OBJECTIVOS: Caracterizar a qualidade de vida e bem-estar dos idosos do ponto de vista dos próprios. MÉTODOS: Foi caracterizada uma amostra de 1.665 idosos com 75 anos ou mais, residentes em 13 Distritos de Portugal. Destes, 311 sujeitos foram excluídos por incapacidade de expressarem opiniões. O instrumento utilizado na recolha dos dados dos 1.354 sujeitos (81,3%) da amostra foi o EASYcare (Sistema de Avaliação dos Idosos). A análise estatística baseou-se na Análise em Componentes Principais e Análise de Clusters. RESULTADOS: Os estudos estatísticos realizados revelaram que o EASYcare possui boas qualidades psicométricas (a de Cronbach =0,92) e permitiram encontrar quatro factores: actividades de vida diária, bem-estar, mobilidade e comunicação. A análise de clusters identificou quatro grupos em 81,3% da amostra: autónomos (62,8%), quase autónomos (8,5%), quase dependentes (4,3%) e dependentes (3,2%). CONCLUSÕES: A qualidade de vida, para a maioria dos idosos, pode ser considerada bastante positiva, sendo que uma minoria apresentou problemas de diminuição cognitiva grave ou algum grau de dependência.

OBJETIVO: Analisar os municípios brasileiros segundo morbidade e desempenho do controle da vigilância epidemiológica de tuberculose e Aids. MÉTODOS: Análise exploratória de dois grupos de clusters não hierárquicos de dados de vigilância epidemiológica de tuberculose e Aids, e indicadores operacionais do Programa Nacional de Controle de Tuberculose, período de 2001 a 2003. A distribuição foi estratificada nas regiões metropolitanas e municípios prioritários, segundo o tamanho da população. A associação entre clusters de morbidade e desempenho foi avaliada pelo qui-quadrado, com análise de resíduos para identificar associações significantes. RESULTADOS: Dos cinco clusters de morbidade, a situação epidemiológica preocupante ocorre nos municípios com alta incidência de Aids, com alta ou baixa incidência de tuberculose, predominantes no Sudeste e Sul do Brasil, e nos grandes municípios. Dos seis clusters de desempenho do programa, desempenhos moderado e regular estão significantemente associados aos municípios prioritários, de regiões metropolitanas e com mais de 80 mil habitantes. Clusters regular e fraco concentram 10% dos municípios com abandono de tratamento elevado e baixa taxa de cura. O cluster "sem dados" está associado ao cluster de incidência muito baixa de tuberculose e Aids. CONCLUSÕES: Os achados refletem inadequação da vigilância à realidade epidemiológica do Brasil: precários fatores sociais associados à tuberculose e Aids e desempenho insuficiente do programa de controle.

OBJETIVO: Analisar a ocorrência espacial e temporal da dengue e sua associação com a heterogeneidade de características do ambiente urbano. MÉTODOS: Foram georreferenciados 1.212 casos de dengue registrados no Sistema de Informação de Agravos de Notificação entre 1998 e 2006, no município de Niterói, RJ, segundo setores censitários. Os setores foram classificados em áreas homogêneas para a ocorrência da doença: favela, estaleiro e urbano. Os casos foram agrupados em cinco períodos: dois interepidêmicos 1998-2000 e 2003-2005; três epidêmicos 2001, 2002 e 2006 e analisados por meio de operações entre camadas em ambiente sistema de informação geográfica. Para identificação de conglomerados com maior intensidade de casos, utilizou-se o método de kernel. O método de varredura espacial de Kulldorff foi usado para confirmação estatística desses clusters. RESULTADOS: Do total de casos, 57% eram do sexo feminino. As faixas etárias com maior concentração de casos foram de 20-29 anos (20,5%) e de 30-39 anos (17,7%). O setor favela morro apresentou somente 11% dos domicílios atendidos por serviço de coleta de lixo, o maior percentual de não alfabetizados (8,7%) e de chefes de família com rendimentos menores de 1 salário mínimo (29,5%). Os casos permaneceram nos setores denominados favelas. No primeiro ano epidêmico e nos períodos interepidêmicos o maior número de casos estava situado nos setores favelas morro e favela plana; no segundo e terceiro ano de epidemia, situavam-se no setor favela plana. CONCLUSÕES: A parcela economicamente ativa foi a mais atingida na área de estudo. Os setores censitários mostram heterogeneidade espacial em relação às condições de vida e dentro de alguns setores, há diferenciais na distribuição espacial e temporal do risco de ocorrência da dengue.

