OBJECTIVE To analyze the effects of acupressure at the SP6 point on labor duration and cesarean section rates in parturients served in a public maternity hospital.METHODS This controlled, randomized, double-blind, pragmatic clinical trial involved 156 participants with gestational age ≥ 37 weeks, cervical dilation ≥ 4 cm, and ≥ 2 contractions in 10 min. The women were randomly divided into an acupressure, placebo, or control group at a university hospital in an inland city in the state of Sao Paulo, Brazil, in 2013. Acupressure was applied to the SP6 point during contractions for 20 min.RESULTS The average labor duration was significantly different between the SP6 acupressure group [221.5 min (SD = 162.4)] versus placebo [397.9 min (SD = 265.6)] and versus control [381.9 min (SD = 358.3)] (p = 0.0047); however, the groups were similar regarding the cesarean section rates (p = 0.2526) and Apgar scores in the first minute (p = 0.9542) and the fifth minute (p = 0.7218) of life of the neonate.CONCLUSIONS The SP6 acupressure point proved to be a complementary measure to induce labor and may shorten the labor duration without causing adverse effects to the mother or the newborn. However, it did not affect the cesarean section rate.

OBJECTIVE To estimate the incidence and predicting factors associated with falls among older inpatients.METHODS Prospective cohort study conducted in clinical units of three hospitals in Cuiaba, MT, Midwestern Brazil, from March to August 2013. In this study, 221 inpatients aged 60 or over were followed until hospital discharge, death, or fall. The method of incidence density was used to calculate incidence rates. Bivariate analysis was performed by Chi-square test, and multiple analysis was performed by Cox regression.RESULTS The incidence of falls was 12.6 per 1,000 patients/day. Predicting factors for falls during hospitalization were: low educational level (RR = 2.48; 95%CI 1.17;5.25), polypharmacy (RR = 4.42; 95%CI 1.77;11.05), visual impairment (RR = 2.06; 95%CI 1.01;4.23), gait and balance impairment (RR = 2.95; 95%CI 1.22;7.14), urinary incontinence (RR = 5.67; 95%CI 2.58;12.44) and use of laxatives (RR = 4.21; 95%CI 1.15;15.39) and antipsychotics (RR = 4.10; 95%CI 1.38;12.13).CONCLUSIONS The incidence of falls of older inpatients is high. Predicting factors found for falls were low education level, polypharmacy, visual impairment, gait and balance impairment, urinary incontinence and use of laxatives and antipsychotics. Measures to prevent falls in hospitals are needed to reduce the incidence of this event.

The authors have standardized methods for evaluation of the activity of the glucose-6-phosphate dehydrogenase and of glutathione reductase. The general principle of the first method was based on methemoglobin formation by sodium nitrite followed by stimulation of the glucose-6-phosphate dehydrogenase with methylene blue. Forty six adults (23 males and 23 females) were studied. Subjects were not G6PD deficient and were aged 20 to 30 years. The results showed that methemoglobin reduction by methylene blue was 154.40 and 139.90 mg/min (p<0.05) for males and females, respectively, in whole blood, and 221.10 and 207.85 mg/min (n.s.), respectively, in washed red cells. These data showed that using washed red cells and 0.7g% sodium nitrite concentration produced no differences between sexes and also shortened reading time for the residual amount of methemoglobin to 90 minutes. Glutathione reductase activity was evaluated on the basis of the fact that cystamine (a thiol agent) binds to the SH groups of hemoglobin, forming complexes. These complexes are reversed by the action of glutathione reductase, with methemoglobin reduction occurring simultaneously with this reaction. Thirty two adults (16 males and 16 females) were studied. Subjects were not G6PD deficient and were aged 20 to 30 years. Methemoglobin reduction by cystamine was 81.27 and 91.13 mg/min (p<0.01) for males and females, respectively. These data showed that using washed red cells and 0.1 M cystamine concentration permits a reading of the residual amount of methemoglobin at 180 minutes of incubation. Glutathione reductase activity was evaluated by methemoglobin reduction by cystamine in 14 females before and after treatment with 10 mg riboflavin per day for 8 days. The results were 73.69 and 94.26 jug/min (p<0.01) before and after treatment, showing that riboflavin treatment increase glutathione reductase activity even in normal individuals. Three Black G6PD-deficient individuals (2 males and 1 female) were also studied. The G6PD and glutathione reductase were partially activated, the change being more intense in males. On the basis of race and of the laboratory characteristics observed, it is possible to suggest that the G6PD deficiency of these individuals is of the African type and that the female is heterozygous for this deficiency. Analysis of the results as a whole permitted us to conclude that the methods proposed here were efficient for evaluating the activity of the glucose-6-phosphate dehydrogenase and of glutathione reductase. The latter is dependent on the pentose pathway, which generates NADPH, and on riboflavin, a FAD precursor vitamin.

