In order to upgrade the reliability of xenodiagnosis, attention has been directed towards population dynamics of the parasite, with particular interest for the following factors: 1. Parasite density which by itself is not a research objective, but by giving an accurate portrayal of parasite development and multiplication, has been incorporated in screening of bugs for xenodiagnosis. 2. On the assumption that food availability might increase parasite density, bugs from xenodiagnosis have been refed at biweekly intervals on chicken blood. 3. Infectivity rates and positives harbouring large parasite yields were based on gut infections, in which the parasite population comprised of all developmental forms was more abundant and easier to detect than in fecal infections, thus minimizing the probability of recording false negatives. 4. Since parasite density, low in the first 15 days of infection, increases rapidly in the following 30 days, the interval of 45 days has been adopted for routine examination of bugs from xenodiagnosis. By following the enumerated measures, all aiming to reduce false negative cases, we are getting closer to a reliable xenodiagnostic procedure. Upgrading the efficacy of xenodiagnosis is also dependent on the xenodiagnostic agent. Of 9 investigated vector species, Panstrongylus megistus deserves top priority as a xenodiagnostic agent. Its extraordinary capability to support fast development and vigorous multiplication of the few parasites, ingested from the host with chronic Chagas' disease, has been revealed by the strikingly close infectivity rates of 91.2% vs. 96.4% among bugs engorged from the same host in the chronic and acute phase of the disease respectively (Table V), the latter comporting an estimated number of 12.3 x 10[raised to the power of 3] parasites in the circulation at the time of xenodiagnosis, as reported previously by the authors (1982).

A specific antiserum to Candida albicans serotype A was prepared adsorbing a total antiserum with Candida albicans serotype B cells. This specific antiserum was used for serotyping C. albicans strains obtained from patients in different hospitals of Havana City, Cuba. Two hundred strains (95.2%) were serotype A, the remaining 10 (4.8%) were serotype B. Results were also correlated with strains isolated from the specimen origin, sex and race of the patient. The usefulness of this specific antiserum to determine C. albicans serotypes and its therapeutic value are pointed out.

Vector species has not hitherto been studied as influencing metacyclogenesis of Trypanosoma cruzi, while the role of the parasite strain has been frequently stressed as of dominant importance in this process. In order to fill this gap in our knowledge, metacyclogenesis was monitored in nine triatomine species. The first part of this paper presents photographs of the main and intermediate parasite stages in each vector species studied. In the second part of the study the proportional distribution of all these forms, as seen in Giemsa stained smears is summarized, thus providing an opportunity to analyze both: the length of time between the ingestion of the blood trypomastigotes and the appearance of metacyclic forms and the rates of developmental stages leading to these latter. The most remarkable observation was that metacyclogenesis rates in vivo appear to be vector dependent, reaching 50 in Rhodnius neglectus, 37 in its congener R. prolixus and being dramatically lower in the majority of Triatoma species (5 in T. sordida, 3 in T. brasiliensis and 0 in T. pseudomaculata) at the 120th day of infection. These observations suggest that through screening of different vector species it is possible to find some that are capable of minimizing or maximizing metacyclic production.

A recombinant-antigen enzyme immunoassay (EIA), BioSCREEN TM anti-Treponema pallidum, was compared favorably with the T. pallidum hemagglutination test, in the detection of specific antibodies in different groups of sera from patients with primary (n = 38), secondary (n = 10), early latent (n = 28) and congenital syphilis (n = 2), patients with leptospirosis ( n= 8), infectious mononucleosis (n = 7), hepatitis (n = 9), diabetes mellitus (n = 11), rheumatoid arthritis (n = 13), leprosy (n = 11), tuberculosis (n = 9), HIV/Aids ( n= 12), systemic lupus erythematosus (n = 4), rheumatic fever (n = 3), old-persons (n = 9), pregnant women (n = 29) and blood donors (n = 164). The coincidence between them was 95.1%. The sensitivity and specificity of the EIA were 93.3% and 95.5%, respectively. Fifteen serum specimens belonging to old-persons, pregnant women, blood donors, and patients with human leptospirosis, hepatitis, diabetes mellitus, tuberculosis and rheumatic fever gave false-positive results by Venereal Disease Research Laboratory and/or Rapid Plasma Reagin. The EIA can be used as alternative method for the serological confirmation of syphilis.