OBJETIVO: Comparar o desempenho de Unidades Básicas de Saúde segundo a implantação de novos arranjos e estratégias de atenção primária e saúde mental. PROCEDIMENTOS METODOLÓGICOS: Pesquisa avaliativa, com triangulação de métodos e referencial teórico da hermenêutica crítica em seis Unidades Básicas de Saúde dos dois distritos de saúde mais populosos de Campinas, SP, 2007. As Unidades Básicas de Saúde foram analisadas segundo resolutividade da clínica, articulação entre as redes de atenção primária e saúde mental e implantação de estratégias de promoção à saúde. Foram definidos dois grupos pela técnica de clusters: um com maior e outro com menor grau de implantação das ações. Os grupos foram comparados a partir da melhora do seguimento clínico, dada pela ocorrência de acidente vascular cerebral; avaliação da dispensação de medicamentos psiquiátricos; grupos focais com trabalhadores, usuários e agentes comunitários de saúde; e entrevistas com usuários e familiares. Empregaram-se estratégias de pesquisa inclusivas e participativas. ANÁLISE DOS RESULTADOS: Não houve modelos puros, mas um mosaico de propostas organizacionais. Foram identificados avanços positivos no grupo com maior implantação de estratégias inovadoras em relação à melhor integração dos agentes comunitários nas equipes das Unidades; à percepção de melhora da assistência pelos trabalhadores e agentes; e à facilidade para encaminhamentos e assistência de casos de saúde mental. As dificuldades identificadas em ambos os grupos foram: comunicação entre os níveis de atenção e dentro das equipes, na implantação do apoio matricial, e ações de promoção à saúde incipientes. CONCLUSÕES: São necessários o desenvolvimento e a implantação de mecanismos de fixação de profissionais na Atenção Básica nas grandes cidades. Os ACS são imprescindíveis para viabilizar o trabalho territorial proposto pela ESF, utilizando mecanismos de integração dos ACS às equipes de saúde para contrabalançar a tendência ao isolamento. Os arranjos pesquisados mostraram-se potentes para produzir essa integração.

OBJECTIVE: To identify clusters of the major occurrences of leprosy and their associated socioeconomic and demographic factors. METHODS: Cases of leprosy that occurred between 1998 and 2007 in São José do Rio Preto (southeastern Brazil) were geocodified and the incidence rates were calculated by census tract. A socioeconomic classification score was obtained using principal component analysis of socioeconomic variables. Thematic maps to visualize the spatial distribution of the incidence of leprosy with respect to socioeconomic levels and demographic density were constructed using geostatistics. RESULTS: While the incidence rate for the entire city was 10.4 cases per 100,000 inhabitants annually between 1998 and 2007, the incidence rates of individual census tracts were heterogeneous, with values that ranged from 0 to 26.9 cases per 100,000 inhabitants per year. Areas with a high leprosy incidence were associated with lower socioeconomic levels. There were identified clusters of leprosy cases, however there was no association between disease incidence and demographic density. There was a disparity between the places where the majority of ill people lived and the location of healthcare services. CONCLUSIONS: The spatial analysis techniques utilized identified the poorer neighborhoods of the city as the areas with the highest risk for the disease. These data show that health departments must prioritize politico-administrative policies to minimize the effects of social inequality and improve the standards of living, hygiene, and education of the population in order to reduce the incidence of leprosy.

OBJECTIVE To analyze the spatial distribution of homicide mortality in the state of Bahia, Northeastern Brazil. METHODS Ecological study of the 15 to 39-year old male population in the state of Bahia in the period 1996-2010. Data from the Mortality Information System, relating to homicide (X85-Y09) and population estimates from the Brazilian Institute of Geography and Statistics were used. The existence of spatial correlation, the presence of clusters and critical areas of the event studied were analyzed using Moran’s I Global and Local indices. RESULTS A non-random spatial pattern was observed in the distribution of rates, as was the presence of three clusters, the first in the north health district, the second in the eastern region, and the third cluster included townships in the south and the far south of Bahia. CONCLUSIONS The homicide mortality in the three different critical areas requires further studies that consider the socioeconomic, cultural and environmental characteristics in order to guide specific preventive and interventionist practices.

