INTRODUCTION: Chagas' disease is a major public health problem in Brazil and needs extensive and reliable information to support consistent prevention and control actions. This study describes the most common causes of death associated with deaths related to Chagas' disease (underlying or associated cause of death). METHODS: Mortality data were obtained from the Mortality Information System of the Ministry of Health (approximately 9 million deaths). We analyzed all deaths that occurred in Brazil between 1999 and 2007, where Chagas' disease was mentioned on the death certificate as underlying or associated cause (multiple causes of death). RESULTS: There was a total of 53,930 deaths related to Chagas' disease, 44,543 (82.6%) as underlying cause and 9,387 (17.4%) as associated cause. The main diseases and conditions associated with death by Chagas' disease as underlying cause included direct complications of cardiac involvement, such as conduction disorders/arrhythmias (41.4%) and heart failure (37.7%). Cerebrovascular disease (13.2%), ischemic heart disease (13.2%) and hypertensive diseases (9.3%) were the main underlying causes of deaths in which Chagas' disease was identified as an associated cause. CONCLUSIONS: Cardiovascular diseases were often associated with deaths related to Chagas' disease. Information from multiple causes of death recorded on death certificates allows reconstruction of the natural history of Chagas' disease and suggests preventive and therapeutic potential measures more adequate and specifics.

OBJETIVO: A religião tem um importante papel na vida de indivíduos encarcerados. A saúde mental e a possibilidade de reabilitação parecem ser favorecidas através da religião. Entretanto poucos estudos no Brasil abordaram a saúde mental em população carcerária feminina e a relacionaram com a religiosidade. O objetivo deste estudo é verificar o perfil de saúde mental e a relação entre religião, religiosidade e saúde mental numa amostra de mulheres encarceradas em São Paulo. MÉTODOS: Foram entrevistadas 358 mulheres, detentas da Penitenciária Feminina da Capital (São Paulo-SP). Foi aplicado um questionário amplo que abrangeu dados sociodemográficos e culturais (incluindo a religiosidade), assim como itens sobre saúde mental e perfil criminal. A avaliação psicopatológica foi feita com o General Health Questionnaire (GHQ-12). Análises estatísticas comparando as variáveis com o GHQ-12 foram realizadas. Foi utilizada também a análise de regressão logística multivariada para respostas dicotômicas para as variáveis que possivelmente interagiram com a saúde mental. RESULTADOS: A idade média da amostra foi 30,7 anos ± 8,5. A prevalência de possível caso psiquiátrico foi 26,6%. CONCLUSÕES: Uma maior religiosidade pessoal associou-se a menor freqüência de possível transtorno mental. Não foram encontradas associações entre tipo de crime e saúde mental.

Objetivo Analisar a associação entre sobrepeso, obesidade e transtornos mentais comuns em profissionais nutricionistas da rede pública de hospitais do município do Rio de Janeiro. Métodos Estudo seccional, com 289 nutricionistas de hospitais públicos do município do Rio de Janeiro, de outubro de 2011 a agosto de 2012. Foi utilizado o índice de massa corporal (kg/m2) pela aferição de peso e altura, e os transtornos mentais comuns (TMC) pelo General Health Questionnaire (GHQ-12). Variáveis sociodemográficas (SES), laborativas e de saúde também foram incluídas no estudo. Resultados As prevalências de sobrepeso, de obesidade e de TMC foram de 32,3%, 15,3% e 37,7%, respectivamente. A análise múltipla não apresentou associação significativa após o ajuste pelas variáveis SES, laborativas e de saúde (OR = 0,60; IC95% 0,32-1,10 para sobrepeso e OR = 1,09; IC95% 0,50-2,37 para obesidade). Conclusão Não encontramos associação entre sobrepeso, obesidade e TMC. Entretanto, as prevalências desses eventos na população estudada foram consideradas altas, o que aponta para a necessidade de estratégias de prevenção e promoção da saúde por se tratar de uma população de trabalhadoras envolvidas com o cuidado da população.

