OBJETIVO: Verificar a validade da classificação do estado nutricional e da estatura para idade quando utilizadas massa corporal e estatura autorreferidas por adolescentes, bem como relacionar essa validade com percepção e com satisfação corporal. MÉTODOS: 304 adolescentes antes de terem suas medidas de massa e estatura aferidas autorreferiram essas medidas, foram questionados sobre a percepção de seu corpo e a classificação da imagem corporal foi obtida com o uso de uma escala de silhuetas corporais. Também foram aplicados testes específicos para avaliar a satisfação corporal e o comportamento alimentar. RESULTADOS: As classificações feitas com base nos dados autorreferidos e aferidos tiveram concordância alta (K = 1,00) para a classificação da estatura por idade e moderada (K < 0,75) para a classificação de índice de massa corporal. No entanto, adolescentes que se consideravam "abaixo do peso" subestimaram o índice de massa corporal autorrelatado e, os que se percebiam como "gordos", o superestimaram. As meninas e os que se classificaram como obesos tenderam a subestimar o índice de massa corporal, e os meninos, a superestimá-lo. CONCLUSÃO: Os adolescentes autorrelataram suas medidas de massa e estatura de maneira adequada. No entanto, o sexo, a percepção e a classificação da imagem corporal foram variáveis que influenciaram na obtenção de respostas não válidas.

OBJETIVO: Identificar a influência de variáveis antropométricas sobre a insatisfação corporal (IC) e o comportamento alimentar inadequado (CAI) em atletas adolescentes competitivos. MÉTODOS: Participaram 580 jovens (464 meninos e 116 meninas) com idades entre 10 e 19 anos, de ambos os sexos. Aplicou-se o Body Shape Questionnaire para mensurar a IC. Utilizou-se o Eating Attitudes Test para avaliar CAI. Foram aferidas dobras cutâneas para estimar o percentual de gordura (%G). Mensuraram-se peso e estatura para calcular o IMC. RESULTADOS: Os resultados evidenciaram que a IC no sexo feminino foi modulada apenas pelo %G, ao contrário do sexo masculino, em que IMC e %G, juntos, explicaram parte de sua variância (p < 0,05). O CAI no sexo masculino foi pouco influenciado pelo %G. CONCLUSÃO: Pode-se concluir que o %G foi a única variável que influenciou a IC em ambos os sexos. Ademais, os CAI em jovens atletas parecem não ser influenciados pelas características antropométricas mensuradas neste estudo.

OBJECTIVE: Risk stratification of patients with nonsustained ventricular tachycardia (NSVT) and chronic chagasic cardiomyopathy (CCC). METHODS: Seventy eight patients with CCC and NSVT were consecutively and prospectively studied. All patients underwent to 24-hour Holter monitoring, radioisotopic ventriculography, left ventricular angiography, and electrophysiologic study. With programmed ventricular stimulation. RESULTS: Sustained monomorphic ventricular tachycardia (SMVT) was induced in 25 patients (32%), NSVT in 20 (25.6%) and ventricular fibrillation in 4 (5.1%). In 29 patients (37.2%) no arrhythmia was inducible. During a 55.7-month-follow-up, 22 (28.2%) patients died, 16 due to sudden death, 2 due to nonsudden cardiac death and 4 due to noncardiac death. Logistic regression analysis showed that induction was the independent and main variable that predicted the occurrence of subsequent events and cardiac death (probability of 2.56 and 2.17, respectively). The Mantel-Haenszel chi-square test showed that survival probability was significantly lower in the inducible group than in the noninductible group. The percentage of patients free of events was significantly higher in the noninducible group. CONCLUSION: Induction of SMVT during programmed ventricular stimulation was a predictor of arrhythmia occurrence cardiac death and general mortality in patients with CCC and NSVT.