OBJECTIVE To evaluate factors associated with users’ satisfaction in the Tuberculosis Control Program. METHODS A cross-sectional study of 295 patients aged ≥ 18 years, with two or more outpatient visits in the Tuberculosis Control Program, in five cities in the metropolitan region of Rio de Janeiro, RJ, Southeastern Brazil, in 2010. Considering an estimated population of 4,345 patients, the sampling plan included 15 health care units participating in the program, divided into two strata: units in Rio de Janeiro City, selected with probability proportional to the monthly average number of outpatient visits, and units in the other four cities. In the units, four temporal clusters of five patients each were selected with equal probability, totaling 300 patients. A questionnaire investigating the users’ clinical and sociodemographic variables and aspects of care and service in the program relevant to user satisfaction was applied to the patients. Descriptive statistics about users and their satisfaction with the program were obtained, and the effects of factors associated with satisfaction were estimated. RESULTS Patients were predominantly males (57.7%), with a mean age of 40.9 and with low level of schooling. The mean treatment time was 4.1 months, mostly self-administered (70.4%). Additionally, 25.8% had previously been treated for tuberculosis. There was a high level of satisfaction, especially regarding medication provision, and respect to patients by the health professionals. Patients who were younger (≤ 30), those on self-administered treatment, and with graduate level, showed less satisfaction. Suggestions to improve the services include having more doctors (70.0%), and offering exams in the same place of attendance (55.1%). CONCLUSIONS Patient satisfaction with the Tuberculosis Control Program was generally high, although lower among younger patients, those with university education and those on self-administered treatment. The study indicates the need for changes to structural and organizational aspects of care, and provides practical support for its improvement.

OBJECTIVE To analyze temporal trends and distribution patterns of unsafe abortion in Brazil. METHODS Ecological study based on records of hospital admissions of women due to abortion in Brazil between 1996 and 2012, obtained from the Hospital Information System of the Ministry of Health. We estimated the number of unsafe abortions stratified by place of residence, using indirect estimate techniques. The following indicators were calculated: ratio of unsafe abortions/100 live births and rate of unsafe abortion/1,000 women of childbearing age. We analyzed temporal trends through polynomial regression and spatial distribution using municipalities as the unit of analysis. RESULTS In the study period, a total of 4,007,327 hospital admissions due to abortions were recorded in Brazil. We estimated a total of 16,905,911 unsafe abortions in the country, with an annual mean of 994,465 abortions (mean unsafe abortion rate: 17.0 abortions/1,000 women of childbearing age; ratio of unsafe abortions: 33.2/100 live births). Unsafe abortion presented a declining trend at national level (R2: 94.0%, p < 0.001), with unequal patterns between regions. There was a significant reduction of unsafe abortion in the Northeast (R2: 93.0%, p < 0.001), Southeast (R2: 92.0%, p < 0.001) and Central-West regions (R2: 64.0%, p < 0.001), whereas the North (R2: 39.0%, p = 0.030) presented an increase, and the South (R2: 22.0%, p = 0.340) remained stable. Spatial analysis identified the presence of clusters of municipalities with high values for unsafe abortion, located mainly in states of the North, Northeast and Southeast Regions. CONCLUSIONS Unsafe abortion remains a public health problem in Brazil, with marked regional differences, mainly concentrated in the socioeconomically disadvantaged regions of the country. Qualification of attention to women’s health, especially to reproductive aspects and attention to pre- and post-abortion processes, are necessary and urgent strategies to be implemented in the country.

