Puffer fish can be poisonous due to the presence of the potent neurotoxins such as Tetrodotoxin (TTX) and Saxitoxin (STX) found in its tissues. The authors report 27 human poisonings from ingestion of puffer fish in patients treated at Toxicology Centers in the states of Santa Catarina and Bahia, Brazil, between 1984 and January 2009. Poisonings were classified as moderate (52%) and severe (33%), two deaths were observed. Early diagnosis is very important to ensure respiratory support.

SUMMARY The role of trace elements in dengue virulence is not yet known. The present study assessed the serum levels of two micronutrients, copper and iron, in cases of dengue fever. The study involved 96 patients of whom 48 had either severe or non-severe forms of dengue (with and without warning signs), and the remaining 48 were patients with other febrile illnesses (OFI), used as controls. Serum levels of copper and iron were evaluated at admission and by the time of defervescence using commercially available kits. At admission, no difference in the level of serum copper was observed between cases and controls. In the group of dengue cases, the copper level was found to be significantly decreased in severe and non-severe cases with warning signs, compared to non-severe cases without warning signs. In contrast, by the time of defervescence the copper level was found to be increased in all dengue cases compared to OFI controls, but no difference was observed among dengue cases. Unlike OFI controls, dengue cases showed an increasing pattern of copper levels from admission until defervescence. On the other hand, no such significant differences were observed in the serum level of iron in the clinical groups, except for a decreased iron level found in severe cases, compared to non-severe dengue without warning signs. The results show that copper is associated with dengue severity and this finding emphasizes the need to investigate the involvement of trace elements in disease severity so as to improve the prognosis of dengue.

A pesquisa objetivou caracterizar a distribuição e evolução do número de pacientes internados por doença de Chagas no Brasil na última década. Procurou também discutir os limites e possibilidades da Autorização de Internação Hospitalar(AIH) na informação epidemiológica. Constatou-se que em números absolutos e relativos, as internações por tripanosomíase são pouco significativas dada a magnitude da doença no pais. A maior parte das hospitalizações com este diagnóstico ocorreu em São Paulo (com quase metade dos pacientes internados no país), Minas Gerais, Goiás e Distrito Federal. Quanto à natureza do hospital, verificou-se um aumento da importância do setor público e uma grande participação dos hospitais universitários nas internações por esta causa. Em termos de despesas com estas hospitalizações, observa-se que a hegemonia do Estado de São Paulo é reforçada e que o custo médio é muito variado de acordo com a localização e a natureza do hospital. O trabalho concluiu que a utilização mais apropriada da fonte AIH seria no estudo de doenças que não apresentam uma gama muito variada deformas clínicas, sendo suficientes as informações disponíveis no banco de dados SINTESE.

Paracoccidioidomycosis is a systemic fungal infection caused by Paracoccidioides brasiliensis. As infectious diseases can cause DNA damage, the authors aimed at analyzing DNA breakage in peripheral blood cells of patients with paracoccidioidomycosis by using the comet assay. The results suggested that paracoccidioidomycosis does not cause genotoxicity.

OBJECTIVE: To assess the risk factors, lipid and apolipoprotein profile, hemostasis variables, and polymorphisms of the apolipoprotein AI-CIII gene in early coronary artery disease (CAD). METHODS: Case-control study with 112 patients in each group controlled by sex and age. After clinical evaluation and nutritional instruction, blood samples were collected for biochemical assays and genetic study. RESULTS: Familial history of early CAD (64 vs 39%), arterial hypertension (69 vs 36%), diabetes mellitus (25 vs 3%), and previous smoking (71 vs 46%) were more prevalent in the case group (p<0.001). Hypertension and diabetes were independent risk factors. Early CAD was characterized by higher serum levels of total cholesterol (235 ± 6 vs 209 ± 4 mg/dL), of LDL-c (154 ± 5 vs 135 ± 4 mg/dL), triglycerides (205 ± 12 vs 143 ± 9 mg/dL), and apolipoprotein B (129 ± 3 vs 105 ± 3 mg/dL), and lower serum levels of HDL-c (40 ± 1 vs 46 ± 1 mg/dL) and apolipoprotein AI (134 ± 2 vs 146 ± 2mg/dL) [p<0.01], in addition to an elevation in fibrinogen and D-dimer (p<0.02). The simultaneous presence of the rare alleles of the APO AI-CIII genes in early CAD are associated with hypertriglyceridemia (p=0.03). CONCLUSION: Of the classical risk factors, hypertension and diabetes mellitus were independently associated with early CAD. In addition to an unfavorable lipid profile, an increase in the thrombotic risk was identified in this population. An additive effect of the APO AI-CIII genes was observed in triglyceride levels.