To determine the prevalence rates and serovar distribution of Chlamydia trachomatis cervical infections in Cuban women, two different groups were selected. Group I consisted of 60 human immunodeficiency virus (HIV-1) seropositive women from different regions of Cuba and group II of 60 randomly selected women HIV seronegative and apparently healthy. C. trachomatis was detected in cervical scrapes by mean of nested polymerase chain reaction (PCR) specific for major out membrane protein. The overall prevalence rate of C. trachomatis in cervical scrapes determined by nested PCR was 10% in group I and the estimated prevalence was 6.6% for group II; 83.3% of HIV seropositive women with C. trachomatis infection reported history of pelvic inflammatory disease followed by cervicitis (50%). The control group C. trachomatis-infected women referred a history of cervicitis in 75% of cases. Other reports in the latter group included infertility and pelvic inflamatory disease in 50%. The present study is the first report of C. trachomatis prevalence in Cuba. It showed that there was not significantly difference in the prevalence rate of C. trachomatis between both groups.

Four methods (chromogenic, acidimetric, inhibition, and iodometric) for demonstration of the beta-lactamase production by 70 isolates of Neisseria gonorrhoeae, were evaluated in Cuba. There was 100% correlation between all beta-lactamase methods and the standardized penicillin dilution susceptibility test for penicillinase-non-producing N. gonorrhoeae. For penicillinase-producing N. gonorrhoeae strains, there was a perfect correlation between the chromogenic method and penicillin susceptibility testing, but one and two strains failed to give a positive result for beta-lactamase with the inhibition/acidimetric and the iodometric methods, respectively. There was a high concordance between the chromogenic method, considered as gold standard and the rest of penicillinase tests evaluated: Kappa Index (KI) = 0.98 for inhibition/acidimetric methods and KI = 0.97 for the iodometric method. The four methods evaluated were accurate, reproducible, easily readable, economical, and ease to use for screening primary isolates of N. gonorrhoeae in Cuba. We recommended the use of the inhibition method, when testing the penicillinase activity in gonococcal isolates in provincial and municipal reference laboratories.

OBJETIVO: Comparar a qualidade de imagens ultrassonográficas do abdome de crianças, obtidas com e sem a instituição de jejum prévio. MATERIAIS E MÉTODOS: Trata-se de estudo prospectivo, incluindo crianças com até 12 anos de idade. Os pacientes foram examinados sequencialmente por dois utrassonografistas e as imagens foram classificadas em escores: 1 (não visualizado ou parcialmente visualizado, inadequada para diagnóstico); 2 (suficientes para diagnóstico); 3 (excelentes). As imagens foram ainda classificadas como "diagnósticas" ou "não diagnósticas". RESULTADOS: Foram examinados 77 pacientes, sendo 47 meninos e 30 meninas, com idades entre 0 e 12 anos (mediana de 1 ano). Jejum se mostrou vantajoso de forma estatisticamente significativa apenas na avaliação da vesícula biliar, por apenas um dos avaliadores (p = 0,032). Depois de agrupadas em "diagnóstica" ou "não diagnóstica", nenhuma diferença foi observada entre os grupos. CONCLUSÃO: A instituição de jejum não afetou de forma significativa a qualidade das imagens de ultrassonografias abdominais obtidas em crianças.

OBJETIVO: avaliar a aplicação de um programa de intervenção multidisciplinar educativo em mulheres com gestação de alto risco devido a doenças endócrinas. MÉTODOS: avaliamos retrospectivamente a aplicação de um programa educativo multidisciplinar em 185 gestantes com doenças endócrinas referenciadas para uma maternidade especializada em gestação de alto risco. As gestantes receberam atenção pré-natal multidisciplinar por times compostos por endocrinologistas, obstetras, ultrassonografistas, enfermeiras e nutricionistas. Informações orais e escritas sobre hábitos saudáveis, cuidados com diabetes, uso de adoçantes artificiais e exercícios na gestação foram passadas na primeira consulta endocrinológica. Plano nutricional individualizado foi feito em primeira visita à nutricionista. Nas reavaliações mensais com nutricionista e quinzenais com endocrinologista, as informações sobre mudanças saudáveis no estilo de vida eram reforçadas e o peso registrado. O grau de aderência à dieta e atividade física foi autorreferido. Comparou-se o peso semanal antes e após a intervenção multidisciplinar, peso fetal ao nascimento, taxa de macrossomia e baixo peso, e a frequência de parto cesário nas quatro categorias de índice de massa corpórea (IMC) pré-gestacional (<18,5; 18,5 a 24,9; 25 a 29,9 e >30 kg/m²). RESULTADOS: a principal patologia de encaminhamento foi o diabetes (84,9%). Um terço das gestantes (31,2%) era composto por portadoras de sobrepeso e 42,5% tinham obesidade pré-gestacional. A maior parte das gestantes foi vista pela primeira vez pela equipe multidisciplinar no terceiro trimestre (64,1%), e 50,5% delas excederam o ganho de peso recomendado para toda a gestação à primeira avaliação. Gestantes obesas excederam o ganho de peso recomendado em 62,5% dos casos. Após a intervenção multidisciplinar, o percentual de gestantes que excedeu o ganho de peso semanal recomendado reduziu em todas as categorias de IMC pré-gestacional (IMCPG), embora diferença estatisticamente significativa tenha sido encontrada apenas no grupo com IMCPG normal (40,6 versus 21,9%, p=0,03). Média do peso fetal ao nascer foi similar entre as categoriais de IMC (p=0,277); entretanto, macrossomia foi mais frequente nas mulheres com sobrepeso e obesidade pré-gestacional. Cesariana foi a via mais frequente de parto, independentemente do IMCPG. CONCLUSÕES: em gestações de alto risco devido a doenças endócrinas, abordagem multidisciplinar educativa limita o excessivo ganho de peso semanal apesar da idade gestacional avançada.

