OBJECTIVE: To evaluate the efficacy and tolerability of telmisartan, given once a day to patients with mild to moderate hypertension, as well as to assess the 24-hour blood pressure profile with ABPM. METHODS: Initially, 163 patients over 18 were selected, regardless of sex, with blood pressure levels >140/90mmHg at visit 1, which was confirmed at visit 2. One hundred thirty-four patients completed the study. After a 4-week placebo run-in phase, telmisartan 40mg/daily was given for 6 weeks. In those patients whose blood pressure (BP) levels were lower than 140/90mmHg, the same dosage was kept for an additional period of 6 weeks. For those who had BP higher than 140/90mmHg, the dosage was increased to 80mg/daily. Sixty-two patients were included in a subgroup that underwent ABPM 3 different times during the study. RESULTS: In the overall group, blood pressure reduction ranged from 162.3±14.5/101.3±5.75 mmHg (baseline) to 147.3±20.1/90.8±10.9 mmHg (week 12) (p<0.05). Mean blood pressure decreases were 14.4mmHg for systolic BP and 10.3mmHg for diastolic BP, after 12 weeks of active treatment. A subanalysis showed that 47 (35%) patients took telmisartan 40mg throughout the study and 81 (65%) had the dosage increased to 80mg daily. Using ABPM, an 8-mmHg reduction in systolic BP as well as a 5-mmHg reduction in diastolic BP were observed, when compared with baseline values in the final 6 hours (18-24 hours after the last dose of study medication). CONCLUSION: Our results confirm that telmisartan given once a day is effective in reducing blood pressure levels in mild to moderate hypertensive patients. This reduction occurs in a sustained and gradual manner during a 24-hour period confirmed by ABPM.

FUNDAMENTO: Diferenças entre regiões geográficas em relação à características de pacientes e desfechos, particularmente em síndromes coronarianas agudas, tem sido demonstradas em testes clínicos. Os desfechos clínicos após intervenções coronarianas percutâneas com o stent eluidor de Zotarolimus em uma população real foram analisados com o tempo. OBJETIVO: A influência da localização geográfica sobre os desfechos clínicos com o stent eluidor de Zotarolimus foi avaliada em três regiões: Pacífico Asiático, Europa e América Latina. MÉTODOS: Um total de 8.314 pacientes (6.572 da Europa, 1.522 do Pacífico Asiático e 220 da América Latina) foram acompanhados por 1 ano; 2.116 desses (1.613, 316, e 187, respectivamente) foram acompanhados por 2 anos. Características dos pacientes e lesões, terapia antiplaquetária dupla e desfechos clínicos foram comparados entre a América Latina e as outras duas regiões. RESULTADOS: Os pacientes da América Latina apresentavam a maior proporção de fatores de risco e infarto do miocárdio prévio. O uso da terapia antiplaquetária dupla declinou rapidamente na América Latina, de 44,9% em 6 meses para 22,5% em 1 ano e 7,8% em 2 anos (Europa: 87,4%, 61,5%, 19,7%; Pacífico Asiático: 82,4%, 67,0%, 45,7%, respectivamente). Não houve diferenças significantes entre a América Latina e a Europa ou Pacífico Asiático para qualquer desfecho em qualquer ponto do tempo. A incidência de trombose de stent provável e definitiva pelo Academic Research Consortium foi baixa (<1,2%) entre todos os pacientes em 1 ano e 2 anos. CONCLUSÃO: Os desfechos clínicos foram comparáveis entre os pacientes da América Latina e Europa, e América Latina e Pacífico Asiático, a despeito dos subgrupos clínicos menos favoráveis na América Latina, perfil de risco mais elevado e menor uso acentuado de terapia antiplaquetária dupla com o tempo.