ABSTRACT OBJECTIVE To describe the spatial patterns of leprosy in the Brazilian state of Tocantins. METHODS This study was based on morbidity data obtained from the Sistema de Informações de Agravos de Notificação (SINAN – Brazilian Notifiable Diseases Information System), of the Ministry of Health. All new leprosy cases in individuals residing in the state of Tocantins, between 2001 and 2012, were included. In addition to the description of general disease indicators, a descriptive spatial analysis, empirical Bayesian analysis and spatial dependence analysis were performed by means of global and local Moran’s indexes. RESULTS A total of 14,542 new cases were recorded during the period under study. Based on the annual case detection rate, 77.0% of the municipalities were classified as hyperendemic (> 40 cases/100,000 inhabitants). Regarding the annual case detection rate in < 15 years-olds, 65.4% of the municipalities were hyperendemic (10.0 to 19.9 cases/100,000 inhabitants); 26.6% had a detection rate of grade 2 disability cases between 5.0 and 9.9 cases/100,000 inhabitants. There was a geographical overlap of clusters of municipalities with high detection rates in hyperendemic areas. Clusters with high disease risk (global Moran’s index: 0.51; p < 0.001), ongoing transmission (0.47; p < 0.001) and late diagnosis (0.44; p < 0.001) were identified mainly in the central-north and southwestern regions of Tocantins. CONCLUSIONS We identified high-risk clusters for transmission and late diagnosis of leprosy in the Brazilian state of Tocantins. Surveillance and control measures should be prioritized in these high-risk municipalities.

OBJECTIVE To analyze if the distribution of specialized care services for HIV/AIDS is associated with AIDS rates. METHODS Ecological study, for which the distribution of 10 specialized care services in the Ceara state, Northeastern Brazil, was obtained, and the mean rates of the disease were estimated per mesoregion. We evaluated 7,896 individuals who had been diagnosed with AIDS, were aged 13 years or older, lived in Ceara, and had been informed of their condition between 2001 and 2011. Maps were constructed to verify the relationship between the distribution of AIDS cases and institutionalized support networks in the 2001-2006 and 2007-2011 periods. BoxMap and LisaMap were used for data analysis. The Voronoi diagram was applied for the distribution of the studied services. RESULTS Specialized care services concentrated in AIDS clusters in the metropolitan area. The Noroeste Cearense and west of the Sertoes Cearenses had high AIDS rates, but a low number of specialized care services over time. Two of these services were implemented where clusters of the disease exist in the second period. The application of the Voronoi diagram showed that the specialized care services located outside the metropolitan area covered a large territory. We identified one polygon that had no services. CONCLUSIONS The scenario of AIDS cases spread away from major urban areas demands the creation of social support services in areas other than the capital and the metropolitan area of the state; this can reduce access barriers to these institutions. It is necessary to create specialized care services for HIV/AIDS in the Noroeste Cearense and north of Jaguaribe.

ABSTRACT OBJECTIVE To describe the spatial distribution of avoidable hospitalizations due to tuberculosis in the municipality of Ribeirao Preto, SP, Brazil, and to identify spatial and space-time clusters for the risk of occurrence of these events. METHODS This is a descriptive, ecological study that considered the hospitalizations records of the Hospital Information System of residents of Ribeirao Preto, SP, Southeastern Brazil, from 2006 to 2012. Only the cases with recorded addresses were considered for the spatial analyses, and they were also geocoded. We resorted to Kernel density estimation to identify the densest areas, local empirical Bayes rate as the method for smoothing the incidence rates of hospital admissions, and scan statistic for identifying clusters of risk. Softwares ArcGis 10.2, TerraView 4.2.2, and SaTScanTM were used in the analysis. RESULTS We identified 169 hospitalizations due to tuberculosis. Most were of men (n = 134; 79.2%), averagely aged 48 years (SD = 16.2). The predominant clinical form was the pulmonary one, which was confirmed through a microscopic examination of expectorated sputum (n = 66; 39.0%). We geocoded 159 cases (94.0%). We observed a non-random spatial distribution of avoidable hospitalizations due to tuberculosis concentrated in the northern and western regions of the municipality. Through the scan statistic, three spatial clusters for risk of hospitalizations due to tuberculosis were identified, one of them in the northern region of the municipality (relative risk [RR] = 3.4; 95%CI 2.7–4,4); the second in the central region, where there is a prison unit (RR = 28.6; 95%CI 22.4–36.6); and the last one in the southern region, and area of protection for hospitalizations (RR = 0.2; 95%CI 0.2–0.3). We did not identify any space-time clusters. CONCLUSIONS The investigation showed priority areas for the control and surveillance of tuberculosis, as well as the profile of the affected population, which shows important aspects to be considered in terms of management and organization of health care services targeting effectiveness in primary health care.