PURPOSE: To evaluate the frequency of fear of needles and the impact of a multidisciplinary educational program in women with pre-gestational and gestational diabetes taking insulin during pregnancy. METHODS: The short Diabetes Fear of Injecting and Self-testing Questionnaire (D-FISQ), composed by two subscales that access fear of self injection (FSI) and fear of self testing (FST), was administered twice during pregnancy to 65 pregnant women with pre-gestational and gestational diabetes: at the first endocrine consult and within the last two weeks of pregnancy or postpartum. An organized multidisciplinary program provided diabetes education during pregnancy. Statistical analysis was carried out by Wilcoxon and McNemar tests and Spearman correlation. A p<0.05 was considered to be significant. RESULTS: Data from the short D-FISQ questionnaire shows that 43.1% of pregnant women were afraid of needles in the first evaluation. There was a significant reduction in scores for FSI and FST subscales between the first and second assessments (first FSI 38.5% compared with second 12.7%, p=0.001; first FST 27.7% compared with second FST 14.3%, p=0.012). CONCLUSIONS: The fear of needles is common in pregnant women on insulin therapy and an organized multidisciplinary educational diabetes program applied during pregnancy reduces scores of such fear.

Objetivo Apresentar e validar um registro eletrônico de saúde (RES) multifuncional para atendimento ambulatorial a portadoras de endocrinopatias na gestação e comparar a taxa de preenchimento de informações de saúde com o prontuário convencional. Métodos Desenvolvemos um RES denominado Ambulatório de Endocrinopatias na Gestação eletrônico (AMBEG) para registro sistematizado das informações de saúde. O AMBEG foi utilizado para atendimento obstétrico e endocrinológico de gestantes acompanhadas no ambulatório de endocrinopatias na gestação na maternidade referência em gestação de alto risco na Bahia, no período de janeiro de 2010 a dezembro de 2013. Aleatoriamente foramselecionadas 100 pacientes atendidas como AMBEG e 100 pacientes atendidas comprontuário convencional comregistro em papel e comparou-se a taxa de preenchimento de informações clínicas. Resultados Foram realizados 1461 atendimentos com o AMBEG: 253, 963 e 245 respectivamente, admissões, consultas de seguimento e puerpério. Eram portadoras de diabetes 77,2% e sendo 60,1% portadoras de diabetes pré-gestacional. O AMBEG substituiu, satisfatoriamente, o prontuário convencional. O percentual de informações clínicas registradas em ambos os prontuários foi significativamente maior no AMBEG: queixas clínicas (100 versus 87%, p < 0,01), altura uterina (89 versus 75%, p = 0,01), ganho de peso total (91 versus 40%, p < 0,01) e dados específicos sobre o diabetes (dieta, esquema de insulina, controle glicêmico e manejo de hipoglicemias) revelando diferença significativa (p < 0,01). A possibilidade de exportar dados clínicos para planilhas facilitou e agilizou a análise estatística de dados. Conclusões O AMBEG é uma ferramenta útil no atendimento clínico a mulheres portadoras de endocrinopatias na gestação. A taxa de preenchimento de informações clínicas foi superior à do prontuário convencional.
