Those over sixty years of age accounted for 6.6% of the total population of Brazil in 1985, in the Federal Republic of Germany this proportion was 20.3% in 1984. As early as 1950 it had been 14.5%. This proportion will not even be reached in Brazil in the year 2000 when persons aged sixty years and older are only projected to make up 8.8% of the total population. Similarly, in 1982/84 life expectancy at birth in the Federal Republic was 70.8 years for men and 77.5 for women; in Brazil the figures for 1980/85 were, by contrast, "only" 61.0 and 66.0. Against this background it is easy to understand why the discussion concerning an ageing society with its many related medical, economic, individual and social problems has been so slow in coming into its own in Brazil. As important as a more intensive consideration of these aspects may be in Brazil at present, they are, nevertheless, only one side of the story. For a European historical demographer with a long-term perspective of three of four hundred years, the other side of the story is just as important. The life expectancy which is almost ten years lower in Brazil is not a result of the fact that no one in Brazil lives to old age. In 1981 people sixty-five years and older accounted for 34.4% of all deaths! At the same time infants accounted for only 22.1% of total mortality. They are responsible, along with the "premature" deaths among youths and adults, for the low, "average" life expectancy figure. In Europe, by contrast, these "premature" deaths no longer play much of a role. In 1982/84 more than half of the women (52.8%) in the Federal Republic of Germany lived to see their eightieth birthdays and almost half of the men (47.3%) lived to see their seventy-fifth. Our biological existence is guaranteed to an extent today that would have been unthinkable a few generations ago. Then, the classic troika of "plague, hunger and war" threatened our forefathers all the time and everywhere. The radical transition from the formerly uncertain to a present-day certain lifetime, which is the result of the repression of "plague, hunger and war", led to unexpected consequences for our living together. Our forefathers were forced to live in closely knit Gemeinschaften in the interest of physical survival and to subordinate their egoistic goals to a common value, but now these pressures have, for the most part, fallen away. Correspondingly, this much more certain EGO has taken center stage. An ever greater number of us chooses to live life as single beings: the number of marriages is lower every year; the number of divorces is on the increase; in Berlin (West) more than half (sic! 52.3%) of all households are already composed on only one person. For the last dozen years the annual number of births in the Federal Republic has been insufficient to ensure population replacement. Not a population explosion but rather the opposite, a population implosion, is our problem. Human beings do not appear to be "social animals", as was axiomatically assumed for so long. They were only forced to behave as such for as long as "plague, hunger and war" forced them to do so. When these life endangering conditions no longer exist and life becomes certain even without their being integrated into a Gemeinschaft then humans suddenly show themselves more and more to be independent single beings. It is not the percentage of the population that is over sixty or sixty-five that is decisive in this context but rather how certain adults perceive their biological lives to be, since they are the ones who organize their lives, who build communities or who are ever more often willing only to enter into means-to-an-end personal unions without lasting or close ties and mutual responsibilities. There are many signs which seem to point to a development in this direction in Brazil as well. More and more adults in Brazil are caught up in the deep-seated transition from an uncertain to a certain lifetime. A third of them die after having reached their sixty-fifth birthday. It therefore seems to me to be high time that one began to give more consideration to the other side of the story in Brazil as well. And who is more suited intensively to consider the long-term perspectives than those engaged in the public health sector in whose competence, after all, such aspects, as "life certainty", "life expectancy" and "age at death" belong?

Individual cancer susceptibility seems to be related to factors such as changes in oncogenes and tumor suppressor genes expression, and differences in the action of metabolic enzymes and DNA repair regulated by specific genes. Epidemiological studies on genetic polymorphisms of human xenobiotics metabolizing enzymes and cancer have revealed low relative risks. Research considering genetic polymorphisms prevalence jointly with environmental exposures could be relevant for a better understanding of cancer etiology and the mechanisms of carcinogenesis and also for new insights on cancer prognosis. This study reviews the approaches of molecular epidemiology in cancer research, stressing case-control and cohort designs involving genetic polymorphisms, and factors that could introduce bias and confounding in these studies. Similarly to classical epidemiological research, genetic polymorphisms requires considering aspects of precision and accuracy in the study design.

