Until 1999 the endemic cases of Sylvatic Yellow Fever were located in the states of northern, midwestern and pre-Amazon regions. Since then, the disease progressively expanded its territory of occurrence, cases being registered beyond the traditional boundaries of endemism. The São Paulo State is considered to be part of this context, since after decades without registration of autochthonous cases of the disease, it reported, in 2000 and 2008-2009, epizootic occurrence in non-human primates and 30 cases in humans. Facts like these, added to the increase in incidences of serious adverse effects resulting from the Yellow Fever vaccination, have highlighted the importance of defining priority municipalities for vaccination against the disease in the state. Two groups of municipalities, some affected and some non-affected by YF, were compared for environmental variables related to the eco-epidemiology of the disease according to literature. The Multiple Correspondence Analysis (MCA) was used to pinpoint the factor able to differentiate the two groups of municipalities and define the levels of risk. The southeast region of the São Paulo State was considered to be the area with a higher number of municipalities classified as high risk and should be considered a priority for the application of prevention measures against Yellow Fever.

Enteric parasitosis remains an important public health problem in many areas around the world including in Brazil, and it is frequently associated with poverty and lack of sanitation facilities. Research carried out over the course of a year revealed that 96.6% (28/29) of children randomly selected from a 'landless farm workers' settlement in Araras, São Paulo, aged 4 - 15 years, presented Giardia intestinalis cysts. After referral to the neighborhood Health Office, all the children received tinidazole, given as a single dose of 50 mg/kg and 12 months later, new fecal samples were collected and analyzed. Despite the low adherence to the study, a high percentage (64.3% - 9/14) of the children remained positive for the parasite. This study showed a high positivity of giardiasis in child residents of the settlement, even after treatment; adults were not sensitized to the study and did not collected and/or deliver children fecal samples. The precarious living conditions are consistent with a high susceptibility to parasitic diseases, suggesting that the treatment of the infected individuals without identifying and eradicating the means of contamination is simply a palliative measure.

Background: The investigation of stable coronary artery disease (CAD) and its treatment depend on risk stratification for decision-making on the need for cardiac catheterization and revascularization. Objective: To analyze the procedures used in the diagnosis and invasive treatment of patients with CAD, at the Brazilian Unified Health System (SUS) in the cities of Curitiba, São Paulo and at InCor-FMUSP. Methods: Retrospective, descriptive, observational study of the diagnostic and therapeutic itineraries of the Brazilian public health care system patient, between groups submitted or not to prior noninvasive tests to invasive cardiac catheterization. Stress testing, stress echocardiography, perfusion scintigraphy, catheterization and percutaneous or surgical revascularization treatment procedures were quantified and the economic impact of the used strategies. Results: There are significant differences in the assessment of patients with suspected or known CAD in the metropolitan region in the three scenarios. Although functional testing procedures are most often used the direct costs of these procedures differ significantly (6.1% in Curitiba, 20% in São Paulo and 27% in InCor-FMUSP). Costs related to the procedures and invasive treatments represent 59.7% of the direct costs of SUS in São Paulo and 87.2% in Curitiba. In InCor-FMUSP, only 24.3% of patients with stable CAD submitted to CABG underwent a noninvasive test before the procedure. Conclusion: Although noninvasive functional tests are the ones most often requested for the assessment of patients with suspected or known CAD most of the costs are related to invasive procedures/treatments. In most revascularized patients, the documentation of ischemic burden was not performed by SUS.

The detection of IgM antibodies for Schistosoma mansoni using gut-associated antigens (IgM-IFT) was compared to the parasitological Kato-Katz method for study of the transmission of schistosomiasis in an urban area in Campinas. About 400 schoolchildren whose ages ranged from 6 to 18 years, were observed for a period of two years. Blood samples on filter paper and fecal samples were collected, at intervals of six months. Serological (IgM-IFT) prevalence rates of 1.2%, 4.3%, 3.6%, 2.9% and 3.4% were obtained in five surveys carried out. S. mansoni eggs were detected in only one child out of the 225 children (0.4%) who were submitted to the Kato-Katz method (three slides for each fecal sample) in the 1st survey. Sixty eight children who submitted five blood samples, one for each survey, were found IFT negative throughout the study. No child was found to be IFT positive in all five surveys, and only four children showed IFT positive results in at least four surveys. Seroconversion from IFT negative to positive was observed from the 1st to the 2nd survey in six chidren, from the 2nd to the 3rd survey in three children, from the 3rd to the 4th survey in four children, and from the 4th to the 5th survey in two cases. However, confirmation of S. mansoni infection using the fecal examination was not possible in any of the cases. Also, in most of them the IFT result oscillated from negative to positive and vice versa. Our data implied that recent transmission of schistosomiasis in the study area was not possible.

