Pesquisamos os anticorpos anti-HBc e anti-HCV em amostras de soros provenientes de 799 candidatos a doadores, que tiveram suas unidades de sangue ou derivados transfundidas a 111 receptores. O anti-HBc e o anti-HCV foram reagentes, em respectivamente 9 e 2,1% dos doadores testados. Observamos que entre os 111 receptores, 44 haviam recebido pelo menos uma unidade anti-HBc positiva e 67 haviam sido transfundidos somente com unidades anti-HBc negativas. Houve um risco 4,5 vezes maior de aquisição de hepatite por vírus C pelos receptores que receberam pelo menos uma unidade anti-HBc positiva Se a pesquisa do anti-HBc fosse realizada na triagem sorológica dos doadores de sangue, cerca de 56% dos casos de HVC nos receptores saiam evitados. A população de receptores que recebeu pelo menos uma unidade anti-HCV reagente, apresentou um risco 29 vezes maior de adquirir esta hepatite, quando comparada aos receptores transfundidos com todas as unidades anti-HCV negativas. A realização do teste para a pesquisa do anti-HCV na triagem dos doadores de sangue, preveniria 79% dos casos de HVC pós-transfusionais. Os candidatos a doadores brasileiros parecem ser acometidos simultânea ou sequencialmente, pelos vírus B e C das hepatites, pois, 44,4% dos doadores anti-HCV positivos, também foram anti-HBc positivos. A realização dos testes para as pesquisas dos anticorpos anti-HBc e anti-HCV, nas triagens hemoterápicas, está indicada para prevenir a transmissão de hepatites pós-transfusionais, em nosso meio.

En una localidad con población rural dispersa ("La Bolsa", 3ª seccional, dpto. de Artigas, Uruguay) se estudió, en ámbitos peridomiciliarios, el perfil alimentario de T. rubrovaria (triatomíneo silvestre y potencial vector secundario de colonización intradomiciliaria), utilizándose la técnica de doble difusión en agar, enfrentando contenido promesentérico frente a un panel de 13 sueros. Se pudo detectar en 120 insectos 251 identificaciones de fuente hematofágica con alimentación predominante en mamíferos (73%), pero marcado eclectismo alimentario (mamíferos, aves, reptiles y cucarachas), incluyendo hematofagia sobre seres humanos en un 8% de las identificaciones totales. La micropredación de hemolinfa lo ubicaría en una situación evolutiva primitiva, intermedia entre predator/entomófago y triatomineo/hematófago, que comparte con T. circunmaculata. Los mamíferos detectados con mayor frecuencia fueron dasipódidos y bóvidos, aunque la fuente hematofágica, salvo en los adultos alados, se constituye en un fenómeno de proximidad ocasional por cohabitación de un mismo habitat. La frecuencia de alimentación sobre hombre, hallada en un ambiente peridomiciliario, aporta un importante elemento a su capacidad vectorial potencial. En el análisis espacial de las dietas se muestra al peridomicilio como un área de interacción de hospederos domésticos, silvestres y sinantrópicos. La infección tripanosómica fue mínima comparada con las altas tasas de infección de ámbitos silvestres.

The determination of aminotranferases levels is very useful in the diagnosis of hepatopathies. In recent years, an elevated serum ALT level in blood donors has been associated with an increased risk of post-transfusion hepatitis (PTH). The purpose of the study was to research the factors associated with elevated ALT levels in a cohort of voluntary blood donors and to evaluate the relationship between increased ALT levels and the development of hepatitis C (HCV) infection. 166 volunteer blood donors with elevated ALT at the time of their first donation were studied. All of the donors were questioned about previous hepatopathies, exposure to hepatitis, exposure to chemicals, use of medication or drugs, sexual behaviour, contact with blood or secretions and their intake of alcohol. Every three months, the serum levels of AST, ALT, alkaline phosphatase, gamma glutamyl transpeptidase, cholesterol, triglyceride and glycemia are assessed over a two year follow-up. The serum thyroid hormone levels as well as the presence of auto-antibodies were also measured. Abdominal ultrasound was performed in all patients with persistently elevated ALT or AST levels. A needle biopsy of liver was performed in 9 donors without definite diagnostic after medical investigation. The presence of anti-HCV antibodies in 116 donors were assayed again the first clinical evaluation. At the end of follow-up period (2 years later) 71 donors were tested again for the presence of anti-HCV antibodies. None of donors resulted positive for hepatitis B or hepatitis C markers during the follow-up. Of the 116 donors, 101 (87%) had persistently elevated ALT serum levels during the follow-up. Obesity and alcoholism were the principal conditions related to elevated ALT serum levels in 91/101 (90.1%) donors. Hypertriglyceridemia, hypercholesterolemia, hypothyroidism and diabetes mellitus also were associated with increased ALT levels. Only 1/101 (0.9%) had mild chronic active non A-G viral hepatitis and 3/101 (2.9%) had liver biopsy with non-specific reactive hepatitis. The determination of ALT levels was not useful to detect donors infected with HCV at donation in Brazil, including the initial seronegative anti-HCV phase.