OBJECTIVE: To assess intermediate-term outcome in children who have undergone orthotopic heart transplantation. METHODS: We carried out a longitudinal and prospective study between October '92 and June '99 comprising 20 patients with ages ranging from 12 days to 7 years (mean of 2.8 years). We employed a double immunosuppression protocol with cyclosporine and azathioprine and induction therapy with polyclonal antithymocyte serum. Survival and complications resulting from the immunosuppression protocol were analyzed. RESULTS:The double immunosuppression protocol and the induction therapy with polyclonal antithymocyte serum resulted in an actuarial survival curve of 90% and 78.2% at 1 and 6 years, respectively, with a mean follow-up period of 3.6 years. One patient died due to acute rejection 40 days after transplantation; another patient died 2 years after transplantation due to lymphoproliferative disorder; a third patient died because of primary failure of the graft; and a fourth patient died due to bronchopneumonia. The major complications were as follows: acute rejection, infection, nephrotoxicity, and systemic hypertension. The means of rejection and infection episodes per patient were 2.9 and 3.4, respectively. After one year of transplantation, a slight reduction in the creatinine clearance and systemic hypertension were observed in 7 (38.9%) patients. CONCLUSION: Heart transplantation made life possible for those patients with complex congenital heart diseases and cardiomyopathies in refractory congestive heart failure constituting a therapeutical option for this group of patients in the terminal phase.

OBJECTIVE: To assess the intraobserver reliability of the information about the history of diagnosis and treatment of hypertension. METHODS: A multidimensional health questionnaire, which was filled out by the interviewees, was applied twice with an interval of 2 weeks, in July '99, to 192 employees of the University of the State of Rio de Janeiro (UERJ), stratified by sex, age, and educational level. The intraobserver reliability of the answers provided was estimated by the kappa statistic and by the coefficient of intraclass correlation (CICC). RESULTS: The general kappa (k) statistic was 0.75 (95% CI=0.73-0.77). Reliability was higher among females (k=0.88, 95% CI=0.85-0.91) than among males (k=0.62, 95% CI=0.59-0.65).The reliability was higher among individuals 40 years of age or older (k=0.79; 95% CI=0.73-0.84) than those from 18 to 39 years (k=0.52; 95% CI=0.45-0.57). Finally, the kappa statistic was higher among individuals with a university educational level (k=0.86; 95% CI=0.81-0.91) than among those with high school educational level (k=0.61; 95% CI=0.53-0.70) or those with middle school educational level (k=0.68; 95% CI=0.64-0.72). The coefficient of intraclass correlation estimated by the intraobserver agreement in regard to age at the time of the diagnosis of hypertension was 0.74. A perfect agreement between the 2 answers (k=1.00) was observed for 22 interviewees who reported prior prescription of antihypertensive medication. CONCLUSION: In the population studied, estimates of the reliability of the history of medical diagnosis of hypertension and its treatment ranged from substantial to almost perfect reliability.

O objetivo principal foi verificar as razões da não procura pelo tratamento da incontinência urinária (IU) entre mulheres incontinentes, usuárias de uma Unidade Básica de Saúde em Campinas, SP. Trata-se de um estudo descritivo e transversal, no qual foram abordadas 213 mulheres que compareceram ao serviço para realizar o exame de citologia oncótica, sendo incluídas apenas 35 que eram incontinentes. Utilizaram-se três questionários: o ICIQ-SF, o King's Health Questionnaire e um instrumento elaborado para esse estudo. Grande parte dos sujeitos (45,7%) não conhecia nenhuma forma de tratamento para a IU e mais da metade (65,7%) não buscou tratamento para o problema, sendo as principais razões apontadas o fato de achar normal a perda de urina, não considerá-la algo importante e a questão do médico dizer que não era necessário. Conclui-se que o desconhecimento sobre os tipos de tratamento pode contribuir para não procurarem ajuda profissional.

Objective: To assess the application of aponeurotic sling by a modified technique with direct visualization of needles in patients with stress urinary incontinence. Methods: we applied the Kings Health Questionnaire (KHQ) for quality of life, gynecological examination, urinalysis I and urine culture approximately seven days prior to the urodynamic study (UDS) and the one-hour PAD test in patients undergoing making aponeurotic sling with its passing through the retropubic route with direct visualization of the needle, PAD test and King's Helth Questionnaire before and after surgery. Results: The mean age was 50.6 years, BMI of 28 and Leak Pressure (LP) 58,5cm H2O; 89% were Caucasian. Forty-six of them were monitored for three and six months, 43 for 12 months. The objective cure rate at 12 months postoperatively was approximately 93.5%. In evaluating quality of life, we observed a significant improvement in 12 months postoperatively compared with the preoperative period. There was no no urethral/bladder injury. As adverse results, we had one persistent urinary retention (2.3%), who was submitted to urethrolysis, currently without incontinence. Conclusion: The proposed procedure is safe as for the risk of bladder or urethral injuries, promoting significant improvement in quality of life and objective cure.

