OBJETIVO: Analisar a qualidade de vida relacionada à saúde de pacientes com osteoporose e compará-la com a população geral. MÉTODOS: Foi realizado um estudo descritivo transversal com 60 pacientes do sexo feminino no serviço de reumatologia de um hospital universitário na Espanha, de abril a outubro de 2003. Foi aplicado o questionário Short Form-36, abordando dados demográficos, características clínicas e dados sobre estilos de vida relacionados à saúde. As pacientes foram classificadas em grupos etários. Foram utilizados os seguintes testes estatísticos: qui-quadrado, modelo linear geral, t de Student. RESULTADOS: As entrevistadas tinham idade média de 65,57 (DP: ±9,7 anos), e tempo de diagnóstico médio de 3,4±2,84 anos. As melhores pontuações foram obtidas nas dimensões aspectos sociais (89), aspectos emocionais (72,2) e saúde mental (63). As mais baixas foram em estado geral de saúde (45,1), capacidade funcional (47,7), dor (52,3) e aspectos físicos (59,9). As pontuações médias dos pacientes resultaram inferiores às pontuações conhecidas da população geral espanhola nas dimensões capacidade funcional, aspectos físicos, dor e estado geral de saúde. As máximas diferenças entre as pontuações médias do SF-36 dos pacientes e os valores populacionais espanhóis correspondem ao grupo de idade de 55 a 64 anos. Nas demais dimensões do SF-36, as pontuações foram inferiores ou similares aos valores populacionais espanhóis. Não se encontraram associações significativas entre as dimensões do SF-36 estudadas e os dados clínicos, demográficos e de estilos de vida analisados. CONCLUSÕES: Os pacientes apresentaram baixa qualidade de vida, sobretudo nas dimensões mais relevantes da enfermidade, quando comparada com valores da população espanhola em geral. As áreas físicas foram as mais afetadas.

Patients with megaesophagus (ME) have increased prevalence of cancer of the esophagus. In contrast, a higher incidence of colorectal cancer is not observed in patients with megacolon (MC). MC is very common in some regions of Brazil, where it is mainly associated with Chagas disease. We reviewed the pathology records of surgical specimens of all patients submitted for surgical resection of MC in the Hospital das Clínicas of the Faculty of Medicine of Ribeirão Preto (HC-FMRP), from the University of São Paulo. We found that 894 patients were operated from 1952 until 2001 for MC resection. Mucosal ulcers, hyperplasia and chronic inflammation were frequently found, while polyps were uncommon. No patients with MC presented any type of colonic neoplasm. This observation reinforces the hypothesis that MC has a negative association with cancer of the colon. This seems to contradict the traditional concept of carcinogenesis in the colon, since patients with MC presents important chronic constipation that is thought to cause an increase in risk for colon cancer. MC is also associated with other risk factors for cancer of colon, such as hyperplasia, mucosal ulcers and chronic inflammation. In ME these factors lead to a remarkable increase in cancer risk. The study of mucosal cell proliferation in MC may provide new insights and useful information about the role of constipation in colonic carcinogenesis.

INTRODUCTION: The main extra-hepatic manifestation of hepatitis C is mixed cryoglobulinemia (MC). The aim of this study was to evaluate its prevalence among patients with chronic hepatitis C (CHC), to correlate its presence to host and virological variables and to the response to combined therapy with interferon-alpha and ribavirin. CASUISTIC AND METHODS: 202 CHC naive patients (136 with chronic hepatitis and 66 with cirrhosis) were consecutively evaluated for the presence of cryoglobulins. Cryoprecipitates were characterized by immunoelectrophoresis and classified according to the Brouet's criteria. RESULTS: The prevalence of MC was 27% (54/202), and 24% of them (13/54) showed major clinical manifestation of the disease. Even though type III MC was more frequent (78%), symptomatic MC was more common in type II MC. The presence of cirrhosis (RR = 2.073; IC95% = 1.029 - 4.179; p = 0.041), and age of the patients (RR = 1.035; IC95% = 1.008 - 1.062; p = 0.01) were independently associated with the presence of cryoglobulins. No relationship was found with viral load and genotype. 102 patients were treated with interferon alpha and ribavirin. Among these, 31 had MC. Sustained virological response (around 30%) was similar in patients with and without MC (p = 0.971). CONCLUSION: MC represents a prevalent complication in patients with CHC, specially older and cirrhotic patients. Only 24% of these patients show clinical manifestation of the disease, specially those with type II MC. The presence of MC did not affect the response to therapy.