Little is known about transmission and drug resistance of tuberculosis (TB) in Bauru, State of São Paulo. The objective of this study was to evaluate risk factors for transmission of Mycobacterium tuberculosis strains in this area. Strains were collected from patients attended at ambulatory services in the region and susceptibility towards the main first line antibiotics was determined and fingerprinting performed. A total of 57 strains were submitted to susceptibility testing: 23 (42.6%) were resistant to at least one drug while 3 (13%) were resistant against both rifampicin and isoniazide. Resistant strains had been isolated from patients that had not (n = 13) or had (n = 9) previously been submitted to anti-TB treatment, demonstrating a preoccupying high level of primary resistance in the context of the study. All strains were submitted to IS6110 restriction fragment length polymorphism (IS6110-RFLP) and double repetitive element PCR (DRE-PCR). Using IS6110-RFLP, 26.3% of the strains were clustered and one cluster of 3 patients included 2 HIV-infected individuals that had been hospitalized together during 16 days; clustering of strains of patients from the hospital was however not higher than that of patients attended at health posts. According to DRE-PCR, 55.3% belonged to a cluster, confirming the larger discriminatory power of IS6110-RFLP when compared to DRE-PCR, that should therefore be used as a screening procedure only. No clinical, epidemiological or microbiological characteristics were associated with clustering so risk factors for transmission of TB could not be defined in the present study.

Schistosomal myeloradiculopathy (SMR) is a form of schistosomiasis that is not linked with a high worm burden but rather is found in patients who have been sporadically exposed to Schistosoma mansoni. This paper aims to determine the occurrence of SMR in a low-endemic area with urban transmission in Campinas, São Paulo, Brazil. A retrospective study was performed, identifying confirmed cases in the two largest public hospitals on the region. Patients were diagnosed with SMR using standardised criteria, common clinical parameters, evidence of schistosomal infection and exclusion of other causes of myelopathy. A total of 27 patients were identified; 19 (85.2%) were men and four (14.8%) were women, ranging from 13-57 years of age (mean = 31.2; standard deviation = 12.8). Patients were classified as autochthonous (n = 14; 51.9%) or allochthonous (n = 11; 40.7%) and epidemiological data could not be obtained for two patients (7.4%). The clinical parameters of these patients were not different from previous studies. The sensitivity of serum immune reactions, cerebrospinal fluid immune reactions and parasitological stool examinations in identifying infected individuals was 87.5%, 93.8% and 40%, respectively. The epidemiological importance of these findings and their relationship with the control policies of schistosomiasis are discussed.

A distribuição de Culex (Culex) quinquefasciatus Say (1823) ao longo das margens do rio Pinheiros e os principais fatores que levam à proliferação da espécie foram estudados efetuando-se coletas semanais de mosquitos adultos, no período de um ano, em três pontos eqüidistantes às margens do rio. Para as coletas de mosquitos utilizou-se aspirador à bateria, por um período de cinco minutos. Os mosquitos foram identificados, diferenciados segundo o sexo e contados. Para verificação do estado fisiológico, as fêmeas foram separadas em vazias, com sangue e com ovos. Foram coletados 35.684 mosquitos, todos identificados como Cx. quinquefasciatus, sendo 39,4% fêmeas e 60,6% machos. As freqüências tomaram proporções diferentes entre os pontos de coletas e, em uma série temporal. O ambiente impactado do rio Pinheiros representa um excelente criadouro de Cx. quinquefasciatus, confirmado pela ocorrência de picos acentuados na freqüência de mosquitos, com desenvolvimento de forma explosiva e sobreposições entre as gerações, após as chuvas e em épocas de verão.

The objective of this study was to determined species composition and community structure of Carabidae and Staphylinidae in five areas of forest fragment and soybean/corn crops or orange orchard, from December 2004 to May 2007. Beetles were captured in pitfall traps distributed along two parallel transects of 200 m in length, placed across crop land/forest boundary fragment, with 100 m each. The Shannon-Wiener diversity and evenness indexes and Morisita similarity index were calculated. The carabids Abaris basistriatus Chaudoir, Calosoma granulatum Perty, Megacephala brasiliensis Kirby, Odontochila nodicornis (Dejean) and Selenophorus seriatoporus Putzeys. are dominant and are widely distributed in northeastern São Paulo state, Brazil. Point-scale species diversity was greatest at the transition between forest fragment and cultivated area. The carabid and staphylinid communities of the forest fragment were more similar to the community of orange orchard than that of soybean/corn crops.

The aim of this study is to investigate the floristic composition of an Atlantic rain forest fragment located in Cananéia, São Paulo, Brazil, and to contribute to the knowledge on Atlantic forest through the comparative analysis of this and other floristic surveys both on the southern and southeastern Brazil, in different soil and relief types. We surveyed 215 species in 132 genera and 51 families. Classification and ordination analysis were applied to a binary matrix in order to analyze the similarity among 24 surveys, including the present one, of Atlantic forest from the south and southeast coast of Brazil. Higher floristic similarity was observed among this area and the ones where there was marine influence and more rugged relief. The surveys in areas with greater marine influence (sandy soil) were separated from those in other conditions, possibly indicating a species replacement gradient from the steep slopes towards the lowland and were probably related to different edaphic conditions. A latitudinal gradient was found among the surveys apparently confirming a continuous species replacement along the Atlantic forest, related to a restricted distribution of the species. This suggests that it is essential to preserve areas from the whole Atlantic coast. Atlantic forest distribution is quite complex and its composition cannot be adequately represented by small localized areas.

