OBJETIVO: Identificar a fauna flebotomínea e alguns aspectos do seu comportamento em ambientes florestal e antrópico. MÉTODOS: As coletas de flebotomíneos foram feitas na Fazenda Peroba, no município de Bandeirantes (23º 6' Latitude Sul; 50º 22' Longitude Oeste), na mesorregião do norte pioneiro paranaense. Para as coletas, foram instaladas armadilhas luminosas de Falcão, na mata, no domicílio, em abrigos de animais domésticos, com periodicidade mensal, das 17h às 7h, de março de 1997 a fevereiro de 1998. RESULTADOS: Coletaram-se 3.655 flebotomíneos de 13 espécies. Predominaram Nyssomyia whitmani e Nyssomyia neivai, representando 81,0% (2.977 exemplares) do total de flebotomíneos coletados. Do total dessas duas espécies, 2.552 (85,7%) foram coletados no ambiente antrópico, dos quais 2.332 (91,3%) na pocilga. N. whitmani e N. neivai foram mais freqüentes de fevereiro a maio entre 20h e 21h, quando foram coletados 90,4% dos exemplares. CONCLUSÕES: As cinco espécies capturadas, N. whitmani, N. neivai, Migonemyia migonei, Pintomyia pessoai e Pintomyia fischeri, têm potencial para transmitir a leishmaniose tegumentar. Ressalta-se a importância das duas primeiras, cujo comportamento é semelhante em relação à sazonalidade, ritmo horário e dominância no ambiente antrópico. Além disso, N. whitmani, apresentou as freqüências mais elevadas.

OBJETIVO: Estudar a distribuição espacial da população de Aedes albopictus, visando fornecer subsídios para construção de planos de amostragem e decisão sobre o controle deste vetor em levantamentos em campo. MÉTODOS: O estudo foi realizado em área urbana próxima de resquícios de vegetação primária, no distrito Picadinha, distante 20 km do município de Dourados, MS. Dez amostragens foram realizadas entre 28/1/2003 e 9/4/2003, utilizando-se armadilhas do tipo ovitrampas e monitoradas semanalmente, distribuídas em uma área amostral fixa com 10 pontos de coleta por amostragem, espaçadas em 300 m. Foram calculados os índices de agregação e ajuste às seguintes distribuições teóricas de freqüência: binomial negativa, binomial positiva e de Poisson. RESULTADOS: A variância amostral foi superior à média nas amostragens, resultando nos índices de razão variância-média sempre acima da unidade (32,066, 29,410, 14,444, 58,840, 56,042, 111,262, 70,140, 50,701, 93,221 e 8,481). O índice de Morisita apresentou valores significativamente acima da unidade em todas as amostragens (6,275, 3,947, 1,484, 3,725, 3,014, 5,450, 3,214, 3,886, 3,954 e 5,810). O parâmetro K resultou em valores entre 0 e 8 (0,174, 0,309, 1,867, 0,332, 0,449, 0,203, 0,408, 0,314, 0,306 e 0,200). Os testes do qui-quadrado de ajuste às distribuições binominal negativa, binomial positiva e de Poisson não foram significativos. CONCLUSÕES: A população de Aedes albopictus da localidade estudada apresentou distribuição espacial padrão agregada. Isso implica que ao encontrar alguns indivíduos do vetor em um determinado local, é provável que outros sejam encontrados nas áreas circunvizinhas, preconizando a aplicação de inseticidas sem a necessidade de amostrar outros pontos dessa localidade.

