OBJECTIVE: To investigate the quality of life, life satisfaction, happiness and demands of work in workers with different work schedules. METHODS: The survey was carried out on professional workers in social care. Some were shiftworkers whose schedule included night shifts (N=311), some were shiftworkers without night shifts (N=207) and some were non-shiftworkers (N=1,210). Surveys were mailed and the response rate was 86%. For the purpose of this study several variables were selected from the Survey: The Quality of Life Profile, which measures importance, satisfaction, control and opportunities in nine domains of life plus measures of happiness, life satisfaction and demands of work. RESULTS: While both groups of shiftworkers, compared to non-shiftworkers, reported needing more physical effort to complete their work, and reported 'being' more physically tired, no differences were found in reports of overall happiness, life satisfaction or total quality of life. However, night-shiftworkers reported greater percentage of time unhappy than the other two groups of workers. In analyses of the quality of life, night-shiftworkers were less satisfied with domains of spiritual 'being' and physical and community 'belonging' than day-shiftworkers and non-shiftworkers. They also reported having fewer opportunities to improve their physical 'being', leisure, and personal growth than the other two groups. CONCLUSIONS: Quality of life in specific domains in night-shiftworkers was rated worse than in other groups of workers. Domain-based quality of life assessment gives more information regarding the particular needs of workers than overall or global measures of well-being.

OBJECTIVE: To determine the best cut-offs of body mass index for identifying alterations of blood lipids and glucose in adolescents. METHODS: A probabilistic sample including 577 adolescent students aged 12-19 years in 2003 (210 males and 367 females) from state public schools in the city of Niterói, Southeastern Brazil, was studied. The Receiver Operating Characteristic curve was used to identify the best age-adjusted BMI cut-off for predicting high levels of serum total cholesterol (>150mg/dL), LDL-C (>100mg/dL), serum triglycerides (>100mg/dL), plasma glucose (>100mg/dL) and low levels of HDL-C (< 45mg/dL). Four references were used to calculate sensitivity and specificity of BMI cut-offs: one Brazilian, one international and two American. RESULTS: The most prevalent metabolic alterations (>50%) were: high total cholesterol and low HDL-C. BMI predicted high levels of triglycerides in males, high LDL-C in females, and high total cholesterol and the occurrence of three or more metabolic alterations in both males and females (areas under the curve range: 0.59 to 0.67), with low sensitivity (57%-66%) and low specificity (58%-66%). The best BMI cut-offs for this sample (20.3 kg/m² to 21.0 kg/m²) were lower than those proposed in the references studied. CONCLUSIONS: Although BMI values lower than the International cut-offs were better predictor of some metabolic abnormalities in Brazilian adolescents, overall BMI is not a good predictor of these abnormalities in this population.

OBJETIVO: As mortes por causas externas representam um dos mais importantes desafios para a saúde pública, sendo a segunda causa de óbito no Brasil. O objetivo do estudo foi analisar os diferenciais de mortalidade por causas externas segundo raça/cor da pele. MÉTODOS:Estudo descritivo realizado em Salvador (BA), com 9.626 registros de óbitos por causas externas entre 1998 e 2003. Dados foram obtidos do Instituto Médico Legal e Instituto Brasileiro de Geografia e Estatística. O indicador "anos potenciais de vida perdidos" foi utilizado para identificar diferenciais entre grupos etários, de raça/cor da pele e sexo. RESULTADOS: As mortes por causas externas determinaram perda de 339.220 anos potenciais de vida, dos quais 210.000 foram devidos aos homicídios. Indivíduos negros morreram em idades mais precoces e perderam 12,2 vezes mais anos potenciais de vida devido a mortes por homicídio que indivíduos brancos. Embora a população negra (pardos e pretos) fosse três vezes maior que a população branca, o número de anos perdidos daquela foi 30 vezes superior. A população de pretos era 11,4% menor que a população branca, mas apresentou anos perdidos quase três vezes mais. Mesmo após a padronização por idade, mantiveram-se as diferenças observadas no indicador de anos potenciais perdidos/100.000 hab e nas razões entre estratos segundo raça/cor. CONCLUSÕES: Os resultados mostram diferenciais na mortalidade por causas externas segundo raça/cor da pele em Salvador. Os negros tiveram maior perda de anos potenciais de vida, maior número médio de anos não vividos e morreram, em média, em idades mais precoces por homicídios, acidentes de trânsito e demais causas externas.