1. In a series of 21 normal cases we found for fatty acids per 100 cc. of plasma an average of 332 mgm., being 314 mgm. for the male sex and 350 mgm. for the female sex. 2.- For lecithin, in four normal cases we found per 100 cc. of plasma 182 mgm. estimated by the contents is phosphorus which ranged from 6.12 to 9.0 mgrs. 3.- Cholesterol in 20 normal cases showed 172 mgm. per 100 cc. of plasma. The averages were 151 mgm. for men and 194 mgm. for women. 4.- The readings of the fractions were 2.01 for the ratio fatty acids divided by lecithins 0.90 for lecithin divided by cholesterol and 1.93 for fatty acids divided by cholesterol. 5.- On comparing the results obtained by us with those reported in foreign literature an absolute conformity is noted chiefly with the values supplied by Bloor and Horiuchi for the ratios among the various lipoid fractions. The average for Cholesterol is comparable with that obtained by Myers but is slightly under that of Bloor's. The lecithin contents found by us did not reach such high values as those supplied by foreign authors.

With the aim to evaluate the circulating cathodic antigen (CCA) levels in relation to the different clinical phases of Schistosoma sp. infection a sandwich ELISA using monoclonal antibody 5H11 was performed. The sera of three groups of 25 Brazilian patients with acute, intestinal and hepatosplenic forms of S. mansoni infection were tested and compared to a non-infected control group. Patients and control groups were matched for age and sex and the number of eggs per gram of feces was equally distributed among the three patient groups. Sensitivity of 100%, 72%, 52% of the assay was observed for the intestinal, hepatosplenic and acute toxemic groups respectively. The specificity was 100%. Intestinal and hepatosplenic groups presented CCA levels significantly higher in comparison to those observed for acute patients (F-ratio = 2,524; p = 0.000 and F-ratio = 6,314; p = 0.015 respectively). There was no significant difference of CCA serum levels between hepatosplenic and intestinal groups (F-ratio = 1,026; p = 0.316).

Soil transmitted helminth (STH) infection are endemic in developing countries. A study was carried out of sewage farms, streams and vegetables to determine the sources and routes of STH infection in Sanliurfa, Turkey. Stool samples from farmhouse inhabitants as well as soil and vegetable samples from the gardens were collected and examined. In addition, water samples from streams and vegetable samples from the city market were collected and examined. One hundred and eighty-seven (59.5%) of a total of 314 samples, including 88.4% of the stool samples, 60.8% of the water samples, 84.4% of the soil samples and 14% of the vegetable samples, were found to be positive for STH eggs. These results indicate that the water, soil and vegetables are heavily contaminated, and suggest a vicious circle between humans and the environment. Improving environmental sanitation is imperative for the control of soil-transmitted helminthiasis in Sanliurfa.

Intravenous drug injection has been reported as the main risk factor for hepatitis C virus (HCV) infection. The aim of the present study was to describe the prevalence and the epidemiological profile of HCV infection among abusers of illegal injected and non-injected drugs in Cuiabá, state of Mato Grosso, Central Brazil. A cross-sectional study including 314 male drug users from eight detoxification centres was performed. Out of 314 subjects studied, 48 (15.2%) were intravenous drug users. Participants were interviewed and had blood samples taken and tested for the presence of anti-HCV antibodies. Positive samples were tested for the presence of HCV RNA. Genotyping was performed on HCV RNA-positive samples. The overall prevalence of anti-HCV antibodies was 6.4% (n = 20). Out of 20 anti-HCV antibody-positive subjects, 16 (80%) were also HCV RNA-positive. Genotype 1 predominated (75%), followed by 3a (25%). Subtype 1a was more common than 1b. HCV infection was more prevalent among intravenous drug users (33%) than non-injecting users (1.5%). Logistic regression analyses showed independent associations between HCV infection and intravenous drug use, imprisonment and increasing age. In the present study, injecting drug use was the factor most strongly associated to HCV infection and inhaling or sniffing did not represent an increased susceptibility to infection.