OBJECTIVE: To assess the behavior of cardiac variables in animals exposed to cigarette smoke. METHODS: Two groups of Wistar rats were studied as follows: control group (C), comprising 28 animals; and smoking group (S), comprising 23 animals exposed to cigarette smoke for 30 days. Left ventricular cardiac function was assessed in vivo with transthoracic echocardiography, and myocardial performance was analyzed in vitro in preparations of isolated left ventricular papillary muscle. The cardiac muscle was assessed in isometric contractions with an extracellular calcium concentration of 2.5 mmol/L. RESULTS: No statistical difference was observed in the values of the body variables of the rats and in the mechanical data obtained from the papillary muscle between the control and smoking groups. The values of left ventricular systolic diameter were significantly greater in the smoking animals than in the control animals (C= 3.39 ± 0.4 mm and S= 3.71 ± 0.51 mm, P=0.02). A significant reduction was observed in systolic shortening fraction (C= 56.7 ± 4.2% and S= 53.5 ± 5.3%, P=0.02) and in ejection fraction (C= 0.92 ± 0.02 and S= 0.89 ± 0.04, P=0.01). CONCLUSION: The rats exposed to cigarette smoke had a reduction in left ventricular systolic function, although their myocardial function was preserved.

OBJECTIVE: To assess the influence of skeletal muscle mass on ventilatory and hemodynamic variables during exercise in patients with chronic heart failure (CHF). METHODS: Twenty-five male patients underwent maximum cardiopulmonary exercise testing on a treadmill with a ramp protocol and measurement of the skeletal muscle mass of their thighs by using magnetic resonance imaging. The clinically stable, noncachectic patients were assessed and compared with 14 healthy individuals (S) paired by age and body mass index, who underwent the same examinations. RESULTS: Similar values of skeletal muscle mass were found in both groups (CHF group: 3863 ± 874 g; S group: 3743 ± 540 g; p = 0.32). Significant correlations of oxygen consumption in the anaerobic threshold (CHF: r = 0.39; P= 0.02 and S: r = 0.14; P = 0.31) and of oxygen pulse also in the anaerobic threshold (CHF: r = 0.49; P = 0.01 and S: r =0.12; P = 0.36) were found only in the group of patients with chronic heart failure. CONCLUSION: The results obtained indicate that skeletal muscle mass may influence the capacity of patients with CHF to withstand submaximal effort, due to limitations in their physical condition, even maintaining a value similar to that of healthy individuals. This suggests qualitative changes in the musculature.

FUNDAMENTO: O sobrepeso e a obesidade são um importante problema de saúde pública na sociedade pela sua associação com diversas doenças crônicas. OBJETIVO: O objetivo deste estudo é determinar a prevalência e a distribuição de sobrepeso e obesidade, usando diferentes medidas antropométricas, e identificar o melhor indicador antropométrico intimamente relacionado aos fatores de risco de Doenças Cardiovasculares (DCV) em população iraniana urbana. MÉTODOS: O presente estudo transversal foi realizado com 991 homens e 1.188 mulheres de 15 a 64 anos. Foram medidos Índice de Massa Corporal (IMC), Circunferência Abdominal (CA), Relação Cintura-Quadril (RCQ), Relação Cintura-Altura (RCA) e porcentagem de gordura corporal. Foi obtida amostra de sangue em jejum. Foram avaliados os fatores de risco cardiovascular, incluindo glicemia de jejum, triglicerídeos, colesterol total (col-T), colesterol de baixa densidade (LDL-colesterol) e colesterol da lipoproteína de alta densidade (HDL-colesterol). RESULTADOS: Em relação ao IMC, 49% dos homens e 53% das mulheres estavam acima do peso ou obesos, e 10,2% dos homens e 18,6% das mulheres encontravam-se obesos. Tanto nos homens quanto nas mulheres, a prevalência de sobrepeso esteve maior entre aqueles com 40-49 anos de idade, e a prevalência de obesidade esteve maior entre aqueles com 50 anos ou mais. Usando a análise de regressão múltipla, IMC, RCA e RCQ explicaram o maior percentual de variação de triglicerídeos, razão entre col-T e HDL-colesterol e LDL-colesterol em homens, respectivamente, ao passo que RCQ explicou o maior percentual de variação de triglicerídeos e CA explicou o maior percentual de variação da razão entre col-T e HDL-colesterol e LDL-colesterol em mulheres. CONCLUSÃO: Nossos dados indicam que RCQ e RCA foram os indicadores antropométricos que melhor previram fatores de risco cardiovascular em homens e RCQ e CA, em mulheres.