The aim of this study was to analyze the prevalence of Toxocara spp. antibodies in children from two different socioeconomic classes in the Presidente Prudente municipality, São Paulo State, Brazil, and the protective and risk factors associated with toxocariasis. One hundred and twenty-six middle-class (MC) and 126 disadvantaged children (DC) were included in this study. Anti-Toxocara ELISA test was performed in order to evaluate seroprevalence. A survey was applied to the children's guardians/parents in order to analyze the protective and risk factors. The overall prevalence was 11.1%, and of 9.5% (12/126) and 12.7% (16/126) for MC and DC subgroups, respectively. Toxocara seropositivity was inversely proportional to the family income. A high household income was considered a protective factor for toxocariasis in the total population and in both MC and DC subgroups. Being a girl was considered a protective factor for the total population and for both subgroups. Whilst being an owner of cat was a risk factor for children belonging to the total and for both MC and DC subgroups, having dog was considered as a risk factor for only the MC. Epidemiologic protective/factor risks can be distinct depending on the strata of the same population. Thus, it is relevant to evaluate these factors independently for different socioeconomic classes in order to design future investigations and programs for preventing the infection of human beings by Toxocara spp. and other geohelminths.

O Programa de Controle da Esquistossomose (PCE) vem sendo desenvolvido em áreas da Bahia pela Fundação Nacional de Saúde (FNS). Em 1989, ações de Informação, Educação, Comunicação e Mobilização Comunitária (IEC/MC) foram iniciadas. Neste estudo avalia-se o impacto epidemiológico do IEC/MC, adotando-se um desenho quasi-experimental, comparando-se prevalências de infecção por S. mansoni em áreas IEC/MC com estimativas de áreas referentes. Os dados são secundários, coletados rotineiramente pela FNS. Verificou-se uma redução da prevalência da esquistossomose em todas as áreas, que alcança maior intensidade nas áreas com IEC/MC. Aparentemente, ações de controle rotineiras realizadas isoladamente são mais efetivas entre escolares e pessoas do sexo masculino, enquanto que nas áreas com IEC/MC, observou-se maior impacto entre as mulheres, refletindo, provavelmente, as distintas estratégias adotadas. Aponta-se para a necessidade de estudos de avaliação qualitativos, além de estimativas do custo-benefício e custo-efetividade de modo a aprimorar o processo de tomada de decisões.

Foi feito um inventário de seis hectares de floresta de. terra firme na área de influência da Estrada Cuiabá-Porto Velho (BR-364). Destes seis hectares, dois foram feitos no Município de Jaru, vicinal 605 e quatro na área do Projeto Machadinho, vicinais MC-2, MA-9. A flonesta apresenta uma altura média de 15m e um total de 278 espécies diferentes, representadas por 2.235 indivíduos e 57 famílias nos seis hectares estudados. Os hectares I e II do Município de Jaru apresentaram, respectivamente, uma diversidade florística de 113 e 136 espécies de 10cm ou mais de diâmetro (DAP), enquanto que os hectares III, IV, V e VI da área do Projeto Machadinho apresentaram respectivamente, 103, 115, 122 e. 121 espécies. A espécie mais importante no hectare I é o breu manga (Tetragastris altíssima (Aubl.) Swartz) com o IVI (Índice de Valor de Importância) de 12,07, o que representa 4,0%do total; no hectare II, muiraquatiara (Astronium lecointelDucke) com o IVI 28,94, ou se,ja 9,6% do total; no hectare III, Macrolobiumsp., com o IVI de 28,94, representando 9,6% do total; no hectare IV, pau d'alho (Gallesia integrifolia(Sprenq.) Harms), com IVI de 39,41, representando 13,1% do total; no hectare V, tauari (Couratari macrospermaA.S. Smith), com IVI de 11,32, ou seja, 3,8% do total; no hectare VI, violeta Peltogyne catingae Ducke subsp. glabra(W. Rodr.) M.F. da Silva), com o IVI de 11,68, representando 3,9% do total. As seis famílias mais importantes em ordem de importância são: Leguminosae, Moraceae, Sapotaceae, Lecythidaceae, Bursenaceae e Pal mae. Os dados quantitativos da regeneração natural indicaram que as espécies que apre-sentaram maior número de indivíduos regenerando, foram: Duguetia flagellaresHuber e Maquirasp. no Município de Jaru e Coussareasp. e. Micrandrasp. na área do Projeto Macha dinho, em Ariquemes.

