OBJETIVO: Revisar a literatura científica sobre a continuidade dos padrões alimentares da infância à adolescência. MÉTODOS: Foram realizadas buscas nas bases de dados MEDLINE/PubMed, Lilacs e SciELO utilizando os seguintes termos: "tracking", "dietary patterns" e "childhood/adolescence" e sinônimos. Foram encontrados 45 resumos e, após aplicação dos critérios de inclusão, 13 artigos foram incluídos. A continuidade do padrão foi avaliada por três principais análises estatísticas: coeficientes de correlação (Pearson ou Spearman), coeficiente kappa e análise de probabilidades. RESULTADOS: A continuidade do padrão alimentar foi de fraca a moderada entre os períodos infância-infância e infância-adolescência. Parece não haver continuidade na adolescência. CONCLUSÕES: Os padrões alimentares da infância podem persistir até a adolescência, embora no transcorrer da adolescência possam ser alterados ou descontinuados.

OBJECTIVE: To identify factors associated to medicine use among children from the 2004 Pelotas Birth Cohort, Brazil. METHODS: Prospective study to evaluate medicine use in children aged 3, 12 and 24 months regardless of the reasons, therapeutic indication or class. The study included 3,985 children followed up at three months of age, 3,907 at 12 months, and 3,868 at the last follow-up time of 24 months. Mothers were interviewed to collect information on medicine use during the recall period of 15 days prior to the interview. The outcome was studied according to sociodemographic and perinatal variables, mother's perception of child's health and breastfeeding status. Crude and adjusted analyses were performed by Poisson regression following a hierarchical model. RESULTS: The prevalence of medicine use ranged from 55% to 65% in the three follow-ups. After controlling for confounders, some variables remained associated to medicine use only at the three-month follow-up with greatest use among children of younger mothers, those children who had intrapartum complications, low birthweight, were never breastfed and were admitted to a hospital. Greatest medicine use was also associated with being a firstborn child at 3 and 12 months; mother's perception of their child health as fair or poor and children whose mothers have private health insurance at 12 and 24 months; highest maternal education level at all follow-up times. CONCLUSIONS: Different variables influence medicine use among children during the first two years of life and they change as the child ages especially maternal factors and those associated to the child's health problems.

OBJECTIVE To assess the increased diagnostic yield for pulmonary tuberculosis using bronchial washing cultures compared with sputum cultures. METHODS Study conducted with 61 adults in Lima, Peru, from January 2006 to December 2007. The yield of sputum cultures was compared with the yield of acid-fast bacilli smears and cultures of bronchial washing for diagnosing pulmonary tuberculosis in suspected cases of clinical tuberculosis with negative acid fast bacilli sputum smears. RESULTS Twenty seven (95%CI 32;58) of the cases were eventually diagnosed with smear-negative pulmonary tuberculosis. Bronchial washing samples detected 23 (95%CI 72;99) of the smear-negative pulmonary tuberculosis cases compared with 15 (95%CI 37;74) for sputum cultures (p = 0.02). The incremental diagnostic yield of acid fast bacilli smear and culture of bronchial washing specimens over sputum culture was 44% (95%CI 25;65). CONCLUSIONS In function of the epidemiological context and the resources available, bronchoscopy should be deployed as part of a comprehensive work up that optimizes smear-negative pulmonary tuberculosis diagnosis and minimizes risk and costs.

OBJETIVO : Analisar fatores associados à baixa adesão ao tratamento medicamentoso em idosos. MÉTODOS : Estudo transversal de base populacional, com amostra representativa de 1.593 indivíduos com 60 anos ou mais, residentes na região urbana de Bagé, RS, em 2008. A amostragem foi realizada em múltiplos estágios. Os dados foram coletados em entrevistas individuais nos domicílios. Analisou-se a associação entre a baixa adesão referida ao tratamento medicamentoso mensurado pelo Brief Medication Questionnaire (BMQ) e fatores demográficos, socioeconômicos, comportamentais e de saúde, assistência e prescrição. Foi utilizado modelo de regressão de Poisson para estimar as razões de prevalência bruta e ajustada, os respectivos intervalos de confiança de 95% e p-valor (teste de Wald). RESULTADOS : Cerca de 78,0% dos indivíduos referiram ter usado algum medicamento nos sete dias precedentes à entrevista. Desses, cerca de 1 / 3 foram considerados com baixa adesão ao tratamento. Os fatores significativamente associados à baixa adesão foram: idade (65 a 74 anos), não ter plano de saúde, ter que comprar (totalmente ou em parte) os seus medicamentos, ter três ou mais morbidades, possuir incapacidade instrumental para a vida diária e usar três ou mais medicamentos. CONCLUSÕES : A utilização elevada de medicamentos, decorrente da alta prevalência de doenças crônico-degenerativas em idosos, e o acesso ao tratamento devem ser considerados pelos profissionais de saúde para adoção de estratégias que visem diminuir a baixa adesão ao tratamento, aumentando a resolutividade terapêutica e a qualidade de vida desses pacientes.