Environmental changes from water resource developmental projects affect the epidemiology of water-associated diseases, as well as malaria and schistosomiasis. Aiming to investigate the occurrence and distribution of freshwater snails of medical and veterinary importance in the area of influence of the Peixe Angical hydroelectric dam, a survey has been conducted over four years (2004-2008). The study has revealed the occurrence of populations of Biomphalaria straminea (Dunker) in all municipalities surrounding the lake. Studies on parasite-mollusc compatibility were undertaken using 35 populations of B. straminea, descendants of specimens obtained from that area and three strains of Schistosoma mansoni (Sambon) (BH, CM and CMO). The main results are as follows: (i) among the 1,314 specimens used, eight had been infected (infection index of 0.6%) with only the BH strain, (ii) for B. straminea populations, the mortality index was 6.8% and, depending on the strain used, the indexes were 4.6%, 8.49% and 19% with BH, CM and CMO strains, respectively, (iii) the infection indexes varied according to the B. straminea populations, ranging from 0-12.5% and (iv) the duration of the precercarial period varied from 25-49 days. These results, in addition to environmental and social changes that took place in the Peixe Angical dam region, indicate the possibility of B. straminea emerging as a schistosomiasis vector in this area.

Estudo feito entre estudantes da EEUSP para saber se eles eram fumantes habituais ou ocasionais, e se esse hábito havia sido adquirido antes ou depois do ingresso na Escola. Do total de 314 estudantes matriculados em 1998, 80,9% responderam ao questionário, dos quais 49,2% já haviam experimentado fumar, e dentre esses, 38 continuavam a fumar na data do estudo. Entre os fumantes encontrados, 16 haviam começado a fumar dos 16 aos 20 anos, 12 com menos de 15 e três com 21 anos ou mais. Ao final, propõe-se que a EEUSP coloque maior quantidade de placas de "proibido fumar" no ambiente da escola; realize seminários periódicos sobre tabagismo para incentivar docentes e alunos a deixarem de fumar; participe ativamente do Dia Mundial sem Tabaco ou inclua essa atividade no programa do CCEx. Enfim, que se divulgue os dados e estatísticas da OMS sobre as conseqüências do tabagismo.

Este artigo teve como objetivos relatar a experiência da formação do grupo de idosos Feliz Idade, desenvolvido por enfermeiras do Programa Saúde da Família em Fortaleza-CE, bem como conhecer a importância do cuidado de enfermagem para a promoção da saúde na vida dos idosos. A seleção inicial dos participantes do grupo ocorreu entre os 314 idosos pertencentes aos programas de Hipertensão Arterial e Diabetes Mellitus. Para formação do grupo, as enfermeiras contaram com a colaboração de médico, fisioterapeuta, educador físico e agente de saúde. As atividades desenvolvidas no grupo foram caminhadas, oficinas, passeios e terapia comunitária. O grupo final foi formado por 124 idosos. Após a inserção destes idosos no grupo, ocorreram melhorias no aspecto biológico, como maior flexibilidade e melhor mobilidade articular, além da ampliação das interações sociais. Observa-se que as atividades, assim realizadas, favoreceram a promoção da saúde na terceira idade.

Abstract OBJECTIVE To map the sub processes related to turnover of nursing staff and to investigate and measure the nursing turnover cost. METHOD This is a descriptive-exploratory study, classified as case study, conducted in a teaching hospital in the southeastern, Brazil, in the period from May to November 2013. The population was composed by the nursing staff, using Nursing Turnover Cost Calculation Methodology. RESULTS The total cost of turnover was R$314.605,62, and ranged from R$2.221,42 to R$3.073,23 per employee. The costs of pre-hire totaled R$101.004,60 (32,1%), and the hiring process consumed R$92.743,60 (91.8%) The costs of post-hire totaled R$213.601,02 (67,9%), for the sub process decreased productivity, R$199.982,40 (93.6%). CONCLUSION The study identified the importance of managing the cost of staff turnover and the financial impact of the cost of the employee termination, which represented three times the average salary of the nursing staff.