In the last years, new techniques of neuroimages and histopathological methods have been added to the management of cerebral mass lesions in patients with AIDS. Stereotactic biopsy is necessary when after 14 days of empirical treatment for Toxoplasma gondii encephalitis there is no clinical or neuroradiologic improvement. We report a woman with AIDS who developed a single focal brain lesion on the right frontal lobe. She presented a long history of headache and seizures. After two weeks of empirical treatment for toxoplasma encephalitis without response, a magnetic resonance image with spectroscopy was performed and showed a tumoral pattern with a choline peak, diminished of N-acetyl-aspartate and presence of lactate. A stereotactic biopsy was performed. Histopathological diagnosis was a diffuse oligodendroglioma type A. A microsurgical resection of the tumor was carried out and antiretroviral treatment was started. To date she is in good clinical condition, with undetectable plasma viral load and CD4 T cell count > 200 cell/uL.

We detected Toxoplasma gondii oocysts in feces of experimentally infected cats, using a Kato Katz approach with subsequent Kinyoun staining. Animals serologically negative to T. gondii were infected orally with 5x10² mice brain cysts of ME49 strain. Feces were collected daily from the 3rd to the 30th day after challenge. Oocysts were detected by qualitative sugar flotation and the quantitative modified Kato Katz stained by Kinyoun (KKK). In the experimentally infected cats, oocysts were detected from the 7th to 15th day through sugar flotation technique, but oocysts were found in KKK from the 6th to 16th day, being sensitive for a larger period, with permanent documentation. The peak of oocysts excretion occurred between the 8th to 11th days after challenge, before any serological positive result. KKK could be used in the screening and quantification of oocysts excretion in feces of suspected animals, with reduced handling of infective material, decreasing the possibility of environmental and operator contamination.

The authors report two cases of adrenal insufficiency secondary to infiltration of the adrenal glands by Paracoccidioides brasiliensis. The first patient had been treated for a chronic multifocal form of paracoccidiodomycosis 11 years ago. The diagnosis of the mycosis was done simultaneous with that of the adrenal insufficiency in the second patient. In both patients the diagnosis was done by direct visualization of fungus in adrenal biopsies. They were treated with hormonal supplements and itraconazol by 12 and six months, without relapses during the follow-up period.

This work compared the time at which negative seroconversion was detected by conventional serology (CS) and by the ELISA-F29 test on a cohort of chronic chagasic patients treated with nifurtimox or benznidazole. A retrospective study was performed using preserved serum from 66 asymptomatic chagasic adults under clinical supervision, and bi-annual serological examinations over a mean follow-up of 23 years. Twenty nine patients received trypanocide treatment and 37 remained untreated. The ELISA-F29 test used a recombinant antigen which was obtained by expressing the Trypanosoma cruzi flagellar calcium-binding protein gene in Escherichia coli. Among the untreated patients, 36 maintained CS titers. One patient showed a doubtful serology in some check-ups. ELISA-F29 showed constant reactivity in 35 out of 37 patients and was negative for the patient with fluctuating CS. The treated patients were divided into three groups according to the CS titers: in 13 they became negative; in 12 they decreased and in four they remained unchanged. ELISA-F29 was negative for the first two groups. The time at which negativization was detected was significantly lower for the ELISA-F29 test than for CS, 14.5 ± 5.7 and 22 ± 4.9 years respectively. Negative seroconversion was observed in treated patients only. The results obtained confirm that the ELISA-F29 test is useful as an early indicator of negative seroconversion in treated chronic patients.

We present a case of histoplasmosis with multiple pulmonary nodules in a patient with a history of melanoma. This case closely simulated malignancy, including the presence of feeding vessel sign, which occurs in pulmonary metastasis. We emphasize the need to be aware of this infection in areas where histoplasmosis is endemic.