OBJETIVO: Descrever a composição, características ecológicas e comportamentais e infectividade das espécies de anofelinos em reservas indígenas da região Amazônica. MÉTODOS: O estudo foi conduzido no ano de 2002 em aldeias das reservas indígenas Nhamundá-Mapuera e Cuminapanema no estado do Pará. Foram realizadas três coletas de duas semanas em cada reserva, com capturas de adultos e de imaturos. Anofelinos adultos foram capturados com capturador de Castro por atração humana nos ambientes intra e peridomiciliares, no período das 18h às 21h e das 18 às 06h e avaliados para verificação da paridade e infectividade para plasmódios por dissecção e ELISA. As coleções hídricas próximas às aldeias foram pesquisadas utilizando conchas de 500 ml, sendo 20 conchadas a cada 10 m, cobrindo-se a extensão máxima de 200 m de perímetro do criadouro. RESULTADOS: Foram capturadas 8.668 fêmeas somando-se as coletas das duas reservas. Anopheles darlingi foi a espécie predominante, com maior freqüência no peridomicílio. Na reserva Mapuera, a atividade hematofágica concentrou-se entre as 20h e 24h e, em Cuminapanema, manteve-se elevada até as 24h, diminuindo após esse horário e voltando a elevar-se no início da manhã. Das 6.350 fêmeas de An. darlingi examinadas, 18 estavam infectadas por Plasmodium vivax VK 247, VK 210, P. falciparum e P. malariae. Outras 1.450 fêmeas de outras espécies foram examinadas, mas nenhuma foi encontrada infectada. An. nuneztovari e Chagasia bonnae foram as espécies mais freqüentes nos criadouros das aldeias Mapuera e Cuminapanema, respectivamente. Imaturos de An. darlingi não foram localizados em Mapuera e foram capturados em apenas uma das coletas da reserva Cuminapanema. CONCLUSÕES: As populações de An. darlingi das duas reservas apresentaram comportamento exofílico e intensa atividade noturna. A ocorrência de imaturos foi pouco freqüente e a densidade larvária foi baixa. As características comportamentais dos vetores não se mostraram favoráveis às atividades usuais de controle vetorial.

Os Autores estudaram a resposta imune celular e os níveis de anticorpos na vacinação e re-vacinação canina. A vacina utilizada foi a Fuenzalida & Palacios, empregada rotineiramente na imunização humana e canina, no Brasil. A avaliação da resposta mediada por células foi feita por testes de inibição de migração de leucócitos periféricos. Os níveis de anticorpos foram determinados por provas de soro-neutralização e fixação do complemento. Os cinco cães envolvidos no experimento receberam um esquema inicial de 5 doses e uma dose de reforço 210 dias após. Todos os animais exibiram níveis moderados de anticorpos após o esquema inicial e uma nítida resposta secundária após o reforço. A resposta imune celular, contudo, ocorreu apenas após a imunização inicial, não sendo detectada em resposta à dose de reforço. Concluem os Autores que a resposta imune celular ocorre na primo-vacinação anti-rábica canina nas condições experimentais empregadas e que embora a resposta secundária não tenha sido obtida neste estudo sua existência não pode ser descartada e deve ser reestudada em outras condições.

Se realizó examen físico de las uñas a 210 ancianos y a aquellos que presentaron lesiones sugestivas de onicomicosis se les realizó toma de muestra con el objetivo de conocer los principales agentes causales, las características e incidencia de las lesiones. Se confirmó el diagnóstico mediante el aislamiento del agente causal en 74 de los casos procedentes principalmente de las uñas de los pies, para una incidencia de 35,2. La tinea pedis se presentó en el 25,7% de los casos, mientras que la enfermedad asociada más frecuente fue la Diabetes mellitus; entre las características clínicas de las uñas predominaron el engrosamiento, la pérdida del brillo y la aparición de estrías longitudinales. Existió una correspondencia significativa entre el examen microscópico directo y el cultivo. La especie de dermatofito predominate fue Trichophyton rubrum, mientras que Candida parapsilosis fue la más aislada entre las especies de Candida.

Aiming at demonstrating a decrease of acquired immunity after chemotherapeutic cure, a group of mice was infected with 25 Schistosoma mansoni cercariae (LE strain). A part of these animals was treated with 400 mg/kg oxamniquine, at 120 days after infection. Challenge infections were carried out at 45, 90 and 170-day-intervals after treatment (185, 210 and 290 days after primoinfection, respectively). Recovery of worms at 20 days after reinfections showed that a residual immunity remains up to 90 days after treatment, and disappears at 170 days after cure. Using the ELISA method, it was possible to detect a decrease of antibody levels (total IgG) in the treated group, when antigens from different evolutive stages of S. mansoni were used. The epidemiological implications of the present results, and the possible mechanisms involved in the decrease of acquired immunity after treatment are discussed.

