OBJECTIVE To analyze the content of policies and action plans within the public healthcare system that addresses the issue of violence against women.METHODS A descriptive and comparative study was conducted on the health policies and plans in Catalonia and Costa Rica from 2005 to 2011. It uses a qualitative methodology with documentary analysis. It is classified by topics that describe and interpret the contents. We considered dimensions, such as principles, strategies, concepts concerning violence against women, health trends, and evaluations.RESULTS Thirteen public policy documents were analyzed. In both countries’ contexts, we have provided an overview of violence against women as a problem whose roots are in gender inequality. The strategies of gender policies that address violence against women are cultural exchange and institutional action within the public healthcare system. The actions of the healthcare sector are expanded into specific plans. The priorities and specificity of actions in healthcare plans were the distinguishing features between the two countries.CONCLUSIONS The common features of the healthcare plans in both the counties include violence against women, use of protocols, detection tasks, care and recovery for women, and professional self-care. Catalonia does not consider healthcare actions with aggressors. Costa Rica has a lower specificity in conceptualization and protocol patterns, as well as a lack of updates concerning health standards in Catalonia.

OBJECTIVE To analyze the relationship between gender violence and suicidal ideation in women with HIV. METHODS A cross-sectional study with 161 users of specialized HIV/AIDS care services. The study investigated the presence of gender violence through the Brazilian version of the World Health Organization Violence against Women instrument, and suicidal ideation through the Suicidal Ideation Questionnaire. Statistical analyses were performed with the SPSS software, using the Chi-square test and Poisson multiple regression model. RESULTS Eighty-two women with HIV reported suicidal ideation (50.0%), 78 (95.0%) of who had suffered gender violence. Age at first sexual intercourse < 15 years old, high number of children, poverty, living with HIV for long, and presence of violence were statistically associated with suicidal ideation. Women who suffered gender violence showed 5.7 times more risk of manifesting suicidal ideation. CONCLUSIONS Women with HIV showed a high prevalence to gender violence and suicidal ideation. Understanding the relationship between these two grievances may contribute to the comprehensive care of these women and implementation of actions to prevent violence and suicide.

OBJECTIVE To analyze the correlation between sociodemographic factors and working conditions of bus workers in a metropolitan area and violence against them.METHODS This cross-sectional study used a nonprobabilistic sample estimated according to the number of workers employed in bus companies located in three cities in the Belo Horizonte metropolitan region in 2012 (N = 17,470). Face-to-face interviews were conducted using a digital questionnaire. The factors associated with violence were analyzed in two stages using Poisson regression, according to each level. The magnitude of the association was evaluated using prevalence ratios with robust variance and a statistical significance of 5%, and 95% confidence intervals were obtained.RESULTS The study sample comprised 782 drivers and 691 fare collectors; 45.0% participants reported at least one act of violence in the workplace in the last 12 months, with passengers being predominantly responsible. The age of the bus workers was inversely associated with violence. Chronic diseases, sickness absenteeism, and working conditions were also associated with violence.CONCLUSIONS The findings on the correlation between violence and working conditions are essential for implementing prevention strategies by transportation service managers.

OBJECTIVE To assess the prevalence and factors associated with intimate partner violence after the diagnosis of sexually transmitted diseases.METHODS This cross-sectional study was conducted in Fortaleza, CE, Northeastern Brazil, in 2012 and involved 221 individuals (40.3% male and 59.7% female) attended to at reference health care units for the treatment of sexually transmitted diseases. Data were collected using a questionnaire applied during interviews with each participant. A multivariate analysis with a logistic regression model was conducted using the stepwise technique. Only the variables with a p value < 0.05 were included in the adjusted analysis. The odds ratio (OR) with 95% confidence interval (CI) was used as the measure of effect.RESULTS A total of 30.3% of the participants reported experiencing some type of violence (27.6%, psychological; 5.9%, physical; and 7.2%, sexual) after the diagnosis of sexually transmitted disease. In the multivariate analysis adjusted to assess intimate partner violence after the revelation of the diagnosis of sexually transmitted diseases, the following variables remained statistically significant: extramarital relations (OR = 3.72; 95%CI 1.91;7.26; p = 0.000), alcohol consumption by the partner (OR = 2.16; 95%CI 1.08;4.33; p = 0.026), history of violence prior to diagnosis (OR = 2.87; 95%CI 1.44;5.69; p = 0.003), and fear of disclosing the diagnosis to the partner (OR = 2.66; 95%CI 1.32;5.32; p = 0.006).CONCLUSIONS Individuals who had extramarital relations, experienced violence prior to the diagnosis of sexually transmitted disease, feared disclosing the diagnosis to the partner, and those whose partner consumed alcohol had an increased likelihood of suffering violence. The high prevalence of intimate partner violence suggests that this population is vulnerable and therefore intervention efforts should be directed to them. Referral health care services for the treatment of sexually transmitted diseases can be strategic places to identify and prevent intimate partner violence.