FUNDAMENTO: A obesidade tem sido identificada como importante fator de risco no desenvolvimento de doenças cardiovasculares, porém outros fatores exercem influência, combinados ou não à obesidade, e devem ser considerados na estratificação de risco cardiovascular em pediatria. OBJETIVO: Analisar a associação entre medidas antropométricas e fatores de risco cardiovascular, investigar os determinantes para as mudanças da pressão arterial (PA) e propor uma equação de predição para circunferência de cintura (CC) em crianças e adolescentes. MÉTODOS: Foram avaliadas 1.950 crianças e adolescentes, com idade entre 7-18 anos. Foi investigada a gordura visceral pela CC e a relação cintura-quadril, PA e índice de massa corporal (IMC). Em uma subamostra selecionada aleatoriamente desses voluntários (n = 578), foram medidos o colesterol total, a glicemia e os triglicerídeos. RESULTADOS: A CC se correlacionou positivamente com o IMC (r = 0,85; p < 0,001) e a PA (PAS r = 0,45 e PAD = 0,37; p < 0,001). A glicemia e os triglicerídeos apresentaram correlação fraca com a CC (r = 0,110; p = 0,008 e r = 0,201; p < 0,001, respectivamente). O colesterol total não se correlacionou com nenhuma variável. Idade, IMC e CC foram preditores significativos nos modelos de regressão para PA (p < 0,001). Propõe-se uma equação de predição da CC para crianças e adolescentes: meninos: y = 17,243 + 0,316 (altura em cm); meninas: y = 25,197 + 0,256 (altura em cm). CONCLUSÃO: A CC está associada com fatores de risco cardiovascular e apresenta-se como fator preditor de risco para hipertensão em crianças e adolescentes. A equação de predição para CC proposta em nosso estudo deve ser testada em futuros trabalhos.

Fundamento: Estima-se que a prevalência de hipertensão em crianças e adolescentes varie entre 1-13%. O excesso de peso e a obesidade central estão relacionados aos níveis pressóricos em adultos e podem ser importantes na patogênese precoce da HAS quando presentes na infância. Objetivos: Identificar a associação entre variáveis antropométricas e níveis pressóricos em escolares de 5.ª a 8.ª séries e avaliar qual medida obteve maior correlação com a medida dos níveis pressóricos. Métodos: Estudo transversal contemporâneo com amostra de base populacional probabilística por conglomerados em escolas públicas do ensino fundamental de Porto Alegre, de alunos matriculados entre a 5.ª e a 8.ª série. Foram coletados dados sobre fatores de risco familiares e antropometria. A análise estatística incluiu correlações e ajuste dos intervalos de confiança para conglomerados. Resultados: A média de idade dos participantes foi de 12,57 (± 1,64) anos, dos quais 55,2% eram do sexo feminino. Encontraram-se 11,3% da amostra com níveis pressóricos alterados e 16,2% com valores limítrofes. Das variáveis antropométricas analisadas, a que demonstrou maior correlação com valores pressóricos aumentados foi o diâmetro do quadril (r = 0,462, p < 0,001) seguido de circunferência abdominal menor (r = 0,404, p < 0,001) e prega cutânea abdominal (r = 0,291, p < 0,001). Conclusão: Foi observada associação entre as circunferências da cintura e dobras cutâneas e níveis pressóricos aumentados nos escolares da amostra. Portanto, é de fundamental importância que a aferição da pressão arterial e as medidas de cintura e quadril sejam rotina nos serviços de saúde de forma precoce a fim de prevenir essa condição patológica.

