To determine the effects of combined therapy of gliclazide and bedtime insulin on glycemic control and C-peptide secretion, we studied 25 patients with type 2 diabetes and sulfonylurea secondary failure, aged 56.8 ± 8.3 years, with a duration of diabetes of 10.6 ± 6.6 years, fasting plasma glucose of 277.3 ± 64.6 mg/dl and a body mass index of 27.4 ± 4.8 kg/m². Patients were submitted to three therapeutic regimens lasting 2 months each: 320 mg gliclazide (phase 1), 320 mg gliclazide and bedtime NPH insulin (phase 2), and insulin (phase 3). At the end of each period, glycemic and C-peptide curves in response to a mixed meal were determined. During combined therapy, there was a decrease in all glycemic curve values (P<0.01). Twelve patients (48%) reached fasting plasma glucose <140 mg/dl with a significant weight gain of 64.8 kg (43.1-98.8) vs 66.7 kg (42.8-101.4) (P<0.05), with no increase in C-peptide secretion or decrease in HbA1. C-Peptide glucose score (C-peptide/glucose x 100) increased from 0.9 (0.2-2.1) to 1.3 (0.2-4.7) during combined therapy (P<0.01). Despite a 50% increase in insulin doses in phase 3 (12 U (9-30) vs 18 U (11-60); P<0.01) only 3 patients who responded to combined therapy maintained fasting plasma glucose <140 mg/dl (P<0.02). A tendency to a higher absolute increase in C-peptide (0.99 (0.15-2.5) vs 0.6 (0-2.15); P = 0.08) and C-peptide incremental area (2.47 (0.22-6.2) vs 1.2 (0-3.35); P = 0.07) was observed among responders. We conclude that combined therapy resulted in a better glucose response to a mixed meal than insulin alone and should be tried in type 2 diabetic patients before starting insulin monotherapy, despite difficulties in predicting the response.

Agaricus blazei Murill is a native Brazilian mushroom which functions primarily as an anticancer substance in transplanted mouse tumors. However, the mechanism underlying this function of A. blazei Murill remains obscure. The present study was carried out to investigate the effect of fraction FA-2-b-ß, an RNA-protein complex isolated from A. blazei Murill, on human leukemia HL-60 cells in vitro. Typical apoptotic characteristics were determined by morphological methods using DNA agarose gel electrophoresis and flow cytometry. The growth suppressive effect of fraction FA-2-b-ß on HL-60 cells in vitro occurred in a dose- (5-80 µg/mL) and time-dependent (24-96 h) manner. The proliferation of HL-60 cells (1 x 10(5) cells/mL) treated with 40 µg/mL of fraction FA-2-b-ß for 24-96 h and with 5-80 µg/mL for 96 h resulted in inhibitory rates ranging from 8 to 54.5%, and from 4.9 to 86.3%, respectively. Both telomerase activity determined by TRAP-ELISA and mRNA expression of the caspase-3 gene detected by RT-PCR were increased in HL-60 cells during fraction FA-2-b-ß treatment. The rate of apoptosis correlated negatively with the decrease of telomerase activity (r = 0.926, P < 0.05), but correlated positively with caspase-3 mRNA expression (r = 0.926, P < 0.05). These data show that fraction FA-2-b-ß can induce HL-60 cell apoptosis and that the combined effect of down-regulation of telomerase activity and up-regulation of mRNA expression of the caspase-3 gene could be the primary mechanism of induction of apoptosis. These findings provide strong evidence that fraction FA-2-b-ß could be of interest for the clinical treatment of acute leukemia.