Serovars of a total of 5,490 Salmonella strains isolated during the period of 1991-95, from human infections (2,254 strains) and from non-human materials (3,236 strains) were evaluated. In the studied period, 81 different serovars were determined among human isolates. Salmonella Enteritidis corresponded to 1.2% in 1991, 2% in 1992, 10.1% in 1993, 43.3% in 1994, and 64.9% in 1995 of all isolates. A significant rise on the isolation of this serovar was seen since 1993 linked to food poisoning outbreaks. It is reported also an increase on the isolation of S. Enteritidis from blood cultures, associated mainly with patients with immunodeficiency syndrome. S. Enteritidis was prevalent among one hundred and thirty different serovars isolated from non-human sources. Increasing number of isolation of this serovar was seen from shell eggs, breeding flocks and from environmental samples. It is also reported a contamination of commercial feed stuffs by S. Enteritidis which represents a major concern for Brazilian poultry industry.

Introduction More than half of the malaria cases reported in the Americas are from the Brazilian Amazon region. While malaria is considered endemic in this region, its geographical distribution is extremely heterogeneous. Therefore, it is important to investigate the distribution of malaria and to determine regions whereby action might be necessary. Methods Changes in malaria indicators in all municipalities of the Brazilian Amazon between 2003-2004 and 2008-2009 were studied. The malaria indicators included the absolute number of malaria cases and deaths, the bi-annual parasite incidence (BPI), BPI ratios and differences, a Lorenz curve and Gini coefficients. Results During the study period, mortality from malaria remained low (0.02% deaths/case), the percent of municipalities that became malaria-free increased from 15.6% to 31.7%, and the Gini coefficient increased from 82% to 87%. In 2003, 10% of the municipalities with the highest BPI accumulated 67% of all malaria cases, compared with 2009, when 10% of the municipalities (with the highest BPI) had 80% of the malaria cases. Conclusions This study described an overall decrease in malaria transmission in the Brazilian Amazon region. As expected, an increased heterogeneity of malaria indicators was found, which reinforces the notion that a single strategy may not bring about uniformly good outcomes. The geographic clustering of municipalities identified as problem areas might help to define better intervention methods.

OBJETIVO: Estudar a população de uma casa de custódia quanto a aspectos criminais, diagnóstico clínico e perfil da vítima. MÉTODOS: Foram examinados os prontuários de 269 pacientes durante o ano de 2005. Considerou-se apenas a população do gênero masculino cujos casos já tinham laudo anexado ao prontuário psiquiátrico-criminal. RESULTADOS: Foi encontrado predomínio de transtornos psicóticos (58%). O crime mais freqüente foi contra a vida (52,8%), sendo o grupo dos pacientes psicóticos o que teve maior associação com esse tipo de crime (p < 0,05). Desses crimes, 89,7% resultaram em morte e em 34,5% a vítima era um parente próximo. Os sujeitos com retardo mental cometeram proporcionalmente mais crimes sexuais quando comparados com os pacientes psicóticos e considerando somente crime sexual ou contra a vida (p < 0,05). Em 78,5% dos crimes sexuais as vítimas tinham idade inferior a 14 anos. CONCLUSÃO: A população estudada é semelhante às de outras instituições com o mesmo perfil. Os achados em relação às características das vítimas, tanto nos casos de homicídio pelos psicóticos como nos crimes sexuais dos sujeitos com retardo mental, indicam que aspectos da vítima têm papel importante no crime.

OBJETIVO: Estudar, mediante uma revisão sistemática da literatura científica, a relação esquizofrenia, psicose e violência. MÉTODOS: Realizou-se uma busca eletrônica por meio das bases de dados Medline, SciELO e Lilacs, até a data de junho de 2006, considerando artigos de línguas inglesa e portuguesa. RESULTADOS: Um número de pesquisas revela associação entre esquizofrenia e comportamento violento, principalmente quando existe comorbidade com abuso de substâncias. Aspectos específicos dos delírios são relacionados com comportamento violento, como maior grau de convicção e presença de delírios de controle e perseguição. CONCLUSÕES: Apesar de limitações metodológicas, alguns aspectos da psicopatologia aguda da psicose e da comorbidade de abuso de substâncias parecem estar fortemente relacionados à presença de comportamento violento entre pacientes psicóticos. De acordo com os estudos analisados, somente uma pequena parcela da violência social poder ser atribuída a esse grupo de pacientes. Novas pesquisas deverão futuramente permitir prever antecipadamente o risco de um comportamento violento, permitindo com isso intervenções preventivas e redução do processo de estigmatização.

OBJETIVO: Estudar os aspectos do delírio que podem estar relacionados à ocorrência de crime violento por pacientes delirantes. MÉTODOS: Estudo caso-controle retrospectivo comparando dois grupos de 30 pacientes psicóticos delirantes. O grupo estudado consiste de pacientes delirantes internados em uma Casa de Custódia do estado de São Paulo, Brasil, e o grupo comparado consiste de pacientes de enfermarias psiquiátricas comuns. Foram utilizadas as escalas PANSS, MINI e MMDAS. RESULTADOS: Em relação às dimensões do delírio, o grupo-caso teve menor pontuação em "inibição de ação por causa do delírio" e "afeto negativo". CONCLUSÃO: Delírios que induzem a inibição de ações aparentemente também reduzem o potencial de ações violentas e, ao contrário do que se afirma correntemente, pacientes delirantes assustados ou com outros afetos negativos associados ao delírio parecem cometer menos atos violentos. Portanto, fatores intrínsecos inerentes a algumas dimensões do delírio podem ser relevantes na ocorrência de crimes violentos cometidos por pacientes psicóticos.

