Palicourea marcgravii é considerada a principal planta tóxica de interesse pecuário no Brasil, porém, até o momento, é conhecido apenas o quadro agudo da intoxicação. Esse estudo avaliou as alterações clínico-patológicas de 10 caprinos cronicamente intoxicados por P. marcgravii. Os animais receberam, diariamente, 0,2g/kg de peso corporal da planta fresca por 6 a 38 dias. Os caprinos apresentaram como principais sinais clínicos anorexia, apatia, taquicardia, arritmia, pulso venoso jugular positivo e decúbito. Nas necropsias, os achados macroscópicos foram hidropericárdio, palidez dos rins e do miocárdio, atrofia gelatinosa da gordura cardíaca, evidenciação do padrão lobular hepático e edema pulmonar. Os principais achados microscópicos foram tumefação e vacuolização de cardiomiócitos, necrose de fibras cardíacas e infiltrado inflamatório mononuclear no miocárdio. Nos rins foi encontrada marcante degeneração hidrópico-vacuolar difusa. Os achados demonstraram nos caprinos cronicamente intoxicados, quadro clínico-patológico com características distintas do observado na forma aguda da intoxicação por P. marcgravii. Essas observações comprovam o risco para caprinos da ingestão da planta, mesmo que em pequenas doses, resultando no surgimento de quadro clínico e graves lesões ainda pouco conhecidas, e que poderiam ser confundidas com outras enfermidades.

Insulin induces tyrosine phosphorylation of Shc in cell cultures and in insulin-sensitive tissues of the intact rat. However, the ability of insulin receptor (IR) tyrosine kinase to phosphorylate Shc has not been previously demonstrated. In the present study, we investigated insulin-induced IR tyrosine kinase activity towards Shc. Insulin receptor was immunoprecipitated from liver extracts, before and after a very low dose of insulin into the portal vein, and incubated with immunopurified Shc from liver of untreated rats. The kinase assay was performed in vitro in the presence of exogenous ATP and the phosphorylation level was quantified by immunoblotting with antiphosphotyrosine antibody. The results demonstrate that Shc interacted with insulin receptor after infusion of insulin, and, more important, there was insulin receptor kinase activity towards immunopurified Shc. The description of this pathway in animal tissue may have an important role in insulin receptor tyrosine kinase activity toward mitogenic transduction pathways.

To investigate the role of nitric oxide in human sepsis, ten patients with severe septic shock requiring vasoactive drug therapy and mechanical ventilation were enrolled in a prospective, open, non-randomized clinical trial to study the acute effects of methylene blue, an inhibitor of guanylate cyclase. Hemodynamic and metabolic variables were measured before and 20, 40, 60, and 120 min after the start of a 1-h intravenous infusion of 4 mg/kg of methylene blue. Methylene blue administration caused a progressive increase in mean arterial pressure (60 [55-70] to 70 [65-100] mmHg, median [25-75th percentiles]; P<0.05), systemic vascular resistance index (649 [479-1084] to 1066 [585-1356] dyne s-1 cm-5 m-2; P<0.05) and the left ventricular stroke work index (35 [27-47] to 38 [32-56] g m-1 m-2; P<0.05) from baseline to 60 min. The pulmonary vascular resistance index increased from 150 [83-207] to 186 [121-367] dyne s-1 cm-5 m-2 after 20 min (P<0.05). Mixed venous saturation decreased from 65 [56-76] to 63 [55-69]% (P<0.05) after 60 min. The PaO2/FiO2 ratio decreased from 168 [131-215] to 132 [109-156] mmHg (P<0.05) after 40 min. Arterial lactate concentration decreased from 5.1 ± 2.9 to 4.5 ± 2.1 mmol/l, mean ± SD (P<0.05) after 60 min. Heart rate, cardiac filling pressures, cardiac output, oxygen delivery and consumption did not change. Methylene blue administration was safe and no adverse effect was observed. In severe human septic shock, a short infusion of methylene blue increases systemic vascular resistance and may improve myocardial function. Although there was a reduction in blood lactate concentration, this was not explained by an improvement in tissue oxygenation, since overall oxygen availability did not change. However, there was a significant increase in pulmonary vascular tone and a deterioration in gas exchange. Further studies are needed to demonstrate if nitric oxide blockade with methylene blue can be safe for patients with septic shock and, particularly, if it has an effect on pulmonary function.