OBJETIVO : Analisar a trajetória e satisfação profissional de egressos de cursos de doutorado na área da saúde. MÉTODOS : Estudo exploratório com 827 egressos dos cursos de doutorado da Fundação Oswaldo Cruz nas áreas da saúde coletiva, biociências e atenção à saúde, entre 1984 e 2007. Os sujeitos foram agrupados em três coortes temporais. Foi analisado o perfil dos egressos; mapeadas suas trajetórias profissionais, suas percepções sobre a formação recebida; suas motivações para escolha da instituição para realizar o doutorado; e as avaliações efetuadas sobre os cursos. Utilizou-se questionário em formato eletrônico para preenchimento on-line para coleta de dados. RESULTADOS : Existiram diferenças entre as coortes de egressos, relacionadas ao período em que cursaram o doutorado, a distintas trajetórias de formação e processos de trabalho entre egressos das áreas de biociências e saúde e as peculiaridades das diferentes áreas em que a instituição oferece cursos de doutorado: saúde coletiva, biociências e atenção à saúde. CONCLUSÕES : Os resultados possibilitam ampliar o conhecimento das instâncias de gestão acadêmica sobre os processos de formação, estabelecendo uma “linha de base” para o acompanhamento da trajetória dos egressos e contribuir com subsídios para o aprimoramento dos processos de acompanhamento de egressos dos programas de pós-graduação.

OBJECTIVE To analyze the evolution of catastrophic health expenditure and the inequalities in such expenses, according to the socioeconomic characteristics of Brazilian families.METHODS Data from the National Household Budget 2002-2003 (48,470 households) and 2008-2009 (55,970 households) were analyzed. Catastrophic health expenditure was defined as excess expenditure, considering different methods of calculation: 10.0% and 20.0% of total consumption and 40.0% of the family’s capacity to pay. The National Economic Indicator and schooling were considered as socioeconomic characteristics. Inequality measures utilized were the relative difference between rates, the rates ratio, and concentration index.RESULTS The catastrophic health expenditure varied between 0.7% and 21.0%, depending on the calculation method. The lowest prevalences were noted in relation to the capacity to pay, while the highest, in relation to total consumption. The prevalence of catastrophic health expenditure increased by 25.0% from 2002-2003 to 2008-2009 when the cutoff point of 20.0% relating to the total consumption was considered and by 100% when 40.0% or more of the capacity to pay was applied as the cut-off point. Socioeconomic inequalities in the catastrophic health expenditure in Brazil between 2002-2003 and 2008-2009 increased significantly, becoming 5.20 times higher among the poorest and 4.17 times higher among the least educated.CONCLUSIONS There was an increase in catastrophic health expenditure among Brazilian families, principally among the poorest and those headed by the least-educated individuals, contributing to an increase in social inequality.

OBJECTIVE To analyze the variation in the proportion of households living below the poverty line in Brazil and the factors associated with their impoverishment. METHODS Income and expenditure data from the Household Budget Survey, which was conducted in Brazil between 2002-2003 (n = 48,470 households) and 2008-2009 (n = 55,970 households) with a national sample, were analyzed. Two cutoff points were used to define poverty. The first cutoff is a per capita monthly income below R$100.00 in 2002-2003 and R$140.00 in 2008-2009, as recommended by the Bolsa Família Program. The second, which is proposed by the World Bank and is adjusted for purchasing power parity, defines poverty as per capita income below US$2.34 and US$3.54 per day in 2002-2003 and 2008-2009, respectively. Logistic regression was used to identify the sociodemographic factors associated with the impoverishment of households. RESULTS After subtracting health expenditures, there was an increase in households living below the poverty line in Brazil. Using the World Bank poverty line, the increase in 2002-2003 and 2008-2009 was 2.6 percentage points (6.8%) and 2.3 percentage points (11.6%), respectively. Using the Bolsa Família Program poverty line, the increase was 1.6 (11.9%) and 1.3 (17.3%) percentage points, respectively. Expenditure on prescription drugs primarily contributed to the increase in poor households. According to the World Bank poverty line, the factors associated with impoverishment include a worse-off financial situation, a household headed by an individual with low education, the presence of children, and the absence of older adults. Using the Bolsa Família Program poverty line, the factors associated with impoverishment include a worse-off financial situation and the presence of children. CONCLUSIONS Health expenditures play an important role in the impoverishment of segments of the Brazilian population, especially among the most disadvantaged.