El picudo, Conotrachelus dubiae O'Brien 1995, es una de las plagas mas importantes del camu camu Myrciaria dubia H.B.K. Mc Vaugh en la Amazonía Peruana. El objetivo del presente estudio fue determinar el ciclo biológico de este insecto bajo condiciones de laboratorio y describir su comportamiento y fluctuación en condiciones de campo en Pucallpa, Ucayali, Perú. El porcentaje de eclosión de larvas fue de 87%, la duración del periodo de incubación de los huevos fue de 5,5±0,9 (4 a 7) días, del estado larval en el fruto 22,2±1,9 (20 a 25) días y en el suelo (fase pre-pupa), 54,4±5,5 (46 a 67) días, del periodo pupal 11,8±0,9 (9 a 13) días y la longevidad del adulto fue de 51,8±18,9 (9 a 75) días. Los adultos se alimentaron de frutos de diferentes diámetros y estados de maduración y de botones florales, ramas tiernas y flores. No se registró la presencia de adultos de C. dubiae en frutos secos, ni en la base del tallo, sino en ritidomas. La mayor actividad de alimentación y de reproducción de los adultos fue entre 18:30 a 22:00 h. Los adultos fueron observados en el cultivo durante todo el año, encontrándose con mayor frecuencia en los meses de enero a marzo en pisos bajos inundables y entre octubre a diciembre en tierra firme no inundable, coincidiendo con la fase de floración y fructificación de la planta.

OBJETIVO: Os meios de contraste (MC) introduzem alterações em alguns parâmetros sangüíneos, adquirindo, assim, mais importância na angiocardiografia pediátrica. MÉTODOS: Estudamos a presença e a severidade das mudanças no hematócrito, hemoglobina, leucócitos, sódio, potássio, cálcio, osmolalidade e viscosidade, em 35 crianças submetidas a angiocardiografia com ioxaglato, identificando, também, as variáveis independentes responsáveis por essas alterações. As amostras sangüíneas foram colhidas no início do procedimento (S1), no fim (S2) e 2h após (S3). RESULTADOS: Hematócrito: S1= 47,3±6,9%; S2= 40,7±7,4% (p<0,001) (relacionado com o ioxaglato r=0,37, p<0,05). Hemoglobina: S1= 15±2,1g%; S2= 13,2±2,4g% (p<0,001). Leucócitos: S1= 7940±3040 leucócitos/mm³; S2= 6950± 2700/mm³ (NS); S3= 10830± 4690, (p<0,001). Identificamos a duração do procedimento (r=0,38, p<0,05) e o volume total de soro glicosado (r=0,49, p<0,05). Sódio: S1= 134,5±0,4mEq/L; S2= 130,7±0,4mEq/L (p<0,001) relacionado à solução glicosada (r=0,61, p<0,01). Potássio: S1= 4,22± 0,45mEq/L; S2= 3,83±0,4mEq/L (p<0,001). Nenhuma variável foi isolada. Cálcio; S1= 9,13±1,03mg%; S2= 8,4±0,91mg% (relacionado ao ioxaglato r=0,43, p<0,01). Osmolalidade: S1= 293,3±12,5mOsm/kg; S2= 300,6±13,3mOsm/kg (p<0,001). Nenhuma das variáveis independentes foi isolada. Viscosidade: S1= 3,36±0,81; S2= 3,09±0,74 (p<0,01); S3= 3,87±0,89, p<0,001 (devido a uma regressão linear indireta com o MC). CONCLUSÃO: Variações significativas nas variáveis dependentes medidas foram observadas durante e após o procedimento. O uso do ioxaglato foi parcialmente relacionado a mudanças no hematócrito/hemoglobina, cálcio total e viscosidade mas não as das variáveis restantes.

