Recent studies have shown differences in the epidemiology of invasive infections caused by Candida species worldwide. In the period comprising August 2002 to August 2003, we performed a study in Santa Casa Complexo Hospitalar, Brazil, to determine Candida species distribution associated with candidemia and their antifungal susceptibility profiles to amphotericin B, fluconazole and itraconazole. Antifungal susceptibility was tested according to the broth microdilution method described in the NCCLS (M27A-2 method). Only one sample from each patient was analyzed (the first isolate). Most of the episodes had been caused by species other than C. albicans (51.6%), including C. parapsilosis (25.8%), C. tropicalis (13.3%), C. glabrata (3.3%), C. krusei (1.7%), and others (7.5%). Dose-dependent susceptibility to itraconazole was observed in 14.2% of strains, and dose-dependent susceptibility to fluconazole was found in 1.6%. Antifungal resistance was not found, probably related to low use of fluconazole. Further epidemiological surveillance is needed.

Candida guilliermondii is one of the components of human microbiota. This yeast has been infrequently associated with human infections, which may be related to its low pathogenicity. The aim of this study was to provide clinical and epidemiological data for patients infected with C. guilliermondii at Santa Casa Complexo Hospitalar, Brazil. From October 1997 to October 2003, C. guilliermondii was isolated from clinical samples from 11 patients. Three patients were excluded because the isolation of the yeast represented colonisation. Specimens from the eight patients included in the study corresponded to blood (n = 5), ascitis fluid (n = 2), and oesophagus biopsy (n = 1). Three patients (37.5%) had major immunosuppressed conditions, including solid organ transplantation, AIDS, and leukaemia. Previous use of antibiotics occurred in 87.5%. Main invasive medical procedures were central venous catheter (50.0%), abdominal surgery (25.0%), and peritoneal dialysis (50.0%). No susceptibility data was obtained. Although risk factors for candidaemia were similar amongst patients infected by with C. guilliermondii or other Candida species, mortality associated with C. guilliermondii was significantly lower.

Toxoplasmosis is considered one of the opportunistic infections for individuals with the Acquired Immunodeficiency Syndrome (AIDS), and is also a major cause of morbidity and mortality. The aim of this study was to evaluate the prevalence of neurotoxoplasmosis, ocular toxoplasmosis and antibodies for Toxoplasma gondii in HIV-positive patients attending the SAE (Specialized Assistance Service for HIV/AIDS), as well as to associate their serological profile with epidemiological and clinical data. A total of 250 patients participated in the study from December, 2009 to November, 2010. Serological analysis was performed using the indirect immunofluorescent technique; epidemiological data were gathered by a questionnaire, and clinical history was based on the analysis of medical charts. Prevalence of seropositivity was 80%, with history of neurotoxoplasmosis in 4.8% and of ocular toxoplasmosis in 1.6% of the patients. The Highly Active Antiretroviral Treatment (HAART) was not used by 32% of the patients, 18.4% of the patients had CD4+ T- lymphocyte count less than 200 cells/mm³ and 96.8% of them were not aware of the modes of disease transmission. These findings led us to conclude that the study population is at high risk of clinical toxoplasmosis, because of both reactivation of infection in the seropositive patients who do not make a regular use of HAART, and primo-infection in seronegative patients worsened by an unawareness of the modes of infection reported in this study.

O trabalho apresenta um estudo retrospectivo de setenta e sete casos de latrodectismo no Estado da Bahia, Brasil, de agosto de 1980 a julho de 1990. Os dados foram levantados nos livros de registro e arquivo de fichas do CIAVE. O agente etiológico em 28% dos acidentes aracnídeos foi a espécie L. curacaviensis e a maior incidência foi registrada no meio urbano (57%), em indivíduos do sexo masculino (70%) e faixa etária de 10 a 29 anos (58%). Os principais sinais locais foram dor (56%), pápula eritematosa (21%) e edema discreto (17%), e os sistêmicos foram dor em membros inferiores (29%), tremores e contraturas (29%), sudorese (28%) parestesia em membros (21%) e dor abdominal (17%). O tratamento foi sintomático em 67% dos casos e específico em 21%. O tempo de permanência hospitalar após o uso do soro antilatrodectus foi menor que 24 horas em 64% dos casos.