OBJETIVO: avaliar a qualidade de vida de mulheres no climatério, atendidas em um hospital-escola na cidade do Recife, Pernambuco, empregando o Medical Outcome Study 36-item Short Form Health Survey (MOS SF-36 Health Survey), o Women's Health Questionnaire (WHQ) e o índice de Blatt-Kupperman modificado. MÉTODOS: em estudo descritivo, transversal, foram avaliadas 233 mulheres atendidas entre fevereiro e junho de 2006. Em amostragem de conveniência, foram incluídas mulheres com idade entre 40 e 65 anos e concordância em participar da pesquisa, excluindo-se a com história prévia de ooforectomia bilateral, terapia hormonal no semestre antecedente à pesquisa e doenças descompensadas. Calculou-se o tamanho amostral, admitindo prevalência de sintomas climatéricos em 4% e precisão igual a 2,5%. Foram analisadas: saúde geral, componente físico e componente mental, obtidos com o MOS SF-36 Health Survey; qualidade de saúde pelo WHQ; e sintomatologia climatérica pelo índice de Blatt-Kupperman modificado. Os dados foram analisados com o programa Statistical Package for Social Sciences (SPSS), versão 13.0. RESULTADOS: a qualidade de vida foi classificada como ruim. Pelo MOS SF-36 Health Survey, identificou-se maior prejuízo no componente mental (18,5 versus 27,7% do físico), maiores perdas nas funções sociais (80,2%) e limitações por problemas emocionais (78,61%). Pelo WHQ, houve maior acometimento de distúrbios do sono (69,7%), sintomas somáticos (69,1%) e vasomotores (68,8%), sendo considerados regulares a função sexual e os sintomas menstruais. Os sintomas de deficiência estrogênica foram acentuados para 53% das mulheres. O aumento dos sintomas de hipoestrogenismo se acompanhou de piora da saúde geral e da saúde menopausal. CONCLUSÕES: pareceu plausível supor que a menopausa se configurou realmente como um evento biopsicossocial, mais do que orgânico, derivado predominantemente da deficiência estrogênica.

OBJECTIVE: To analyze the strengths and limitations of the Family Health Strategy from the perspective of health care professionals and the community. METHODS: Between June-August 2009, in the city of Vespasiano, Minas Gerais State, Southeastern Brazil, a questionnaire was used to evaluate the Family Health Strategy (ESF) with 77 healthcare professionals and 293 caregivers of children under five. Health care professional training, community access to health care, communication with patients and delivery of health education and pediatric care were the main points of interest in the evaluation. Logistic regression analysis was used to obtain odds ratios and 95% confidence intervals as well as to assess the statistical significance of the variables studied. RESULTS: The majority of health care professionals reported their program training was insufficient in quantity, content and method of delivery. Caregivers and professionals identified similar weaknesses (services not accessible to the community, lack of healthcare professionals, poor training for professionals) and strengths (community health worker-patient communications, provision of educational information, and pediatric care). Recommendations for improvement included: more doctors and specialists, more and better training, and scheduling improvements. Caregiver satisfaction with the ESF was found to be related to perceived benefits such as community health agent household visits (OR 5.8, 95%CI 2.8;12.1), good professional-patient relationships (OR 4.8, 95%CI 2.5;9.3), and family-focused health (OR 4.1, 95%CI 1.6;10.2); and perceived problems such as lack of personnel (OR 0.3, 95%CI 0.2;0.6), difficulty with access (OR 0.2, 95%CI 0.1;0.4), and poor quality of care (OR 0.3, 95%CI 0.1;0.6). Overall, 62% of caregivers reported being generally satisfied with the ESF services. CONCLUSIONS: Identifying the limitations and strengths of the Family Health Strategy from the healthcare professional and caregiver perspective may serve to advance primary community healthcare in Brazil.

