Patients undergoing neurosurgery are predisposed to a variety of complications related to mechanical ventilation (MV). There is an increased incidence of extubation failure, pneumonia, and prolonged MV among such patients. The aim of the present study was to assess the influence of extubation failure and prolonged MV on the following variables: postoperative pulmonary complications (PPC), mortality, reoperation, tracheostomy, and duration of postoperative hospitalization following elective intra-cranial surgery. The study involved a prospective observational cohort of 317 patients submitted to elective intracranial surgery for tumors, aneurysms and arteriovenous malformation. Preoperative assessment was performed and patients were followed up for the determination of extubation failure and prolonged MV (>48 h) until discharge from the hospital or death. The occurrence of PPC, incidence of death, the need for reoperation and tracheostomy, and the length of hospitalization were assessed during the postoperative period. Twenty-six patients (8.2%) experienced extubation failure and 30 (9.5%) needed prolonged MV after surgery. Multivariate analysis showed that extubation failure was significant for the occurrence of death (OR = 8.05 [1.88; 34.36]), PPC (OR = 11.18 [2.27; 55.02]) and tracheostomy (OR = 7.8 [1.12; 55.07]). Prolonged MV was significant only for the occurrence of PPC (OR = 4.87 [1.3; 18.18]). Elective intracranial surgery patients who experienced extubation failure or required prolonged MV had a higher incidence of PPC, reoperation and tracheostomy and required a longer period of time in the ICU. Level of consciousness and extubation failure were associated with death and PPC. Patients who required prolonged MV had a higher incidence of extubation failure.

Because the superficial lymphatics in the lungs are distributed in the subpleural, interlobular and peribroncovascular interstitium, lymphatic impairment may occur in the lungs of patients with idiopathic interstitial pneumonias (IIPs) and increase their severity. We investigated the distribution of lymphatics in different remodeling stages of IIPs by immunohistochemistry using the D2-40 antibody. Pulmonary tissue was obtained from 69 patients with acute interstitial pneumonia/diffuse alveolar damage (AIP/DAD, N = 24), cryptogenic organizing pneumonia/organizing pneumonia (COP/OP, N = 6), nonspecific interstitial pneumonia (NSIP/NSIP, N = 20), and idiopathic pulmonary fibrosis/usual interstitial pneumonia (IPF/UIP, N = 19). D2-40+ lymphatic in the lesions was quantitatively determined and associated with remodeling stage score. We observed an increase in the D2-40+ percent from DAD (6.66 ± 1.11) to UIP (23.45 ± 5.24, P = 0.008) with the advanced process of remodeling stage of the lesions. Kaplan-Meier survival curves showed a better survival for patients with higher lymphatic D2-40+ expression than 9.3%. Lymphatic impairment occurs in the lungs of IIPs and its severity increases according to remodeling stage. The results suggest that disruption of the superficial lymphatics may impair alveolar clearance, delay organ repair and cause severe disease progress mainly in patients with AIP/DAD. Therefore, lymphatic distribution may serve as a surrogate marker for the identification of patients at greatest risk for death due to IIPs.

Exposure to air pollutants is associated with hospitalizations due to pneumonia in children. We hypothesized the length of hospitalization due to pneumonia may be dependent on air pollutant concentrations. Therefore, we built a computational model using fuzzy logic tools to predict the mean time of hospitalization due to pneumonia in children living in São José dos Campos, SP, Brazil. The model was built with four inputs related to pollutant concentrations and effective temperature, and the output was related to the mean length of hospitalization. Each input had two membership functions and the output had four membership functions, generating 16 rules. The model was validated against real data, and a receiver operating characteristic (ROC) curve was constructed to evaluate model performance. The values predicted by the model were significantly correlated with real data. Sulfur dioxide and particulate matter significantly predicted the mean length of hospitalization in lags 0, 1, and 2. This model can contribute to the care provided to children with pneumonia.

The biological nitrogen fixation is an alternative to supply the nitrogen needed for maize. The objective of this study was to evaluate the development and yield of maize in response to inoculation with Azospirillum associated with nitrogen fertilization. We conducted two field experiments in the summer harvest, the first in the 2000/2001 crop year in the region of Marechal Cândido Rondon, under conventional tillage, and second in the 2002/2003 agricultural year in the region of Cascavel, under no tillage. The experimental design in both experiments was a randomized complete block, with four replications, 2x2x2 factorial, with two levels of nitrogen at sowing (zero and 20 kg ha-1), two levels of inoculum (zero and 200 g ha-1) and two levels of nitrogen in topdressing (zero and 100 kg ha-1). There was evaluated the height of ear insertion, total plant height, leaf N content, shoot dry biomass and grain yield. The height of ear insertion and total plant height were not influenced by the factors under study. Nitrogen fertilization at sowing increased the leaf N content, causing the opposite effect when combined with inoculation. Inoculation with Azospirillum in the absence of nitrogen, provide productivity increases of 15.4% and 7.4% for 2000/2001 and 2002/2003 crops, respectively. The inoculation provided productivity similar to that obtained with 100 kg ha-1 in topdressing in crop 2000/2001, while in association with the topdressing, reduced productivity and shoot dry biomass in crop 2002/2003.