Adenovirus are important pathogen primarily associated to respiratory infections of children and military personnel, even though it is also associated to cases of conjunctivitis and keratoconjunctivitis. We analyzed respiratory secretion collected from subjects with and without respiratory infection symptoms, being 181 civilians and 221 military subjects. The samples were inoculated in HEp-2 and/or A549 tissue cultures for viral isolation. Samples presenting cytopathogenic effect (CPE) in any tissue culture were tested by a polymerase chain reaction (PCR) assay to confirm adenovirus isolation. The isolates confirmed as adenovirus were further analyzed by restriction endonuclease assay for determination of viral species. Three isolates were identified as specie A (two from civilian and one from military), one isolate from military was identified as specie C, and one isolate from civilian was identified as specie D. For two isolates the specie could not be identified.

The effects of eucalyptol were evaluated against the house fly, Musca domestica L., and blow fly, Chrysomya megacephala (F.). The bioassay of adults, using topical application, indicated that M. domestica males were more susceptible than females, with the LD50 being 118 and 177 µg/fly, respectively. A higher LD50 of C. megacephala was obtained; 197 µg/fly for males and 221 µg/fly for females. Living flies of both species yielded a shorter life span after being treated with eucalyptol. The bioassay of larvae, using the dipping method on the third instar, showed that M. domestica was more susceptible than C. megacephala, with their LC50 being 101 and 642 µg/µl, respectively. The emergence of adults, which had been treated with eucalyptol in larvae, decreased only in M. domestica. Having the volatile property, fumigation or impregnated paper test of eucalyptol or the efficacy of repellence or attractiveness merits further investigations to enhance bio-insecticidal efficacy.

Foi escolhida, para demolição, uma casa no município de Pereiro, território de prevalência de Triatoma, pseudo maculata, onde raros exemplares de Triatoma, braziliensis são encontrados. Numa área de 13 casas, onde vivem 74 pessoas, foram capturados 41 T., pseudo maculata que apresentaram taxas de infecção de 2,4%. Em Pereiro a infecção hümana é de 2,2% e é baixa a infecção de animais: 2,0% pelo xenodiagnóstico. A casa selecionada foi desinsetizada mais de um ano antes da demolição e 20 dias antes da demolição uma captura mostrou 20 T., pseudo maculata, sendo 50% infectados. Durante a domolição estavam presentes: 6 pessoas, 1 cão. 1 gato, 1 porco e 30 galinhas; na ocasião foram capturados 221 triatomíneos, no teto (0,5 por m²); e nas paredes internas e externas (14 por m²); 180 exemplares foram examinados e apresentaram 7,2% de infecção. Concluiu-se que os métodos normais de captura mostram pequena parte da colônia de triatomíneos numa casa habitada e a taxa de infecção do T. pseudomaculata observada revela sua capacidade de transmissão do Trypanossoma cruzi, pois 20% dos exemplares infectados foram encontrados no quarto.

ln order to study the measles antibody behavior of three vaccination schedules, 684 children were divided into 4 Groups: Group A (341 vaccinated children under the age of one); Group B (101 children at the age of one); Group C (74 children under the age of one and one at the age of one); Group D (163 unvaccinated children with a history of measles in the past - Group control). Children of Group A presented lower rates and 25.9% of the age group under two did not show any measles antibodies. In Group B, all the children presented antibodies. In Group C onby 4.0% did not. In all age groups, the geometric mean HI antibody titers of Group A were lower than the valuesfound in the other groups. The age at vaccination was the factor of greater influence on the results of this study.

INTRODUÇÃO: O presente estudo buscou identificar os efeitos adversos e o efeito protetor da vacina contra influenza, em idosos acima de 60 anos, aplicada pela rede pública no município de Tubarão, Santa Catarina. MÉTODOS: Foi realizada uma coorte prospectiva de pacientes durante o período de maio a setembro de 2008. Foram recrutados 341 idosos, sendo 289 vacinados na rede pública (VSUS) e 52 não vacinados (NV). A incidência do efeito protetor foi verificada através da comparação dos grupos VSUS com os NV. RESULTADOS: Um percentual de 22,5% de pacientes apresentou pelo menos um efeito adverso. Comparando o grupo VSUS e NV quanto à incidência de sintomas gripais durante o inverno, a presença de pelo menos um sintoma foi verificada em 47% e 28,8% (RR = 1,11; 1,02-1,22; p = 0,0156), respectivamente. CONCLUSÕES: Os resultados apontam uma baixa incidência de efeitos adversos. A maior incidência de sintomas gripais no grupo VSUS comparado ao NV pode estar relacionado às características da população que costuma fazer a vacina contra influenza.