A manutenção ou a elevação dos níveis de tecnologia no cultivo da soja no Cerrado é fundamental à produtividade econômica, a qual tem se revelado superior à obtida na Região Sul do Brasil. Entre as contribuições da pesquisa ao desempenho destacado da cultura da soja no Cerrado estão as novas cultivares, que representam papel fundamental para a estabilidade produtiva. A cultivar de soja BRS Celeste é uma opção para atender à demanda dos produtores. Apresenta ciclo semelhante ao da BR/EMGOPA 314 (Garça Branca), porém com produtividade superior, e com resistência às principais doenças, em especial ao cancro-da-haste (Diaporthe phaseolorum f. sp. meridionalis).

O objetivo deste trabalho foi avaliar o padrão espacial da variabilidade genética entre plantas, dentro de três populações naturais de barueiro (Dipteryx alata Vogel), pela genotipagem por RAPD e técnicas de autocorrelação espacial. Os cinco iniciadores RAPD permitiram a codificação de 45 locos, utilizados nas análises de diversidade, estrutura e distribuição espacial da variabilidade genética entre populações. As populações apresentaram diversidade genética (Hs) com valor médio 0,314. Verificou-se que 12% da variação total se encontra entre as populações, o que indica que estas mantêm um considerável nível de variabilidade genética. Foi observada tendência de autocorrelação espacial positiva nas primeiras classes de distâncias, nas três populações, o que indica a formação de grupos de vizinhança com estruturação familiar, dentro das populações de barueiro. Entretanto, o tamanho desses grupos de vizinhança varia entre as populações; isso mostra que outros processos ecológicos influenciaram a distribuição espacial da variabilidade genética. As populações naturais de barueiro apresentam consideráveis níveis de diversidade genética, com base nos 45 locos RAPD avaliados.

O objetivo deste trabalho foi avaliar o desempenho de 12 combinações de três clones de painel (PB 311, PB 314 e RRIM 600) com quatro clones de copa (F 4512, MDF 180, IAN 6158 e IAN 6543) de seringueira cultivados no sudoeste do Estado do Mato Grosso. Utilizou-se o delineamento experimental de blocos ao acaso, com quatro repetições. Foram avaliados: produção de borracha seca, percentagem de plantas aptas à sangria, secamento de painel, estado nutricional, teor de borracha seca, plasticidade Wallace (P0 e P30), índice de retenção de plasticidade e viscosidade Mooney. As combinações das copas IAN 6543 e MDF 180 com os painéis PB 311, RRIM 600 e PB 314 apresentaram estado nutricional adequado, com produtividade de borracha seca acima de 1,41 Mg ha-1 por ano. O clone de copa IAN 6158 propiciou, em oito anos, as maiores quantidades de plantas aptas à sangria. As combinações painel/copa apresentaram baixa incidência de secamento de painel, exceto PB 311 x IAN 6158. As borrachas das 12 combinações painel/copa apresentam boa resistência à degradação térmica e alta retenção de plasticidade.

The objective of this work was to evaluate the efficacy of eugenol as an anesthetic for juvenile common snook, and to determine the minimum effective concentration for use in handling procedures. In the first trial, juvenile common snook were subjected to immersion baths at 25, 50, 75, 100, 125, and 150 mg L-1 eugenol concentrations, after which induction and recovery times were evaluated. In the second experiment, the lethal exposure time (LT50) at 75 mg L-1 was estimated. Minimum effective eugenol concentration was 50 mg L-1, andthe stage of deep anesthesia and recovery were, respectively, reached at 126.3 and 208.8 s. At 75 mg L-1, LT50 was 1,314 s, and induction time and recovery were also satisfactory; however, fish cannot tolerate over 229 s exposure.

In 1787, Lavoisier and coworkers published a 314-page book entitled Méthode de nomenclature chimique in which a novel system for the naming of compounds and elements was presented. The Brazilian chemist Vicente Coelho de Seabra Telles (1764-1804) was responsible for translating it into Portuguese. Telles proposed the adoption not only of the Latin etymology, but decided to use Latin suffixes as well, because of the similarity between Latin and the Portuguese language. In doing so, he made the names of the compounds in Portuguese bear a closer resemblance to the names in Latin than to those in French or Spanish.