Foi realizado um estudo descritivo, a partir das informações provenientes de órgãos oficiais e dos atestados de óbito, sobre a mortalidade por pneumonia entre os anos de 1979 e 1985 no Município de Belo Horizonte, MG (Brasil). Os dados revelaram que a taxa de mortalidade chegou a ser 35 vezes superior àquela dos países desenvolvidos e que a redução anual da mortalidade no período em questão foi 2/3 daquela obtida nesses mesmos países. Em 1985, estas disparidades também ocorreram dentro do próprio município pois, nas zonas de maior renda familiar média mensal as taxas foram menos elevadas, embora estatisticamente não significativas. (Z = 1,2, p > 0,05).

Através de um estudo tipo caso-controle, foi comparada uma amostra de óbitos pós-neonatais por pneumonia ocorridos na Região Metropolitana do Rio de Janeiro, Brasil (1986-1987) e controles sadios, moradores na vizinhança. Os fatores de risco investigados foram variáveis relacionadas à história gestacional da mãe e ao nascimento da criança, às condições sociais da família e à utilização de serviços de saúde. Na primeira etapa de análise, através de um modelo de regressão logística univariada, foram estimados os coeficientes de cada variável independente, o risco relativo e seus limites de confiança. O peso ao nascer e a idade do desmame mostraram-se das mais fortemente associadas com a variável dependente. Na segunda etapa, foi feito o ajuste pelo modelo de regressão logística múltipla e somente 4 variáveis permaneceram estatisticamente associadas com a mortalidade: idade do desmame, peso ao nascer, número de moradores da casa e aplicação da vacina BCG. Conclui-se que a mortalidade por pneumonia em menores de um ano está fortemente associada às condições sociais da família, em particular da mãe.

Estudou-se a letalidade hospitalar devido à diarréia ou pneumonia em menores de um ano de idade na Região Metropolitana do Rio de Janeiro cujo óbito ocorreu entre abril/86 e maio/87. Foram investigados possíveis fatores prognósticos de letalidade hospitalar em relação a condições socioeconômicas, biológicas e amamentação aos 30 dias de vida. Foi utilizada metodologia de caso-controle, sendo casos as crianças internadas por diarréia ou por pneumonia que morreram, e controles aquelas que sobreviveram. Referências são feitas aos fenômenos de causalidade reversa e "overmatching" como possíveis viéses neste tipo de estudo. Razão de produtos cruzados (RPC) foi utilizada para estimar os riscos relativos, através de regressão logística não condicional. Os principais fatores prognósticos encontrados foram prematuridade, baixo peso ao nascer, mau estado geral e déficit peso/idade na hospitalização. Nas crianças com pneumonia a duração do aleitamento materno esteve associado com a letalidade (RPC=2,0). As condições biológicas evidenciaram-se como os principais fatores prognósticos de letalidade hospitalar por diarréia ou pneumonia.

A programme for the control of respiratory diseases in children was conceived for the State of S. Paulo, Brazil, in 1986. Its progress thereafter and the epidemiology of the diseases concerned are examined. Apart from an inquiry into the 64 existing State local health authorities, a sample of 18,255 cases of children assisted by the programme at different levels, including both in-patient and outpatient care, is analysed. Each case record included information about identification (child, doctor and health facility), reasons for calling, diagnoses made and outcome of treatment. Further data were also sought from hospitals and from State mortality records. The programme was found to be poorly implemented in the State but, where implemented, it showed itself capable of resolving problems (only 0.5% of the cases could not be handled) as also of changing ongoing trends (more than 50% reduction in hospital admission rates). Individual assessment of each item of the programme indicated its bottlenecks. Regarding the epidemiology of respiratory diseases, it is observed that the major burden to health services comes from children aged less than five, and that the most important diseases are wheezing illnesses and pneumonia. Morevoer, they were found to be significantly associated (p = 0.000) so that a child in the community presenting wheezing diseases is 5 times more likely to develop pneumonia than a child with any other respiratory diagnosis. Similarly, among the under five deaths it was found that the risk for pneumonia is 3 times greater for children who died presenting wheezing diseases than it is for children with any other sort of diagnosis. In conclusion, the programme is deemed to be efficient and effective but its efficacy is marred by administrative flaws. The successful control of respiratory problems in childhood is related to a proper appreciation of the importance of wheezing diseases.