We show here a simplified RT-PCR for identification of dengue virus types 1 and 2. Five dengue virus strains, isolated from Brazilian patients, and yellow fever vaccine 17DD as a negative control, were used in this study. C6/36 cells were infected and supernatants were collected after 7 days. The RT-PCR, done in a single reaction vessel, was carried out following a 1/10 dilution of virus in distilled water or in a detergent mixture containing Nonidet P40. The 50 µl assay reaction mixture included 50 pmol of specific primers amplifying a 482 base pair sequence for dengue type 1 and 210 base pair sequence for dengue type 2. In other assays, we used dengue virus consensus primers having maximum sequence similarity to the four serotypes, amplifying a 511 base pair sequence. The reaction mixture also contained 0.1 mM of the four deoxynucleoside triphosphates, 7.5 U of reverse transcriptase, 1U of thermostable Taq DNA polymerase. The mixture was incubated for 5 minutes at 37ºC for reverse transcription followed by 30 cycles of two-step PCR amplification (92ºC for 60 seconds, 53ºC for 60 seconds) with slow temperature increment. The PCR products were subjected to 1.7% agarose gel electrophoresis and visualized by UV light after staining with ethidium bromide solution. Low virus titer around 10 3, 6 TCID50/ml was detected by RT-PCR for dengue type 1. Specific DNA amplification was observed with all the Brazilian dengue strains by using dengue virus consensus primers. As compared to other RT-PCRs, this assay is less laborious, done in a shorter time, and has reduced risk of contamination

Visceral larva migrans (VLM) is a clinical syndrome caused by infection of man by Toxocara spp, the common roundworm of dogs and cats. Tissue migration of larval stages causes illness specially in children. Because larvae are difficult to detect in tissues, diagnosis is mostly based on serology. After the introduction of the enzyme-linked immunosorbent assay (ELISA) using the larval excretory-secretory antigen of T. canis (TES), the diagnosis specificity was greatly improved although cross-reactivity with other helminths are still being reported. In Brazil, diagnosis is routinely made after absorption of serum samples with Ascaris suum antigens, a nematode antigenicaly related with Ascaris lumbricoides which is a common intestinal nematode of children. In order to identify T. canis antigens that cross react to A. suum antigens we analyzed TES antigen by SDS-PAGE and Western blotting techniques. When we used serum samples from patients suspected of VLM and positive result by ELISA as well as a reference serum sample numerous bands were seen (molecular weight of 210-200 kDa, 116-97 kDa, 55-50 kDa and 35-29 kDa). Among these there is at least one band with molecular weight around 55-66 kDa that seem to be responsible for the cross-reactivity between T. canis e A. suum once it disappears when previous absorption of serum samples with A. suum antigens is performed

We assessed the frequency of serological markers of hepatitis B virus (HBV) and hepatitis C virus (HCV) infections in 365 alcoholics by determining, by ELISA, the presence of HBsAg, anti-HBc, anti-HBs and anti-HCV. Fifty patients were cirrhotics and 315 had no evidence of hepatic cirrhosis; of the latter HBsAg was assessed in all, anti-HBc and anti-HBs in 130, and anti-HCV in 210. Among the alcoholics the frequencies of HBsAg (1.9%), anti-HBc (28.3%) and anti-HCV (3.8%) were higher (p<0.001) than among the controls (N=17,059), 0.4%, 4.0% and 0.4% respectively. The frequency of positive HBsAg was higher (p<0.001) in the cirrhotic patients (8.0%) than in alcoholics without cirrhosis (0.95%) and in controls (0.4%), and similar between the latter; of anti-HBc in alcoholics without cirrhosis (28.5%) was similar in cirrhotics patients (28.0%) and higher (p<0.001) than in the controls (4.0%); of anti-HBs in alcoholics without cirrhosis (20.8%) was similar to that of the cirrhotic patients (10.0%), and the anti-HCV was similar between alcoholics with (6.0%) and without cirrhosis (3.3%) and higher (p<0.001) than in controls (0.4%). We concluded that: a) alcoholics with or without cirrhosis have similar frequencies of infection with HBV and HCV between them, and higher than in nonalcoholics; b) alcoholics without cirrhosis had a frequency of HBV active infection (HBsAg+) which was similar to the controls, whereas among those who progressed to cirrhosis this frequency was significantly higher, what suggests that HBV may be implicated in the pathogenesis of cirrhosis in a few alcoholic individuals.