We determined the effect of fish oil (FO) ingestion on colonic carcinogenesis in rats. Male Wistar rats received 4 subcutaneous injections (40 mg/kg body weight each) of 1,2-dimethylhydrazine (DMH) at 3-day intervals and were fed a diet containing 18% by weight FO (N = 10) or soybean oil (SO, N = 10) for 36 weeks. At sacrifice, the colon was removed, aberrant crypt foci were counted and the fatty acid profile was determined. Intestinal tumors were removed and classified as adenoma or carcinoma. Liver and feces were collected and analyzed for fatty acid profile. FO reduced the mean (± SEM) number of aberrant crypt foci compared to SO (113.55 ± 6.97 vs 214.60 ± 18.61; P < 0.05) and the incidence of adenoma (FO: 20% vs SO: 100%), but carcinoma occurred equally in FO and SO rats (2 animals per group). The polyunsaturated fatty acid (PUFA) profile of the colon was affected by diet (P < 0.05): total ω-3 (FO: 8.18 ± 0.97 vs SO: 1.71 ± 0.54%) and total ω-6 (FO: 3.83 ± 0.59 vs SO: 10.43 ± 1.28%). The same occurred in the liver (P < 0.05): total ω-3 (FO: 34.41 ± 2.6 vs SO: 6.46 ± 0.59%) and total ω-6 (FO: 8.73 ± 1.37 vs SO: 42.12 ± 2.33%). The PUFA profile of the feces and liver polyamine levels did not differ between groups (P > 0.05). In conclusion, our findings indicate that chronic FO ingestion protected against the DMH-induced preneoplastic colon lesions and adenoma development, but not against carcinoma in rats.

Our objective was to evaluate the association of rs12255372 in theTCF7L2 gene with type 2 diabetes mellitus (T2DM) in the world population. We carried out a survey of the literature about the effect of rs12255372 on genetic susceptibility to T2DM by consulting PubMed, the Cochrane Library, and Embase from 2006 to 2012, and then performed a meta-analysis of all the studies in order to evaluate the association between rs12255372 and T2DM. A total of 33 articles including 42 studies (with 34,076 cases and 36,192 controls) were confirmed to be eligible and were included in the final meta-analysis: 6 studies conducted on Europeans, 14 on Caucasians, 17 on Asians, 2 on Africans, and 3 on Americans. Overall, the effect size was as follows: for the variant allele T (OR = 1.387, 95%CI = 1.351-1.424), for the TT genotype (OR = 1.933, 95%CI = 1.815-2.057), for the GT genotype (OR = 1.363, 95%CI = 1.315-1.413), for the dominant model (OR = 1.425, 95%CI = 1.344-1.510), and for the recessive model (OR = 1.659, 95%CI = 1.563-1.761). In summary, by pooling all available qualified data from genetic studies on rs12255372 and T2DM, we have confirmed that rs12255372 is significantly associated with susceptibility to T2DM in the global population.

High plasma levels of homocysteine (Hcy) promote the progression of neurodegenerative diseases. However, the mechanism by which Hcy mediates neurotoxicity has not been elucidated. We observed that upon incubation with Hcy, the viability of a neuroblastoma cell line Neuro2a declined in a dose-dependent manner, and apoptosis was induced within 48 h. The median effective concentration (EC50) of Hcy was approximately 5 mM. Glyceraldehyde-3-phosphate dehydrogenase (GAPDH) nuclear translocation and acylation has been implicated in the regulation of apoptosis. We found that nuclear translocation and acetylation of GAPDH increased in the presence of 5 mM Hcy and that higher levels of acetyltransferase p300/CBP were detected in Neuro2a cells. These findings implicate the involvement of GAPDH in the mechanism whereby Hcy induces apoptosis in neurons. This study highlights a potentially important pathway in neurodegenerative disorders, and a novel target pathway for neuroprotective therapy.

O presente trabalho visou a aplicação da β-1,3 glucanase lítica, obtida do microrganismo Cellulosimicrobium cellulans 191, na produção de protoplastos e na lise da parede celular de leveduras. A preparação bruta da enzima foi capaz de lisar as leveduras Kluyveromyces lodderi, Saccharomyces cerevisiae (Fleischmann e Itaiquara), S. cerevisiae KL-88, S. diastaticus NCYC 713, S. cerevisiae NCYC 1001, Candida glabrata NCYC 388, Kluyveromyces marxianus NCYC 587 e Hansenula mrakii NCYC 500. A β-1,3 glucanase purificada foi capaz de lisar as leveduras Saccharomyces cerevisiae KL-88, Saccharomyces capensis, Debaromyces vanriji, Pachysolen tannophillus, Kluyveromyces drosophilarum, Candida glabrata, Hansenula mrakii e Pichia membranaefaciens e formar protoplastos de Saccharomyces cerevisiae KL-88.