INTRODUCTION: The objective was to identify space and space-time risk clusters for the occurrence of deaths in a priority city for the control of tuberculosis (TB) in the Brazilian Northeast. METHODS: Ecological research was undertaken in the City of São Luis/Maranhão. Cases were considered that resulted in deaths in the population living in the urban region of the city with pulmonary TB as the basic cause, between 2008 and 2012. To detect space and space-time clusters of deaths due to pulmonary TB in the census sectors, the spatial analysis scan technique was used. RESULTS: In total, 221 deaths by TB occurred, 193 of which were due to pulmonary TB. Approximately 95% of the cases (n=183) were geocoded. Two significant spatial clusters were identified, the first of which showed a mortality rate of 5.8 deaths per 100,000 inhabitants per year and a high relative risk of 3.87. The second spatial cluster showed a mortality rate of 0.4 deaths per 100,000 inhabitants per year and a low relative risk of 0.10. A significant cluster was observed in the space-time analysis between 11/01/2008 and 04/30/2011, with a mortality rate of 8.10 deaths per 100,000 inhabitants per year and a high relative risk (3.0). CONCLUSIONS: The knowledge of priority sites for the occurrence of deaths can support public management to reduce inequities in the access to health services and permit an optimization of the resources and teams in the control of pulmonary TB, providing support for specific strategies focused on the most vulnerable populations.

Entre novembro de 1987 e agosto de 1988, realizou-se um censo da frota artesanal da Rapôsa, principal comunidade pesqueira do litoral maranhense. Esta frota está composta por 221 embarcações) das quais 186 operam na pesca. É uma frota estruturalmente heterogênea, segundo o tipo de casco e o tipo de propulsão. As variações dos diferentes tipos, são representáveis pelo comprimento do barco. Definiram-se 4 estratos dentro desta frota, que são: 1. Bianas abertas com vela biana; 2. 3iana aberta com vela curicaca; 3. Guanas fechadas e botes com potência de motor menor de 40 HP e 4. Botes com mais de 40 HP. As possíveis medidas de poder de pesca são o comprimento da embarcação e o comprimento de rede, as quais servirão de base para avaliação dos recursos pesqueiros explorados pela frota analisada, principalmente Scomberomorus brasiliensis, Macrodon ancylodone Cynoscion acoupa.

OBJETIVO: Traçar o perfil do uso de álcool e tabaco entre universitários do curso de graduação em Psicologia do Centro de Ciências Humanas e Naturais da Universidade Federal do Espírito Santo. MÉTODOS: Trata-se de um estudo de corte transversal, com abordagem quantitativa. A população estudada foi de alunos matriculados no referido curso no segundo semestre do ano de 2010, constituindo uma amostra de 221 estudantes. O questionário utilizado foi o mesmo utilizado no I Levantamento Nacional entre Universitários, proposto pela Secretaria Nacional de Políticas sobre Drogas. RESULTADOS: A maioria dos estudantes era solteira (90,1%), do sexo feminino (81%), estava na faixa etária de 18 a 24 anos (81%), era da raça/cor caucasoide/branca (57,5%) e 55% referiram pertencer às classes econômicas B1 e B2. Encontrou-se maior prevalência de álcool (85,07%) e tabaco (33,07%) na frequência uso na vida, sendo o uso de álcool maior que na população geral. As substâncias associadas ao uso de álcool na vida foram a maconha (p-valor = 0,007), os tranquilizantes (p = 0,011) e os anfetamínicos (p = 0,045). Já para o uso de tabaco na vida, as substâncias mais associadas foram a maconha (p = 0,0001), os inalantes (p = 0,0001), os alucinógenos (p = 0,0001) e os anfetamínicos (p = 0,001). CONCLUSÃO: É necessária maior abordagem nos currículos de graduação, especialmente da Psicologia, sobre o consumo de substâncias psicoativas e seus impactos para o indivíduo, a família e a sociedade, bem como a criação de programas preventivos específicos para estudantes universitários.