Neste trabalho são relatadas pesquisas parasitológicas e epiâemiológicas realizadas numa provável fonte de infecção de Histoplasmose da área rural do D. F. - Brasília, onde 14 pessoas contraíram a doença. Os estudos clínico, imunológico e radiológico foram anteriormente descritos. Os autores conseguiram isolar o H. capsulatum do solo da caverna e das vísceras e do sangue de morcegos (Phyllostomus hastatus hastatus, Pallas 1767) que nela habitavam. Resultaram negativas as tentativas de isolamento do fungo de animais sentinelas (cobaios), assim como não se obteve neles a viragem dos testes intradérmicos com histoplasmina. Em impressões de vísceras dos morcegos, constataram-se formas semelhantes as do T. gondii que posteriormente foram isoladas, em camundongos jovens, por inoculação de vísceras maceradas e sangue. Foram encontrados 2 ectoparasitos nos morcegos: Boophilus microplus e um díptero da família Streblidae. O ácaro albergava tripomastigotos do tipo cruzi, não sendo porém conseguido seu isolamento. No tubo digestivo dos quirópteros, foram retirados nematódeos (Histiostrongylus octaeantus) e cestódeos do gênero Mathevotaenia. Foram capturados, em torno da entrada da caverna, 2 exemplares de Cercomys cunicularis apereoide não sendo examinados sob o ponto de vista parasitológico. Testes intradérmicos realizados em 826 habitantes da área resultaram positivos em 184 (22,27%). A gruta, fonte da infecção, está localizada em uma formação calcárea, pertencente à série Bambuí, acreditando-se, pelos aspectos tectônicos, ser da idade siluriana. No Brasil, o isolamento de H. capsulatum de solo, guanos de morcegos e vísceras de roedores já tinham sido realizados; contudo, esta foi a primeira vez que se conseguiu isolá-lo do solo de uma caverna, fonte de infecção, e das vísceras e sangue de morcegos. Os resultados obtidos com os testes intradérmicos com histoplasmina demonstraram a prevalência de histoplasmose na área rural do DF e, conseqüentemente, a possibilidade de ocorrerem outra microepidemia ou aparecimento de casos isolados de histoplasmose-doença.

O reconhecimento das espécies Triatoma maculata (Erichson, 1848) e Triatoma pseudomaculata (Correia e Espínola, 1964), tem sido e continua sendo objeto de muita controvérsia. Essas espécies foram também confundidas pelo autor durante algum tempo. Possivelmente, a razão dessa confusão está na precária descrição de certas estruturas dessas espécies, de modo que não se pode ter certeza de sua distribuição no Brasil. Um estudo comparativo cuidadoso envolvendo ambas as espécies está aqui relatado e concluiu-se que é o T. pseudomaculata e não o T. maculata que está sendo encontrado nos Estados do Piauí, Ceará, Rio Grande do Norte, Paraíba, Pernambuco, Alagoas, Bahia, Minas Gerais, Goiás e Distrito Federal - Brasília.

Esta investigación tiene por objeto analizar las características epidemiológicas, clínicas, electrocardiográficas y radiológicas de una población de seropositivos y seronegativos al Trypanosoma cruzi en una área rural del Departamento San Miguel, provincia de Corrientes, Argentina. Se seleccionaron 132 pobladores (58 hombres y 74 mujeres) de todos los grupos etareos para la realización de un examen clínico y cardiovascular. Se registraron síntomas y signos compatibles con afección cardíaca, tensión arterial, electrocardiograma de 12 derivaciones y radiografía anteroposterior de tórax. En la signo-sintomatología, anomalías electrocardiográficas y en las alteraciones radiográficas detectadas, no se observaron diferencias estadísticamente significativas entre seropositivos y seronegativos. Se concluye que si bien en la población objeto de estudio la infección chagásica no se asoció a mayor prevalencia de cardiopatía, cabe destacar que los pacientes chagásicos pertenecían a un grupo etareo muy joven con un 54,0% de ellos menores de 20 años y que el 45,0% de los pacientes mayores de 41 años tenían alteraciones electrocardiográficas.

A partir da indicação clínica de Rotavírus em fezes de 485 crianças, investigou-se a presença de oocistos de Cryptosporidium sp. Não houve diferenças significativas entre a positividade de Cryptosporidium sp e rotavírus com a consistência das fezes. Cryptosporidium sp deve ser incluído na investigação diagnóstica dos quadros diarréicos em crianças.