Background:The risk factors that characterize metabolic syndrome (MetS) may be present in childhood and adolescence, increasing the risk of cardiovascular disease in adulthood.Objective:Evaluate the prevalence of MetS and the importance of its associated variables, including insulin resistance (IR), in children and adolescents in the city of Guabiruba-SC, Brazil.Methods:Cross-sectional study with 1011 students (6–14 years, 52.4% girls, 58.5% children). Blood samples were collected for measurement of biochemical parameters by routine laboratory methods. IR was estimated by the HOMA-IR index, and weight, height, waist circumference and blood pressure were determined. Multivariate logistic regression models were used to examine the associations between risk variables and MetS.Results:The prevalence of MetS, IR, overweight and obesity in the cohort were 14%, 8.5%, 21% and 13%, respectively. Among students with MetS, 27% had IR, 33% were overweight, 45.5% were obese and 22% were eutrophic. IR was more common in overweight (48%) and obese (41%) students when compared with eutrophic individuals (11%; p = 0.034). The variables with greatest influence on the development of MetS were obesity (OR = 32.7), overweight (OR = 6.1), IR (OR = 4.4; p ≤ 0.0001 for all) and age (OR = 1.15; p = 0.014).Conclusion:There was a high prevalence of MetS in children and adolescents evaluated in this study. Students who were obese, overweight or insulin resistant had higher chances of developing the syndrome.

Background: Heart failure prediction after acute myocardial infarction may have important clinical implications. Objective: To analyze the functional echocardiographic variables associated with heart failure in an infarction model in rats. Methods: The animals were divided into two groups: control and infarction. Subsequently, the infarcted animals were divided into groups: with and without heart failure. The predictive values were assessed by logistic regression. The cutoff values predictive of heart failure were determined using ROC curves. Results: Six months after surgery, 88 infarcted animals and 43 control animals were included in the study. Myocardial infarction increased left cavity diameters and the mass and wall thickness of the left ventricle. Additionally, myocardial infarction resulted in systolic and diastolic dysfunction, characterized by lower area variation fraction values, posterior wall shortening velocity, E-wave deceleration time, associated with higher values of E / A ratio and isovolumic relaxation time adjusted by heart rate. Among the infarcted animals, 54 (61%) developed heart failure. Rats with heart failure have higher left cavity mass index and diameter, associated with worsening of functional variables. The area variation fraction, the E/A ratio, E-wave deceleration time and isovolumic relaxation time adjusted by heart rate were functional variables predictors of heart failure. The cutoff values of functional variables associated with heart failure were: area variation fraction < 31.18%; E / A > 3.077; E-wave deceleration time < 42.11 and isovolumic relaxation time adjusted by heart rate < 69.08. Conclusion: In rats followed for 6 months after myocardial infarction, the area variation fraction, E/A ratio, E-wave deceleration time and isovolumic relaxation time adjusted by heart rate are predictors of heart failure onset.

We studied the reproductive biology of a population of Pseudis minuta Günther, 1858 from Reserva Biológica do Lami (30º 15' S; 51º 05' W), Porto Alegre, southern Brazil. We assessed the spatial and temporal distribution of individuals (males, females, juveniles) and explored potential relationships with environmental variables. Field activities encompassed bimonthly surveys in three semi-permanent ponds, each one during approximately two days and two nights, from August 2004 to July 2005. We recorded differences in the sites used by males, females and juveniles, with males occupying deeper and more distant places from the border. The temporal distributions of individuals, calling sites and amplectant pairs indicated that the reproductive activity of P. minuta is related to some of the studied abiotic factors. Calling males presented statistical differences in relation to non-calling males for all daily abiotic variables analyzed (air temperature, water temperature, relative humidity and rainfall), as well as to monthly temperature and rainfall. The number of active males, females and juveniles was influenced by at least one of the daily or monthly environmental variables analyzed. We conclude that the reproduction in this species is seasonal and may be partially determined by abiotic factors.