Crianças com SAVA podem ter deficiências auditivas moderadas a severas durante fases precoces da infância, porém sua audição residual permite que elas desenvolvam linguagem oral com aparelhos auditivos convencionais e possam estar completamente integradas a condições escolares regulares. Contudo, estas crianças apresentam uma deterioração de sua habilidade auditiva com o decorrer do tempo e o implante coclear está sendo utilizado como uma opção para manter a habilidade auditiva. OBJETIVO: Avaliação da habilidade auditiva de 3 crianças com SAVA submetidas a implante coclear. MATERIAIS: Estudo retrospectivo baseado em revisão de prontuários. RESULTADOS: Em reconhecimento de palavras em campo aberto paciente 1, 80%, paciente 2, 87,5%, paciente 3, 4%. CONCLUSÃO: Os pacientes com aqueduto vestibular alargado são considerados bons candidatos para implante coclear pelos principais centros de implante coclear do mundo, por desenvolverem, em sua maioria, bons resultados de percepção de fala, o que leva estes pacientes a uma boa inserção social.

A goiaba é uma fruta que apresenta alta perecibilidade, sendo o armazenamento em Atmosfera Controlada (AC) uma técnica que pode prolongar a vida pós-colheita, mantendo sua qualidade. O objetivo deste trabalho foi determinar a melhor condição de armazenamento em AC para a conservação da qualidade da goiaba, cultivar 'Paluma'. O experimento foi conduzido no Delineamento em Blocos ao Acaso, com os tratamentos: [1] 20,9 kPa O2 + 0,03 kPa CO2 (AR - testemunha); [2] 1,0 kPa O2 + 2,0 kPa CO2; [3] 2,0 kPa O2 + 2,0 kPa CO2; [4] 3,0 kPa O2 + 2,0 kPa CO2; e [5] 3,0 kPa O2 + 4,0 kPa CO2. Todos os frutos foram armazenados à temperatura de 8°C (±0,2) sob umidade relativa de 95% (±2,0). Após 28 dias de armazenamento, os frutos foram avaliados quanto às suas características físico-químicas. A melhor conservação da goiaba cultivar 'Paluma' em AC foi obtida com O2 entre 1 e 2 kPa, combinado com 2 kPa de CO2. Estas condições reduziram a degradação da coloração verde da epiderme, a perda de firmeza e de acidez e mantiveram a coloração da polpa com mais eficiência; porém, a alta ocorrência de podridões durante a vida de prateleira é o fator que limita o período de conservação desta goiaba.

A relevância da autogestão tem despertado estudos e discussões sobre o tema no âmbito da academia, principalmente no que diz respeito à sua proposta de mudança social. A partir de uma revisão da literatura e da análise de algumas experiências autogestionárias, defende-se a ideia de que a gestão autônoma repousa sobre três pilares: 1) autonomia e equidade no processo decisório; 2) valores humanistas e 3) disponibilidade de conhecimento técnico-administrativo para os membros da organização. O objetivo deste artigo é, portanto, analisar a experiência do Programa de Autogestão da Habitação, da Secretaria Municipal de Habitação de Belo Horizonte, à luz destes pilares definidos pelos autores. De forma geral, percebeu-se que a vivência dos pilares não se dá de forma completa, o que de certa maneira impede que a autogestão seja exercida integralmente. No entanto, argumenta-se que tal forma de gestão cria um vínculo identitário-afetivo das pessoas com o imóvel e contribui para o desenvolvimento da cidadania, os quais se destacam como aspectos positivos do programa.

