Com uma incidência de 3 a 10% dos casos e letalidade próxima a 70%, o comprometimento pulmonar constitui uma das manifestações mais graves da malária por Plasmodium falciparum. Embora sua patogênese não esteja perfeitamente esclarecida, reconhece-se que a hiperativação do sistema imune por antígenos liberados pelo Plasmodium falciparum desempenhe um importante papel no desencadeamento e agravamento das lesões. A estrutura alvo parece ser o endotélio capilar, responsável pelo fluxo de líquidos para o espaço intersticial. Essas células são ativadas por ação de citocinas, produzidas por linfócitos e macrófagos durante a resposta imune, e passam a expressar em sua membrana celular receptores e moléculas de aderência que facilitam a sequestração de eritrócitos parasitados e também a aderência de células capazes de produzir mediadores inflamatórios. A reação inflamatória e a lesão endotelial que se seguem, juntamente com as alterações hemodinâmicas induzidas pelo bloqueio capilar devido ao acúmulo local de eritrócitos e células inflamatórias causam alterações de permeabilidade vascular e, consequentemente, acúmulo de líquido nos espaços intersticiais e alvéolos. Nos casos mais graves, as manifestações clínicas assemelham-se às do quadro da Síndrome do desconforto respiratório do adulto. Comprometimento pulmonar grave pode se instalar rapidamente em qualquer estágio da evolução clínica da malária, mesmo após a cura parasitológica, desconhecendo-se os fatores desencadeantes. Hiperparasitismo, insuficiência renal e gravidez constituem fatores predisponentes. O prognóstico dependerá da rapidez com que o diagnóstico for estabelecido e o correto tratamento instituído. Além do tratamento instituído contra o parasita, especial atenção deverá ser dispensada à monitorização hemodinâmica, se possível através de cateter de Swan-Ganz, à manutenção de adequada oxigenação e balanço hídrico, e ao controle de outras complicações, frequentemente associadas ao comprometimento pulmonar. O esclarecimento da patogenia do comprometimento pulmonar associado à malária deverá concorrer para a racionalização da conduta terapêutica e, consequentemente, melhorar o prognóstico dos indivíduos acometidos por esta complicação

Cryptococcus neoformans is the fifth most common opportunistic agent of infection in patients with AIDS in the USA, exceeded only by Candida species, Pneumocystis carinii, cytomegalovirus and Mycobacterium avium1, 2, 6, 10, 11. In Brazil is the sixth, exceeded by Candida species, P. carinii, Mycobacterium species, Toxoplasma gondii, and herpes simplex virus (AIDS, Boletim Epidemiológico, set/nov 96, Ministério da Saúde, Brasil). During 30 years, the treatment of C. neoformans meningitis was based on the use of amphotericin B with or without flucytosine13. Nowadays, with the immunodepression caused by human immunodeficiency virus (HIV) infection and the availability of new antifungal drugs as the triazoles, the concept related to cure and relapses of cryptococcosis has been altered7, 20. Patients are treated with amphotericin B with or without flucytosine as initial therapy, but maintenance therapy is always necessary in AIDS patients with C. neoformans infections

The authors studied 70 leprosy patients and 20 normal individuals, comparing the traditional sera collection method and the finger prick blood with the conservation on filter paper for specific antibodies against the native phenolic glycolipid-I (PGL-I) from Mycobacterium leprae. The finger prick blood dried on filter paper was eluated in phosphate buffer saline (PBS) containing 0.5% gelatin. The classical method for native PGL-I was performed for these eluates, and compared with the antibody determination for sera. It was observed that there is a straight correlation comparing these two methods; although the titles found for the eluates were lower than those obtained for serology. This blood collection method could be useful for investigation of new leprosy cases in field, specially in contacts individuals.

A serosurvey of varicella has been carried out in children attending the public school network of São Paulo city, Brazil, from 1992 to 1994. This study was performed in order to establish the age related prevalence of antibodies against varicella-zoster virus (VZV) and its age specific transmission dynamics pattern in these children. Among 2500 schools in the city of São Paulo public network, 304 were randomly selected; 7 children of a given age (ranging from 1 to 15 years) were randomly selected in each school, and blood samples were obtained by fingerprick into filter paper. Blood eluates were analyzed for the presence of antibodies to VZV by ELISA. Proportion of seropositivity were calculated for each age group. Samples consisted of 1768 individuals in 1992, 1758 in 1993, and 1817 in 1994, resulting in 5343 eluates. A high proportion of seropositive children from 1 to 3 years of age was observed, ascending until 10 years of age and reaching a plateau around 90% afterwards. VZV transmission in this community was similar along the three years of the study. In children attending public schools in the city of São Paulo, contact with VZV occurs in early childhood. If immunization against VZV is considered it should be introduced as soon as possible.