FUNDAMENTO: A sedação durante a cineangiocoronariografia tem sido pouco estudada e saber qual é a melhor droga para sedar esses pacientes é um questionamento importante. OBJETIVO: Avaliar a qualidade da sedação e os efeitos sobre a freqüência cardíaca (FC) e a pressão arterial (PA) do midazolam e do diazepam, associados ou não a clonidina, em pacientes com suspeita de doença coronariana. MÉTODOS: Foi desenvolvido ensaio clínico prospectivo, duplo-cego, randomizado, controlado, com 160 pacientes divididos em cinco grupos de 32 pacientes cada, de acordo com o fármaco utilizado: grupo C (clonidina 0,5 µg/kg); grupo M (midazolam 40 µg/kg); grupo MC (associação de midazolam 40 µg/kg e clonidina 0,5 µg/kg); grupo D (diazepam 40 µg.kg); e grupo DC (associação de diazepam 40 µg/kg e clonidina 0,5 µg/kg). A sedação foi avaliada com base na escala de Ramsay e no consumo de meperidina 0,04 mg.kg-1. A PA invasiva, a FC e o escore de sedação foram analisados a cada cinco minutos em quatro diferentes momentos. RESULTADOS: Os pacientes que utilizaram midazolam apresentaram maiores escores de sedação e variação da FC e da PA (p < 0,05). Os que utilizaram diazepam ou clonidina tiveram menores escores de sedação e mais satisfatórios para a realização do exame e apresentaram menor variação da PA e da FC (p > 0,05). CONCLUSÃO: O midazolam foi associado a maior efeito sedativo e cardiovascular enquanto o diazepam causou menor efeito sedativo e cardiovascular. A clonidina e o diazepam tiveram efeitos semelhantes na PA, na FC e na sedação.

