OBJETIVO: Analisar fatores associados à sobrevida no primeiro ano de vida. MÉTODOS: Estudo de coorte histórica foi realizado com dados dos sistemas de informação de nascimento e mortalidade sobre 90.153 registros de nascidos vivos e 1.053 registros de óbitos de menores de um ano em hospitais de Florianópolis e São José, SC, entre 1999 e 2006. Foram estimadas curvas de sobrevida (Kaplan-Meier) para grupos de peso ao nascer, período (quadriênios) e tipo de maternidade. Foram calculadas razões de riscos proporcionais para óbitos utilizando regressão de Cox. RESULTADOS: A sobrevida (98,8%) não se modificou entre os grupos de peso, mas aumentou nos grupos de menos de 2.000 g (77,7% para 81,2%, p = 0,029) entre os quadriênios de 1999 a 2002 e 2003 a 2006. Houve aumento de menores de 2.000 g no segundo quadriênio estudado. O tipo de hospital foi associado significativamente à probabilidade de sobrevida. CONCLUSÕES: Há maior probabilidade de sobrevida entre nascidos em hospitais privados e no hospital de ensino para todos os grupos de peso e para o grupo de menos de 2000 g. A sobrevida dos grupos de peso abaixo de 2000 g aumentou no quadriênio mais recente. Entretanto, o coeficiente de mortalidade infantil não diminuiu nesse período, pois a prevalência dos nascidos em grupos de menor peso também aumentou.

OBJECTIVE To analyze the methodology used for assessing the spatial distribution of specialized cardiac care units. METHODS A modeling and simulation method was adopted for the practical application of cardiac care service in the state of Santa Catarina, Southern Brazil, using the p-median model. As the state is divided into 21 health care regions, a methodology which suggests an arrangement of eight intermediate cardiac care units was analyzed, comparing the results obtained using data from 1996 and 2012. RESULTS Results obtained using data from 2012 indicated significant changes in the state, particularly in relation to the increased population density in the coastal regions. The current study provided a satisfactory response, indicated by the homogeneity of the results regarding the location of the intermediate cardiac care units and their respective regional administrations, thereby decreasing the average distance traveled by users to health care units, located in higher population density areas. The validity of the model was corroborated through the analysis of the allocation of the median vertices proposed in 1996 and 2012. CONCLUSIONS The current spatial distribution of specialized cardiac care units is more homogeneous and reflects the demographic changes that have occurred in the state over the last 17 years. The comparison between the two simulations and the current configuration showed the validity of the proposed model as an aid in decision making for system expansion.

The clinical and laboratory data of a disease in a resident of Ribeira Valley, São Paulo State, southeastern Brazil, caused by an agent close or identical to Caraparu, a Group C arbovirus, was described. Although there is evidence of an intensive circulation of several arboviruses in the area, no diagnosis of human disease by these agents has been made, except the encephalitis cases caused by Rocio virus during an epidemic in 1975-1977. An antigenic difference between Caraparu strains isolated in São Paulo and in Pará States and a close antigenic relationship between Caraparu strain from São Paulo and Bruconha virus were suggested by the serological tests.

The presence of IgM antibodies to Rocio in sera of two children from rural area of Ribeira Valley, Brazil, was detected by MAC-ELISA. This new arbovirus of the Flaviviridae family was responsible for an extensive encephalitis epidemic that occurred in the region in 1975-1977. Since 1980 no human disease caused by this virus has been diagnosed. An improvement on surveillance of Rocio infections and on the researches for virus identification in suspected vectors and reservoirs is necessary.

Moluscos de concha bastante semelhante à de Oncomelania nosophora, transmissora do Schistosoma japonicum, foram coletados nos municípios de Pedro Leopoldo e Lagoa Santa, MG, Brasil. Estes moluscos foram identificados, através dos caracteres conquiliológicos e anatômicos da parte mole, como Idiopyrgus souleyetianus. É também fornecida a relação dos outros hidrobioides já identificados no Brasil.