Neste estudo investigou-se o potencial imunomodulador do levamisole e da mistura BCG/Mycobacterium leprae em pacientes virchovianos inativos, utilizando como parâmetro a reação de Mitsuda. Vinte pacientes, classificados como Mitsuda histologicamente negativos há 10 anos, foram divididos em três grupos: cinco pacientes que foram somente reavaliados frente a mitsudina: oito pacientes que receberam levamisole e, sete que receberam a mistura de BCG vivo mais M. leprae morto. Os resultados mostraram que: 1) o levamisole não alterou a reatividade à mitsudina em nenhum dos casos estudados; 2) as modificações da reatividade verificadas com o uso da mistura (tres casos) ou aquelas que ocorreram espontaneamente (tres casos) foram sempre de pequena amplitude e refletiram variações próprias de pacientes com algum grau de resistência ao Mycobacterium leprae.

Charcoal is an important energy raw material and its properties are influenced by the wood's anatomical and chemical composition and the production process. The aim of this study was to evaluate the anatomical characteristics, calorific power and volatiles and ash content of carbonized wood from Byrsonima spicata, Calophyllum brasiliense, Cecropia sciadophylla, Cochlospermum orinocense and Schefflera morototoni. The calorific power varied from 26,878 to 31,117 kJ kg-1; the content of volatile materials ranged from 20.9 to 31.7%; ash content ranged from 0.1 to 3.8%; and carbon content varied from 68.2 to 75.3%. Anatomical structures of charcoal can be used for species identification. The studied species are not indicated for charcoal production because the levels of ash and volatile compounds are higher than those recommended for charcoal produced for household use. In addition, the calorific power and level of carbon content are insufficient for use in the steel industry.

We describe here two patients with angiographic diagnosis of intrastent restenosis and regional myocardial ischemia. One stent restenosis was located in a native coronary artery and the other in a vein graft. Both were treated with cutting balloon angioplasty (CBA), inflated at low pressures. Angiographic success was obtained and both patients were discharged in the day after the procedure. Cutting balloon angioplasty using low inflation pressures achieved important luminal gains, in these two cases of intrastent restenosis. Further studies are necessary before the effectiveness of this procedure can be precisely defined.

OBJECTIVE: To assess the effect of a new feed soy product fermented by Enterococcus faecium and Lactobacillus jugurti on the serum lipid levels of rabbits with induced hypercholesterolemia. METHODS: Thirty-two rabbits were divided into 4 groups as follows: 1) control (C); 2) hypercholesterolemic (H); 3) hypercholesterolemic + fermented product (HPF); and 4) control + fermented product (CPF). The H and HPF groups were fed with a diet with 0.15% (p/p) cholesterol in the first 15 days. C and CPF groups received regular food preparation. The HPF and CPF groups received 10 mL daily of the fermented 30 days. Blood samples were drawn at the beginning of the study and at the 15th and 30th days. Concentrations of total cholesterol, HDL-cholesterol, and triglycerides were analyzed. RESULTS: After 15 days, the HPF group showed a total cholesterol concentration lower (18.4%) than that of the H group (p=0.05), but this difference disappeared after 30 days. No change was observed in total cholesterol levels of C and CPF groups. After 15 days, the HDL-cholesterol was higher (17.8%) in the HPF group, but the triglyceride levels remained unchanged in all groups during the same period of time. CONCLUSION: The soy fermented product caused an 18.4% reduction in total cholesterol and a 17.8% increase in the HDL-fraction. It may, therefore, be a possible coadjutor in the treatment of hypercholesterolemia.