OBJECTIVE: To estimate the prevalence and analyze risk factors associated to osteoporosis and low-trauma fracture in women. METHODS: Cross-sectional study including a total of 4,332 women older than 40 attending primary care services in the Greater São Paulo, Southeastern Brazil, between 2004 and 2007. Anthropometrical and gynecological data and information about lifestyle habits, previous fracture, medical history, food intake and physical activity were obtained through individual quantitative interviews. Low-trauma fracture was defined as that resulting from a fall from standing height or less in individuals 50 years or older. Multiple logistic regression models were designed having osteoporotic fracture and bone mineral density (BMD) as the dependent variables and all other parameters as the independent ones. The significance level was set at p<0.05. RESULTS: The prevalence of osteoporosis and osteoporotic fractures was 33% and 11.5%, respectively. The main risk factors associated with low bone mass were age (OR=1.07; 95% CI: 1.06;1.08), time since menopause (OR=2.16; 95% CI: 1.49;3.14), previous fracture (OR=2.62; 95% CI: 2.08;3.29) and current smoking (OR=1.45; 95% CI: 1.13;1.85). BMI (OR=0.88; 95% CI: 0.86;0.89), regular physical activity (OR=0.78; 95% CI: 0.65;0.94) and hormone replacement therapy (OR=0.43; 95% CI: 0.33;0.56) had a protective effect on bone mass. Risk factors significantly associated with osteoporotic fractures were age (OR=1.05; 95% CI: 1.04;1.06), time since menopause (OR=4.12; 95% CI: 1.79;9.48), familial history of hip fracture (OR=3.59; 95% CI: 2.88;4.47) and low BMD (OR=2.28; 95% CI: 1.85;2.82). CONCLUSIONS: Advanced age, menopause, low-trauma fracture and current smoking are major risk factors associated with low BMD and osteoporotic fracture. The clinical use of these parameters to identify women at higher risk for fractures might be a reasonable strategy to improve the management of osteoporosis.

OBJETIVO: Desenvolver um modelo preditivo de óbito hospitalar com base nos dados do Sistema de Informações Hospitalares do Sistema Único de Saúde. MÉTODOS: Estudo transversal com dados de 453.515 autorizações de internação de 332 hospitais do Rio Grande do Sul no ano de 2005. A partir da razão entre óbitos observados e óbitos esperados elaborou-se um ranking ajustado dos hospitais que foi comparado ao ranking bruto da taxa de mortalidade. Utilizou-se regressão logística para desenvolvimento do modelo preditivo de probabilidade para óbito hospitalar segundo sexo, idade, diagnóstico e uso de unidade de terapia intensiva. Foram obtidos os intervalos com 95% de confiança para 206 hospitais com mais de 365 internações por ano. RESULTADOS: Obteve-se um índice de risco para mortalidade hospitalar. A ordenação dos hospitais utilizando apenas a taxa de mortalidade bruta diferiu da ordenação quando se utiliza o ranking ajustado pelo modelo preditivo de probabilidade. Dos 206 hospitais analisados, 40 hospitais apresentaram mortalidade observada significativamente superior à esperada e 58 hospitais com mortalidade significativamente inferior à esperada. Uso de unidade de terapia intensiva apresentou maior peso para a composição do índice de risco, seguida pela idade e diagnóstico. Quando os hospitais atendem pacientes com perfis muito diferentes, o ajuste de risco não resulta numa indicação definitiva sobre qual prestador é o melhor. Os hospitais de grande porte apresentaram, no conjunto, maior número de óbitos do que seria esperado de acordo com as características das internações. CONCLUSÕES: O índice de risco de óbito hospitalar mostrou-se preditor adequado para o cálculo dos óbitos esperados, podendo ser aplicado na avaliação do desempenho hospitalar. Recomenda-se que, ao comparar hospitais, seja utilizado o ajuste pelo modelo preditivo de probabilidade de risco, estratificando-se pelo porte do hospital.