Com o objetivo de verificar a maneira mais eficaz para descobrir e tratar casos de tuberculose pulmonar, um modelo matemático foi aplicado à demanda do Centro de Saúde de Ribeirão Preto, São Paulo (Brasil). As 30.855 pessoas residentes com 15 ou mais anos de idade que se matricularam entre maio de 1973 e abril de 1974 foram submetidas ao teste tuberculínico, abreugrafia e exame bacteriológico do escarro (nos sintomáticos). Os resultados forneceram a composição epidemiológica da demanda, constituída por cinco categorias mutuamente exclusivas: 1 - não infectado: 61,14%; 2 - infectados: 37,84%; 3 - suspeitos à abreugrafia: 0,83%; 4 - casos positivos apenas à cultura: 0,07%; 5 - casos baciloscopia-positivos: 0,12%. Paralelamente calcularam-se os custos médios unitários dos exames e do tratamento no Centro de Saúde. Com esses dados, o modelo matemático proposto por Revelle e Male, modificado neste trabalho, foi aplicado para o cálculo do custo por caso ativo tratado segundo todas as combinações lógicas possíveis dos exames citados. A estratégia mais adequada à demanda estudada, nas condições administrativas vigentes, definida por critérios econômico, epidemiológico e clínico foi a seguinte: inicialmente teste tuberculínico em toda a demanda; a seguir, baciloscopia de escarro apenas nos reatores, complementada por cultura dos escarros negativos; tratamento dos positivos à bacteriologia. Sendo disponível a abreugrafia, este exame poderá ser encaixado nesta seqüência supra, entre o teste tuberculínico e a baciloscopia.

Em 1988 e no primeiro semestre de 1989 cinco desinfetantes de uso doméstico foram divulgados através de publicação televisiva. Para avaliar as propriedades antimicrobianas desses produtos os mesmos foram testados por um método qualitativo (Diluição-Uso com 10 carreadores, método convencional e outro simplificado adaptado) e outro quantitativo, também adaptado. Os compostos ativos dos produtos descritos nos respectivos rótulos foram: 1- Paraclorofenol (O- Benzil) 0,1%; 2 - Éter 2,4,4' Cloro (III) 2' hidroxifenilico 0,1%; 3 - N-alquildimetil benzil amônio-Cloreto de alquil dimetil etil benzil amonio 50% - 1,6%; 4 - formaldeído 37% (solução de 0,3%); 5 - Sem informação. Os microrganismos utilizados foram: Staphylococcus aureus ATCC 6538, Pseudomonas aeruginosa ATCC 15442 e Salmonella choleraesuis ATCC 10708. No método qualitativo, a cepa de pseudomonas foi recuperada dos desinfetantes 1, 2 e 3. Todos os desinfetantes mostraram um efeito germicida de 5,0 (99,999% de redução) em 15 segundos frente às três cepas. O desinfetante 3 estava contaminado com Enterobacter sp na ordem de 10(4) células por ml. Este contaminante foi sensível diante dos desinfetantes 1, 4 e 5, frente à metodologia qualitativa, e relativamente resistente frente ao desinfetante 2, na metodologia quantitativa.

OBJETIVO: A susceptibilidade dos insetos tem sido um dos mais importantes aspectos a ser monitorados em programas de saúde pública que tratam do controle de vetores. O estudo objetiva avaliar a susceptibilidade de larvas de Aedes aegypti a inseticidas químicos em áreas sujeitas ou não a controle. MÉTODOS: Bioensaios foram realizados com concentração de diagnóstico e concentração múltipla, segundo padrão da Organização Mundial de Saúde para as coletas de larvas de Aedes aegypti, em uma área não sujeita -- Campinas, SP -- e em uma outra área sujeita -- Campo Grande, MS -- a tratamentos químicos de controle. RESULTADOS: Larvas de Aedes aegypti coletadas em Campinas indicaram resistência potencial à concentração-diagnóstico (CD) de 0,04 ppm do organofosforado temephos. O teste de concentração múltipla registrou sobrevivência de 24,5% à concentração de 0,0125 ppm. A susceptibilidade dessa mesma linhagem foi avaliada para o organofosforado fenitrothion (CD=0,08 ppm) e o piretróide cipermetrina (CD=0,01 ppm), resultando em valores normais para essas concentrações. Larvas de Ae. aegypti coletadas em Campo Grande mostraram susceptibilidade normal ao temephos (CD=0,04 ppm) e à cipermetrina (CD=0,01 ppm). Também foram estabelecidas as CL50 e as CL95 de cipermetrina 25 CE, cyfluthrin 5 CE, betacyfluthrin 1,25 SC e propoxur 20 CE para Ae. aegypti. Com base nos dados da linhagem-padrão Rockefeller, foram estimadas as razões de resistência de 2,9, 2,2, 2,4 e 1,3, respectivamente, pela CL50, e de 3,5, 2,6, 3,9 e 1,3 pela CL95. CONCLUSÃO: Os resultados reforçam a necessidade de avaliações prévias e monitoramento da efetividade dos inseticidas que devem ser usados em programas de controle de mosquitos.