The goal of this article is to present a snapshot of an ongoing debate within epidemiology, pitching opposing sides in the struggle to define the path it should follow in the years to come. The debate among epidemiologists in the mid-90s pitted those who defended the idea that epidemiology should necessarily deal with a wide context against those who believed that science and public health are better served by focusing on the individual level. Ian Hacking’s concept of styles of reasoning was used as a theoretical tool. The literature was reviewed using a core set of articles as an entry point, seeking articles that cited them, and then back-tracking the citations of the resulting set in the Scopus database. The main arguments are presented according to levels (ontological, epistemological, axiological and pragmatic), in order to show an even deeper disagreement, in the very conception of science and its relation to social issues and public policy.

Os resíduos gerados em domicílios incluem diversos produtos, como pesticidas, produtos farmacêuticos, detergentes, óleos de cozinha, metais pesados contidos em baterias e outros utensílios. Esses resíduos são lançados continuamente em aterro sanitário ou lixões em cidades como Manaus. O chorume produzido nesses aterros, quando não tratados, contamina recursos hídricos superficiais e subterrâneos. Neste estudo foi feita uma avaliação das conseqüências da liberação do chorume no sistema hídrico da bacia do Tarumã-Açu. Amostras de água e sedimento foram coletadas nos igarapés Matrinxã, Acará, Bolívia, bacia do Tarumã-Açu e dentro do aterro sanitário (Manaus - Amazonas - Brasil) em março 2001. As amostras de água foram filtradas em filtro Milipore (0,45 mm de poro) e, em seguida, tratadas com HNO3 concentrado. As amostras de sedimento foram peneiradas em malha de 0,053 mm e digeridas com HCl:HNO3 (1:3) a 150ºC. As concentrações de alguns metais pesados (Co, Cu, Fe, Cr, Ni, Mn, Pb e Zn) foram determinadas nas amostras de água e sedimento por espectrometria de absorção atômica de chama. Os resultados revelaram que a concentração dos metais pesados é muito acima dos permitidos pela resolução 357/2005 do CONAMA em praticamente todos os locais amostrados, mostrando que o Aterro Sanitário é um dos principais responsáveis pelo impacto ambiental observado nos corpos hídricos estudados. As análises dos componentes principais (PCA) e hierárquica de cluster (HCA), revelam que os pontos de coleta localizados dentro do aterro sanitário apresentam características diferentes dos outros locais amostrados. Além disso, o HCA e PCA mostraram que existe uma similaridade entre os pontos de coleta localizados fora do aterro o que permite afirmar que o chorume do aterro se dissolve por todo corpo hídrico estudado.

O diagnóstico da fertilidade e teores de elementos-traço (ETs) em solos é importante, pois estes dados são escassos na literatura para áreas de transição Pantanal-Cerrado-Floresta Amazônica. Esse trabalho avaliou diversos parâmetros relacionados à fertilidade, teores biodisponíveis de Fe, Mn, Zn, Cu e B e semitotais de As, Cd, Hg e Pb de solos do Vale do Alto Guaporé, região sudoeste do estado de Mato Grosso. Foram coletadas amostras de solos (0-0,20 e 0,20-0,40 m de profundidade) em áreas de vegetação nativa (VN), pastagem (AP), cultura anual (CA) e garimpo de ouro (G). As amostras foram analisadas conforme métodos de rotina para avaliação da fertilidade do solo e ETs pelo método SW-3051A e os resultados médios comparados com os valores de referência de qualidade (VRQ) para solos estipulados pela Companhia de Tecnologia de Saneamento Ambiental (CETESB). Teores mais elevados de As e Hg foram verificados em VN e G com médias, respectivamente, iguais a 43,9 e 101,13 para o As; e 0,12 e 0,14 mg kg-1 para o Hg. Exceto Pb, vários locais de amostragem apresentaram teores dos ETs superiores ao VRQ: 46% em VN; 60% em G; 28% em CA; e 44% em AP, para o As; 20,8; 50; 55; e 22% em VN, G, CA e AP, respectivamente, para o Cd; 75; 65 e 67% das áreas de VN, G e CA e AP, respectivamente, para o Hg. A saturação por bases foi alta (60-80%) em 51,5% das amostras, enquanto o P foi baixo em todas áreas. Valores de referência de qualidade de solo para o As e Hg devem ser estipulados para solos dessa região, tendo em vista que os teores observados em áreas nativas foram superiores ao VRQ.

