Certain host single nucleotide polymorphisms (SNPs) affect the likelihood of a sustained virological response (SVR) to treatment in subjects infected with hepatitis C virus (HCV). SNPs in the promoters of interleukin (IL)-10 (-1082 A/G, rs1800896), myxovirus resistance protein 1 (-123 C/A, rs17000900 and -88 G/T, rs2071430) and tumour necrosis factor (TNF) (-308 G/A, rs1800629 and -238 G/A, rs361525) genes and the outcome of PEGylated α-interferon plus ribavirin therapy were investigated. This analysis was performed in 114 Brazilian, HCV genotype 1-infected patients who had a SVR and in 85 non-responders and 64 relapsers. A significantly increased risk of having a null virological response was observed in patients carrying at least one A allele at positions -308 [odds ratios (OR) = 2.58, 95% confidence intervals (CI) = 1.44-4.63, p = 0.001] or -238 (OR = 7.33, 95% CI = 3.59-14.93, p < 0.001) in the TNF promoter. The risk of relapsing was also elevated (-308: OR = 2.87, 95% CI = 1.51-5.44, p = 0.001; -238: OR = 4.20, 95% CI = 1.93-9.10, p < 0.001). Multiple logistic regression of TNF diplotypes showed that patients with at least two copies of the A allele had an even higher risk of having a null virological response (OR = 16.43, 95% CI = 5.70-47.34, p < 0.001) or relapsing (OR = 6.71, 95% CI = 2.18-20.66, p = 0.001). No statistically significant association was found between the other SNPs under study and anti-HCV therapy response.

Induction of apoptosis by tumor necrosis factor (TNF) is modulated by changes in the expression and activity of several cell cycle regulatory proteins. We examined the effects of TNF (1-100 ng/ml) and butyrolactone I (100 µM), a specific inhibitor of cyclin-dependent kinases (CDK) with high selectivity for CDK-1 and CDK-2, on three different cancer cell lines: WEHI, L929 and HeLa S3. Both compounds blocked cell growth, but only TNF induced the common events of apoptosis, i.e., chromatin condensation and ladder pattern of DNA fragmentation in these cell lines. The TNF-induced apoptosis events were increased in the presence of butyrolactone. In vitro phosphorylation assays for exogenous histone H1 and endogenous retinoblastoma protein (pRb) in the total cell lysates showed that treatment with both TNF and butyrolactone inhibited the histone H1 kinase (WEHI, L929 and HeLa) and pRb kinase (WEHI) activities of CDKs, as compared with the controls. The role of proteases in the TNF and butyrolactone-induced apoptosis was evaluated by comparing the number and expression of polypeptides in the cell lysates by gel electrophoresis. TNF and butyrolactone treatment caused the disappearance of several cellular protein bands in the region between 40-200 kDa, and the 110- 90- and 50-kDa proteins were identified as the major substrates, whose degradation was remarkably increased by the treatments. Interestingly, the loss of several cellular protein bands was associated with the marked accumulation of two proteins apparently of 60 and 70 kDa, which may be cleavage products of one or more proteins. These findings link the decrease of cyclin-dependent kinase activities to the increase of protease activities within the growth arrest and apoptosis pathways induced by TNF.

We demonstrated that 4 mM butyrate induces apoptosis in murine peritoneal macrophages in a dose- and time-dependent manner as indicated by studies of cell viability, flow cytometric analysis of annexin-V binding, DNA ladder pattern and the determination of hypodiploid DNA content. The activity of caspase-3 was enhanced during macrophage apoptosis induced by butyrate and the caspase inhibitor z-VAD-FMK (100 µM) inhibited the butyrate effect, indicating the major role of the caspase cascade in the process. The levels of butyrate-induced apoptosis in macrophages were enhanced by co-treatment with 1 µg/ml bacterial lipopolysaccharide (LPS). However, our data indicate that apoptosis induced by butyrate and LPS involves different mechanisms. Thus, LPS-induced apoptosis was only observed when macrophages were primed with IFN-gamma and was partially dependent on iNOS, TNFR1 and IRF-1 functions as determined in experiments employing macrophages from various knockout mice. In contrast, butyrate-induced macrophage apoptosis was highly independent of IFN-gamma priming and of iNOS, TNFR1 and IRF-1 functions.