OBJETIVO: Quantificar a influência do diurético na capacidade funcional em portadores de insuficiência cardíaca congestiva (ICC) descompensada, através do teste de caminhada. MÉTODOS: Estudamos 10 pacientes internados, com idade média de 47 anos, sendo cinco do sexo masculino, com ICC descompensada, em classe funcional III e IV (NYHA), submetidos ao teste de caminhada de 6 e 9min na admissão e alta. Foram obtidos registros na admissão e alta, do peso, do ecocardiograma, sódio, potássio, uréia, creatinina séricos, hematócrito e hemoglobina. O tratamento instituído foi o aumento da dose prévia de furosemida EV e/ou VO, associado ou não a diurético tiazídico, tendo sido mantidas as doses prévias de digital, captopril ou da associação de nitrato e hidralazina. RESULTADOS: O período de compensação variou entre 4 a 30 dias (média 8,7±7,8 dias). Ao ecocardiograma bidimensional apresentaram diâmetro do ventrículo esquerdo que variou de 47 a 81mm e a fração de ejeção de 0,26 a 0,74. A distância caminhada em 6min passou de 193,4±71,5m para 341,8±67,7m (p<0,00002 ) e em 9min passou de 268,1±119,6m para 518,0±114,8m (p<0,00005 ). Não houve diferença estatística entre os valores, na admissão e alta, do hematócrito, hemoglobina, uréia, creatinina e sódio. O potássio sérico médio à admissão era de 4,0±0,91mEq/l e na alta 4,69±1,00mEq/l (p= 0,01) e o peso dos pacientes na admissão e na alta foi de 58,9±6,42kg para 52,9±5,31kg, respectivamente (p<0,0006 ). CONCLUSÃO: A compensação da ICC com o uso de diurético induziu a uma melhora importante, a curto prazo, da capacidade física dos pacientes, demonstrada pelo aumento da distância caminhada em 6 e 9min. O diurético melhorou significativamente o desempenho físico.

OBJECTIVE - The aim of this paper is to describe the prevalence of T. Cruzi infection in patients of from Mulungu do Morro, a rural tropical region of Northeastern Brazil. METHODS - A cross-sectional study was performed. After randomly selecting samples of the population, and obtaining their consents , patients completed pretested epidemiological and clinical questionnaires. Serum samples from all patients were collected and screened for the presence of T. cruzi antibodies. RESULTS - Of 694 patients examined, 174 patients (25.1%) tested had a positive serology for Chagas' disease. Of the study population, 341 patients were male with 27% Chagas' disease prevalence, without a statistical difference. Illiteracy was the only variable related to T. cruzi infection in our population. CONCLUSION - In conclusion, our study points to the high prevalence of Chagas' disease among patients in Mulungu do Morro, suggesting that this region has a high frequency of infection and probably active vectorial transmission.

OBJECTIVE: To obtain information about the profile and behavior of a population with ischemic heart disease undergoing cine coronary angiography and to determine disease severity. METHODS: Retrospective study assessing patients hospitalized at InCor from 1986 to 1995, in which the variables age, sex, and number of major coronary arteries with obstruction degree > 40% were analyzed. RESULTS: We studied 18,221 patients and observed a significant increase in the number of females (22.8% to 25.2%, P=0.001) and an increase in age (57.1±29.3 to 60.4±10.7 years, P=0.0001). A significant increase in the incidence of multivessel disease was observed, which was more frequent among males (69.2% and 64.5%) and among the older patients (59.8±9.8 and 56.8±10.7 years, P=0.0001). A reduction in the incidence of single-vessel disease was also observed (66.2% vs 69.2% and 33.8% vs 30.5%, respectively, P<0.0001). CONCLUSION: A change in the profile of the population studied was observed as follows: patients undergoing cine coronary angiography at InCor were older, had a greater number of impaired major coronary arteries, and the number of females affected increased, leading to indices suggestive of a poorer prognosis.

OBJETIVO: Avaliar os custos hospitalares da cirurgia de revascularização do miocárdio em pacientes coronarianos eletivos e relacioná-los com o número de enxertos realizados. MÉTODOS: Estudo descritivo prospectivo, realizado no Instituto Dante Pazzanese de Cardiologia, nos meses de abril, maio e junho de 2005, em pacientes coronarianos de ambos os sexos, de qualquer idade. Foram excluídos os pacientes atendidos na emergência, os que tinham outras alterações cardíacas associadas e os casos de reoperação. Foram elaborados formulários apropriados para a fase inicial de internação no pré-operatório, para o centro cirúrgico, para o pós-operatório imediato e para a fase de internação final na enfermaria. RESULTADOS: O procedimento foi realizado em 103 pacientes, com o custo médio total de R$ 6.990,30, com um mínimo de R$ 5.438,69, um máximo de R$ 11.778,96, desvio padrão de R$ 1.035,47 e intervalo de confiança de 95% de R$ 6.790,33-R$ 7.190,27, correspondendo, em dólares, a média de US$ 2.784,98, mínimo de US$ 2.166,81, máximo de US$ 4.692,81, desvio padrão de US$ 412,54 e intervalo de confiança de 95% de US$ 2.705,31-US$ 2.864,67. O custo médio total para a cirurgia de três a cinco pontes foi maior (R$ 7.148,05) que para a cirurgia de uma a duas pontes (R$ 6.659,29) e a diferença foi significante (p < 0,05). CONCLUSÃO: Os maiores custos médios foram no centro cirúrgico (R$ 4.627,97) e no pós-operatório imediato (R$ 1.221,39), seguidos pela enfermaria depois do pós-operatório imediato (R$ 840,04) e pela enfermaria inicial de pré-operatório (R$ 300,90).