OBJETIVO: Investigar a influência de fatores socioeconômicos e gestacionais sobre a hospitalização por pneumonia no período pós-neonatal. MATERIAL E MÉTODO: Longitudinal. Crianças com idade entre 28 e 364 dias, nascidas na cidade de Pelotas, RS (Brasil), em 1993. A definição de caso foi a permanência em ambiente hospitalar por um período igual ou superior a 24 horas em conseqüência de pneumonia. Foi aplicado delineamento longitudinal. RESULTADOS: Dentre as 5.304 crianças da coorte, 152 (2,9%) foram hospitalizadas por pneumonia no período. O valor preditivo positivo do diagnóstico clínico comparado com o radiológico alcançou 76%. A análise através de regressão logística mostrou que a classe social e a escolaridade materna estiveram forte e inversamente associadas à admissão hospitalar. Filhos de mães adolescentes tiveram risco duplicado à internação; paridade igual ou superior a três representou risco 2,8 vezes maior em relação às mães primíparas; ganho de peso inferior a 10 kg durante a gestação implicou risco cerca de 40% maior à hospitalização. CONCLUSÕES: A classe social e a escolaridade materna foram os principais determinantes da hospitalização. Idade e paridade materna e o ganho de peso durante a gestação foram também fatores de risco importantes.

INTRODUCTION: Although obesity is well recognized as a current public health problem, its prevalence and impact among pregnant women have been less investigated in Brazil. The objective of the study was to evaluate the impact of pre-obesity and obesity among pregnant women, describing its prevalence and risk factors, and their association with adverse pregnancy outcomes. METHODS: A cohort of 5,564 pregnant women, aged 20 years or more, enrolled at aproximately 20 to 28 weeks of pregnancy, seen in prenatal public clinics of six state capitals in Brazil were followed up, between 1991 and 1995. Prepregnancy weight, age, educational level and parity were obtained from a standard questionnaire. Height was measured in duplicate and the interviewer assigned the skin color. Nutritional status was defined using body mass index (BMI), according to World Health Organization (WHO) criteria. Odds ratios and 95% confidence interval were calculated using logistic regression. RESULTS: Age-adjusted prevalences (and 95% CI) based on prepregnancy weight were: underweight 5.7% (5.1%-6.3%), overweight 19.2% (18.1%-20.3%), and obesity 5.5% (4.9%-6.2%). Obesity was more frequently observed in older black women, with a lower educational level and multiparous. Obese women had higher frequencies of gestational diabetes, macrosomia, hypertensive disorders, and lower risk of microsomia. CONCLUSIONS: Overweight nutritional status (obesity and pre-obesity) was seen in 25% of adult pregnant women and it was associated with increased risk for several adverse pregnancy outcomes, such as gestational diabetes and pre-eclampsia.

OBJETIVO: Investigar os efeitos causados pela poluição atmosférica na morbidade por pneumonia e por gripe em idosos entre 1996 e 1998. MÉTODOS: Foram obtidos dados diários de atendimentos por pneumonia e gripe para idosos em pronto-socorro médico de um hospital-escola de referência no Município de São Paulo, SP, Brasil. Os níveis diários de CO, O3, SO2, NO2 e PM10 foram obtidos na Companhia de Tecnologia de Saneamento Ambiental, e os dados diários de temperatura e umidade relativa do ar foram obtidos no Instituto Astronômico e Geofísico da USP. Para verificar a relação existente entre pneumonia e gripe e poluição atmosférica, utilizou-se o modelo aditivo generalizado de regressão de Poisson, tendo como variável dependente o número diário de atendimentos por pneumonia e gripe e como variáveis independentes as concentrações médias diárias dos poluentes atmosféricos. A análise foi ajustada para sazonalidade de longa duração (número de dias transcorridos), sazonalidade de curta duração (dias da semana), temperatura mínima, umidade média, períodos de rodízio e os atendimentos por doenças não-respiratórias em idosos. RESULTADOS: O3 e SO2 estão diretamente associados à pneumonia e à gripe, independentemente das variáveis de controle. Porém, na análise conjunta, eles perdem sua significância estatística. Pôde-se observar que um aumento interquartil (25%-75%) para o O3 (38,80 mig/m³) e SO2 (15,05 mig/m³) levaram a um acréscimo de 8,07% e 14,51%, respectivamente, no número de atendimentos por pneumonia e gripe em idosos. CONCLUSÕES: Os resultados sugerem que a poluição atmosférica promove efeitos adversos para a saúde de idosos.