OBJECTIVE To estimate rates of non-adherence to telemedicine strategies aimed at treating drug addiction. METHODS A systematic review was conducted of randomized controlled trials investigating different telemedicine treatment methods for drug addiction. The following databases were consulted between May 18, 2012 and June 21, 2012: PubMed, PsycINFO, SciELO, Wiley (The Cochrane Library), Embase, Clinical trials and Google Scholar. The Grading of Recommendations Assessment, Development and Evaluation was used to evaluate the quality of the studies. The criteria evaluated were: appropriate sequence of data generation, allocation concealment, blinding, description of losses and exclusions and analysis by intention to treat. There were 274 studies selected, of which 20 were analyzed. RESULTS Non-adherence rates varied between 15.0% and 70.0%. The interventions evaluated were of at least three months duration and, although they all used telemedicine as support, treatment methods differed. Regarding the quality of the studies, the values also varied from very poor to high quality. High quality studies showed better adherence rates, as did those using more than one technique of intervention and a limited treatment time. Mono-user studies showed better adherence rates than poly-user studies. CONCLUSIONS Rates of non-adherence to treatment involving telemedicine on the part of users of psycho-active substances differed considerably, depending on the country, the intervention method, follow-up time and substances used. Using more than one technique of intervention, short duration of treatment and the type of substance used by patients appear to facilitate adherence.
Diagnosis of cytomegalovirus infections by qualitative and quantitative PCR in HIV infected patients

A high incidence of cytomegalovirus (CMV) infections is observed in Brazil. These viruses are causatives of significant morbidity and mortality among patients with advanced human immunodeficiency virus (HIV) infection. This work, shows the application of a PCR on determination of CMV load in the buffy coat and plasma. We analyzed the samples of 247 HIV infected patients in order to diagnose CMV infection and disease. We developed a semi-quantitative PCR that amplifies part of the glycoprotein B (gB) gene of CMV. The semi-quantitative PCR was carried out only in positive clinical samples in a qualitative PCR confirmed by a nested-PCR. CD4 lymphocyte count, HIV viral load and CMV disease symptom were correlated with CMV load. CMV genome was detected in the buffy coat of 82 of 237 (34.6%) patients, in 10 of these the CMV load was determined varying between 928 and 332 880 viral copies/mug DNA. None of these 237 patients developed any suggestive manifestation of CMV disease. For the other 10 HIV infected patients selected based on the suspicion of CMV disease, CMV genome was detected in only one case. This patient presented a high CMV load, 8 000 000 copies/mug DNA, and developed a disseminated form of CMV disease including hepatitis and retinitis. Our results were greatly influenced by the impact of the highly active antiretroviral therapy that reduced incidence of CMV viremia and occurrence of CMV disease in the HIV infected patients.

Os aa. determinaram o diâmetro médio dos granulomas esquistossomóticos em suas várias fases evolutivas (necrótico-exsudativa, produtiva e em cura pór fibrose) em 332 casos de esquistossomose hepática humana: 167 na forma aguda, toxêmica e 165 nas formas crônicas (hepatesplênica, miliar). Foram medidos 286 granulomas em punções biópsias da primeira e 165 em punções biópsias e biópsias cirúrgicas da segunda.

O diagnóstico parasitológico deve ser realizado de maneira apropriada, com maior sensibilidade e especificidade para a detecção dos parasitas intestinais, uma vez que dele dependerá o tratamento específico. Foi desenvolvido um estudo comparativo para avaliar a concordância entre os métodos Kato-Katz e coprotest® na detecção de helmintos em 332 indivíduos do município de Pedro de Toledo. Destacou-se uma diferença significativa para Trichuris trichiura, 16,2% no Kato-Katz e 7,5% no coprotest®. Devido a essa diferença compararam-se amostras positivas e negativas do método de coprotest® com número de ovos por grama de fezes (opg) obtido pelo método de Kato-Katz. Quando o método de coprotest® era negativo, contaram-se 65 opg de Trichuris trichiura pelo Kato-Katz e quando o coprotest® era positivo, esse número foi maior, 199 opg. O coprotest® mostrou-se inferior ao Kato-Katz nas infecções de baixa carga parasitária.