OBJECTIVE: Selecting controls is one of the most difficult tasks in the design of case-control studies. Hospital controls may be inadequate and random controls drawn from the base population may be unavailable. The aim was to assess the use of hospital visitors as controls in a case-control study on the association of organochlorinated compounds and other risk factors for breast cancer conducted in the main hospital of the "Instituto Nacional de Câncer" -- INCA (National Cancer Institute) in Rio de Janeiro (Brazil). METHODS: The study included 177 incident cases and 377 controls recruited among female visitors. Three different models of control group composition were compared: Model 1, with all selected visitors; Model 2, excluding women visiting relatives with breast cancer; and Model 3, excluding all women visiting relatives with any type of cancer. Odds ratios (OR) and 95% confidence intervals were calculated to test the associations. RESULTS: Age-adjusted OR for breast cancer associated with risk factors other than family history of cancer, except smoking and breast size, were similar in the three models. Regarding family history of all cancers, except for breast cancer, there was a decreased risk in Models 1 and 2, while in Model 3 there was an increased risk, but not statistically significant. Family history of breast cancer was a risk factor in Models 2 and 3, but no association was found in Model 1. In multivariate analysis a significant risk of breast cancer was found when there was a family history of breast cancer in Models 2 and 3 but not in Model 1. CONCLUSIONS: These results indicate that while investigating risk factors unrelated to family history of cancer, the use of hospital visitors as controls may be a valid and feasible alternative.

INTRODUCTION: Although obesity is well recognized as a current public health problem, its prevalence and impact among pregnant women have been less investigated in Brazil. The objective of the study was to evaluate the impact of pre-obesity and obesity among pregnant women, describing its prevalence and risk factors, and their association with adverse pregnancy outcomes. METHODS: A cohort of 5,564 pregnant women, aged 20 years or more, enrolled at aproximately 20 to 28 weeks of pregnancy, seen in prenatal public clinics of six state capitals in Brazil were followed up, between 1991 and 1995. Prepregnancy weight, age, educational level and parity were obtained from a standard questionnaire. Height was measured in duplicate and the interviewer assigned the skin color. Nutritional status was defined using body mass index (BMI), according to World Health Organization (WHO) criteria. Odds ratios and 95% confidence interval were calculated using logistic regression. RESULTS: Age-adjusted prevalences (and 95% CI) based on prepregnancy weight were: underweight 5.7% (5.1%-6.3%), overweight 19.2% (18.1%-20.3%), and obesity 5.5% (4.9%-6.2%). Obesity was more frequently observed in older black women, with a lower educational level and multiparous. Obese women had higher frequencies of gestational diabetes, macrosomia, hypertensive disorders, and lower risk of microsomia. CONCLUSIONS: Overweight nutritional status (obesity and pre-obesity) was seen in 25% of adult pregnant women and it was associated with increased risk for several adverse pregnancy outcomes, such as gestational diabetes and pre-eclampsia.

OBJECTIVE: To evaluate possible adverse reproductive outcomes in an area adjacent to a petrochemical plant in southern Brazil. METHODS: A review of 17,113 birth records of the main hospital of the municipality of Montenegro, southern Brazil, from 1983 to 1998 was carried out. Three groups of cases were selected: (1) newborns with major congenital malformations; (2) newborns with low birth weight (<2,500 g); and (3) stillborns (>500 g). A control was assigned to each case. Controls were the first newborns weighing > or = 2,500 g without malformations and of case-matching sex. Mother's residence during pregnancy was used as an exposure parameter. Statistical analyses were performed using Chi-square test or Fisher test, odds ratio, 0.05 significance level, and 95% confidence interval. RESULTS: For unadjusted analysis, it was found a correlation between low birth weight and geographical proximity of mother's residence to the petrochemical plant (OR = 1.66; 95% CI = 1.01--2.72) or residence on the way of preferential wind direction (OR = 1.62; 95% CI = 1.03--2.56). When other covariates were added in the conditional logistic regression (maternal smoking habits, chronic disease and age), there was no association. CONCLUSIONS: Despite final results were negative, low birth weight could be a good parameter of environmental contamination and should be closely monitored in the studied area.