OBJECTIVE: To identify factors that lead people to visit a doctor in Brazil and assess differences between socioeconomic groups. METHODS: A cross-sectional study comprising 1,260 subjects aged 15 or more was carried out in southern Brazil. Demographic, socioeconomic, health needs and regular source of care data were analyzed concerning visits to a doctor within two months from the interview. Adjusted prevalence ratios and 95% confidence intervals were calculated using Poisson regression. RESULTS: Adjusted PR showed that women having stressful life events, health insurance, and a regular doctor increased the outcome. A dose-related response was found with self-reported health, and the probability of visiting a doctor increased with health needs. Analysis in the chronic disease group revealed that uneducated lower income subjects had a 62% reduction in the chance of visiting a doctor compared to uneducated higher income ones. However, as it was seen a significant interaction between income and education, years of schooling increased utilization in this group. CONCLUSIONS: Results suggest the existence of health inequity in the poorest group that could be overcome with education. Specific measures reinforcing the importance of having a regular doctor may also improve access in the underserved group.

OBJECTIVE: Blood donors in Brazil have been routinely screened for HTLV-I/II since 1993. A study was performed to estimate the prevalence of HTLV-I/II infection in a low risk population and to better understand determinants associated with seropositivity. METHODS: HTLV-I/II seropositive (n=135), indeterminate (n=167) and seronegative blood donors (n=116) were enrolled in an open prevalence prospective cohort study. A cross-sectional epidemiological study of positive, indeterminate and seronegative HTLV-I/II subjects was conducted to assess behavioral and environmental risk factors for seropositivity. HTLV-I/II serological status was confirmed using enzyme-linked immunosorbent assay (EIA) and Western blot (WB). RESULTS: The three groups were not homogeneous. HTLV-I/II seropositivity was associated to past blood transfusion and years of schooling, a marker of socioeconomic status, and use of non-intravenous illegal drugs. CONCLUSIONS: The study results reinforce the importance of continuous monitoring and improvement of blood donor selection process.

OBJECTIVE: Odorous compounds produced at the biosolids composting plant in Bariloche (NW Patagonia) attract a variety of insects, mainly belonging to the order Diptera. In order to characterize these flies, collected specimens were taxonomically identified, their community characteristics were described and their sanitary and synanthropic importance and autochthonous or introduced character were determined. METHODS: Sampling was performed from October 1999 until March 2000. Adults were collected using an entomological net, and larvae and puparia were obtained from the composting material and incubated to obtain adults. Richness, abundance and sex ratio were calculated. RESULTS: A total of 9 taxa of Diptera were identified: Sarconesia chlorogaster, Phaenicia sericata, Calliphora vicina, Cochliomya macellaria, Ophyra sp, Muscina stabulans, Musca domestica, Sarcophaga sp and Fannia sp. Specimens of Anthomyiidae, Acaliptratae and one larva of Eristalis tenax were also found. Ophyra sp. was the most abundant taxa. All the captured Diptera belonged to introduced taxa. Most of them are considered to be eusynanthropic and/or hemisynanthropic and have sanitary importance as they may cause myiasis and pseudomyiasis. The high number of females registered and the finding of immature stages indicated that flies can develop their complete life cycle on biosolid composting windrows. CONCLUSIONS: The characterization of flies obtained in this study may be useful for defining locations of urban or semi-urban composting facilities. It also highlights the importance of sanitary precautions at such plants.

OBJECTIVE: To assess factors associated with infant feeding practices on the first day at home after hospital discharge. METHODS: A total of 209 women, who had a child aged four months or less and were living in Itapira, Brazil, were interviewed during the National Immunization Campaign Day in 1999. Statistical analysis was performed using the Chi-square test and a logistic regression model was used for verifying an association between dependent and independent variables. RESULTS: Women aged 25.5 years on average and 18.2% were teenagers. Fifty-three percent of the women delivered vaginally and most vaginal deliveries (78.5%) took place in the public hospital. The prevalence of exclusive breastfeeding on the first day at home was 78.1% and 11.6% of the infants were receiving formula at this time. The only factor associated with EBF on the first day at home was being a teenaged-primiparous mother (OR=9.40; 95% CI: 1.24-71.27). This association remained statistically significant even after controlling for type of delivery and hospital where the birth took place. Feeding formula on the first day at home was only significantly associated with the hospital (i.e., birth at the city hospital was a protective factor (OR=0.33; 95% CI: 0.13-0.86), even after controlling for vaginal delivery. CONCLUSIONS: On the first day at home after hospital discharge, teenaged-primiparous mothers were more likely to exclusive breastfeeding as well as those infants born in the municipal public hospital. Further studies are needed from a multidisciplinary approach.