The purpose of the present study was to compare the susceptibility to four antifungal agents of 69 Cryptococcus neoformans strains isolated from AIDS patients with that of 13 C. neoformans strains isolated from the environment. Based on the NCCLS M27-A methodology the Minimal Inhibitory Concentrations (MICs) obtained for amphotericin B, itraconazole and ketoconazole were very similar for clinical and environmental isolates. Clinical isolates were less susceptible to fluconazole than environmental isolates. The significance of these findings and aspects concerning the importance, role and difficulties of C. neoformans susceptibility testing are also discussed.

We present the case of a 15-year-old patient infected with HTLV-1 who developed a cutaneous T-cell lymphoma, confirmed by histopathological and immunohistochemical examination, as well as clinically and hematologically confirmed leukemia. The patient died 3 months after initial presentation of the disease. The rarity of the disease in this age group justifies the present report.

Chagas disease has been almost entirely eradicated from the arid zones in Central and Northeastern Brazil where rare or no autochthonous cases have been reported. However, in the last 10 years the disease has increasingly been registered in the Amazon Region. Aiming to investigate the possibility of the occurrence of autochthonous cycle of Chagas disease in Roraima, triatomine collections, vectorial susceptibility studies (this one to be shown elsewhere), parasitological and serological analyses were conducted in three agricultural settlement areas (Rorainópolis, Passarão Project and Ilha Community). Blood-donor candidates were also investigated. This is the first epidemiological survey on Chagas disease conducted in agricultural settlements in Roraima. Triatomine species found were Triatoma maculata, Rhodnius pictipes, Rhodnius robustus and Panstrongylus geniculatus. Trypanosoma cruzi detection analyses included xenodiagnosis, indirect immunofluorescence, indirect hemaglutination, ELISA and kinetoplast PCR amplification. Natural triatomine infection was not found in intestinal contents. Twenty-five adult settlers (1.4% out of 1821, all > 15 year-old, 20 migrants) presented anti-T. cruzi antibodies. Two migrant settlers (from Minas Gerais and Maranhão) tested positive for more than two serological tests, besides either being positive for xenodiagnosis or PCR. Results show that Chagas disease is not endemic in the areas studied. However, all elements of the transmission cycle are present, demanding for an adequate and continuous vigilance.

Even though Chagas disease is rare in the Brazilian Amazon, the conditions for the establishment of domiciliated cycles prevail in many areas where triatomines are of frequent occurrence. In Roraima, a previous serological and entomological survey in three agricultural settlements showed the existence of all transmission cycle elements, i.e., individuals infected by Trypanosoma cruzi, triatomine species previously found harboring T. cruzi in the broader Amazon region of neighboring countries and, domicile/ peridomicile conditions favorable to triatomine colonization. Triatoma maculata was the most frequent species, found in chicken houses in the peridomicile and sporadically within residences. Aiming to investigate the possibility of T. maculata to possess the potentiality to transmit T. cruzi in the area, bionomic characteristics were studied under laboratory conditions. These were feeding frequency, time for defecation after a blood meal, time elapsed in voluntary fasting pre- and pos-ecdysis, moulting time periods, pre-oviposition and oviposition periods and index of oviposition, incubation period, egg viability, longevity and mortality rate. Results show that the Passarão population of T. maculata should be considered a potential vector of T. cruzi since it shows a capacity to infest artificial ecotopes in the peridomicile, to carry out large number of meals during the nymphal cycle, to have a relatively short developmental cycle capable of producing 2.9 generations/year, to blood source eclecticism, to defecate immediately after the blood meal while still on the host and to the fact that has been previously found naturally infected by T.cruzi.

Introduction: The purpose of measuring the burden of disease involves aggregating morbidity and mortality components into a single indicator, the disability-adjusted life year (DALY), to measure how much and how people live and suffer the impact of a disease. Objective: To estimate the global burden of disease due to AIDS in a municipality of southern Brazil. Methods: An ecological study was conducted in 2009 to examine the incidence and AIDS-related deaths among the population residing in the city of Tubarao, Santa Catarina State, Brazil. Data from the Mortality Information System in the National Health System was used to calculate the years of life lost (YLL) due to premature mortality. The calculation was based on the difference between a standardized life expectancy and age at death, with a discount rate of 3% per year. Data from the Information System for Notifiable Diseases were used to calculate the years lived with disability (YLD). The DALY was estimated by the sum of YLL and YLD. Indicator rates were estimated per 100,000 inhabitants, distributed by age and gender. Results: A total of 131 records were examined, and a 572.5 DALYs were estimated, which generated a rate of 593.1 DALYs/100,000 inhabitants. The rate among men amounted to 780.7 DALYs/100,000, whereas among women the rate was 417.1 DALYs/100,000. The most affected age groups were 30-44 years for men and 60-69 years for women. Conclusion: The burden of disease due to AIDS in the city of Tubarao was relatively high when considering the global trend. The mortality component accounted for more than 90% of the burden of disease.