It is estimated that 5 to 8 million individuals with chest pain or other symptoms suggestive of myocardial ischemia are seen each year in emergency departments (ED) in the United States 1,2, which corresponds to 5 to 10% of all visits 3,4. Most of these patients are hospitalized for evaluation of possible acute coronary syndrome (ACS). This generates an estimated cost of 3 - 6 thousand dollars per patient 5,6. From this evaluation process, about 1.2 million patients receive the diagnosis of acute myocardial infarction (AMI), and just about the same number have unstable angina. Therefore, about one half to two thirds of these patients with chest pain do not have a cardiac cause for their symptoms 2,3. Thus, the emergency physician is faced with the difficult challenge of identifying those with ACS - a life-threatening disease - to treat them properly, and to discharge the others to suitable outpatient investigation and management.

FUNDAMENTO: A hipertensão arterial pertence ao grupo de doenças cardiovasculares de maior mortalidade no mundo e pode se iniciar na infância. OBJETIVO: Avaliar a prevalência de níveis pressóricos elevados em crianças da rede pública de ensino e sua associação com indicadores antropométricos. MÉTODOS: Trata-se de um estudo transversal, composto por uma amostra aleatória de 750 escolares, entre 6-9 anos de idade, da rede de ensino municipal de João Pessoa, Paraíba. Os dados foram coletados por uma equipe previamente treinada. A pressão foi aferida com técnica auscultatória com auxílio de estetoscópio e esfigmomanômetro aneroide. Os dados antropométricos coletados foram as medidas do peso, da estatura e da circunferência abdominal. Foram feitos testes de associação qui-quadrado e t de Student para comparações de médias, ambos com nível de significância de 5,0%. Foram construídos três modelos de regressão logística, relacionando nível pressórico elevado com as variáveis antropométricas para encontrar um melhor modelo de predição. RESULTADOS: A prevalência de níveis pressóricos elevados foi de 13,6%. A variável índice de massa corporal (IMC) apresentou associação significativa com o aumento da pressão arterial (p < 0,0001) e a maior razão de chances (OR = 1,17). A elevação dos níveis pressóricos também ocorreu com o aumento do peso (p < 0,0001) e da circunferência abdominal (p < 0,0001). CONCLUSÃO: A associação do excesso de peso com a elevação da pressão arterial identificada destaca a necessidade de intervenção e medidas de controle do estado nutricional, como educação alimentar, para prevenção e tratamento da obesidade como fator de risco das doenças cardiovasculares na faixa etária pediátrica e futura. Foram produzidos antígenos solúveis de P. brasiliensis, H. capsulatum e A. fumigatus e padronizados nas técnicas de imunodifusão dupla (IDD) e imunoeletroosmoforese (IEOF). A especificidade dos antígenos foi testada utilizando-se 96 soros de pacientes com paracoccidioidomicose, histoplasmose, aspergilose, candidíase sistêmica, esporotricose, tuberculose, neoplasia pulmonar, leishmaniose tegumentar e visceral e em 18 indivíduos sadios. Na IDD, a especificidade foi de 100% usando-se como critério de positividade linhas de precipitação com identidade total com soro de referência. Entretanto na IEOF, a especificidade variou de acordo com o antígeno testado, sendo observadas reações cruzadas com antígeno de P. brasiliensis frente a soros de pacientes com histoplasmose (16,7%) e leishmaniose tegumentar (10%) e com antígeno de H. capsulatum frente a soros de pacientes com paracoccidioidomicose (31,8%) e leishmaniose tegumentar (10%). Ambas as técnicas mostraram a mesma sensibilidade para o sorodiagnóstico de paracoccidioidomicose, histoplasmose e aspergilose, respectivamente 100%, 83,3% e 100%. A grande sensibilidade e especificidade da IDD observadas nos soros desses pacientes, aliadas à fácil reprodutibilidade e baixo custo, fazem esta técnica muito apropriada como procedimento de rotina, para a triagem de pacientes sintomáticos respiratórios.