OBJETIVO: Determinação da freqüência das alterações cardíacas e sua evolução nas crianças expostas ao HIV-1 por via perinatal. MÉTODOS: Realizada avaliação seqüencial clínico-cardiológica, eletrocardiográfica e ecocardiográfica Doppler em 84 crianças expostas ao HIV-1. RESULTADOS: Grupo I (sororreversão) 43 crianças (51,2%). Ausência de alterações clínicas. ECG: distúrbio de condução de ramo direito 5 casos. ECO: CIA (1 caso) e CIV (1 caso). Grupo II 41 infectados (48,8%) com 51,2% de alterações cardiológicas. Crianças assintomáticas ou com sintomas leves, sem imunossupressão: alterações clínico-ecocardiográficas ausentes; ECG: distúrbio de condução de ramo direito (2 casos). Crianças com comprometimento clínico-imunológico moderado e severo: Alterações encontradas: 1) Clínicas (31,7%): taquicardia isolada (1 caso), ICC (12 casos). 2) Eletrocardiográficas (43,9%): taquicardia sinusal associada a outras alterações (10 casos), distúrbio de condução de ramo direito (5 casos), BDAS (1 caso), HBAD (1 caso), alterações da repolarização ventricular (11 casos), SVD (2 casos), SVE (1 caso), desvio do AQRS para direita (1 caso), arritmias (3 casos). 3) Ecocardiográficas (26,8%): miocardiopatia dilatada (5 casos), derrame pericárdico com tamponamento (2 casos), hipertensão pulmonar (2 casos) e prolapso da valva mitral (1 caso). CONCLUSÃO: O envolvimento cardíaco foi uma característica do grupo infectado. Houve maior prevalência de alterações nas crianças pertencentes à categoria clínico-imunológica mais avançada. Os achados clínicos, eletrocardiográficos e ecocardiográficos mais freqüentes foram, respectivamente, ICC, alterações da repolarização ventricular e miocardiopatia dilatada. Esta foi reversível em um caso. As alterações eletrocardiográficas foram significantemente mais freqüentes que as clínicas e ecocardiográficas.

FUNDAMENTO: Diferentes abordagens de enfermagem no manejo de pacientes com insuficiência cardíaca (IC) tem demonstrado benefícios na redução da morbidade e mortalidade. Entretanto, a combinação de educação intra-hospitalar com contato telefônico após a alta hospitalar tem sido pouco explorada. OBJETIVO: Comparar dois grupos de intervenção de enfermagem entre pacientes hospitalizados devido à IC descompensada: o grupo intervenção (GI) recebeu intervenção educativa de enfermagem durante a hospitalização, seguida de monitorização por telefone após a alta hospitalar e o grupo controle (GC) recebeu apenas a intervenção hospitalar. Os desfechos foram conhecimento da IC e autocuidado, número de visitas à emergência, re-hospitalizações e morte em um período de três meses. MÉTODOS: Ensaio clínico randomizado. Pacientes adultos com IC e fração de ejeção do ventrículo esquerdo (FEVE) < 45% que podiam ser contatados por telefone após a alta foram estudados. O conhecimento da IC foi avaliado por meio de um questionário padronizado que também incluía questões referentes ao conhecimento do autocuidado, o qual foi respondido durante o período de hospitalização e três meses depois. Para os pacientes do grupo GI, os contatos foram realizados por meio de telefonemas e as entrevistas finais foram conduzidas em ambos os grupos ao final do estudo. RESULTADOS: Quarenta e oito pacientes foram alocados no GI e 63 no grupo GC. A idade média (63 ± 13 anos) e FEVE (aproximadamente 29%) eram similares nos dois grupos. Os escores para conhecimento da IC e autocuidado foram similares na avaliação basal. Três meses depois, ambos os grupos demonstraram melhora significativa dos escores de conhecimento da IC e autocuidado (P < 0,001). Outros desfechos foram similares. CONCLUSÃO: A intervenção educativa de enfermagem intra-hospitalar beneficiou todos os pacientes com IC em relação ao conhecimento da doença e autocuidado, independente do contato telefônico após a alta hospitalar.

