Sixteen S. mansoni infected and untreated patients (5 with recent infection and 11 with chronic disease) were evaluated for their in vitro natural killer (NK) activity against the NK sensitive target K562 cell line. NK levels in 9 out of 11 patients (82%) with chronic disease were significantly lower (mean = 15 ± 6%),compared with patients recently infected (mean = 41 ± 9% p < 0.001) and with the control group (mean = 38 ± 13% p < 0.001). However, both patients and controls NK activity was stimulated by soluble adult worm antigens (SAWA), indicating that NK function even in the chronic stage of the infection is able to respond to the parasite antigens. These results suggest the possibility of NK cell participation as effector mechanism against S. mansoni.

A 27 year Old male developed seizures after receiving a single 20 mg/kg dose of praziquantel for the treatment of an intestinal Hymenolepis nana infection. On further clinical and laboratorial evaluations, he was found to suffer from an until then asymptomatic parenchymal brain cysticercosis. Praziquantel must be used with caution in those areas where cysticercosis represents a mayor public health problem. The occurrence of unexpected seizures in an individual being treated with the compound, must prompt clinicians to rule out cysticercosis of the CNS.

Two patients from the gold mines of Bolivar State, Venezuela, presenting cutaneous leishmaniasis in the genital region, an unusual location, are described. The first patient showed an ulcerated lesion of the glans penis. Leishmanin skin test was positive. A biopsy specimen revealed a granulomatous infiltrate containing Leishmania parasites. In the second patient, Leishmanin skin test was positive, HIV and VDRL were negative. Leishmania parasites were present in a biopsy of an ulcerated lesion in the scrotum, with an indurated base, infiltrative borders with an yellowish exudate. Patients were treated with meglumine antimoniate and the lesions healed.

A leptospirosis clinical-epidemiological study was made in humans and reservoirs in the state of Yucatán, México. Interviews and serological analyses were made on 400 persons from an open population, 439 probable cases of leptospirosis and 1060 animal reservoirs (cows, pigs, dogs, rats and opossums). IgM Leptospira DipstickTM and Microscopic Agglutination Test (MAT) was used to detect human antibodies to leptospiras and serovar respectively. Leptospirosis incidence in humans was 2.2/100,000 inhab. in 1998, 0.7/100,000 in 1999 and 0.9/100,000 in 2000. Overall seroprevalence was 14.2%, relatively unchanged from seroprevalences observed 20 years ago. Highest seropositivity was found in people over 56 years of age, predominating males over females. Predominant serovars in the open population were tarassovi, hardjo, pomona and panama. Leptospirosis cases were most frequent in rural areas, and the anicteric course predominated over the icteric. The panama, icterohaemorrhagiae and pomona serovars predominated in both anicteric and icteric courses. Dogs, pigs and rodents had the highest seropositivity among the reservoirs. Contact with rodents and natural water sources were significant factors (p £ 0.05). Human cases (74%) occurred during the rainy season. It is concluded that leptospirosis is still a serious illness with important clinical and epidemiological implications in the state of Yucatán, Mexico.

A case of mycotic bovine nasal granuloma in a 10 year-old Jersey cow, produced by Drechslera halodes is presented. Histopathological sections showed abundant hyaline and pigmented extra and intracellular fungal structures together with a polymorphic cellular granuloma formed by neutrophils, lymphocytes, plasmocytes, histiocytes and giant cells of the Langhans type. It is the first case of mycotic bovine nasal granuloma recognized in Uruguay although this disease seems to be frequent according to the opinion of veterinarian specialists. Another similar clinical case also in a Jersey cow from the same dairy house with an intense cellular infiltrate rich in eosinophils without granulomatous image, together with extracellular hyaline and fuliginous fungal forms, is also referred for comparative purposes. Geotrichum sp. was isolated. The need of an early diagnosis and treatment of the disease is stressed.

We report a human case of polycystic hidatidosis due to Echinococcus vogeli from Contamana (Department of Loreto) village located in the central jungle of Peru. The patient is a 44 year-old lady, teacher, who carried a painless liver mass since a year ago. She was submitted to abdominal surgery and the liver mass was removed and showed multiple cysts containing colorless liquid as is showed in the polycystic hidatidosis. The morphology and measure of the hooks obtained from the liquid contained in the cysts are from Echinococcus vogeli. It is the first report of this parasitism in Perú.

The authors report two cases of adrenal insufficiency secondary to infiltration of the adrenal glands by Paracoccidioides brasiliensis. The first patient had been treated for a chronic multifocal form of paracoccidiodomycosis 11 years ago. The diagnosis of the mycosis was done simultaneous with that of the adrenal insufficiency in the second patient. In both patients the diagnosis was done by direct visualization of fungus in adrenal biopsies. They were treated with hormonal supplements and itraconazol by 12 and six months, without relapses during the follow-up period.

SUMMARY Chagas disease (CD) is an endemic anthropozoonosis from Latin America of which the main means of transmission is the contact of skin lesions or mucosa with the feces of triatomine bugs infected by Trypanosoma cruzi. In this article, we describe the first acute CD case acquired by vector transmission in the Rio de Janeiro State and confirmed by parasitological, serological and PCR tests. The patient presented acute cardiomyopathy and pericardial effusion without cardiac tamponade. Together with fever and malaise, a 3 cm wide erythematous, non-pruritic, papule compatible with a "chagoma" was found on his left wrist. This case report draws attention to the possible transmission of CD by non-domiciled native vectors in non-endemic areas. Therefore, acute CD should be included in the diagnostic workout of febrile diseases and acute myopericarditis in Rio de Janeiro.