OBJECTIVE To evaluate factors associated with users’ satisfaction in the Tuberculosis Control Program. METHODS A cross-sectional study of 295 patients aged ≥ 18 years, with two or more outpatient visits in the Tuberculosis Control Program, in five cities in the metropolitan region of Rio de Janeiro, RJ, Southeastern Brazil, in 2010. Considering an estimated population of 4,345 patients, the sampling plan included 15 health care units participating in the program, divided into two strata: units in Rio de Janeiro City, selected with probability proportional to the monthly average number of outpatient visits, and units in the other four cities. In the units, four temporal clusters of five patients each were selected with equal probability, totaling 300 patients. A questionnaire investigating the users’ clinical and sociodemographic variables and aspects of care and service in the program relevant to user satisfaction was applied to the patients. Descriptive statistics about users and their satisfaction with the program were obtained, and the effects of factors associated with satisfaction were estimated. RESULTS Patients were predominantly males (57.7%), with a mean age of 40.9 and with low level of schooling. The mean treatment time was 4.1 months, mostly self-administered (70.4%). Additionally, 25.8% had previously been treated for tuberculosis. There was a high level of satisfaction, especially regarding medication provision, and respect to patients by the health professionals. Patients who were younger (≤ 30), those on self-administered treatment, and with graduate level, showed less satisfaction. Suggestions to improve the services include having more doctors (70.0%), and offering exams in the same place of attendance (55.1%). CONCLUSIONS Patient satisfaction with the Tuberculosis Control Program was generally high, although lower among younger patients, those with university education and those on self-administered treatment. The study indicates the need for changes to structural and organizational aspects of care, and provides practical support for its improvement.

OBJETIVO: desenvolvimento da versão em português brasileiro do Endometriosis Health Profile Questionnaire (EHP-30), adaptação transcultural ao Brasil e avaliação das medidas psicométricas do EHP-30 Português em amostra brasileira. MÉTODOS: o instrumento original em inglês foi vertido para o português, seguindo diretrizes internacionais, passando por todas as etapas de tradução, retrotradução e comparação das versões para adaptação transcultural, validade de face e de conteúdo. O EHP-30 Português foi aplicado a uma amostra de 54 pacientes com diagnóstico de endometriose para análise de consistência interna, usando o alfa de Cronbach. A fidedignidade teste-reteste foi avaliada pelo coeficiente de correlação intraclasse (ICC). Para a avaliação de validade de construto convergente foi testada correlação entre o EHP-30 Português, WHOQOL-Bref e o Inventário de Depressão de Beck (BDI). RESULTADOS: a avaliação da consistência interna apresentou valores de α=0,8 a 0,9, sugerindo homogeneidade entre as questões. A fidedignidade teste-reteste apresentou ICC de 0,8 a 0,9, demonstrando estabilidade do instrumento. Na validação de construto, demonstraram-se fortes correlações da escala de auto-imagem do EHP-30 com os domínios físico (-0,6) e psicológico (-0,6) do WHOQOL-Bref e da escala de suporte social com o BDI (0,5), evidenciando-se, assim, boa correlação com outros instrumentos de avaliação de qualidade de vida. CONCLUSÕES: o EHP-30 Português mostrou ser um instrumento de fácil e rápida aplicação e bem aceito pelas pacientes, apresentando bom desempenho psicométrico, com medidas de fidedignidade adequadas (consistência interna e fidedignidade teste-reteste) e validade de construto. Estes resultados demonstram que o EHP-30 Português é um instrumento adequado para avaliação de qualidade de vida em mulheres brasileiras com endometriose em ambiente clínico e de pesquisa.

OBJECTIVE: To test discriminant analysis as a method of turning the information of a routine customer satisfaction survey (CSS) into a more accurate decision-making tool. METHODS: A 7-question, 10-multiple choice, self-applied questionnaire was used to study a sample of patients seen in two outpatient care units in Valparaíso, Chile, one of primary care (n=100) and the other of secondary care (n=249). Two cutting points were considered in the dependent variable (final satisfaction score): satisfied versus unsatisfied, and very satisfied versus all others. Results were compared with empirical measures (proportion of satisfied individuals, proportion of unsatisfied individuals and size of the median). RESULTS: The response rate was very high, over 97.0% in both units. A new variable, medical attention, was revealed, as explaining satisfaction at the primary care unit. The proportion of the total variability explained by the model was very high (over 99.4%) in both units, when comparing satisfied with unsatisfied customers. In the analysis of very satisfied versus all other customers, significant relationship was identified only in the case of the primary care unit, which explained a small proportion of the variability (41.9%). CONCLUSIONS: Discriminant analysis identified relationships not revealed by the previous analysis. It provided information about the proportion of the variability explained by the model. It identified non-significant relationships suggested by empirical analysis (e.g. the case of the relation very satisfied versus others in the secondary care unit). It measured the contribution of each independent variable to the explanation of the variation of the dependent one.