Background: Nitric oxide (NO) has been largely associated with cardiovascular protection through improvement of endothelial function. Recently, new evidence about modulation of NO release by microRNAs (miRs) has been reported, which could be involved with statin-dependent pleiotropic effects, including anti-inflammatory properties related to vascular endothelium function. Objective: To evaluate the effects of cholesterol-lowering drugs including the inhibitors of cholesterol synthesis, atorvastatin and simvastatin, and the inhibitor of cholesterol absorption ezetimibe on NO release, NOS3 mRNA expression and miRs potentially involved in NO bioavailability. Methods: Human umbilical vein endothelial cells (HUVEC) were exposed to atorvastatin, simvastatin or ezetimibe (0 to 5.0 μM). Cells were submitted to total RNA extraction and relative quantification of NOS3 mRNA and miRs -221, -222 and -1303 by qPCR. NO release was measured in supernatants by ozone-chemiluminescence. Results: Both statins increased NO levels and NOS3 mRNA expression but no influence was observed for ezetimibe treatment. Atorvastatin, simvastatin and ezetimibe down-regulated the expression of miR-221, whereas miR-222 was reduced only after the atorvastatin treatment. The magnitude of the reduction of miR-221 and miR-222 after treatment with statins correlated with the increment in NOS3 mRNA levels. No influence was observed on the miR-1303 expression after treatments. Conclusion: NO release in endothelial cells is increased by statins but not by the inhibitor of cholesterol absorption, ezetimibe. Our results provide new evidence about the participation of regulatory miRs 221/222 on NO release induction mediated by statins. Although ezetimibe did not modulate NO levels, the down-regulation of miR-221 could involve potential effects on endothelial function.

Background: Systemic Arterial Hypertension (SAH) is one of the main risk factors for Coronary Artery Disease (CAD), in addition to male gender. Differences in coronary artery lesions between hypertensive and normotensive individuals of both genders at the Coronary Computed Tomography Angiography (CCTA) have not been clearly determined. Objective: To Investigate the calcium score (CS), CAD extent and characteristics of coronary plaques at CCTA in men and women with and without SAH. Methods: Prospective cross-sectional study of 509 patients undergoing CCTA for CAD diagnosis and risk stratification, from November 2011 to December 2012, at Instituto de Cardiologia Dante Pazzanese. Individuals were stratified according to gender and subdivided according to the presence (HT +) or absence (HT-) of SAH. Results: HT+ women were older (62.3 ± 10.2 vs 57.8 ± 12.8, p = 0.01). As for the assessment of CAD extent, the HT+ individuals of both genders had significant CAD, although multivessel disease is more frequent in HT + men. The regression analysis for significant CAD showed that age and male gender were the determinant factors of multivessel disease and CS ≥ 100. Plaque type analysis showed that SAH was a predictive risk factor for partially calcified plaques (OR = 3.9). Conclusion: Hypertensive men had multivessel disease more often than women. Male gender was a determinant factor of significant CAD, multivessel disease, CS ≥ 100 and calcified and partially calcified plaques, whereas SAH was predictive of partially calcified plaques.

