuí). As espécies mais consumidas foram: curimatá (Prochilodus nigricans) e acarí-bodó (Liposarcus partialis). Em média, as famílias tiveram alguma refeição constituída de pescado em 6 dias de cada semana. Extrapolando para toda a população do Lago, o consumo diário de pescado é pouco mais de 3 t, chegando a 1.114 t/ano.

Dados de comprimento e de captura e esforço da pesca do surubim-tigre (Pseudoplatystoma tigrinum) obtidos durante o período de 1993 a 1996, no Médio Amazonas, foram utilizados para descrever as pescarias e estimar as taxas de crescimento e mortalidade da espécie. Os parâmetros populacionais médios estimados foram Loo =180,0 cm (comprimento total), Woo = 56,56 kg, k = 0,29 ano-1, C = 0,9; WP = Novembro, M = 0,47 ano-1, Z = l ,42 ano-1, F = 0,95 ano-1 e Lc = 87,0 cm. A análise do rendimento por recruta mostra que o estoque está sendo explorado em seu limite de sustentabilidade.

Analisaram-se aspectos da biologia reprodutiva do tambaqui (Colossoma macropomum) na região do Baixo Amazonas. Foram coletados 1232 exemplares entre outubro de 1994 e fevereiro de 1996, nos mercados e frigoríficos de Santarém no Estado do Pará. Indivíduos entre 20 e 120 cm foram pesados, medidos e observados o sexo e o estágio de maturidade. O tipo de desova e a fecundidade foram determinados através da análise dos ovócitos intraovarianos de 10 fêmeas maduras. Colossoma macropomum possui desova total. Existem variações macro e microscópicas cíclicas dos ovários associadas ao período hidrológico. A desova coincidiu com o início do período das chuvas. A proporção sexual varia com o tamanho dos peixes. A relação comprimento/peso indica diferenças nas estratégias de crescimento para ambos os sexos. O menor macho sexualmente maduro mediu 62 cm e o maior 95 cm, no caso das fêmeas, 70 cm e 112 cm respectivamente. A fecundidade média absoluta calculada para o tambaqui foi de 1.007.349 ovócitos (s = 67.279). Existe uma relação linear entre a fecundidade (F) e o peso total (W1) sendo a equação F = 885347 + 9,16 W1.

OBJECTIVE: To assess whether coronary stenting in diabetic patients provides in-hospital results and clinical evolution similar to those in nondiabetic patients. METHODS: From July `97 to April '99 we performed coronary stent implantation in 386 patients with coronary heart disease, who were divided into two groups: diabetic patients and nondiabetic patients. The in-hospital results and the clinical evolution of each group were retrospectively analyzed. RESULTS: The nondiabetic group comprised 305 (79%) patients and the diabetic group 81 (21%) patients. Basic clinical and angiographic characteristics were similar. Angiographic success was in diabetics = 96.6% vs in nondiabetics = 97.9% (p=ns). Among the major complications in the in-hospital phase, the rate of myocardial infarction was higher in the diabetic group (7.4% vs 1.9%) (p=0.022). In the follow-up, a favorable and homogeneous evolution occurred in regard to asymptomatic patients, myocardial infarction, and death in the groups. A greater need for revascularization, however, existed in the diabetic patients (15% vs 2.4%, p<0.001). CONCLUSION: Coronary stenting in diabetic patients is an efficient procedure, with a high angiographic and clinical success rate similar to that in nondiabetic patients. Diabetic patients, however, had a higher incidence of in-hospital myocardial infarction and a greater need for additional myocardial revascularization.

OBJECTIVE: To analyze clinical and histologic findings of 50 patients with primary neoplams of the heart in a tertiary referral center. METHODS: From 1980 to 1998, we retrospectively analyzed 50 patients, 32 of whom were females, whose ages ranged from 9 to 73 years (mean age = 44.16±18 years). RESULTS: Most tumors were located in the left side of the heart (72%), myxoma being the most common (84%) histologic type. The other histologic types found were as follows: fibroma (4%), lipoma (2%), rhabdomyosarcoma (2%), hemangioma (2%), sarcoma (2%), angiosarcoma (2%), and lymphoma (2%). Diagnosis was established by echocardiography in 94% of the cases. Clinical findings were as follows: dyspnea (36%), weight loss (20%), palpitations (18%), chest pain (16%), fever (8%), and arthralgia (6%). All patients with thromboembolic phenomena (10%) had left atrial myxoma. Approximately 20% of the patients were asymptomatic at the initial clinical assessment. CONCLUSION: Primary cardiac tumors are a rare entity with diverse clinical and histologic findings, requiring, therefore, a high level of clinical suspicion.