Tumor necrosis factor-alpha (TNF-alpha) is one of the most important proinflammatory cytokines which plays a central role in host defense and in the acute inflammatory response related to tissue injury. The major source of TNF-alpha are immune cells such as neutrophils and macrophages. We tested the hypothesis that pentoxifylline, a methylxanthine derivative, down-regulates proinflammatory cytokine expression during acute lung injury in rats. Male Wistar rats weighing 250 to 450 g were anesthetized ip with 50 mg/kg sodium thiopental and randomly divided into three groups: group 1 (N = 7): tidal volume (V T) = 7 ml/kg, respiratory rate (RR) = 50 breaths/min and normal saline infusion; group 2 (N = 7): V T = 42 ml/kg, RR = 9 breaths/min and normal saline infusion; group 3 (N = 7): V T = 42 ml/kg, RR = 9 breaths/min and pentoxifylline infusion. The animals were ventilated with an inspired oxygen fraction of 1.0, a positive end-expiratory pressure of 3 cmH2O, and normal saline or pentoxifylline injected into the left femoral vein. The mRNA of TNF-alpha rapidly increased in the lung tissue within 180 min of ventilation with a higher V T with normal saline infusion. The concentrations of inflammatory mediators were decreased in plasma and bronchoalveolar lavage (BAL) in the presence of higher V T with pentoxifylline infusion (TNF-alpha: plasma, 102.2 ± 90.9 and BAL, 118.2 ± 82.1; IL-1ß: plasma, 45.2 ± 42.7 and BAL, 50.2 ± 34.9, P < 0.05). We conclude that TNF-alpha produced by neutrophil influx may function as an alert signal in host defense to induce production of other inflammatory mediators.

Because low tumor necrosis factor-alpha (TNF-alpha) production has been reported in malnourished children, in contrast with high production of TNF-alpha in experimental protein-energy malnutrition, we reevaluated the production of TNF-alpha in whole blood cultures from children with primary malnutrition free from infection, and in healthy sex- and age-matched controls. Mononuclear cells in blood diluted 1:5 in endotoxin-free medium released TNF-alpha for 24 h. Spontaneously released TNF-alpha levels (mean ± SEM), as measured by enzyme immunoassay in the supernatants of unstimulated 24-h cultures, were 10,941 ± 2,591 pg/ml in children with malnutrition (N = 11) and 533 ± 267 pg/ml in controls (N = 18) (P < 0.0001). TNF-alpha production was increased by stimulation with lipopolysaccharide (LPS), with maximal production of 67,341 ± 16,580 pg/ml TNF-alpha in malnourished children and 25,198 ± 2,493 pg/ml in controls (P = 0.002). In control subjects, LPS dose-dependently induced TNF-alpha production, with maximal responses obtained at 2000 ng/ml. In contrast, malnourished patients produced significantly more TNF-alpha with 0.02-200 ng/ml LPS, responded maximally at a 10-fold lower LPS concentration (200 ng/ml), and presented high-dose inhibition at 2000 ng/ml. TNF-alpha production a) was significantly influenced by LPS concentration in control subjects, but not in malnourished children, who responded strongly to very low LPS concentrations, and b) presented a significant, negative correlation (r = -0.703, P = 0.023) between spontaneous release and the LPS concentration that elicited maximal responses in malnourished patients. These findings indicate that malnourished children are not deficient in TNF-alpha production, and suggest that their cells are primed for increased TNF-alpha production.