The present study intended to analyze the seroprevalence of Helicobacter pylori, IgG, and its relation to dyspepsia in a population from the western Amazon region. During the "Projeto Bandeira Científica", a University of São Paulo Medical School program, in Monte Negro's rural areas, state of Rondônia, 266 blood samples were collected from volunteers. The material was tested for IgG antibodies anti-Helicobacter pylori by ELISA method and the participants were also interviewed on dyspepsia, hygiene and social aspects. Participants aged between five and 81 years old (34 years on average), 149 (56%) were female and 117 (44%) male. We found 210 (78.9%) positive, 50 (18.8%) negative and six (2.3%) undetermined samples. Dyspeptic complaints were found in 226 cases (85.2%). There was no statistical association between dyspepsia and positive serology for H. pylori. We concluded that the seroprevalence in all age categories is similar to results found in other studies conducted in developing countries, including those from Brazil. On the other hand, the seroprevalence found in Monte Negro was higher than that reported in developed countries. As expected, there was a progressive increase in the positivity for H. pylori in older age groups.

SUMMARY To evaluate soil contamination by parasites in different developmental stages in public squares used as recreation and leisure areas for children in Belo Horizonte (MG, Brazil), 210 soil samples and 141 canine fecal samples were collected from 42 squares in the city. These samples were analyzed by the Caldwell and Caldwell technique and the Hoffman, Pons, and Janer technique. Of the samples analyzed, 89 (42.4%) soil samples and 104 (73.5%) fecal samples were contaminated with Ancylostoma sp., Toxocara sp., Trichuris sp., or Dipylidium sp. eggs; Giardia sp. cysts; or Isospora sp. oocysts. The commonest parasite was Ancylostoma sp., found in 85% soil and 99% fecal samples, followed by Toxocara sp., found in 43.7% soil and 30.7% fecal samples.

Parasitic infection is one of the problems that affect human health, especially in developing countries. In this study, all of the fast food shops, restaurants, and roast meat outlets of Khorramabad (Western Iran) and all the staff employed by them, some 210 people, were selected through a census and their stools were examined for the presence of parasites. The parasitological tests of direct wet-mount, Lugol's iodine staining, formaldehyde-ether sedimentation and Trichrome staining techniques were performed on the samples. The data was analyzed with a chi-square test and logistic regression was selected as the analytical model. The results showed 19 (9%) stool specimens were positive for different intestinal parasites. These intestinal parasites included Giardia lamblia2.9%, Entamoeba coli 4.3%, Blastocystis sp. 1.4%, and Hymenolepis nana 0.5%. There was a significant difference between the presence of a valid health card, awareness of transmission of intestinal parasites, participation in training courses in environmental health with intestinal parasites (p < 0.05). No statistically significant difference was found between the rate of literacy and gender among patients infected with intestinal parasites (p > 0.05). To control parasitic infection in food handlers, several strategies are recommended such as stool examinations every three months, public education, application of health regulations, controlling the validity of health cards and training on parasitic infection transmission. In this regard, the findings of the present study can be used as a basis to develop preventive programs targeting food handlers because the spread of disease via them is a common problem worldwide.

Para caracterizar o perfil clínico e demográfico dos portadores da forma digestiva da doença de Chagas atualmente atendidos no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto, foram revistos 377 prontuários de pacientes com resultado positivo para reação sorológica para a doença de Chagas atendidos entre janeiro de 2002 a março de 2003. A idade mediana dos pacientes era de 67 anos e 210 (56%) eram mulheres. Megaesôfago e/ou megacólon chagásicos estavam presentes em 135 pacientes, dos quais, 59% apresentavam cardiopatia. Para 49% dos pacientes com doença digestiva, havia prescrição de pelo menos dois medicamentos para tratamento de doença cardiovascular. Em 66 pacientes, foram detectadas comorbidades crônicas. A população de portadores da forma digestiva da doença de Chagas do HCFMRP é majoritariamente geriátrica e apresenta freqüência elevada de doenças cardiovasculares, o que sugere risco elevado das modalidades de tratamento cirúrgico do megaesôfago e megacólon.

O estudo foi desenvolvido com o objetivo de caracterizar os genótipos da proteína circunsporozoíta de Plasmodium vivax, circulantes em área periférica da Ilha de São Luís, Maranhão. Foram obtidas amostras de sangue para exame parasitológico direto (gota espessa) de 126 indivíduos, dentre os quais, foram coletadas também 109 amostras para diagnóstico molecular, por reação em cadeia da polimerase. O exame parasitológico demonstrou a presença de Plasmodium vivax em 2 indivíduos, sintomáticos, enquanto o estudo molecular foi positivo para o Plasmodium vivax em 7 indivíduos (2 sintomáticos e positivos na gota espessa e 5 assintomáticos e negativos na gota espessa). Em dois havia associação com Plasmodium falciparum. A genotipagem das amostras de Plasmodium vivax revelou a variante VK 210, havendo associação com a variante VK 247 em duas delas.