Eggplant (Solanum melongena) is consumed extensively in Brazil. It has been believed that infusion of a powdered preparation of the fruit may reduce serum cholesterol. However, there are few documented reports on its effects on cholesterol metabolism and its possible hypocholesterolemic effect has not been proved by well-controlled studies. The aim of the present study was to observe the effects of S. melongena on the serum cholesterol and triglycerides of 38 hypercholesterolemic human volunteers ingesting S. melongena infusion for five weeks. Thirty-eight hypercholesterolemic subjects receiving either S. melongena infusion (N = 19) or placebo (N = 19) participated in two clinical experiments in which the effect of S. melongena infusion was studied with (N = 16) or without (N = 38) dietary orientation. Total cholesterol and its fractions, triglycerides, and apolipoproteins A and B were measured in blood at the beginning of the experiment and three and five weeks thereafter. No differences were observed compared to control. Intraindividual analysis showed that S. melongena infusion significantly reduced the blood levels of total and LDL cholesterol and of apolipoprotein B. After dietary orientation, no intra- or intergroup differences were seen for any of the parameters analyzed. The results suggest that S. melongena infusion had a modest and transitory effect, which was not different from that obtained with standard orientation for dyslipidemia patients (diet and physical activities).

The objective of the present study was to determine the effect of cellulose on intestinal iron absorption in rats during recovery from iron deficiency anemia. Twenty-one-day-old male Wistar-EPM rats were fed an iron-free ration for two weeks to induce anemia. At 5 weeks of age, the rats were divided into two groups (both groups receiving 35 mg of elemental iron per kg diet): cellulose group (N = 12), receiving a diet containing 100 g of cellulose/kg and control (N = 12), receiving a diet containing no cellulose. The fresh weight of the feces collected over a 3-day period between the 15th and 18th day of dietary treatment was 10.7 ± 3.5 g in the group receiving cellulose and 1.9 ± 1.2 g in the control group (P<0.001). Total food intake was higher in the cellulose group (343.4 ± 22.0 g) than in the control (322.1 ± 13.1 g, P = 0.009) during the 3 weeks of dietary treatment. No significant difference was observed in weight gain (cellulose group = 132.8 ± 19.2, control = 128.0 ± 16.3 g), hemoglobin increment (cellulose group = 8.0 ± 0.8, control = 8.0 ± 1.0 g/dl), hemoglobin level (cellulose group = 12.3 ± 1.2, control = 12.1 ± 1.3 g/dl) or in hepatic iron levels (cellulose group = 333.6 ± 112.4, control = 398.4 ± 168.0 µg/g dry tissue). We conclude that cellulose does not adversely affect the regeneration of hemoglobin, hepatic iron level or the growth of rats during recovery from iron deficiency anemia.

Pharmacological evidence indicates that the basolateral nucleus of the amygdala (BLA) is involved in the mediation of inhibitory avoidance but not of escape behavior in the elevated T-maze test. These defensive responses have been associated with generalized anxiety disorder (GAD) and panic disorder, respectively. In the present study, we determined whether the BLA plays a differential role in the control of inhibitory avoidance and escape responses in the elevated T-maze. Male Wistar rats (250-280 g, N = 9-10 in each treatment group) were pre-exposed to one of the open arms of the maze for 30 min and 24 h later tested in the model after inactivation of the BLA by a local injection of the GABA A receptor agonist muscimol (8 nmol in 0.2 µL). It has been shown that a prior forced exposure to one of the open arms of the maze, by shortening latencies to withdrawal from the open arm during the test, improves the escape task as a behavioral index of panic. The effects of muscimol in the elevated T-maze were compared to those caused by this GABA agonist in the avoidance reaction generated in the light/dark transition test. This defensive behavior has also been associated with GAD. In the elevated T-maze, intra-BLA injection of muscimol impaired inhibitory avoidance (control: 187.70 ± 14.90 s, muscimol: 37.10 ± 2.63 s), indicating an anxiolytic effect, without interfering with escape performance. The drug also showed an anxiolytic effect in the light/dark transition test as indicated by the increase in the time spent in the lighted compartment (control: 23.50 ± 2.45 s, muscimol: 47.30 ± 4.48 s). The present findings point to involvement of the BLA in the modulation of defensive responses that have been associated with GAD.