Gastrointestinal motility disturbances during endotoxemia are probably caused by lipopolysaccharide (LPS)-induced factors: candidates include nitric oxide (NO), tumor necrosis factor-alpha (TNF-alpha), interleukin-1ß, and interleukin-6. Flow cytometry was used to determine the effects of LPS and these factors on gastric emptying (evaluated indirectly by determining percent gastric retention; %GR) and gastrointestinal transit (GIT) in male BALB/c mice (23-28 g). NO (300 µg/mouse, N = 8) and TNF-alpha (2 µg/mouse, N = 7) increased (P < 0.01) GR and delayed GIT, mimicking the effect of LPS (50 µg/mouse). During early endotoxemia (1.5 h after LPS), inhibition of inducible NO synthase (iNOS) by a selective inhibitor, 1400 W (150 µg/mouse, N = 11), but not antibody neutralization of TNF-alpha (200 µg/mouse, N = 11), reversed the increase of GR (%GR 78.8 ± 3.3 vs 47.2 ± 7.5%) and the delay of GIT (geometric center 3.7 ± 0.4 vs 5.6 ± 0.2). During late endotoxemia (8 h after LPS), both iNOS inhibition (N = 9) and TNF-alpha neutralization (N = 9) reversed the increase of GR (%GR 33.7 ± 2.0 vs 19.1 ± 2.6% (1400 W) and 20.1 ± 2.0% (anti-TNF-alpha)), but only TNF-alpha neutralization reversed the delay of GIT (geometric center 3.9 ± 0.4 vs 5.9 ± 0.2). These findings suggest that iNOS, but not TNF-alpha, is associated with delayed gastric emptying and GIT during early endotoxemia and that during late endotoxemia, both factors are associated with delayed gastric emptying, but only TNF-alpha is associated with delayed GIT.

Allogeneic stem cell transplantation has been increasingly performed for a variety of hematologic diseases. Clinically significant acute graft-versus-host disease (GVHD) occurs in 9 to 50% of patients who receive allogeneic grafts, resulting in high morbidity and mortality. There is no standard therapy for patients with acute GVHD who do not respond to steroids. Studies have shown a possible benefit of anti-TNF-a (infliximab)for the treatment of acute GVHD. We report here on the outcomes of 10 recipients of related or unrelated stem cell transplants who received 10 mg/kg infliximab, iv, once weekly for a median of 3.5 doses (range: 1-6) for the treatment of severe acute GVHD and who were not responsive to standard therapy. All patients had acute GVHD grades II to IV (II = 2, III = 3, IV = 5). Overall, 9 patients responded and 1 patient had progressive disease. Among the responders, 3 had complete responses and 6 partial responses. All patients with cutaneous or gastrointestinal involvement responded, while only 2 of 6 patients with liver disease showed any response. None of the 10 patients had any kind of immediate toxicity. Four patients died, all of them with sepsis. Six patients are still alive after a median follow-up time of 544 days (92-600) after transplantation. Considering the severity of the cases and the bad prognosis associated with advanced acute GVHD, we find our results encouraging. Anti-TNF-a seems to be a useful agent for the treatment of acute GVHD.

The microenvironment of the tumor plays an important role in facilitating cancer progression and activating dormant cancer cells. Most tumors are infiltrated with inflammatory cells which secrete cytokines such as tumor necrosis factor-a (TNF-a). To evaluate the role of TNF-a in the development of cancer we studied its effects on cell migration with a migration assay. The migrating cell number in TNF-a -treated group is about 2-fold of that of the control group. Accordingly, the expression of E-cadherin was decreased and the expression of vimentin was increased upon TNF-a treatment. These results showed that TNF-a can promote epithelial-mesenchymal transition (EMT) of MCF-7 cells. Further, we found that the expression of Snail, an important transcription factor in EMT, was increased in this process, which is inhibited by the nuclear factor kappa B (NFkB) inhibitor aspirin while not affected by the reactive oxygen species (ROS) scavenger N-acetyl cysteine. Consistently, specific inhibition of NFkB by the mutant IkBa also blocked the TNF-a-induced upregulation of Snail promoter activity. Thus, the activation of NFkB, which causes an increase in the expression of the transcription factor Snail is essential in the TNF-a-induced EMT. ROS caused by TNF-a seemed to play a minor role in the TNF-a-induced EMT of MCF-7 cells, though ROS per se can promote EMT. These findings suggest that different mechanisms might be responsible for TNF-a - and ROS-induced EMT, indicating the need for different strategies for the prevention of tumor metastasis induced by different stimuli.