An attempt has been made to characterize the synanthropic trends in Sarcophaginae of three Central European cities, viz. Brno, Bratislava and Budapest. The polar ordination of both sarcophagine taxa and of their taxocenoses revealed clear-cut trends towards culturophily and synanthropy in the male preconnubial aggregations of Sarchophaginae evidencing that this group of high Diptera represent an excellent model for the study of this phenomenon.

Prof. Dr. Hugo de Souza Lopes is one of the authors of the phylogenetic classification of Sarcophagidae, especially Sarcophaginae. In this paper I present the taxonomic key of the tribes of Sarcophaginae according to his opinion; a list of the 48 genera and subgenera and the 356 species described by Prof. Lopes; and a review of subtribal construction of tribe Sarcophagini with a key of the subtribes. One new subtribe Boettcheriiscina Verves, subtr. nov. and two new monotypic genera (Mufindia Verves, gen. nov., and Sabiella Verves, gen. nov.) are described. The role of Prof. Lopes in the knowledge of taxonomy and ecology of American, Oriental, Australian and Oceanic Sarcophagidae is illumined.

The present paper reviews our recent data concerning the use of immunological methods employing monoclonal antibodies and synthetic peptides to study malaria transmission and immunity and to diagnose plasmodial infection. As concerns malaria transmission, we studied the main vectors of human malaria and the plasmodial species transmitted in endemic areas of Rondônia state, Brazil. The natural infection on anopheline was evaluated by immunoradiometric assay (IRMA) using monoclonal antibodies to an immunodominant sporozoite surface antigen (CS protein) demonstrated to be species specific. Our results showed that among six species of Anopheles found infected, An. darlingi was the main vector transmitting Plasmodium falciparum and P. vivax malaria in the immediate vicinity of houses. In order to assess the level of anti-CS antibodies we studied, by IRMA using the synthetic peptide corresponding to the repetitive epitope of the sporozoite CS protein, sera of individuals living in the same areas where the entomological survey has been performed. In this assay the prevalence of anti-CS antibodies was very low and did not reflect the malaria transmission rate in the studied areas. In relation to malaria diagnosis, a monoclonal antibody specific to an epitope of a 50 kDa exoantigen, the major component of supernatant collected at the time of schizont rupture, was used as a probe for the detection of P. falciparum antigens. This assay seemed to be more sensitive than parasitological examination for malaria diagnosis since it was able to detect plasmodial antigens in both symptomatic and asymtomatic individuals with negative thick blood smear at different intervals after a last parasitologically confirmed confirmed attack of malaria.

The Brazilian variant of human immunodeficiency virus type 1 (HIV-1) subtype B, (serotype B"-GWGR), has a tryptophan replacing the proline in position 328 the HIV-1 envelope. A longer median time period from infection to acquired immunodeficiency syndrome (AIDS) for serotype B (B"-GWGR) infected subjects compared to the B-GPGR US/European strain was reported. In a cohort study, in São Paulo city, 10 B"-GWGR patients had a statistically significant increased avidity of the anti-V3 antibodies, from 79% ± 33% to 85% ± 75%, versus from 48% ± 59% to 32% ± 17% for the 10 B-GPGR subjects (p = 0.02). The T CD4+ cells showed a mean increase of + 0.45 cells/month for the B-GPGR subjects and for B"-GWGR the slope was + 1.24 cells/month (p = 0.06), for 62 and 55 months of follow up, respectively. RNA plasma viral load decreased from 3.98 ± 1.75 to 2.16 ± 1.54 log10 in the B"-GWGR group while B-GPGR patients showed one log10 reduction in viral load from 4.09 ± 0.38 to 3.17 ± 1.47 log10 over time (p = 0.23), with a decreasing slope of 0.0042 ± log10,/month and 0.0080 ± log10/month, for B-GPGR and B"-GWGR patients, respectively (p = 0.53). Neither group presented any AIDS defining events during the study, according to Center for Diseases Control criteria. Although the sample size is small, these results may indicate that differences in the pathogenicity of the 2 HIV-1 B serotypes which co-circulate in Brazil may be correlated to the avidity of anti-V3 antibodies.