Sera from 299 fishermen 16 to 80 years old, residents in Cananeia and Iguape counties, southern cost of São Paulo State, Brazil, were studied in order to identify a possible association between the prevalence of specific antibodies to the hepatitis B virus (HBV) and exposure to haematophagus mosquitoes evaluated by the prevalence of arbovirus antibodies. This professional group presented the highest prevalence of arbovirus antibodies (54.1%) in past investigations carried out in this heavily forested region. Detection of antibody to hepatitis B core antigen (anti-HBc) in the sera was done by enzyme immunoassay (Roche). Prevalence of anti-HBc antibodies in this group was 31.4% (94/299) which is very high compared with 7.2% to 15.0% for different groups of healthy adults in State of São Paulo. No significant difference is observed between the prevalences of HBV antibodies in Iguape and Cananeia. Prevalence of anti-HBc and anti-arbovirus antibodies increases with age. There is a concordance in the distribution according to age groups of the frequency of anti-HBc and anti-arbovirus positive sera. Ag HBs was detected in 4% of the studied sera. These results support the hypothesis that the transmission of the hepatitis B virus and the arboviruses may be due to the same factor, one of the possibilities would be by anthropophilic mosquitoes.

The first register of Biomphalaria straminea, vector species of schistosomiasis, in Santa Catarina State, South Brazil is described. The specimens were obtained from two ornamental aquaria of private residences. In both cases the ornamental plants and/or fishes were bought from the same supplier. The presence of this species was later confirmed in the farm where these plants and fishes are cultivated, in the city of Governador Celso Ramos, also in Santa Catarina State. The occurrence in natural environments was later detected in two different places of the Island of Santa Catarina.

In the São Paulo State, Brazil, where the Biomphalaria tenagophila is the intermediate host, the Ribeira Valley is an important endemic schistosomiasis mansoni area. During last eleven years there has been intense control measures focusing on schistosomiasis. The efforts have been concentrated in the municipalities of Pedro de Toledo and Itariri. We determined the susceptibility of B. tenagophila to sympatric strain of S. mansoni, both recently isolated from Itariri field. In 1988, this strain was isolated and maintained in the experimental model: Swiss mice - sympatric B. tenagophila. The second generation of the worm was evaluated. The snail were divided in the three groups of 60 snails each. One group was exposed to 1 miracidium and other to 10. The third group was the control. The mortality and the shedding of cercariae were checked during 78 days. After that, the positive snails were observed until they ceased to shed cercariae. The exposed molluscs showed mortality rates of 23% and 31% and infection indexes were of 8% and 60% to 1 and 10 miracidia respectively. The mortality was of 22% in the control group. The periods of shedding cercariae in the two groups were 82 and 104 days. We can conclude that B. tenagophila is an effective intermediate host to the sympatric strain of S. mansoni sympatric strain

Studies on eight localities on the Island of Santa Catarina revealed the presence of three species of the molluscan family Planorbidae: Biomphalaria tenagophila, Drepanotrema cimex and Biomphalaria oligoza, the first one being naturally infected by Cercaria ocellifera, a furcocercaria with morphological characteristics of Cercaria caratinguensis, and by an unknown furcocercaria. Drepanotrema cimex was infected by a furcocercaria with characteristics of C. caratinguensis and by C. macrogranulosa. No natural infection was found in B. oligoza. B. tenagophila showed no susceptibility to the experimental infection by the BH-MG strain of Schistosoma mansoni from Belo Horizonte and maintained at laboratory in B. glabrata snails.

Objetivando verificar a colonização de Panstrongylus megistus em ecótopos artificiais em Florianópolis foram examinados, de 1985 a 1992, 779 anexos peridomiciliares (524 galinheiros, 46 currais e 209 ranchos) em 9 localidades e 443 domicílios no distrito de Lagoa, todos na Ilha de Santa Catarina. Todo o ecótopo, incluindo forro e porão das casas, era examinado após aplicação de líquido insentífugo (Pirisa a 5%). A pesquisa nos anexos peri-domiciliares revelou 3 galinheiros e um rancho positivo no distrito de Lagoa, onde foram também encontrados 2 domicílios colonizados pelo P. megistus, com a captura de ovos, ninfas e adultos em todos os ecótopos. Pesquisas dirigidas foram realizadas em dois outros domicílios e em uma escola, nos quais os moradores haviam detectado anteriormente exemplares de P. megistus e, em todos os 3, foi confirmada a colonização pelo triatomíneo. Nos 9 ecótopos artificiais foram coletados 559 ovos, 305 ninfas e 24 adultos de P. megistus, com um índice de infecção pelo T. cruzi de 53,3% (182/329). Índice de infecção semelhante, de 56,5% (78/ 138), foi também encontrado nos adultos de P. megistus oriundos dos ecótopos silvestres e capturados nos domicílios pelos moradores, no período de 1983 a 1991. Os testes de precipitina revelaram, em 94,0% dos insetos examinados (170/181), sangue de uma única fonte alimentar e presença de sangue humano em 80,6% (25/31) dos adultos e em 5,8% (1/17) das ninfas capturados nos domicílios. Os resultados encontrados sugerem a necessidade de adoção de medidas de vigilância epidemiológica com a participação da comunidade, face o risco potencial de domiciliação do P. megistus.