FUNDAMENTO: A doença cardiovascular aterosclerótica inicia seu processo na infância precoce e é influenciada ao longo da vida por fatores genéticos e exposição ambiental a fatores de risco potencialmente modificáveis. OBJETIVO: Investigar a prevalência de fatores de risco para aterosclerose com ênfase nos hábitos alimentares em uma cidade de colonização predominantemente italiana. MÉTODOS: Estudo transversal de base populacional, envolvendo 590 estudantes do ensino fundamental com idades entre 9 e 18 anos, com amostra por conglomerado. Foram coletados: dados de identificação, história familiar e história pregressa, além das informações referentes à alimentação dos estudantes. Os hábitos alimentares considerados inadequados incluíram: consumo de fast food, guloseimas, bebidas açucaradas e gorduras de origem animal por quatro ou mais vezes por semana e frutas, hortaliças e leguminosas por menos de quatro vezes por semana. RESULTADOS: A prevalência de excesso de peso entre os estudantes foi 24,6% (n=145); pressão arterial elevada, 11,1% (n=65); tabagismo passivo, 35,4% (n=208); estilo de vida sedentário, 52,3% (n=306); história familiar doenças 1º grau: hipertensão arterial sistêmica, 21,4% e obesidade, 36,5%. Alimentos consumidos por quatro ou mais vezes por semana: fast food, 70,3% (n=411); guloseimas, 42,7% (n=252); bebidas açucaradas, 71% (n=419); e gorduras de origem animal, 24,4% (n=143). Alimentos consumidos por menos de quatro vezes por semana: frutas, 36,8% (n=215); hortaliças, 49,5% (n= 292) e leguminosas, 63,7% (n=374). CONCLUSÃO: São necessárias intervenções que promovam mudanças nos hábitos alimentares dos estudantes: maior consumo de frutas, hortaliças e leguminosas e aumento do nível de atividade física.

Background: Cardiovascular diseases are the current leading causes of death and disability globally. Objective: To assess the effects of a basic educational program for cardiovascular prevention in an unselected outpatient population. Methods: All participants received an educational program to change to a healthy lifestyle. Assessments were conducted at study enrollment and during follow-up. Symptoms, habits, ATP III parameters for metabolic syndrome, and American Heart Association’s 2020 parameters of cardiovascular health were assessed. Results: A total of 15,073 participants aged ≥ 18 years entered the study. Data analysis was conducted in 3,009 patients who completed a second assessment. An improvement in weight (from 76.6 ± 15.3 to 76.4 ± 15.3 kg, p = 0.002), dyspnea on exertion NYHA grade II (from 23.4% to 21.0%) and grade III (from 15.8% to 14.0%) and a decrease in the proportion of current active smokers (from 3.6% to 2.9%, p = 0.002) could be documented. The proportion of patients with levels of triglycerides > 150 mg/dL (from 46.3% to 42.4%, p < 0.001) and LDL cholesterol > 100 mg/dL (from 69.3% to 65.5%, p < 0.001) improved. A ≥ 20% improvement of AHA 2020 metrics at the level graded as poor was found for smoking (-21.1%), diet (-29.8%), and cholesterol level (-23.6%). A large dropout as a surrogate indicator for low patient adherence was documented throughout the first 5 visits, 80% between the first and second assessments, 55.6% between the second and third assessments, 43.6% between the third and fourth assessments, and 38% between the fourth and fifth assessments. Conclusion: A simple, basic educational program may improve symptoms and modifiable cardiovascular risk factors, but shows low patient adherence.

Background:Previous reports have inferred a linear relationship between LDL-C and changes in coronary plaque volume (CPV) measured by intravascular ultrasound. However, these publications included a small number of studies and did not explore other lipid markers.Objective:To assess the association between changes in lipid markers and regression of CPV using published data.Methods:We collected data from the control, placebo and intervention arms in studies that compared the effect of lipidlowering treatments on CPV, and from the placebo and control arms in studies that tested drugs that did not affect lipids. Baseline and final measurements of plaque volume, expressed in mm3, were extracted and the percentage changes after the interventions were calculated. Performing three linear regression analyses, we assessed the relationship between percentage and absolute changes in lipid markers and percentage variations in CPV.Results:Twenty-seven studies were selected. Correlations between percentage changes in LDL-C, non-HDL-C, and apolipoprotein B (ApoB) and percentage changes in CPV were moderate (r = 0.48, r = 0.47, and r = 0.44, respectively). Correlations between absolute differences in LDL-C, non‑HDL-C, and ApoB with percentage differences in CPV were stronger (r = 0.57, r = 0.52, and r = 0.79). The linear regression model showed a statistically significant association between a reduction in lipid markers and regression of plaque volume.Conclusion:A significant association between changes in different atherogenic particles and regression of CPV was observed. The absolute reduction in ApoB showed the strongest correlation with coronary plaque regression.