Between January and March 1998, a cross-sectional survey was carried out in four rural communities in Honduras, Central America. We examined the prevalence and intensity of Ascaris lumbricoides and Trichuris trichiura infections among 240 fecal specimens, and the association between selected socio-demographic variables and infection for 62 households. The overall prevalence of A. lumbricoides and T. trichiura was 45% (95% CI 39.0-51.9) and 38% (95% CI 31.8-44.4) respectively. The most intense infections for Ascaris and Trichuris were found in children aged 2-12 years old. By univariate analysis variables associated with infections of A. lumbricoides were: number of children 2-5 years old (p=0.001), level of formal education of respondents (p=0.01), reported site of defecation of children in households (p=0.02), households with children who had a recent history of diarrhea (p=0.002), and the location of households (p=0.03). Variables associated with both A. lumbricoides and T. trichiura infection included: number of children 6-14 years old (p=0.01, p=0.04, respectively), ownership of a latrine (p=0.04, p=0.03, respectively) and coinfection with either helminth (p=0.001, p=0.001, respectively). By multivariate analysis the number of children 2-5 years living in the household, (p=0.01, odds ratio (OR)=22.2), children with a recent history of diarrhea (p=0.0, OR=39.8), and infection of household members with T. trichiura (p=0.02, OR=16.0) were associated with A. lumbricoides infection. The number of children 6-14 years old in the household was associated with both A. lumbricoides and T. trichiura infection (p=0.04, p=0.01, OR=19.2, OR=5.2, respectively).

The immature stages of Ochlerotatus albifasciatus develop in temporary pools. The present study aims at evaluating the seasonal dynamics of the aquatic stages of this mosquito, also analyzing the relationship among their presence and breeding success to some relevant climatic and environmental variables in the ephemeral rain pools of an urban park. Nineteen cohorts of O. albifasciatus that developed synchronously after rain events were recorded in all seasons. The proportions of mosquito-positive pools were significantly higher during the fall-winter period than in the spring-summer months (p < 0.001). The presence of this mosquito species was positively related to the amount of rain (p < 0.001), whereas negatively correlated to air temperature (p < 0.05) within a 5.2 to 29.7ºC range. The distribution of the number of cohorts per pool throughout the year was grouped (variance/mean: 3.96), indicating that these habitats were not equally suitable as breeding sites. The immature stages of O. albifasciatus were detected in pools belonging to all of the categories of surface area, depth, duration, vegetation cover, and insolation. However, the proportion of pools where immature mosquitoes were detected was positively and significantly related to surface, depth, duration, and vegetation cover. On the other hand, the proportion of mosquito-positive pools was higher at an intermediate insolation degree. Our results suggest that although preimaginal stages were present in all seasons, high temperatures may be unfavorable to larval development, and substrate vegetation may regulate water temperature. The positive relationship between the proportion of mosquito-positive pools and pool size and duration might reflect a strategy of O. albifasciatus to accomplish immature development.

In the present paper, we evaluate the relationship between climate variables and population density of Lutzomyia longipalpis in Montes Claros, an area of active transmission of American visceral leishmaniasis (AVL) in Brazil. Entomological captures were performed in 10 selected districts of the city, between September 2002-August 2003. A total of 773 specimens of L. longipalpiswere captured in the period and the population density could be associated with local climate variables (cumulative rainfall, average temperature and relative humidity) through a mathematical linear model with a determination coefficient (Rsqr) of 0.752. Although based on an oversimplified statistical analysis, as far as the vector is concerned, this approach showed to be potentially useful as a starting point to guide control measures for AVL in Montes Claros.

The aim of the study was to determine the prevalence and variables associated with the pattern of risky health behavior (PRHB) among adolescent students in Cartagena, Colombia. A cross-sectional study was designed to investigate PRHB in a random cluster sample of students from middle and high schools. The associations were adjusted by logistic regression. A total of 2,625 students participated in this research, with ages from 10 to 20 years, mean=13.8 years (SD=2.0), and 54.3% were women. A total of 332 students reported PRHB (12.7%, 95%CI 11.4–14.0). Age over 15 years (OR=2.19, 95%CI 1.72–2.79), not being heterosexual (OR=1.98, 95%CI 1.36-2.87), poor/mediocre academic performance (OR=1.87, 95%CI 1.47–2.38), family dysfunction (OR=1.78, 95%CI 1.40–2.28) and male gender (OR=1.58, 95%CI 1.24–2.01) were associated with PRHB. One in every eight students presented a PRHB. It is important to pay greater attention to students who are over 15 years of age, male, not heterosexual, with a poor/mediocre academic performance and a dysfunctional family.