INTRODUÇÃO: Para investigar susceptibilidade às reações hansênicas, três polimorfismos do gene natural resistance-associated macrophage protein (NRAMP1), foram determinados em 201 indivíduos, atendidos em dois centros de referência no Recife, entre 2007 e 2008, sendo 100 paucibacilares e 101 multibacilares. MÉTODOS: A determinação dos polimorfismos 274C/T, D543N e 1729+55del4 do gene NRAMP1 foi realizada utilizando a técnica do polimorfismo de fragmento de restrição em DNA extraído de sangue periférico e as estimativas das freqüências alélicas e genotípicas foram feitas por contagem direta. RESULTADOS: Os genótipos predominantes foram: CC (51,8%) para 274C/T, GG (86,6%) para D543N e +-TGTG (59,9%) para 1729+55del4. O genótipo mutante 274 TT predominou na negatividade da reação reversa (p=0,03) e na positividade do eritema nodoso (p=0,04). CONCLUSÕES: Nossos resultados sugerem que o polimorfismo 274 C/T do gene NRAMP1 pode auxiliar na determinação da susceptibilidade à reação tipo II em indivíduos com hanseníase.

INTRODUCTION: Hospitals around the world have presented multiresistant Acinetobacter sp. outbreaks. The spread of these isolates that harbor an increasing variety of resistance genes makes the treatment of these infections and their control within the hospital environment more difficult. This study aimed to evaluate the occurrence and dissemination of Acinetobacter sp. multiresistant isolates and to identify acquired resistance genes. METHODS: We analyzed 274 clinical isolates of Acinetobacter sp. from five hospitals in Porto Alegre, RS, Brazil. We evaluated the susceptibility to antimicrobial, acquired resistance genes from Ambler's classes B and D, and performed molecular typing of the isolates using enterobacterial repetitive intergenic consensus-polymerase chain reaction (ERIC-PCR) technique. RESULTS: A high (68%) percentage of multiresistant isolates of Acinetobacter sp. was observed, and 69% were resistant to carbapenems. We identified 84% of isolates belonging to species A. baumannii because they presented the gene blaOXA-51. The gene blaOXA-23 was detected in 62% of the isolates, and among these, 98% were resistant to carbapenems. Using the ERIC-PCR technique, we identified clones of Acinetobacter sp. spread among the four hospitals analyzed during the sampling period. CONCLUSIONS: The data indicate the dissemination of Acinetobacter sp. isolates among hospitals and their permanence in the hospital after one year.

IntroductionFew studies have examined hepatocellular carcinoma (HCC) in Brazil, and the incidence and risk factors for this type of malignancy vary greatly geographically. In this paper, we report several risk factors associated with HCC diagnosed at the University Hospital in Vitória, ES, Brazil.MethodsWe reviewed 274 cases of HCC (January 1993 to December 2011) in which hepatitis B (HBV) and C (HCV) virus infection and chronic alcoholism were investigated. A diagnosis of hepatocellular carcinoma was confirmed by histology or by the presence of a characteristic pattern on imaging.ResultsHCC with associated liver cirrhosis was noted in 85.4% of cases. The mean ages of men and women were 56.6 years and 57.5 years, respectively. The male-to-female ratio was 5.8:1. Associated risk factors included the following: HBV, 37.6% (alone, 23.4%; associated with chronic alcoholism, 14.2%); HCV, 22.6% (alone, 13.5%; associated with chronic alcoholism, 9.1%), chronic alcoholism, 17.1%, non-alcoholic steatohepatitis, 2.6% and cryptogenic, 19.3%. The male-to-female ratio was higher in cases associated with HBV or chronic alcoholism compared with HCV-associated or cryptogenic cases. In 40 cases without associated cirrhosis, the male-to-female ratio and mean age were lower than those in cirrhosis-associated cases.ConclusionsThese results demonstrate that the main risk factor associated with HCC in the State of Espírito Santo is HBV. Chronic alcoholism is an important etiological factor, alone or in association with HBV or HCV infection.

