This article evaluates the impacts of the imposition of tariffs on the Brazilian soluble coffee mainly by European countries as of the 1990s. More particularly, it verifies whether the imposition of discriminatory trade tariffs by the European Union and of non-discriminatory ones by some Eastern European countries reflects on the international demand for this commodity. For this purpose, dynamic models of global demand for Brazilian soluble coffee were estimated for the 1995-2003 period using data from the International Coffee Organization. Findings suggest that existing tariffs significantly account for the reduction of Brazilian share of soluble in the world market.

Durante o período pré-menstrual é comum a ocorrência de disfonia, e são poucas as mulheres que se dão conta dessa variação da voz dentro do ciclo menstrual (Quinteiro, 1989). OBJETIVO: Verificar se há diferença no padrão vocal de mulheres no período de ovulação em relação ao primeiro dia do ciclo menstrual, utilizando-se da análise perceptivo-auditiva, da espectrografia, dos parâmetros acústicos e quando esta diferença está presente, se é percebida pelas mulheres. FORMA DE ESTUDO: Caso-controle. MATERIAL E MÉTODO: A amostra coletada foi de 30 estudantes de Fonoaudiologia, na faixa etária de 18 a 25 anos, não-fumantes, com ciclo menstrual regular e sem o uso de contraceptivo oral. As vozes foram gravadas no primeiro dia de menstruação e no décimo-terceiro dia pós-menstruação (ovulação), para posterior comparação. RESULTADOS: Observou-se durante o período menstrual que as vozes estão rouco-soprosa de grau leve a moderado, instáveis, sem a presença de quebra de sonoridade, com pitch e loudness adequados e ressonância equilibrada. Há pior qualidade de definição dos harmônicos, maior quantidade de ruído entre eles e menor extensão dos harmônicos superiores. Encontramos uma f0 mais aguda, jitter e shimmer aumentados e PHR diminuída. CONCLUSÃO: No período menstrual há mudanças na qualidade vocal, no comportamento dos harmônicos e nos parâmetros vocais (f0,jitter, shimmer e PHR). Além disso, a maioria das estudantes de Fonoaudiologia não percebeu a variação da voz durante o ciclo menstrual.

O zumbido é uma desordem extremamente freqüente em pacientes com perda auditiva, atingindo cerca de 40 milhões de pessoas nos EUA, afetando aproximadamente 1/3 da população acima dos 65 anos de idade. A prática clínica tem demonstrado que pacientes portadores de perda auditiva associada a zumbido beneficiam-se com o uso de próteses auditivas, pois estas, além de melhorarem a compreensão da conversação, aliviam o zumbido. OBJETIVO: No presente trabalho, procurou-se verificar a eficiência da adaptação de próteses auditivas na redução ou eliminação do zumbido em pacientes com perda auditiva. FORMA DE ESTUDO: Estudo de série. MATERIAL E MÉTODO: Foram avaliados 47 indivíduos adultos, sendo 32 do sexo feminino e 15 do sexo masculino, com idades entre 32 e 88 anos, com indicação médica para adaptação de prótese auditiva. Realizou-se análise e avaliação subjetiva da sensação de zumbido por meio da técnica da acufenometria, caracterizando-o quanto ao pitch e loudness, e acompanhamento dos pacientes durante um ano afim de verificar se houve melhora do zumbido após a adaptação da prótese auditiva. RESULTADOS: A maioria dos pacientes (87,2%) referiu melhora do zumbido com o uso da prótese auditiva, sendo que em 51% destes o zumbido desapareceu completamente. O tempo de uso da prótese necessário para a melhora do zumbido foi de 3 a 8 meses para a maioria dos indivíduos. As características da prótese auditiva, como modelo, tecnologia e ventilação, não influenciaram a melhora do zumbido. CONCLUSÃO: A adaptação de próteses auditivas mostrou-se eficaz na redução ou eliminação do zumbido em pacientes portadores de perda auditiva associada a zumbido.

Apesar da crescente experiência adquirida com as campanhas nacionais da voz na triagem de indivíduos com alterações laríngeas, ainda não estabelecemos qual o melhor método de avaliação: triagem fonoaudiológica exclusiva, exame laringoscópico exclusivo ou trabalho conjunto médico/fonoaudiólogo para triagem. OBJETIVO: O objetivo do atual estudo foi avaliar a sensibilidade, especificidade, valor preditivo positivo e negativo da análise perceptivo auditiva da voz em comparação com videolaringoscopia como método de triagem de indivíduos com alterações laringofaríngeas. MATERIAL E MÉTODO: Foram comparados os aspectos vocais (escala GRBASI, pitch, loudness, CPF e ressonância) e videolaringoscópicos de 567 indivíduos que participaram da Campanha da Voz 2005 em um hospital universitário terciário. RESULTADOS: A alteração laríngea mais freqüente foi o refluxo laringofaríngeo (RLF) (43,5%), seguida de lesões benignas (17%) e suspeitas de malignas (1%). A sensibilidade da avaliação perceptivo-auditiva da voz foi 91% para os pacientes com lesões benignas e 100% nas suspeitas de maligna, porém apenas 76% no RLF. Dos exames considerados normais 52% foram flagrados como tendo alterações vocais. O valor preditivo positivo foi de 71% e o negativo foi de 61%. CONCLUSÕES: Apesar de importante, a análise perceptivo-auditiva não deve ser usada como único instrumento de triagem em campanhas de saúde vocal.