OBJECTIVE: To analyze the scoring obtained by an instrument, which evaluates the ability to read and understand items in the health care setting, according to education and age. METHODS: The short version of the Test of Functional Health Literacy in Adults was administered to 312 healthy participants of different ages and years of schooling. The study was conducted between 2006 and 2007, in the city of São Paulo, Southeastern Brazil. The test includes actual materials such as pill bottles and appointment slips and measures reading comprehension, assessing the ability to read and correctly pronounce a list of words and understand both prose passages and numerical information. Pearson partial correlations and a multiple regression model were used to verify the association between its scores and education and age. RESULTS: The mean age of the sample was 47.3 years(sd=16.8) and the mean education was 9.7 years(sd=5; range: 1 - 17). A total of 32.4% of the sample showed literacy/numeracy deficits, scoring in the inadequate and marginal functional health literacy ranges. Among the elderly (65 years or older) this rate increased to 51.6%. There was a positive correlation between schooling and scores (r=0.74; p<0.01) and a negative correlation between age and the scores (r=-0.259; p<0.01). The correlation between the scores and age was not significant when the effects of education were held constant (rp=-0.031, p=0.584). A significant association (B=3.877, Beta =0.733; p<0.001) was found between schooling and scores. Age was not a significant predictor in this model (B=-0.035, Beta=-0.22; p=0.584). CONCLUSIONS: The short version of the Test of Functional Health Literacy in Adults was a suitable tool to assess health literacy in the study population. The high number of individuals classified as functional illiterates in this test highlights the importance of special assistance to help them properly understand directions for healthcare.

OBJECTIVE To investigate the factors related to the granting of preliminary court orders [injunctions] in drug litigations. METHODS A retrospective descriptive study of drug lawsuits in the State of Minas Gerais, Southeastern Brazil, was conducted from October 1999 to 2009. The database consists of 6,112 lawsuits, out of which 6,044 had motions for injunctions and 5,167 included the requisition of drugs. Those with more than one beneficiary were excluded, which totaled 5,072 examined suits. The variables for complete, partial, and suppressed motions were treated as dependent and assessed in relation to those that were independent – lawsuits (year, type, legal representation, defendant, court in which it was filed, adjudication time), drugs (level five of the anatomical therapeutic chemical classification), and diseases (chapter of the International Classification of Diseases). Statistical analyses were performed using the Chi-square test. RESULTS Out of the 5,072 lawsuits with injunctions, 4,184 (82.5%) had the injunctions granted. Granting varied from 95.8% of the total lawsuits in 2004 to 76.9% in 2008. Where there was legal representation, granting exceeded 80.0% and in lawsuits without representation, it did not exceed 66.9%. In public civil actions (89.1%), granting was higher relative to ordinary lawsuits (82.8%) and injunctions (80.1%). Federal courts granted only 68.6% of the injunctions, while the state courts granted 84.8%. Diseases of the digestive system and neoplasms received up to 87.0% in granting, while diseases of the nervous system, mental and behavioral disorders, and diseases of the skin and subcutaneous tissue received granting below 78.6% and showed a high proportion of suspended injunctions (10.9%). Injunctions involving paroxetine, somatropin, and ferrous sulfate drugs were all granted, while less than 54.0% of those involving escitalopram, sodium diclofenac, and nortriptyline were granted. CONCLUSIONS There are significant differences in the granting of injunctions, depending on the procedural and clinical variances. Important trends in the pattern of judicial action were observed, particularly, in the reduced granting [of injunctions] over the period.

OBJECTIVE To analyze the factors associated with stiffness of the great arteries in prepubertal children.METHODS This study with convenience sample of 231 schoolchildren aged 9-10 years enrolled in public and private schools in Vitória, ES, Southeastern Brazil, in 2010-2011. Anthropometric and hemodynamic data, blood pressure, and pulse wave velocity in the carotid-femoral segment were obtained. Data on current and previous health conditions were obtained by questionnaire and notes on the child’s health card. Multiple linear regression was applied to identify the partial and total contribution of the factors in determining the pulse wave velocity values.RESULTS Among the students, 50.2% were female and 55.4% were 10 years old. Among those classified in the last tertile of pulse wave velocity, 60.0% were overweight, with higher mean blood pressure, waist circumference, and waist-to-height ratio. Birth weight was not associated with pulse wave velocity. After multiple linear regression analysis, body mass index (BMI) and diastolic blood pressure remained in the model.CONCLUSIONS BMI was the most important factor in determining arterial stiffness in children aged 9-10 years.