The relationship between lipid serum levels and coronary atherosclerotic plaque fat content was studied in 51 necropsy patients. Serum lipids were measured by standard techniques, during life, in the absence of lipid-lowering drugs. Intima, intimal fat and media areas were measured using a computerized system in cryosections of the odd segments of the right, anterior descending and circumflex coronary arteries stained with Sudan-IV. Mean intimal and lipid areas were 5.74 ± 1.98 and 1.22 ± 0.55 mm² (22.12 ± 8.48%) in 26 cases with high cholesterol (³200 mg/dL) and 4.98 ± 1.94 and 1.16 ± 0.66 mm² (22.75 ± 9.06%) in 25 cases with normal cholesterol (<200 mg/dL; P > 0.05). Patients with high levels of low-density lipoprotein (³130 mg/dL, N = 15) had a higher intima/media area ratio than those with normal levels of low-density lipoprotein (<130 mg/dL, N = 13, P < 0.01). No significant difference in the morphometrical variables was found in groups with high or low serum levels of triglycerides (³200 mg/dL, N = 13 vs <200 mg/dL, N = 36) or high-density lipoprotein (³35 mg/dL, N = 11 vs <35 mg/dL, N = 17). The association between the morphological measurements and serum levels of cholesterol, its fractions, and triglycerides was also tested and the correlation coefficients were low. Although high cholesterol is a risk factor, we show here that in patients with severe atherosclerosis blood cholesterol and triglyceride levels seem to have little influence on coronary lipid content, indicating that other factors may contribute to arterial lipid deposition and plaque formation.

CDKN2A encodes proteins such as p16 (INK4a), which negatively regulate the cell-cycle. Molecular genetic studies have revealed that deletions in CDKN2A occur frequently in cancer. Although p16 (INK4a) may be involved in tumor progression, the clinical impact and prognostic implications in head and neck squamous cell carcinoma (HNSCC) are controversial. The objective of this study was to evaluate the frequency of the immunohistochemical expression of p16 (INK4a) in 40 oropharynx and 35 larynx from HNSCC patients treated in a single institution and followed-up at least for 10 years in order to explore potential associations with clinicopathological outcomes and prognostic implications. Forty cases (53.3%) were positive for p16 (INK4a) and this expression was more intense in non-smoking patients (P = 0.050), whose tumors showed negative vascular embolization (P = 0.018), negative lymphatic permeation (P = 0.002), and clear surgical margins (P = 0.050). Importantly, on the basis of negative p16 (INK4a) expression, it was possible to predict a probability of lower survival (P = 0.055) as well as tumors presenting lymph node metastasis (P = 0.050) and capsular rupture (P = 0.0010). Furthermore, increased risk of recurrence was observed in tumors presenting capsular rupture (P = 0.0083). Taken together, the alteration in p16 (INK4a) appears to be a common event in patients with oropharynx and larynx squamous cell carcinoma and the negative expression of this protein correlated with poor prognosis.

Angiotensin II is a key player in the pathogenesis of renovascular hypertension, a condition associated with endothelial dysfunction. We investigated aliskiren (ALSK) and L-arginine treatment both alone and in combination on blood pressure (BP), and vascular reactivity in aortic rings. Hypertension was induced in 40 male Wistar rats by clipping the left renal artery. Animals were divided into Sham, 2-kidney, 1-clip (2K1C) hypertension, 2K1C+ALSK (ALSK), 2K1C+L-arginine (L-arg), and 2K1C+ALSK+L-arginine (ALSK+L-arg) treatment groups. For 4 weeks, BP was monitored and endothelium-dependent and independent vasoconstriction and relaxation were assessed in aortic rings. ALSK+L-arg reduced BP and the contractile response to phenylephrine and improved acetylcholine relaxation. Endothelium removal and incubation with N-nitro-L-arginine methyl ester (L-NAME) increased the response to phenylephrine in all groups, but the effect was greater in the ALSK+L-arg group. Losartan reduced the contractile response in all groups, apocynin reduced the contractile response in the 2K1C, ALSK and ALSK+L-arg groups, and incubation with superoxide dismutase reduced the phenylephrine response in the 2K1C and ALSK groups. eNOS expression increased in the 2K1C and L-arg groups, and iNOS was increased significantly only in the 2K1C group compared with other groups. AT1 expression increased in the 2K1C compared with the Sham, ALSK and ALSK+L-arg groups, AT2 expression increased in the ALSK+L-arg group compared with the Sham and L-arg groups, and gp91phox decreased in the ALSK+L-arg group compared with the 2K1C and ALSK groups. In conclusion, combined ALSK+L-arg was effective in reducing BP and preventing endothelial dysfunction in aortic rings of 2K1C hypertensive rats. The responsible mechanisms appear to be related to the modulation of the local renin-angiotensin system, which is associated with a reduction in endothelial oxidative stress.