The objectives of this study were to determine the effect of tumor necrosis factor alpha (TNF-α) on intestinal epithelial cell permeability and the expression of tight junction proteins. Caco-2 cells were plated onto Transwell® microporous filters and treated with TNF-α (10 or 100 ng/mL) for 0, 4, 8, 16, or 24 h. The transepithelial electrical resistance and the mucosal-to-serosal flux rates of the established paracellular marker Lucifer yellow were measured in filter-grown monolayers of Caco-2 intestinal cells. The localization and expression of the tight junction protein occludin were detected by immunofluorescence and Western blot analysis, respectively. SYBR-Green-based real-time PCR was used to measure the expression of occludin mRNA. TNF-α treatment produced concentration- and time-dependent decreases in Caco-2 transepithelial resistance and increases in transepithelial permeability to the paracellular marker Lucifer yellow. Western blot results indicated that TNF-α decreased the expression of phosphorylated occludin in detergent-insoluble fractions but did not affect the expression of non-phosphorylated occludin protein. Real-time RT-PCR data showed that TNF-α did not affect the expression of occludin mRNA. Taken together, our data demonstrate that TNF-α increases Caco-2 monolayer permeability, decreases occludin protein expression and disturbs intercellular junctions.

We aimed to investigate the effects of an anti-tumor necrosis factor-α antibody (ATNF) on cartilage and subchondral bone in a rat model of osteoarthritis. Twenty-four rats were randomly divided into three groups: sham-operated group (n=8); anterior cruciate ligament transection (ACLT)+normal saline (NS) group (n=8); and ACLT+ATNF group (n=8). The rats in the ACLT+ATNF group received subcutaneous injections of ATNF (20 μg/kg) for 12 weeks, while those in the ACLT+NS group received NS at the same dose for 12 weeks. All rats were euthanized at 12 weeks after surgery and specimens from the affected knees were harvested. Hematoxylin and eosin staining, Masson's trichrome staining, and Mankin score assessment were carried out to evaluate the cartilage status and cartilage matrix degradation. Matrix metalloproteinase (MMP)-13 immunohistochemistry was performed to assess the cartilage molecular metabolism. Bone histomorphometry was used to observe the subchondral trabecular microstructure. Compared with the rats in the ACLT+NS group, histological and Mankin score analyses showed that ATNF treatment reduced the severity of the cartilage lesions and led to a lower Mankin score. Immunohistochemical and histomorphometric analyses revealed that ATNF treatment reduced the ACLT-induced destruction of the subchondral trabecular microstructure, and decreased MMP-13 expression. ATNF treatment may delay degradation of the extracellular matrix via a decrease in MMP-13 expression. ATNF treatment probably protects articular cartilage by improving the structure of the subchondral bone and reducing the degradation of the cartilage matrix.

O colesteatoma adquirido da orelha média causa erosão óssea, com altas taxas de morbidade e mortalidade. O TNF-alfa (TNF-alfa) lambda uma das principais citocinas envolvidas neste processo. OBJETIVO: Avaliar o papel do TNF-alfa na reabsorsão óssea e a ação dele no colesteatoma. MATERIAL E MÉTODOS: Foi realizado um levantamento e uma revisão crítica da literatura. RESULTADOS: Todos os autores estudados concordam com a importância do TNF-alfa no processo de reabsorção óssea presente no colesteatoma e com o grau de destruição observado. Diferentes trabalhos demonstraram que o TNF-alfa é capaz de provocar erosão óssea, através de diferentes vias de ação. Ele pode estimular a diferenciação e a maturação dos osteoclastos ou, ainda, agir na matriz óssea expondo-a à ação dos osteoclastos. Existe a possibilidade de inibir a ação do TNF-alfa, diminuindo seus efeitos e prevenindo a perda óssea em doenças como a artrite reumatóide. Não existe, entretanto, trabalhos específicos em colesteatoma. Não existe consenso sobre a sua localização. Estas diferenças, provavelmente, ocorrem devido à distribuição dos receptores. CONCLUSÃO: O TNF-alfa, presente no colesteatoma promove a reabsorsão óssea, juntamente com outras citocinas (RANKL e IL-1), estando relacionado com a presença de complicações.