Introduction Eratyrus mucronatus Stål, 1859 is a wild triatomine vector of Trypanosoma cruzi Chagas, 1909. However, little is known regarding the biology and ecoepidemiology of this triatomine in the Brazilian Amazon. The present study describes the biology of E. mucronatus grown under laboratory conditions and the epidemiological aspects of its natural breeding sites. Methods Five colonies were monitored in the field for 3 years. Temperature and humidity measurements were taken in the mornings and afternoons at the natural breeding sites, and the behavior and distribution of the nymphs and adults were observed in the wild colony. We also monitored the life cycle under controlled laboratory conditions. Results Some factors that were considered decisive for the establishment of these colonies were present at all of the colonies studied in the field. These factors included an active termite nest, a vertebrate for repast, and dry and shaded substrates with temperatures of 24-28°C and with humidity of 80-90%. A generation was developed in 274 days under these microclimatic conditions in the laboratory. Conclusions The climatic variables described in the field indicate that these environmental parameters have a limiting effect on the dispersal and colonization of E. mucronatus to new environments. In addition, the long period of development to adulthood demonstrates that only one generation can develop per year even under the more favorable laboratory conditions.

Rheumatic fever is still the most commonly seen rheumatic disease in Brazilian pediatric rheumatology clinics. It remains a significant health problem since subsequent cardiac sequelae represent one of the most important causes of chronic heart disease in children. We reviewed the clinical manifestations of rheumatic fever in 786 patients, followed at seven pediatric rheumatology clinics in the state of São Paulo, Brazil. All patients were diagnosed according to revised Jones' criteria. Regarding major criteria, 396 (50.4%) children exhibited carditis, 453 (57.6%) polyarthritis, 274 (34.8%) chorea, 13 (1.6%) erythema marginatum, and 12 (1.5%) subcutaneous nodules. Valvular lesions documented by echocardiography in the absence of accompanying auscultatory findings were found in 144 (18.3%) patients. Migratory polyarthritis was observed in 290 (64.0%) patients with articular involvement. Documented previous streptococcal infection assessed by serum antistreptolysin (ASO) titers occurred in 531 (67.5%) patients. Even though prophylaxis with benzathine penicillin was recommended to all patients, recurrent attacks were observed in 147 (18.7%). We emphasize the high frequency of chorea, silent carditis and recurrences in our series as well as the variable clinical presentation of arthritis in rheumatic fever. Multicenter studies should be encouraged to improve our understanding of the clinical features of rheumatic diseases in children and adolescents.

Brucellosis and leptospirosis are widely spread bacterial infections and dogs are the most important source of infection and reservoir for diseases. Dogs can disseminate the agents in the environment and transmit them to humans and/or other animals. The objective of this study was assess the occurrence of reactive to antibodies anti-Leptospira spp., Brucella canis and B. abortus in Belém and Castanhal, State of Pará, Amazon, Brazil. A total of 156 samples were randomly collected in the city of Belém and 158 samples in Castanhal. The anti-B. canis antibodies research was performed by Agar Gel Immunodiffusion (AGID) with and without 2-mercaptoethanol serum treatment (AGID-2ME). To assess the anti-B. abortus antibodies, the technique of Fast Seroagglutination with buffered acidified plate antigen (BAPAT) was used. For anti-Leptospira spp. antibodies research, the Microscopic Agglutination Technique (MAT) was used. No animal reacted to Brucella abortus and one animal was reactive to B. canis at the AGID, but it was negative to the AGID-2ME test. Seventeen percent of dogs (47/274) presented anti-Leptospira spp. antibodies, with prevalence of serovar Canicola. The dogs from Belém and Castanhal are not source of infection for B. abortus and B. canis, however, they are reservoirs for different serovars of Leptospira spp.

OBJETIVO: Realizar revisão sistemática da literatura acerca da QV dos cuidadores de idosos com DA. MÉTODOS: Revisão sistemática de literatura por meio do PubMed e da MedLine entre os anos de 1997 e 2008, utilizando os termos quality of life, caregivers, Alzheimer disease. RESULTADOS: Dezesseis estudos entre 274 publicações iniciais e 39 artigos posteriormente incluídos pelas referências bibliográficas foram avaliados. A QV dos cuidadores dos idosos demenciados, na maioria dos estudos, estava comprometida negativamente. Diversos fatores interferem na QV do cuidador, desde a gravidade da doença até alterações comportamentais que esses idosos possam vir a apresentar. CONCLUSÃO: É necessária a realização de mais estudos direcionados a esse tema, a fim de buscar estratégias para melhora da QV desse cuidador.