INTRODUÇÃO: A maioria dos pacientes com doença renal crônica terminal depende de hemodiálise (HD) para a manutenção de sua vida. A análise dos fatores que influenciam na sobrevida pode auxiliar na busca contínua por melhores resultados. MÉTODOS: Analisamos 1.009 pacientes tratados por HD crônica em três unidades de diálise de Santa Maria, RS, Brasil, durante 25 anos (1982-2007). RESULTADOS: A sobrevida (método de Kaplan-Meier) em 1, 2 e 5 anos foi de 91%, 84% e 64%, respectivamente. No modelo proporcional de Cox, tiveram influência estatisticamente significativa sobre o risco de mortalidade: idade ao iniciar HD (aumento de 4,5% por ano a mais; p = 0,0001), presença de diabetes (aumento de 56%; p = 0,001) e ano de início da HD (redução de 5,2% por ano mais tarde; p = 0,0001). A sobrevida foi significativamente melhor para pacientes que iniciaram HD de 1997 a 2007 do que para os que iniciaram de 1982 a 1996, tanto em diabéticos (54% versus 41% em 5 anos; p = 0,01) como não diabéticos (72% versus 65% em 5 anos; p = 0,045), embora, nestes, a idade tenha sido significativamente maior no período mais recente. CONCLUSÕES: A presença de diabete e cada ano a mais na idade determinaram risco significativamente aumentado. Cada ano subsequente do calendário trouxe um risco significativamente menor. Nos anos mais recentes, a melhora de sobrevida foi maior para pacientes diabéticos e idosos, sendo atribuída a avanços diagnósticos e terapêuticos e melhor qualidade global do programa dialítico.

INTRODUÇÃO: Objetivou-se avaliar a dimensão educativa das percepções e atitudes de médicos, enfermeiros e auxiliares de enfermagem na saúde bucal de crianças e adolescentes portadores de insuficiência renal crônica (IRC) em três hospitais do Rio de Janeiro. MÉTODOS Realizou-se uma entrevista com perguntas abertas e fechadas com 43 profissionais de saúde. Os dados foram coletados e tabulados no programa SPSS 13.0. Empregou-se o teste Qui-quadrado, com nível de significância estatística p < 0,05. RESULTADOS: A média de idade foi 36,5 anos (± 11,3), com 80% do sexo feminino. A maioria dos médicos (71,4%, n = 10) e enfermeiros (72,4%, n = 21) acredita que esses pacientes podem ter alguma alteração bucal decorrente da doença, sendo perda de esmalte e descalcificação as mais citadas. Em relação à orientação sobre higiene bucal, pouco mais da metade da amostra respondeu que orienta seus pacientes no sentido de realizar escovação (72,7%, n = 16), usar fio dental (9%, n = 2), fazer bochecho (18,1%, n=4) e limpar a língua (9%, n = 2). Apenas 9% (n = 2) orientam a higiene após o uso de medicamentos. Quanto à necessidade de cuidados diferenciados para esses pacientes, 65,5% dos enfermeiros e auxiliares de enfermagem acreditam que estes devam ocorrer. Para a maioria dos médicos (57,1%, n = 8), não há necessidade de tais cuidados. Apesar disso, 78,6% (n = 11) dos médicos têm o hábito de encaminhar os pacientes a serviços odontológicos. CONCLUSÕES: Diante da metodologia empregada, concluiuse que a maioria dos profissionais de saúde têm algum conhecimento sobre saúde bucal, porém suas atitudes não refletem esse fato.