In order to estimate ages at which etiological agents of systemic mycoses initiate infection, histoplasmin and paracoccidioidin skin tests were performed in 344 children of both sexes, between 2 and 15 years old. They were selected from a statistically significant population sample Gral. San Martín city (Northeast Argentina). Tests were read 48h after injection and considered positive if a 5 mm on larger induration was present. Circulating antibodies were also evaluated by agar gel immunodiffusion. The overall infection rate for H. capsulatum was 9.2%, belonging to children from 4 to 14 years old, without significant differences among sexes. Five children from 2 to 14 years old were positive to paracoccidioidin (1.6%). None of the children had specific antibodies neither signs of active mycosis. Results show H. capsulatum infection can be found from age 4, while for P. brasiliensis the lower limit was two years old. These findings may contribute to better knowledge on infantile fungal infection in a geographical region where no previous references can be found.

Most studies from Argentina have focused on toxocariasis as an environmental problem of big cities, and there are no available data about children infection from small or middle-sized cities. In order to assess the prevalence of anti-Toxocara antibodies in infantile population, 206 children from Resistencia, of both sexes, aged 1-14 years old were studied by Elisa testing with E/S T. canis L2 antigens. Hematological parameters and immunoglobulin levels were determined; five days' stool samples were studied and epidemiological data were obtained by means of a questionnaire to parents. Results showed that 73% of the children had one or more dogs living at home, 57% reported geophagia and 37.9% were positive for Toxocara serology, but there was no significant difference in prevalence neither for boys and girls, nor concerning age. An increased risk of infection was observed in age groups 5-6 and 7-8 for boys, and in age groups 3-4 and 5-6 for girls.

It has been well demonstrated the relationship between the infection with high-risk human papillomavirus (HPVs) genotypes and cervical cancer. In Northeastern Argentina a high incidence of this pathology has been described and therefore a high prevalence of HPV infection is expected. In order to identify HPV genotypes associated with malignant and pre-malignant cervical lesions present in the area, 53 ecto-endo cervical cell specimens obtained from women with cytohistological alterations were studied by a PCR-RFLP technique. Out of 53 patients, 34 (64.2%) were positive for HPV infection, being HPV-16 (32.3%) the most frequently found genotype, followed by HPV-58 (14.7%), -6, -18 and -45 (5.9%), -33, -52, -53, -54, -56, -66, -MM4 and -LVX100 (2.9%). Also 5 cases of infection caused by multiple genotypes were found, which corresponded to 14.7% of the positive cases. Results indicate that besides HPV-16 and -18, the most prevalent high-risk HPV genotypes worldwide, others like -45 and -58 as well as co-infection cases are frequent between women of Northeastern Argentina, and a particular attention should be paid to this circumstance because it could be an epidemiological feature of regional importance and a useful information for a future vaccination program.

This report describes a preliminary characterization of proteolytic activity of proteins isolated from lysate of Giardia trophozoites of an axenic Brazilian strain. Fractions obtained by high-performance liquid chromatography (FPLC) were tested in SDS-polyacrylamide gel for the protein profiles, and the proteases activity was analyzed using gelatin impregnated SDS-PAGE. The proteases characterization was based on inhibition assays employing synthetic inhibitors for cysteine (E-64, IAA), serine (PMSF, TPCK, TLCK, and elastatinal), metalo (EDTA) and aspartic (pepstatin) proteases. Among thirty eluted fractions, polypeptide bands were observed in eight of them, however, proteolytic activity was detected in four ones (F23, F24, F25 and F26). Protein profiles of these fractions showed a banding pattern composed by few bands distributed in the migration region of 45 to < 18 kDa. The zymograms revealed proteolytic activity in all the four fractions assayed, mainly distributed in the migration region of 62 to 35 kDa. Among the profiles, the main pronounced zones of proteolysis were distinguished at 62, 55, 53, 50, 46 and 40 kDa. In inhibition assays, the protease activities were significantly inhibited by cysteine (E-64) and serine proteases (TPCK, TLCK and elastatinal) inhibitors. Gels incubated with other cysteine and serine protease inhibitors, IAA and PMSF, respectively, showed a decrease in the intensity of hydrolysis zones. Indeed, in the assays with the inhibitors EDTA for metalloproteases and pepstatin for aspartic proteases, none inhibition was detected against the substrate. These observations are relevants, especially if we consider that to define the real role of the proteases in host-parasite interaction, the purification of these enzymes for detailed studies may be warranted.