To clarify the existance of reinfection in children bearing an established Ascaris lumbricoides infection, the authors evaluated the weight and the length of worms collected from ten cases of ascaridiasis. The worm burden was greater than 27 worms in nine cases. In seven cases the weight and the length of worms showed little variation, with unimodal distribution of values, suggesting that all the worms in each case belong to the same population, originated from a single brood infection or from successive infections over small time intervals. In three cases there was great variation in worm size indicated by the different values for the means and medians and by the high values for the standard deviation and coefficient of variation. In these three cases there was a bimodal distribution of worm's size suggesting the coexistance of two distinct populations: one, less numerous, composed of mature worms and the other, more numerous, composed ofimmature worms, in two cases, and two distinct populations of immature worms in one case. The existance of worms in different stages of maturation indicates that the less mature population was acquired when the mature worms were established in the gut. These results indicate that the reinfection with Ascaris in children bearing an established infection is not rare and resistance induced by a preexisting infection is not the rule.

Relatam-se oito casos de criptococose não associada à AIDS diagnosticados em Santa Maria, RS, no período 1961-1995. É revisada a literatura sul-riograndense sobre a micose e comentada a prevalência da infecção pela var. neoformans em pacientes sem doença predisponente nas regiões subtropicais.

The frequency of myocarditis associated with meningococcal disease in children was reported only in two autopsied series (United States and South Africa). Here we report the frequency of associated myocarditis in 31 children who died of meningoccal infection at Hospital Infantil N.S. da Glória in Vitória, Espirito Santo State, Brazil. The diagnosis was confirmed by isolation of Neisseria meningitidis . At least three sections of fragments of both atria and ventricles were studied using the Dallas Criteria for the morphologic diagnosis of myocarditis. The mean age was 47.6 ± 39.8 months and the mean survival time after the onset of symptoms was 46.1 ± 26.5h (12-112h). Myocarditis was present in 13 (41.9%) patients, being of minimal severity in 11 cases and of moderate severity in 2 cases. There were no cases with severe diffuse myocarditis. The frequency of myocarditis was not influenced by sex, presence of meningitis, survival time after the onset of symptoms or use of vasoactive drugs. The frequency of myocarditis reported here was intermediate between the values reported in the only two case series published in the literature (57% in the United States and 27% in South Africa). Although our data confirm the high frequency of myocaditis in meningoccal disease, further investigations are necessary to elucidate the contribution of myocarditis to myocardial dysfunction observed in cases of meningococcal infection in children.
Nódulo granulomatoso com Enterobius vermicularis em epíploon simulando metástase de câncer de ovário

Os autores descrevem caso de enterobíase peritoneal apresentando-se como granuloma necrosante, em adolescente com tumor do seio endodérmico do ovário. O diagnóstico foi estabelecido por biópsia de nódulo do epíploon, durante laparotomia para reestadiamento de câncer de ovário após tratamento clínico. Nódulos granulomatosos peritoneais causados por parasitas podem simular metástases, confundindo o estadiamento de neoplasias.

Canindé apresenta uma população de 71.235 habitantes. Em abril de 2001 iniciou a utilização de peixes larvófagos em tanques de cimento, localizados ao nível do solo, como forma de controle biológico para larvas de Aedes aegypti. Durante a visita do agente, ao invés de se tratar os tanques com larvicida, colocou-se um espécime do peixe Betta splendens por depósito. O presente trabalho teve como objetivo avaliar os resultados desta intervenção. Com os levantamentos do número de imóveis e depósitos existentes, estimou-se este número mês a mês determinando então o número de depósitos existentes por imóvel. Com esta estimativa e o número de imóveis visitados mensalmente analisou-se a infestação deste tipo de depósito. Em janeiro de 2001, 70,4% dos tanques examinados apresentavam larvas; e apenas 7,4% em janeiro de 2002. Em dezembro de 2002 este índice caiu para 0,2%. Demonstrou-se com clareza a capacidade do Betta splendens como agente de controle biológico, em tanques de cimento, reduzindo 320 vezes a infestação deste tipo de recipiente de grande volume.

É descrito o primeiro caso de detecção do genótipo 4 do vírus da hepatite C (VHC) em Salvador, BA. Foram utilizados os testes de RT-PCR para detecção do VHC-RNA, e o LIPA para genotipagem. O genótipo 4 responde mal ao tratamento, sendo portanto importante a busca ativa dos contactantes.