OBJECTIVE: To develop and validate a short health literacy assessment tool for Portuguese-speaking adults. METHODS: The Short Assessment of Health Literacy for Portuguese-speaking Adults is an assessment tool which consists of 50 items that assess an individual's ability to correctly pronounce and understand common medical terms. We evaluated the instrument's psychometric properties in a convenience sample of 226 Brazilian older adults. Construct validity was assessed by correlating the tool scores with years of schooling, self-reported literacy, and global cognitive functioning. Discrimination validity was assessed by testing the tool's accuracy in detecting inadequate health literacy, defined as failure to fully understand standard medical prescriptions. RESULTS: Moderate to high correlations were found in the assessment of construct validity (Spearman's coefficients ranging from 0.63 to 0.76). The instrument showed adequate internal consistency (Cronbach's alpha=0.93) and adequate test-retest reliability (intraclass correlation coefficient=0.95). The area under the receiver operating characteristic curve for detection of inadequate health literacy was 0.82. A version consisting of 18 items was tested and showed similar psychometric properties. CONCLUSIONS: The instrument developed showed good validity and reliability in a sample of Brazilian older adults. It can be used in research and clinical settings for screening inadequate health literacy.

OBJECTIVE To evaluate the level of HIV/AIDS knowledge among men who have sex with men in Brazil using the latent trait model estimated by Item Response Theory. METHODS Multicenter, cross-sectional study, carried out in ten Brazilian cities between 2008 and 2009. Adult men who have sex with men were recruited (n = 3,746) through Respondent Driven Sampling. HIV/AIDS knowledge was ascertained through ten statements by face-to-face interview and latent scores were obtained through two-parameter logistic modeling (difficulty and discrimination) using Item Response Theory. Differential item functioning was used to examine each item characteristic curve by age and schooling. RESULTS Overall, the HIV/AIDS knowledge scores using Item Response Theory did not exceed 6.0 (scale 0-10), with mean and median values of 5.0 (SD = 0.9) and 5.3, respectively, with 40.7% of the sample with knowledge levels below the average. Some beliefs still exist in this population regarding the transmission of the virus by insect bites, by using public restrooms, and by sharing utensils during meals. With regard to the difficulty and discrimination parameters, eight items were located below the mean of the scale and were considered very easy, and four items presented very low discrimination parameter (< 0.34). The absence of difficult items contributed to the inaccuracy of the measurement of knowledge among those with median level and above. CONCLUSIONS Item Response Theory analysis, which focuses on the individual properties of each item, allows measures to be obtained that do not vary or depend on the questionnaire, which provides better ascertainment and accuracy of knowledge scores. Valid and reliable scales are essential for monitoring HIV/AIDS knowledge among the men who have sex with men population over time and in different geographic regions, and this psychometric model brings this advantage.

OBJECTIVE To comprehend the perception of body image in adolescence. METHODS A qualitative study was conducted with eight focus groups with 96 students of both sexes attending four public elementary school institutions in the city of Rio de Janeiro, Southeastern Brazil, in 2013. An interview guide with questions about the adolescents’ feelings in relation to: their bodies, standards of idealized beauty, practice of physical exercise and sociocultural influences on self-image. In the data analysis we sought to understand and interpret the meanings and contradictions of narratives, understanding the subjects’ context and reasons and the internal logic of the group. RESULTS Three thematic categories were identified. The influence of media on body image showed the difficulty of achieving the perfect body and is viewed with suspicion in face of standards of beauty broadcast; the importance of a healthy body was observed as standards of beauty and good looks were closely linked to good physical condition and result from having a healthy body; the relationship between the standard of beauty and prejudice, as people who are not considered attractive, having small physical imperfections, are discriminated against and can be rejected or even excluded from society. CONCLUSIONS The standard of perfect body propagated by media influences adolescents’ self-image and, consequently, self-esteem and is considered an unattainable goal, corresponding to a standard of beauty described as artificial and unreal. However, it causes great suffering and discrimination against those who do not feel they are attractive, which can lead to health problems resulting from low self-esteem.

OBJECTIVE To analyze HIV/AIDS positive individual’s perception and attitudes regarding dental services.METHODS One hundred and thirty-four subjects (30.0% of women and 70.0% of men) from Nuevo León, Mexico, took part in the study (2014). They filled out structured, analytical, self-administered, anonymous questionnaires. Besides the sociodemographic variables, the perception regarding public and private dental services and related professionals was evaluated, as well as the perceived stigma associated with HIV/AIDS, through a Likert-type scale. The statistical evaluation included a factorial and a non-hierarchical cluster analysis.RESULTS Social inequalities were found regarding the search for public and private dental professionals and services. Most subjects reported omitting their HIV serodiagnosis and agreed that dentists must be trained and qualified to treat patients with HIV/AIDS. The factorial analysis revealed two elements: experiences of stigma and discrimination in dental appointments and feelings of concern regarding the attitudes of professionals or their teams concerning patients’ HIV serodiagnosis. The cluster analysis identified three groups: users who have not experienced stigma or discrimination (85.0%); the ones who have not had those experiences, but feel somewhat concerned (12.7%); and the ones who underwent stigma and discrimination and feel concerned (2.3%).CONCLUSIONS We observed a low percentage of stigma and discrimination in dental appointments; however, most HIV/AIDS patients do not reveal their serodiagnosis to dentists out of fear of being rejected. Such fact implies a workplace hazard to dental professionals, but especially to the very own health of HIV/AIDS patients, as dentists will not be able to provide them a proper clinical and pharmaceutical treatment.