Cyanobacteria are common in aquatic environments but are also well-adapted to terrestrial habitats where they are represented by a diversified flora. The present study aimed to contribute to our taxonomic knowledge of terrestrial cyanobacteria by way of a floristic survey of the main components of corticolous communities found in seasonal semideciduous forest fragments. Samples of visible growths of Cyanobacteria, algae, and bryophytes found on tree bark were randomly collected and their taxonomies examined. Eighteen species of Cyanobacteria were found belonging to the genera Aphanothece, Chroococcus, Lyngbya, Phormidium, Porphyrosiphon, Hapalosiphon, Hassalia, Nostoc, Scytonema, and Stigonema. Many genera and species observed in the present work have been reported in previous surveys of the aerophytic flora in several regions of the world, although six species were described only on the basis of populations found in the forest fragments studied, which highlights the importance of taxonomic studies of cyanobacteria in these habitats.

This report shows the phlebotomine vectorial role in the endemic cutaneous leishmaniasis situated in the middle and north-eastern region of the São Paulo, Brazil. Analysis of information encloses a sandfly man-biting fauna of four different patches of residual forest. So, using the human-bait and Shannon trap for a period of one year we have caught 16,869 sandflies. The predominance of Lutzomyia intermedia (85,6%) was clear. Moreover, the low density of Lutzomyia whitmani and Lutzomyia pessoai and their epidemiological implications at present and in the past are discussed. Information about the daily activity of some species is given and we emphasize for the first time, the diurnal activity of Lutzomyia firmatoi in the São Paulo State.

Aborda-se o problema da desnutrição no Estado de São Paulo (Brasil) enfocando-se prevalência e determinantes, identificando-se lacunas significativas no conhecimento e sugerindo-se linhas de pesquisa prioritárias. Ainda que não se disponha de informações precisas e atualizadas como seria desejável, os estudos já realizados mostram ser grave o problema da desnutrição no Estado, sendo endêmicas as presenças da desnutrição protéico-energética e da anemia ferropriva. Do ponto de vista descritivo, há premente necessidade de se atualizar as últimas estimativas para a prevalência da desnutrição no Estado, que datam de dez anos atrás, sendo também desejável a obtenção de informações regionalizadas que permitam identificar situações particulares que ensejam uma atenção especial. Do ponto de vista analítico, há necessidade de se expandir os universos relativamente restritos que têm sido enfocados em estudos de causalidade, sendo igualmente preciso um maior avanço na investigação de determinantes imediatos, no estudo da eficácia de intervenções específicas e na análise multidisciplinar de causas básicas da desnutrição.

São apresentados dados de mortalidade infantil, neonatal precoce, neonatal, pós-neonatal e perinatal, para Estados Unidos e Suécia e o Estado de São Paulo em uma série temporal de 1950 a 1982. A Suécia, consistentemente, apresenta os menores coeficientes, sendo que aqueles para o Estado de São Paulo podem ser considerados ainda bastante elevados, pois no início da década de 80 apresentava uma mortalidade infantil igual a daquele país meio século antes e a mortalidade de menores de 1 dia foi maior que a mortalidade para todo o primeiro ano de vida na Suécia. Para as três populações existe declínio de todos os coeficientes, porém, para a Suécia e os Estados Unidos, ainda que com números já bastante baixos em relação ao Estado de São Paulo no início do período estudado, o declínio tem sido maior.

Since the beginning of the seventies the natural transmission of Chagas’ infection has been considered to be under control in the State of São Paulo and not even a case of American Trypanosomiasis, transmitted by triatomine bugs, has been detected by the epidemiological surveillance system. This situation justifies the report of a case of acute Chagas’ disease that occurred in a forest area considered free of domiciliary triatomines along the Southern seacoast of São Paulo State. In May, 1995 the presence of trypomastigote forms of Trypanosoma cruzi had been diagnosed in a retired 57 year-old male patient, born and living in Santos (São Paulo State), complaining of fever, fatigue and malaise. The patient reported that 40 days before he had participated with 17 friends in a 7-day excursion in a forest area of the municipalities of Itanhaém and Peruíbe. During this period the group had been lodged in three houses located within the forest. Eight days after the end of the excursion the patient began to have fever, malaise and fatigue. During the next 31 days he had received medical care both as an inpatient and an outpatient, without any significant improvement. After the detection of T. cruzi trypomastigotes in his blood stream the patient began to be treated with benzonidazole in a hospital but died 8 days after the beginning of treatment. The epidemiological investigation carried out showed no signs of the presence of triatomine bugs in the three houses where the group had been lodged, or any indication of Chagas' infection in other excursionists