This study aimed to estimate the prevalence of paracoccidioidal infection by intradermal reaction (Delayed-Type Hypersensitivity, DTH) to Paracoccidioides brasiliensis in rural areas in Alfenas, Southern Minas Gerais (MG) State, Brazil, and to assess risk factors (gender, occupation, age, alcohol intake and smoking) associated with infection. We conducted a population-based cross-sectional study using intradermal tests with gp 43 paracoccidioidin in 542 participants, who were previously contacted by local health agents and so spontaneously attended the test. Participants underwent an interview by filling out a registration form with epidemiological data and were tested with an intradermal administration of 0.1 mL of paracoccidioidin in the left forearm. The test was read 48 hours after injection and was considered positive if induration was greater than or equal to 5 mm. Out of 542 participants, 46.67% were positive to the skin test. Prevalence increased in accordance with an increase of age. There was statistical significance only for males. Occupation, alcohol intake and smoking habits were not significantly associated with the risk of paracoccidioidomycosis infection. There is relevance of paracoccidioidomycosis infection in such rural areas, which suggests that further epidemiological and clinical studies on this mycosis should be done in the southern part of Minas Gerais State.

With the aim of studying the contamination of soils with eggs of Toxocara spp. in an arid area in the central-western region of Argentina, 76 soil samples were collected from 18 towns belonging to six provinces of central-western Argentina. They were processed by the centrifugal flotation method. No eggs of Toxocara spp. were found. It can be concluded that the negative results are directly related to the characteristics of the environment and climate present in the studied area. The finding of eggs in soils depends on several factors: the presence of canine or feline feces, the hygienic behavior of pet owners, the presence of stray animals without veterinary supervision, the weather and environmental conditions, and laboratory techniques used; and all these circumstances must be considered when comparing the results found in different geographical regions. In order to accurately define the importance of public spaces in the transmission of infection to humans, it is important to consider the role of backyards or green spaces around housing in small towns, where the population is not used to walking pets in public spaces, and in such cases a significant fraction of the population may acquire the infection within households.

Foram estudadas 131 amostras de fezes de gatos de comportamento domiciliado e errante da Região Metropolitana do Rio de Janeiro, obtendo-se uma positividade de 63,4% das amostras, com maior ocorrência no grupo de animais errantes. Foi observado predomínio de parasitismo por Ancylostoma sp (43,5%), Toxocara sp (19,1%) e Cystoisospora sp (43,5%) em ambos os grupos. Também foram encontrados ovos de Uncinaria sp (1,5%), Toxascaris leonina (7,6%), cistos de Giardia sp (6,1%) e esporocistos de Sarcocystis sp (0,8%). A alta prevalência de enteroparasitas na amostra estudada ressalta a importância de um maior controle parasitológico nesses animais, para proteção da saúde animal e humana.

No período de 1994 a 2000, durante a construção e enchimento da represa Yacyretá, foram estudadas em Ituzaingó e Posadas variáveis infecciosas (diarréias e infecções respiratórias), clínico-cirúrgicas (doenças cardiovasculares e politraumatismos) e ambientais (pluviais, temperatura e umidade). As diarréias, em Ituzaingó, tiveram um aumento 6,5%, 78,3% e 13%, respectivamente em 1995, 1996 e 1997 e nos anos seguintes os valores foram similares aos de 1994 e em Posadas mostraram uma tendência ascendente. As infecções respiratórias, em Ituzaingó em 1995, aumentaram 143% e nos períodos subseqüentes voltaram aos limites de 1994 e, em Posadas tiveram valores ascendentes. No Hospital de Ituzaingó, em 1995, as doenças incrementaram 97,6%, mostraram decréscimo em 1996 e atingiram 127% em 2000; os politraumatismos aumentaram 107% em 1995, declinaram 38% em 1996 nos anos seguintes apresentaram tendência ascendente atingindo 33% em 2000. O impacto das variáveis ambientais foi maior em Ituzaingó do que em Posadas. O aumento das doenças relacionou-se com a temperatura máxima, mas não com a umidade.