La más compleja de las interacciones que plantas e insectos han desarrollado durante el transcurso de su evolución, son las agallas. Las especies de insectos galícolas se encuentran en la mayoría de las regiones biogeográficas, principalmente en ambientes xéricos, de los cuales un ejemplo lo constituye la ecorregión del Espinal, ubicada en la Provincia Biogeográfica de la Pampa, Subregión Chaqueña. Schinus fasciculata (Griseb.) I.M. Johnst. (Anacardiaceae) es una especie arbórea o arbustiva representativa de la ecorregión del Espinal que presenta diversas agallas entomógenas. Los objetivos del presente trabajo son identificar las especies de insectos que producen agallas en hojas y tallos de Schinus fasciculata en un relicto de Espinal de la provincia de Córdoba y caracterizar exomorfológicamente las agallas. Se seleccionaron 18 ejemplares de S. fasciculata distribuidos en cuatro transectas de 100 m2. Se caracterizaron cinco morfotipos de agallas, tres en hojas, inducidas por insectos del orden Hemiptera y dos en tallos, originadas por insectos del orden Lepidoptera. Los insectos productores de las mismas fueron identificados a nivel de especie y los distintos morfotipos de agallas fueron únicos para cada especie de insecto inductor.

Con el objeto de aumentar la sensibilidad del diagnóstico histopatológico de lesiones cutáneas y mucocutáneas causada por subespecies del complejo Leismania braziliensis y para lograr una mejor visualización de los parásitos en las lesiones, se evaluó el método de la inmunoperoxidas indirecta para localizar en forma rápida y específica los amastigotas en biopsia de tejido afectado. Los cortes de tejido se fijaron en formol y se incluyeron en parafina; después se evaluaron por inmunohistoquímica usando un antisuero policlonal producido en conejo, como reactivo primario, Se examinaron 265 biopsias de pacientes con lesiones sospechosas de leishmaniasis de la costa Pacífica y región suroriental colombiana. a 1983 (72.8%) pacientes se les estableció el diagnóstico por métodos clínicos y/o poarasitológicos. Los resultados obtenidos por la inmunoperoxidasa en el grupo de pacientes a los cuales se les confirmó la leishmaniasis se compararn con la histopatología convencional, el examen directo de frotis y el aislamiento del parásito por cultivo del aspirado de la lesión. La localización inmunoenzimática de las amastigotas fue más efectivas (61.3%) que la histopatología com hematoxilina y eosina (34.6%), y que el frotis (43,9%). En cambio, el cultivo de aspirado fue más sensible (89.8%). La eficiencia del método de inmunoperoxidasa fue mayor en las lesiones recientes (72.5%) positivos en los casos con menos de tres meses de evolución) que en las lesiones más antiguas (55.6, 37.5 y 21.1% para 3-5.9, 6-11 meses y mayores o iguales a 12 mese, respectivamente). La combinación de frotis e inmunoperoxidasa incrementó el porcentaje de caso diagnosticados a 72.0%, lo que indica la importancia de combinar métodos para obtener una mayor eficiencia de diagnóstico. La especificidad fue de 100% en controles sanos y 92.9% en pacientes con lesiones causadas por agentes etiológicos distintos a leishmania.

Neohilgertia gen. n. proposed for Oxyuridae nematodes from Thylamys venustus cinderellus (Marsupialia: Didelphidae) is described. The hypothesis about the possibility of a secondary parasitism for marsupials and the origin of the genus in the African Sciuridae parasite ancestors is discussed.

Pterigodermatites (P.) spinicaudatis sp.n. from Dromiciops australis is proposed and described. The simple morphology of the ovijector and the presence of a well developed spine between the two cuticular projections at the caudal extremity of the female distinguish the studied nematode from the remainder species of the genus parasitizing South American Edentata, marsupials and cricetid rodents. The distribution area of the hosts of the different species of P. (P.) are given. The studied genus does not parasitize any Australian marsupials. It was found in the endemic South American Microbiotheriidae. This fact suggests from a parasitological point of view that D. australis is not related to the Australian marsupials but to the South American ones.