INTRODUCCIÓN: Considerando la indiscutible evidencia de los efectos adversos del tabaco, para la salud, millones de personas continúan fumando. La situación es de mayor gravedad, ya que muchos de estos fumadores y la mayoría de los nuevos fumadores, tanto en Chile, como en otros países, son adolescentes. Los objetivos de este estudio fueron determinar la prevalencia de tabaquismo en escolares chilenos y cuantificar el impacto relativo que ejercen factores socioeconómicos, socioculturales, familiares, de exposición a medios de comunicación de masas, demográficos, educacionales y psicosociales, sobre la prevalencia de tabaquismo. MATERIAL E MÉTODO: Se seleccionó una muestra aleatoria, representativa de 2.967 escolares de educación básica y media de la Región Metropolitana de Chile, proporcional de acuerdo al curso(IV, VI, y VIII año básico y I y IV año medio), sexo, tipo de colegio y área geográfica. La prevalencia de tabaquismo se determinó mediante un cuestionario autoadministrado. El nivel socioeconómico(NSE) se midió por medio del método de Graffar modificado, midiéndose además, las condiciones familiares, la exposición a medios de comunicación de masas y factores psicosociales. El rendimiento escolar se determinó mediante un test de castellano y matemáticas, al mismo tiempo que se registraron otras variables educacionales. El análisis estadístico incluyó análisis de varianza, test de la t de Student y test de Scheffe para comparación de medias, correlación, regresión múltiple stepwise, chi-cuadrado y la metodología del enfoque de riesgo de OPS/OMS, para el cálculo del riesgo relativo simple (RR). RESULTADOS: La prevalencia de tabaquismo (10,6%) aumentó significativamente con la edad, de 1,3% en los escolares menores de 13 años, a 15,4% y 36,9%, en los adolescentes entre 13-15 años y ³ 16 años, respectivamente (p< 0,001), a la vez que fue mayor en los escolares de sexo femenino. El grado de placer(r=0,499 p<0,001), el grado de rebeldía(r=0,124 p<0,0001), el grado de recreación familiar (r=-0,131 p<0,0001) y el porcentaje de asistencia al colegio(r=-0,118 p< 0,0001), fueron las variables independientes con el mayor poder explicatorio en la varianza del tabaquismo (r²= 0,2860), pero el grado de placer fue la variable con el mayor poder explicatorio en la varianza explicada (89,2%) y con el mayor RR (34,3). El impacto relativo de las variables independientes sobre el tabaquismo experimentó diferencias de acuerdo a la edad, sexo y NSE. CONCLUSIONES: Nuestros resultados han demonstrado que el grado de placer, el grado de rebeldía, el grado de recreación familiar y el porcentaje de asistencia al colegio, fueron las variables independientes que mayormente contribuyeron a explicar el tabaquismo. Estos hallazgos podrían ser de utilidad para las políticas educacionales y de salud focalizadas a la población escolar, con el objeto de prevenir este importante factor de riesgo para la salud de la población.

OBJETIVO: Evaluar la relación entre consumo precoz de tabaco y alcohol y el riesgo de consumir marihuana en escolares. MÉTODOS: Estudio transversal con datos del IV Estudio Nacional de Consumo de Drogas en Población Escolar, año 2001. Se analizó 54,001 escolares de ocho a 20 años de edad. Como variables predictoras se consideró el autoreporte de consumo de tabaco y alcohol, la edad de inicio de consumo de tabaco y/o alcohol, y la intensidad de uso del tabaco. Como variable resultado se utilizó el autoreporte de uso de marihuana y la edad de inicio de consumo de ésta. Para el análisis de los datos se utilizó regresión de Poisson y regresión de Weibull. RESULTADOS: Los escolares presentaron alta prevalencia de consumo de tabaco, alcohol y marihuana (77%, 79% y 23% respectivamente). El consumo de tabaco en presencia de consumo de alcohol fue un factor de riesgo de uso de marihuana (RP=10.4; IC 95%: 8.9;12.2). El inicio tardío de consumo de tabaco (HR=0.85; IC 95%: 0.84;0.86) y alcohol (HR=0.90; IC 95%: 0.89;0.91) resultó ser un factor protector del uso de marihuana. La probabilidad de consumo de marihuana fue mayor en quienes fumaban todos o casi todos los días en relación a quienes fumaban sólo los fines de semana (RP=3.11; IC 95%: 2.96;3.26 vs. RP=1.70; IC 95%: 1.58;1.83). CONCLUSIONES: El riesgo de consumo de marihuana se asoció significativamente con la edad de inicio de consumo de tabaco, la frecuencia de consumo de tabaco y el consumo simultáneo de alcohol. Las estrategias de prevención deberían orientarse a evitar el consumo precoz de tabaco en escolares.