A Insuficiência Cardíaca Congestiva (ICC) é uma patologia com incidência crescente e que representa uma condição de grande impacto na saúde pública, com alta morbidade e mortalidade. O excesso de volume é uma complicação prevalente, presente em 80% dos pacientes atendidos com o diagnóstico de ICC. Estratégias farmacológicas e não farmacológicas no manejo terapêutico da ICC visam ao melhor manejo do volume e à redução no uso de diuréticos. A ultrafiltração extracorpórea tem evidenciado melhor controle de peso, redução nos dias de hospitalização e re-hospitalização dos pacientes com ICC. Neste artigo, relatamos dois casos atendidos no Hospital de Clínicas de Porto Alegre, relativos a pacientes com diagnóstico de ICC tratados com diálise peritoneal.

INTRODUÇÃO: A nutrição desempenha papel fundamental nas doenças renais. A recomendação nutricional deve ofertar uma dieta hiperproteica, adequada em energia e fósforo segundo o Kidney Disease Outcomes Quality Initiative (K/DOQI). É necessário controlar e/ou prevenir as complicações da Doença Renal Crônica (DRC), pois ela impõe desafios clínicos diretamente ligados ao estado nutricional. OBJETIVO: Investigar as associações entre a ingestão energética, proteica e de fósforo em pacientes em hemodiálise (HD). PACIENTES E MÉTODOS: Estudo observacional envolvendo 72 pacientes em HD, em dois hospitais de Porto Alegre/RS/Brasil. Foram coletados dados referentes ao perfil antropométrico (peso, altura e índice de massa corporal - IMC) e do registro alimentar de três dias (ingestão de energia, proteína e fósforo). O teste de correlação de Spearman foi utilizado para avaliar as associações entre as variáveis do registro alimentar (p < 0,05). RESULTADOS: A ingestão energética, proteica e de fósforo foi, respectivamente, de: 28 ± 10,0 kcal/kg, 1,1 ± 0,4 g ptn/kg, 958 ± 374 mg/dia, estando os níveis de energia e proteína abaixo das recomendações do K/DOQI. Observou-se correlação positiva forte entre as variáveis. DISCUSSÃO: O perfil no tratamento nutricional é fundamental, pois houve correlações entre os nutrientes. É importante salientar que as recomendações do K/DOQI não conseguem ser atingidas pelos pacientes. CONCLUSÃO: Há associação positiva entre a ingestão de energia, proteína e fósforo na alimentação de pacientes em HD. O controle desses nutrientes na dieta é essencial, pois auxilia no controle das complicações relacionadas com a DRC. Assim, a avaliação nutricional é importante nas unidades de diálise para definir o perfil dietético dos pacientes e que seja feito o ajuste de suas prescrições.

INTRODUÇÃO: Este artigo enfoca um dos objetivos de um estudo mais amplo sobre a realização de diálise peritoneal (DP) em crianças e adolescentes no domicílio. Descreve e discute os relatos dos cuidadores sobre as reações e dificuldades vivenciadas pela família e pela criança/adolescente com doença renal crônica (DRC) para a realização da DP. MÉTODO: Realizou-se no período de março de 2004 a maio de 2006 estudo descritivo constituído de um universo de 30 crianças e adolescentes portadores de DRC, assistidos pelo HC/UFMG, abordando questões relativas às dificuldades dos cuidadores quanto à aplicação da técnica de DP e as queixas das crianças/adolescentes quanto à DP por meio de entrevista, acompanhamento de consulta de rotina e visita domiciliar. RESULTADOS: As principais queixas foram: limitações que a diálise traz para a vida do paciente e do cuidador e aquelas relativas à diálise peritoneal em si. CONCLUSÃO: O conhecimento da realidade vivenciada pela criança/adolescente com DRC e pela família pode subsidiar ações e medidas a fim de melhorar a qualidade de vida dos envolvidos e contribuir para o sucesso da técnica dialítica.