Toxicological and toxicogenetic effects of aqueous (tea) and hexanica fruit extract of Indigofera suffruticosa Mill, and hydroalcoholic root extract od Solanum agrarium Stendt. Were evaluated in Balb C male mice intraperitoneally exposed. A hepatotoxic effect was observed just for animals treated with aqueous fruit extract of I. suffruticosa. In relation to the toxicogenetic effect, just the group trreated with 12.5% of toxic dose of aqueous fruit extract of I. suffruticosa showed a statistically significant increase in the frequency of cells with chromosome aberrations (cytogenetic effect), although a slight increase was also observed for the highest dose (25% of LF50_ of hydroalcoholic root extract of S. agrarium. The results obtanied show that before S. agrarium is used as medicine and before the wide use of I. suffruticosa in cattle food, careful evaluation must be done.

The immunoreactivity of seven peptides synthesized from Schistosoma mansoni proteins, was evaluated by dot-blot and ELISA assays using two different sensitization methodologies. The best results were obtained on wells of the Costar 3590 microplates coated with peptides P1, P2, P3, P6, and P7 using conventional methodology. The signals increased considerably (p < 0.0003) on wells sensitized with P1 to P6 using alternative methodology. In contrast, the well coated with peptide P7 presented lower signal when compared with conventional methodology (p = 0.0019). These results, establish the basis for the application of synthetic peptides for laboratory diagnosis of schistosomiasis mansoni.

Vaccines have had an unquestionable impact on public health during the last century. The most likely reason for the success of vaccines is the robust protective properties of specific antibodies. However, antibodies exert a strong selective pressure and many microorganisms, such as the obligatory intracellular parasite Trypanosoma cruzi, have been selected to survive in their presence. Although the host develops a strong immune response to T. cruzi, they do not clear the infection and instead progress to the chronic phase of the disease. Parasite persistence during the chronic phase of infection is now considered the main factor contributing to the chronic symptoms of the disease. Based on this finding, containment of parasite growth and survival may be one method to avoid the immunopathology of the chronic phase. In this context, vaccinologists have looked over the past 20 years for other immune effector mechanisms that could eliminate these antibody-resistant pathogens. We and others have tested the hypothesis that non-antibody-mediated cellular immune responses (CD4+ Th1 and CD8+ Tc1 cells) to specific parasite antigens/genes expressed by T. cruzi could indeed be used for the purpose of vaccination. This hypothesis was confirmed in different mouse models, indicating a possible path for vaccine development.

Rhodococcus equi has emerged as an opportunistic pathogen associated with pulmonary, invasive or systemic infections in immunocompromised patients. We report the identification of 51 R. equi isolates found in sputum samples of 546 individuals suspected to have pulmonary tuberculosis in two Public Health Hospital Units in Brazil. The epidemiology of R. equi infection as well as the phenotypic identification and drug susceptibility profile of isolates are described in this paper.

The purpose of this study was to provide information about the genetic diversity and prevalent genotype of Mycobacterium tuberculosis in a low-endemic setting in northwestern state of Paraná in Southern Brazil. We employed spoligotyping and mycobacterial interspersed repetitive units-variable number tandem repeat (MIRU-VNTR) techniques to genotype M. tuberculosisisolates from patients with pulmonary tuberculosis (TB). The 93 isolates analyzed by spoligotyping were divided into 36 different patterns, 30 of which were described in the SITVIT database. Latin American and Mediterranean, Haarlem and T families were responsible for 26.9%, 17.2% and 11.8% of TB cases, respectively. From the 84 isolates analyzed by MIRU-VNTR, 58 shared a unique pattern and the remaining 26 belonged to nine clusters. The MIRU loci 40, 23, 10 and 16 were the most discriminatory. A combination of MIRU-VNTR and spoligotyping resulted in 85.7% discriminatory power (Hunter-Gaston index = 0.995). Thus, combining spoligotyping and MIRU-VNTR typing proved to be most useful for epidemiological study in this low-endemic setting in Southern Brazil. The current study demonstrated that there is significant diversity in circulating strains in the city of Maringá and the surrounding regions, with no single genotype of M. tuberculosispredominating.