OBJETIVO: Estimar la prevalencia y los factores asociados al abuso sexual en niñez y adolescencia. MÉTODOS: Estudio realizado en una muestra de estudiantes del estado de Morelos, México, en 2004-2005. Los participantes (n=1730) pertenecen a una cohorte de 13.293 estudiantes de 12 a 24 años. Los datos fueron colectados mediante la aplicación de un cuestionario conteniendo secciones de escalas validadas. Las variables analizadas fueron: factores sociodemográficos (sexo, zona de habitación, nivel socioeconómico); familiares (educación de los padres, adicciones de los padres, violencia entre padres); psicológicos individuales (autoestima - Inventario de Autoestima de Coopersmith, depresión, consumo de alcohol); violencia intrafamiliar (Escala de Strauss); y abuso sexual. Mediante regresión logística múltiple se evaluaron los factores asociados. Se obtuvieron Razones de Momios (RM) con intervalos de confianza al 95%. RESULTADOS: El 4.7% (n=80) de los (as) estudiantes presentaron intento de abuso y el 2.9% (n=50) fueron víctimas de abuso sexual consumado. Las mujeres tuvieron mayor prevalencia de intento (6.1%). El 3.6% de las mujeres y el 1.9% de los hombres fueron abusados sexualmente. Principal agresor en mujeres fue el novio y en hombres una persona desconocida. Edad promedio de 12.02 años en mujeres y 11.71 en hombres. Factores asociados al abuso: mayor consumo de alcohol padres (RM = 3.37; IC 95% 1.40;8.07); violencia hacia madre (RM=4.49; IC 95%1.54;13.10); ser mujer (RM = 2.47; IC 95%1.17;5.24); ser víctima de violencia intrafamiliar alta (RM=3.58; IC 95%1.32;9.67). Autoestima alta fue un factor protector (RM=0.27; IC 95% 0.09;0.75). CONCLUSIONES: En promedio el abuso sexual se presenta a los 12 años de edad en ambos sexos, siendo más frecuente en el sexo femenino. La mayoría de víctimas no lo denuncia.

La descripción macroscópica del proceso de patogénesis en hamsters inoculados subcutáneamente en nariz con Sporothrix schenckii ó Leishmania mexicana spp. proporcionó bases para diferenciar estos dos microorganismos en un modelo animal utilizado comunmente para estudiarlos. Observaciones secuenciales durante 150 días permitieron afirmar que en las infecciones causadas por estos patógenos se presentaron edema y eritema como signos primarios, seguidos de alopecia, necrosis y ulceración. La producción de pus fué una característica distintiva para el S. schenckii. Estos signos clínicos se observaron más temprano en la esporotricosis que en la infección por L. mexicana, mostrando diferencias estadísticas significantes en días promedio de aparición. El presente trabajo muestra que las lesiones producidas tanto por el S. schenckii como la L. mexicana en este modelo experimental comparten signos clínicos, pero el tiempo de aparición de los mismos y su frecuencia relativa permiten diferenciarlas. Las condiciones de inoculación como: cepa de los microorganismos, dosis del inóculo, sitio y vía de inoculación, deben tenerse presentes en la evaluación de su comportamiento experimental.