OBJETIVOS: Estudar a população internada em um hospital de custódia no Rio de Janeiro quanto a aspectos demográficos, diagnósticos e criminais. MÉTODOS: Todos os internos cumprindo medida de segurança detentiva no Hospital de Custódia e Tratamento Psiquiá­trico Heitor Carrilho, em dezembro de 2007, (n = 177) foram avaliados pelo censo sociodemográfico aplicado por psiquiatras da instituição e tiveram seus prontuários analisados quanto a diagnóstico, tratamento psiquiátrico prévio e tipo de crime. Foi avaliada a relação entre vítima e perpetrador nos homicídios. RESULTADOS: A população é preferencialmente masculina (80%), solteira (72%), com 30 a 39 anos de idade (34%), baixa escolaridade (69%) e inativa (56%). Os diagnósticos mais prevalentes foram transtornos psicóticos (67%), seguidos por retardo mental (15,2%), transtornos em virtude de uso de substâncias psicoativas (7,3%), de personalidade (4,5%) e outros (6,2%). A maioria (71%) já havia recebido tratamento psiquiátrico prévio. O homicídio foi o crime mais comum (44%), seguido por crimes contra o patrimônio (26%), crimes sexuais (11%), crimes relacionados a entorpecentes (11%) e outros. O homicídio intrafamiliar predominou entre os psicóticos e os portadores de retardo mental. Os últimos cometeram proporcionalmente mais crimes sexuais do que os primeiros. CONCLUSÃO: O perfil da população foi compatível com o descrito para outras populações de internos em hospitais de custódia no país.

A survey of the autopsy data on hepatosplenic schistosomiasis during periods, before and after the advent of new chemotherapeutic drugs, revealed that: a) the pathological presentation was the same for the two periods; b) the number of cases in the last five years is progressively decreasing; c) hepatosplenic disease due to schistosomiasis is becoming rare in young people. These data represent a change in the pattern of pathology in schistosomiasis, probably related to new chemotherapy.

About one third of the world population is infected with tubercle bacilli, causing eight million new cases of tuberculosis (TB) and three million deaths each year. After years of lack of interest in the disease, World Health Organization recently declared TB a global emergency and it is clear that there is need for more efficient national TB programs and newly defined research priorities. A more complete epidemiology of tuberculosis will lead to a better identification of index cases and to a more efficient treatment of the disease. Recently, new molecular tools became available for the identification of strains of Mycobacterium tuberculosis (M. tuberculosis), allowing a better recognition of transmission routes of defined strains. Both a standardized restriction-fragment-length-polymorphism-based methodology for epidemiological studies on a large scale and deoxyribonucleic acids (DNA) amplification-based methods that allow rapid detection of outbreaks with multidrug-resistant (MDR) strains, often characterized by high mortality rates, have been developed. This review comments on the existing methods of DNA-based recognition of M. tuberculosis strains and their peculiarities. It also summarizes literature data on the application of molecular fingerprinting for detection of outbreaks of M. tuberculosis, for identification of index cases, for study of interaction between TB and infection with the human immunodeficiency virus, for analysis of the behavior of MDR strains, for a better understanding of risk factors for transmission of TB within communities and for population-based studies of TB transmission within and between countries

Predictions that deforestation would reduce American cutaneous leishmaniasis incidence have proved incorrect. Presentations at a recent international workshop, instead, demonstrated frequent domestication of transmission throughout Latin America. While posing new threats, this process also increases the effectiveness of vector control in and around houses. New approaches for sand fly control and effective targeting of resources are reviewed.

Between March 2000 and December 2001 a survey of the sand flies (Diptera: Phlebotominae) of French Guiana was carried out during 14 nights of captures with CDC light-traps and Malaise traps, and resulted in the collection of 2245 individuals of 38 species. The most abundant species were Lutzomyia (Trichophoromyia) ininii Floch & Abonnenc, Lu.(Psychodopygus) squamiventris maripaensis Floch & Abonnenc, and Lu .(Nyssomyia) flaviscutellata Mangabeira. Half of the collected sand flies females were dissected under field conditions and five species were found harboring Leishmania-like parasites. The Leishmania (Kinetoplastidae: Trypanosomatidae) species were identified by molecular typing, and for the first time Lu. (Nys.) flaviscutellata was found harboring Leishmania (Viannia) guyanensis and Lu. (Tri) ininii harboring unknown Leishmania. The first record for French Guiana of Lu. (Psy.) squamiventris maripaensis harboring L. (V.) naiffi, was also reported. The patterns of diversification of the human cutaneous leishmaniasis transmission in French Guiana are discussed.