1) The first part deals with the different processes which may complicate Mendelian segregation and which may be classified into three groups, according to BRIEGER (1937b) : a) Instability of genes, b) Abnormal segregation due to distur- bances during the meiotic divisions, c) obscured segregation, after a perfectly normal meiosis, caused by elimination or during the gonophase (gametophyte in higher plants), or during zygophase (sporophyte). Without entering into detail, it is emphasized that all the above mentioned complications in the segregation of some genes may be caused by the action of other genes. Thus in maize, the instability of the Al factor is observed only when the gene dt is presente in the homozygous conditions (RHOADES 1938). In another case, still under observation in Piracicaba, an instability is observed in Mirabilis with regard to two pairs of alleles both controlling flower color. Several cases are known, especially in corn, where recessive genes, when homozigous, affect the course of meiosis, causing asynapsis (asyndesis) (BEADLE AND MC CLINTOCK 1928, BEADLE 1930), sticky chromosomes (BEADLE 1932), supermunmerary divisions (BEADLE 1931). The most extreme case of an obscured segregatiou is represented by the action of the S factors in self stetrile plants. An additional proof of EAST AND MANGELSDORF (1925) genetic formula of self sterility has been contributed by the studies on Jinked factors in Nicotina (BRIEGER AND MANGELSDORF (1926) and Antirrhinum (BRIEGER 1930, 1935), In cases of a incomplete competition and selection between pollen tubes, studies of linked indicator-genes are indispensable in the genetic analysis, since it is impossible to analyse the factors for gametophyte competition by direct aproach. 2) The flower structure of corn is explained, and stated that the particularites of floral biology make maize an excellent object for the study of gametophyte factors. Since only one pollen tube per ovule may accomplish fertilization, the competition is always extremely strong, as compared with other species possessing multi-ovulate ovaries. The lenght of the silk permitts the study of pollen tube competitions over a varying distance. Finally the genetic analysis of grains characters (endosperm and aleoron) simpliflen the experimental work considerably, by allowing the accumulation of large numbers for statistical treatment. 3) The four methods for analyzing the naturing of pollen tube competition are discussed, following BRIEGER (1930). Of these the first three are: a) polinization with a small number of pollen grains, b) polinization at different times and c) cut- ting the style after the faster tubes have passe dand before the slower tubes have reached the point where the stigma will be cut. d) The fourth method, alteration of the distatice over which competition takes place, has been applied largely in corn. The basic conceptions underlying this process, are illustrated in Fig. 3. While BRINK (1925) and MANGELSDORF (1929) applied pollen at different levels on the silks, the remaining authors (JONES, 1922, MANGELSDORF 1929, BRIEGER, at al. 1938) have used a different process. The pollen was applied as usual, after removing the main part of the silks, but the ears were divided transversally into halves or quarters before counting. The experiments showed generally an increase in the intensity of competition when there was increase of the distance over which they had to travel. Only MANGELSDORF found an interesting exception. When the distance became extreme, the initially slower tubes seemed to become finally the faster ones. 4) Methods of genetic and statistical analysis are discussed, following chiefly BRIEGER (1937a and 1937b). A formula is given to determine the intensity of ellimination in three point experiments. 5) The few facts are cited which give some indication about the physiological mechanism of gametophyte competition. They are four in number a) the growth rate depends-only on the action of gametophyte factors; b) there is an interaction between the conductive tissue of the stigma or style and the pollen tubes, mainly in self-sterile plants; c) after self-pollination necrosis starts in the tissue of the stigma, in some orchids after F. MÜLLER (1867); d) in pollon mixtures there is an inhibitory interaction between two types of pollen and the female tissue; Gossypium according to BALLS (1911), KEARNEY 1923, 1928, KEARNEY AND HARRISON (1924). A more complete discussion is found in BRIEGER 1930). 6) A list of the gametophyte factors so far localized in corn is given. CHROMOSOME IV Ga 1 : MANGELSDORF AND JONES (1925), EMERSON 1934). Ga 4 : BRIEGER (1945b). Sp 1 : MANGELSDORF (1931), SINGLETON AND MANGELSDORF (1940), BRIEGER (1945a). CHROMOSOME V Ga 2 : BRIEGER (1937a). CHROMOSOME VI BRIEGER, TIDBURY AND TSENG (1938) found indications of a gametophyte factor altering the segregation of yellow endosperm y1. CHROMOSOME IX Ga 3 : BRIEGER, TIDBURY AND TSENG (1938). While the competition in these six cases is essentially determined by one pair of factors, the degree of elimination may be variable, as shown for Ga2 (BRIEGER, 1937), for Ga4 (BRIEGER 1945a) and for Spl (SINGLETON AND MANGELSDORF 1940, BRIEGER 1945b). The action of a gametophyte factor altering the segregation of waxy (perhaps Ga3) is increased by the presence of the sul factor which thus acts as a modifier (BRINCK AND BURNHAM 1927). A polyfactorial case of gametophyte competition has been found by JONES (1922) and analysed by DEMEREC (1929) in rice pop corn which rejects the pollen tubes of other types of corn. Preference for selfing or for brothers-sister mating and partial elimination of other pollen tubes has been described by BRIEGER (1936). 7) HARLAND'S (1943) very ingenious idea is discussed to use pollen tube factors in applied genetics in order to build up an obstacle to natural crossing as a consequence of the rapid pollen tube growth after selfing. Unfortunately, HARLAND could not obtain the experimental proof of the praticability of his idea, during his experiments on selection for minor modifiers for pollen tube grouth in cotton. In maize it should be possible to employ gametophyte factors to build up lines with preference for crossing, though the method should hardly be of any practical advantage.