In order to learn the prevalence of Chagas' infection among students from Santa Cruz de la Sierra's universities, a random sample of 372 new students was drawn. All participants have had electrocardiograms (EKG) and serologic analysis (IHAT). 64/372 (17.2%) had serologic evidence of Chagas' infection, and from those, 10/64 (15.6%) had some EKG alterations. Among students presenting negative serologic test, 31/308 (10.1%) had EKG alterations. There was no statistical association between Chagas' infection and EKG alterations (X2=1.67, p=0.2). There was a positive association between Chagas' infection and intraventricular conduction defects and this association was higher among the students of 19 years of age or less (O.R. 10.4, p<0.05).

Dermatophytoses comprise mycoses which are very frequently diagnosed in the routine of clinical laboratories of Florianópolis, like any other Brazilian cities. However, no clinical or epidemiological studies data have been published for that city so far. To partially clarify these questions, we carried out a study on this subject on patients who sought the mycology services of Hospital of Federal University of Santa Catarina, from January 1995 to November 1996. The most prevalent dermatophyte was Trichophyton rubrum (58.6%), followed by T. mentagrophytes (25.3%), Epidermophyton floccosum (7.2%), Microsporum canis (4.8%), T. tonsurans (1.6%) T. violaceum (1.6%) and M. gypseum (0.8%). The prevalence of T. mentagrophytes was significantly higher for females than for males, with a frequency of 37.3% and 16.0% respectively, which could be explained by higher infection of T. mentagrophytes in feet and nails, which were percentually more affected in females than in males. These results suggest that, in general, the clinical and epidemiological characteristics of dermatophytoses of our study have similar patterns of those occurring in other southern and southeastern Brazilian cities

Direct blood examination and xenodiagnosis of 47 synanthropic rodents (Rattus rattus, R. norvegicus, Mus musculus) captured in the valley of Caracas, Venezuela, revealed trypanosomal infections in 12 R. rattus, 10 with T. lewisi and 2 with T. cruzi. Of the latter the course of parasitemia, the pleomorphism of the bloodstream trypomastigotes, tissue tropism in naturally and experimentally infected rats and mice, host mortality, morphology of fecal parasites in Rhodnius prolixus used for xenodiagnosis, and infectivity of the bug feces for NMRI mice, were all characteristic of Trypanosoma (Schizotrypanum) cruzi. One rat, with a patent parasitemia, had numerous nests of amastigotes in cardiac muscle and moderate parasitism of the smooth muscle of the duodenum and of skeletal muscle. Mice inoculated with fecal flagellates from the bugs had moderate tissue tropism in the same organs and also in the colon and pancreas. The possible role of R. rattus in the establishment of foci of Chagas’ disease in Caracas is discussed

Due to the lack of studies about neurocysticercosis in the South of Brazil, an investigation was conducted to determine the percentage of suspected cases of neurocysticercosis in computed tomography diagnoses in Santa Maria, RS, from January 1997 to December 1998. Of 6300 computed tomographies (CT) of the skull performed at the private Hospital de Caridade Astrogildo de Azevedo, 80, i.e., 1.27% were suspected of neurocysticercosis. Fifty were women (62.5%) and 30 were men (37.5%). The most frequent radiological manifestation indicating neurocysticercosis was the presence of calcifications (isolated or associated), with a 95% rate (76 cases), while the presence of hypodense lesions reached a 5% rate (4 cases). After routine analysis, each CT was evaluated again and the suspected cases were confirmed. The percentage of suspected cases of neurocysticercosis detected by CT in the present study carried out in Santa Maria was considered low (1.27%). This can be explained by the fact that tomography is not accessible to the economically underprivileged population of Santa Maria. We hope that the present study can alert the population and the professionals to the fact that neurocysticercosis is a more frequent disease than indicated by the few diagnoses made.