OBJECTIVE: To assess the in-hospital results and clinical follow-up of young patients (< 50 years) with multivessel coronary artery disease undergoing stent implantation in native coronary arteries and to compare their results with those of patients with single-vessel coronary artery disease. METHODS: We retrospectively studied 462 patients undergoing coronary stent implantation. Patients were divided into 2 groups: group I (G-I) - 388 (84%) patients with single-vessel coronary artery disease; and group II (G-II) - 74 (16%) patients with multivessel coronary artery disease. RESULTS: The mean age of the patients was 45±4.9 years, and the clinical findings at presentation and demographic data were similar in both groups. The rate of clinical success was 95% in G-I and 95.8% in G-II (P=0.96), with no difference in regard to in-hospital evolution between the groups. Death, acute myocardial infarction, and the need for myocardial revascularization during clinical follow-up occurred in 10.1% and 11.2% (P=0.92) in G-I and G-II, respectively. By the end of 24 months, the actuarial analysis showed an event-free survival of 84.6 % in G-I and 81.1% in G-II (P=0.57). CONCLUSION: Percutaneous treatment with coronary stent implantation in young patients with multivessel disease may be safe with a high rate of clinical success, a low incidence of in-hospital complications, and a favorable evolution in clinical follow-up.

Relatamos caso de rara anomalia de artéria coronária direita saindo do terço médio da artéria descendente anterior com lesão obstrutiva ateromatosa proximal, imediatamente antes da emergência da coronária direita. O paciente foi submetido a angioplastia com implante de Stent na descendente anterior com sucesso. Há relatos de apenas 7 casos desta anomalia de distribuição na literatura, porém nenhum com tratamento de revascularização percutânea.

OBJETIVO: Os excelentes resultados obtidos com os stents eluídos com sirolimus (rapamicina) na prevenção da reestenose motivaram a avaliação de outras substâncias que também apresentassem esta propriedade. O batimastat é um bloqueador de alta eficácia da enzima metaloproteinase, com potencial para reduzir a degradação da matriz extracelular e inibir a migração das células musculares lisas, com conseqüente capacidade de controlar a reestenose coronariana. MÉTODOS: De outubro/2001 a abril/2002, foram selecionados prospectivamente, 34 pacientes com lesões "de novo", em artéria coronária nativa, >50% e < 100%, passíveis de tratamento com stents de 3 a 4 mm de diâmetro e de 18 mm de comprimento. O desfecho primário do estudo foi verificar a ocorrência de eventos cardiovasculares maiores (morte de origem cardíaca, infarto agudo do miocárdio e necessidade de revascularização do vaso alvo) aos 30 dias e aos 4 meses e o secundário avaliar a taxa de reestenose coronariana após 4 meses do implante e de trombose subaguda aos 30 dias. RESULTADOS: A taxa de sucesso do procedimento foi de 97,1%. O desfecho primário ocorreu em 2,9% e 27,2% dos pacientes aos 30 dias e aos 4 meses respectivamente. A taxa de reestenose binária ao estudo angiográfico foi de 39,3%. Não houve episódio de trombose subaguda. A análise comparativa entre os grupos que apresentaram ou não reestenose não mostrou diferenças significativas entre ambos, exceto na perda luminal tardia, maior no G-I. CONCLUSÃO: Os stents eluídos com batimastat apresentaram bom perfil de segurança, entretanto, não se mostraram efetivos no controle da reestenose coronariana.

OBJETIVO: Avaliar a influência do diabete melito (DM) nos resultados imediatos do implante de stent coronário (SC), de acordo com o quadro clínico de apresentação. MÉTODOS: Entre janeiro/1997 e dezembro/2003, segundo a Central Nacional de Intervenções Cardiovasculares (CENIC), 11.874 pacientes diabéticos foram submetidos a implante de SC: 7.386 (62,3%) com insuficiência coronária crônica (ICO), 3.142 (26,4%), em síndrome isquêmica instável sem elevação ST (SIASEST) e 1.346 (11,3%), com infarto agudo do miocárdio (IAM) com supradesnivelamento de ST. Estes grupos foram comparados com 48.103 não-diabéticos: 30.980 (64,5%) com ICO, 10.938 (22,7%) em SIASEST e 6.185 (12,8%) com IAM. RESULTADOS: Os diabéticos apresentaram características clínicas e angiográficas de maior risco. Os diabéticos com ICO apresentaram taxa de eventos adversos semelhantes aos não-diabéticos (0,98% x 0,91%, p=0,5971), porém, os diabéticos em SIASEST e IAM apresentaram maior incidência de eventos: 2,76% x 1,46% (p<0,0001) e 7,87% x 4,1% (p<0,0001), respectivamente. A análise multivariada mostrou o DM como preditor independente de risco para eventos adversos maiores na SIASEST (OR: 1,92 IC: 1,46-2,52 p<0,0001) e no IAM (OR: 2,0 IC: 1,57-2,54 p<=0,0001) e não na ICO (OR: 1,08 IC: 0,83-1,42 p=0,5470). CONCLUSÃO: Os pacientes diabéticos portadores de ICO apresentaram evolução hospitalar semelhante aos não diabéticos, porém, os com SIASEST e IAM demonstraram maior taxa de eventos cardíacos adversos comparados com a população não-diabética.