ABSTRACT OBJECTIVE To estimate the prevalence of arterial hypertension and obesity and the population attributable fraction of hypertension that is due to obesity in Brazilian adolescents. METHODS Data from participants in the Brazilian Study of Cardiovascular Risks in Adolescents (ERICA), which was the first national school-based, cross-section study performed in Brazil were evaluated. The sample was divided into 32 geographical strata and clusters from 32 schools and classes, with regional and national representation. Obesity was classified using the body mass index according to age and sex. Arterial hypertension was defined when the average systolic or diastolic blood pressure was greater than or equal to the 95th percentile of the reference curve. Prevalences and 95% confidence intervals (95%CI) of arterial hypertension and obesity, both on a national basis and in the macro-regions of Brazil, were estimated by sex and age group, as were the fractions of hypertension attributable to obesity in the population. RESULTS We evaluated 73,399 students, 55.4% female, with an average age of 14.7 years (SD = 1.6). The prevalence of hypertension was 9.6% (95%CI 9.0-10.3); with the lowest being in the North, 8.4% (95%CI 7.7-9.2) and Northeast regions, 8.4% (95%CI 7.6-9.2), and the highest being in the South, 12.5% (95%CI 11.0-14.2). The prevalence of obesity was 8.4% (95%CI 7.9-8.9), which was lower in the North region and higher in the South region. The prevalences of arterial hypertension and obesity were higher in males. Obese adolescents presented a higher prevalence of hypertension, 28.4% (95%CI 25.5-31.2), than overweight adolescents, 15.4% (95%CI 17.0-13.8), or eutrophic adolescents, 6.3% (95%CI 5.6-7.0). The fraction of hypertension attributable to obesity was 17.8%. CONCLUSIONS ERICA was the first nationally representative Brazilian study providing prevalence estimates of hypertension in adolescents. Regional and sex differences were observed. The study indicates that the control of obesity would lower the prevalence of hypertension among Brazilian adolescents by 1/5.

Cercariae of Schistosoma mansoni inoculated into the peritoneal cavity of naive mice induced host cell adhesion to their surface, but after 90 minutes the number of adherent cells sharply decreased. The cell detachment is progressive and simultaneous to the cercaria-schistosomule transformation. The histological study showed mainly neutrophils in close contact with the larvae. Mononuclear cells and some eosinophils were occasionally seen surrounding the adherent neutrophils. The scanning electron microscopy showed cells displaying twisted microvilli and several microplicae contacting or spreading over the larval surface, and larvae completely surrounded by clusters of cells. These results suggest that the neutrophils recognize molecules on the cercarial surface which induce their spreading

The recognition profile of the tissue cysts antigens by IgG antibodies was studied during acute and chronic human toxoplasmic infection. Thus the IgG response against Toxoplasma gondii was investigated by immunoblotting in two patients accidentally infected with the RH strain as well as in group of naturally infected patients at acute and chronic phase. There was an overall coincidence of molecular mass among antigens of tachyzoites and tissue cysts recognized by these sera, however, they appear not to be the same molecules. The response against tissue cysts starts early during acute infection, and the reactivity of antibodies is strong against a wide range of antigens. Six bands (between 82 and 151 kDa) were exclusively recognized by chronic phase sera but only the 132 kDa band was positive in more than 50% of the sera analysed. A mixture of these antigens could be used to discriminate between the two infection phases. The most important antigens recognized by the acute and the chronic phase sera were 4 clusters in the ranges 20-24 kDa, 34-39 kDa, 58-80 kDa and 105-130 kDa as well as two additional antigens of 18 and 29 kDa. Both accidentally infected patients and some of the naturally infected patients showed a weak specific response against tissue cyst antigens.

We describe the avidity maturation of IgGs in human toxoplasmosis using sequential serum samples from accidental and natural infections. In accidental cases, avidity increased continuously throughout infection while naturally infected patients showed a different profile. Twenty-five percent of sera from chronic patients having specific IgM positive results could be appropriately classified using exclusively the avidity test data. To take advantage of the potentiality of this technique, antigens recognized by IgG showing steeper avidity maturation were identified using immunoblot with KSCN elution. Two clusters of antigens, in the ranges of 21-24 kDa and 30-33 kDa, were identified as the ones that fulfill the aforementioned avidity characteristics.