The use of cryotherapy as an adjunct to systemic antimonial therapy (Clucantime) was studied in 17 patients with a total of23 skin lesions of leishmaniasis in an area where L. braziliensis braziliensis is the species in circulation. Cryotherapy did not speed healing and has been discarded as an auxiliary therapeutic measure in our practice. However this technique may be suitable for species o/Leishmania causing more limited superficial lesions in man without the danger of metastasis.

Foi estudada a suscetibilidade de Dipetalogaster maximus, Rhodnius neglectus, R. prolixus, R. robustus, Triatoma infestans e T. rubrovaria ao Trypanosoma cruzi, através do xenodiagnóstico, em dois pacientes na fase aguda da doença de Chagas. Usaram-se como parâmetros o número de triatomíneos infectados e o número de tripanosomas excretados. A leitura do xenodiagnóstico foi realizada pelos métodos da compressão abdominal e o das dejeções espontâneas. Este foi mais eficiente que aquele, em relação ao número de T. cruzi por lâmina pelo teste de Wilcoxon. Em um dos pacientes, tendo como parâmetro o número de triatomíneos infectados, a suscetibilidade das espécies obedeceu a seguinte ordem: D. maximus e R. neglectus (100%), R. robustus (95%), R. prolixus e T. rubrovaria (90%) e T. infestans (857c). Nesse mesmo paciente, a suscetibilidade das espécies de triatomíneos, avaliada pelo número de T. cruzi excretados, usando-se a compressão abdominal, encontra-se na seguinte ordem: R. neglectus, R. prolixus, D. maximus eR. robustus, T. infestans eT. rubrovaria. Pelo método das dejeções espontâneas, a ordem foi: D. maximus e R. prolixus, T. rubrovaria, R. robustus,T. infestans e R. neglectus. No outro paciente, esse parâmetro, em ambas as técnicas de leitura, mostrou a mesma ordem de suscetibilidade (R. neglectus, T. rubrovaria e T. infestans), porém, o número de tripanosomas excretados foi significativamente maior pelo método das dejeções espontâneas.

O xenodiagnóstico (xeno) clássico e artificial feitos com Dipatalogaster maximus de primeiro estágio foi realizado simultaneamente em 57 pacientes com infecção chagásica crônica (22 do sexo masculino e 35 do sexo feminino, com idades entre 7 e 80 anos). Exceto dois pacientes com clínica de megaesôfago, os demais tinham dois exames sorológicos prévios positivos sendo feita nova sorológia no decorrer do estudo. Os pacientes eram provenientes do ambulatório do Hospital Universitário de Brasília (HUB) ou eram residentes no município de Mambaí, GO. Dos 57 pacientes, 24 (42%) apresentaram xenodiagnósticos positivos. Dos 114 xenodiagnósticos realizados, 36 (32%) foram positivos. A comparação das duas técnicas não mostrou diferença estatisticamente significante (p = 0,42), porém o xeno artificial apresenta vantagem porque o sangue é oferecido aos triatomíneos através de um aparelho enquanto, no xeno clássico, os triatomíneos sugam através da pele do paciente.

Uma paciente com síndrome de imunodeficiência adquirida (SIDA) e doença de Chagas, com xenodiagnóstico positivo, estava em uso prolongado de cetoconazol com o objetivo de suprimir a parasitemia e prevenir a reativação da doença de Chagas. O cetoconazol foi suspenso inadvertidamente após 6 meses de uso. Um mês após, a paciente foi internada com febre, cefaléia, vômitos, taquicardia e hepatoesplenomegalia. Tanto o xenodiagnóstico como o exame de sangue a fresco demonstraram a presença de Trypanosoma cruzi. O tratamento com benzonidazol foi instituído, com supressão da parasitemia. A paciente desenvolveu concomitantemente uma provável neurotoxoplasmose, evoluindo para o óbito em septicemia. À necropsia, não foram encontrados parasitas.

The present open pilot study was conducted to assess the efficacy of azithromycin for the treatment of patients with cutaneous leishmaniasis in Araçuaí and Varzelândia, MG. Twenty-four patients with less of six months of disease evolution were treated after clinical examination, Montenegro test and a biopsy. The treatment schemes consisted of oral doses of 500 mg per day for 3, 5 and 10 days and of 1000 mg for two days. A clinical control was performed monthly and treatment cycles were repeated when necessary until full reepithelialization of the lesions. On the occasion of the final evaluation, 20 patients had completed the study and 17 of them (85%) were cured. The time to obtain a cure was 60 days ifor 6 (30%) patients, 90 days for 7 (35%), and 120 for 4 (20%). The three patients with treatment failure received a pentavalent antimonial for 20 days. No adverse reactions to the medication were observed and a 14 month follow-up did not show recurrence in any patient. These results suggest that azithromycin can be a good therapeutic option for the treatment of cutaneous leishmaniasis caused by Leishmania Viannia brasiliensis.

A partir da emergência de um surto de casos agudos de doença de Chagas em Montalvânia, norte de Minas Gerais, Brasil, e da implantação do controle triatomínico com inseticidas, verifica-se pronta e conseqüente desaparição dos casos e redução da incidência da infecção no Município. É mostrada a evolução da implantação e do controle do Triatoma infestans em Montalvânia e a evolução da sorologia praticada na população em geral, ao longo dos últimos trinta anos.
