Malaria is currently one of the most serious public health problems in Colombia with an endemic/epidemic transmission pattern that has maintained endemic levels and an average of 105,000 annual clinical cases being reported over the last five years. Plasmodium vivax accounts for approximately 70% of reported cases with the remainder attributed almost exclusively to Plasmodium falciparum. A limited number of severe and complicated cases have resulted in mortality, which is a downward trend that has been maintained over the last few years. More than 90% of the malaria cases in Colombia are confined to 70 municipalities (about 7% of the total municipalities of Colombia), with high predominance (85%) in rural areas. The purpose of this paper is to review the progress of malaria-eradication activities and control measures over the past century within the eco-epidemiologic context of malaria transmission together with official consolidated morbidity and mortality reports. This review may contribute to the formulation of new antimalarial strategies and policies intended to achieve malaria elimination/eradication in Colombia and in the region.

Recently, we described the improved immunogenicity of new malaria vaccine candidates based on the expression of fusion proteins containing immunodominant epitopes of merozoites and Salmonella enterica serovar Typhimurium flagellin (FliC) protein as an innate immune agonist. Here, we tested whether a similar strategy, based on an immunodominant B-cell epitope from malaria sporozoites, could also generate immunogenic fusion polypeptides. A recombinant His6-tagged FliC protein containing the C-terminal repeat regions of the VK210 variant of Plasmodium vivax circumsporozoite (CS) protein was constructed. This recombinant protein was successfully expressed in Escherichia coli as soluble protein and was purified by affinity to Ni-agarose beads followed by ion exchange chromatography. A monoclonal antibody specific for the CS protein of P. vivax sporozoites (VK210) was able to recognise the purified protein. C57BL/6 mice subcutaneously immunised with the recombinant fusion protein in the absence of any conventional adjuvant developed protein-specific systemic antibody responses. However, in mice genetically deficient in expression of TLR5, this immune response was extremely low. These results extend our previous observations concerning the immunogenicity of these recombinant fusion proteins and provide evidence that the main mechanism responsible for this immune activation involves interactions with TLR5, which has not previously been demonstrated for any recombinant FliC fusion protein.

In Brazil, carbapenem-resistant Pseudomonas aeruginosa isolates are closely related to the São Paulo metallo-β-lactamase (SPM) Brazilian clone. In this study, imipenem-resistant isolates were divided in two sets, 2002/2003 and 2008/2009, analysed by pulsed field gel electrophoresis and tested for the Ambler class B metallo-β-lactamase (MBL) genes blaSPM-1, blaIMP and blaVIM. The results show a prevalence of one clone related to the SPM Brazilian clone in 2002/2003. In 2008/2009, P. aeruginosa isolates were mostly MBL negative, genetically diverse and unrelated to those that had been detected earlier. These findings suggest that the resistance to carbapenems by these recent P. aeruginosa isolates was not due to the spread of MBL-positive SPM-related clones, as often observed in Brazilian hospitals.

Candida species are an important cause of bloodstream infections (BSI). To evaluate the epidemiological, clinical and microbiological aspects of two cohorts {1994-1999 [period 1 (P1) ]; 2000-2004 [period 2 (P2) ]} of candidaemic patients, we performed a retrospective analysis from a laboratory-based survey. A total of 388 candidaemias were identified, with an incidence of 0.20/1,000 patient-days and a significant increase in P2 vs. P1 (0.25 vs. 0.15, p = 0.04). Cancer and prior antibiotic use were frequent and Candida albicans was the most prevalent species found (42.4%). Resistance to fluconazole was found in 2.47% of the strains. No differences were observed in the species distribution of Candida during the study periods. In the P2 cohort, there were higher prevalence of elderly individuals, cardiac, pulmonary and liver diseases, renal failure, central venous catheters and antibiotic therapy. In P1, there were higher prevalence of neurological diseases and chemotherapy. The crude mortality was 55.4%. In conclusion, our incidence rates remained high. Furthermore, the distribution pattern of Candida species and the fluconazole resistance profile remained unchanged. Moreover, we found a clear trend of higher prevalence of candidaemia among the elderly and among patients with comorbidities. Finally, it is necessary to discuss strategies for the prevention and control of Candida BSI in Brazil.

Over the last decades, Candida spp have been responsible for an increasing number of infections, especially in patients requiring intensive care. Knowledge of local epidemiology and analysis of the spread of these pathogens is important in understanding and controlling their transmission. The aim of this study was to evaluate the genetic diversity of 31 Candida albicans and 17 Candida glabrata isolates recovered from intensive care unit patients from the tertiary hospital in Krakow between 2011-2012. The strains were typed by random amplified polymorphic DNA (RAPD) polymerase chain reaction using five primers (CD16AS, HP1247, ERIC-2, OPE-3 and OPE-18). The results of the present investigation revealed a high degree of genetic diversity among the isolates. No clonal relationship was found among the C. albicans strains, whereas two C. glabrata isolates were identical. The source of Candida infection appeared to be mostly endogenous; however, the presence of two clonal C. glabrata strains suggested the possibility of cross-transmission of these pathogens. Our study confirmed the high discriminatory power of the RAPD technique in the molecular typing of Candida clinical isolates. This method may be applied to the evaluation of transmission routes of pathogenic fungi on a local level.

Abstract Solitary pulmonary nodule corresponds to a common radiographic finding, which is frequently detected incidentally. The investigation of this entity remains complex, since characteristics of benign and malignant processes overlap in the differential diagnosis. Currently, many strategies are available to evaluate solitary pulmonary nodules with the main objective of characterizing benign lesions as best as possible, while avoiding to expose patients to the risks inherent to invasive methods, besides correctly detecting cases of lung cancer so as the potential curative treatment is not delayed. This first part of the study focuses on the epidemiology, the morfological evaluation and the methods to determine the likelihood of cancer in cases of indeterminate solitary pulmonary nodule.

Apple leaf spot (ALS) caused by Colletotrichum spp. is a major disease of apple (Malus domestica) in Southern Brazil. The epidemiology of this disease was studied in experiments carried out in the counties of Passo Fundo and Vacaria, State of Rio Grande do Sul, from February 1998 to October 2000. The disease was found in all the six apple orchards sampled in the growing seasons of 1997/98 and 1998/99. The fungus isolates associated with ALS fit the characteristics of C. gloeosporioides (75%), C. acutatum (8%), and Colletotrichum sp. (17%). The pathogen overwintered in dormant buds and twigs but not in dropped leaves or fruit mummies. Two sprays of copper oxychloride (at 0.3%) reduced the fungus initial inoculum by 65-84.6% in buds and 85.6-93.7% in twigs, but had no effect on the early season progress of the disease. Disease severity increased proportionally to elevation of temperature from 14 to 26-28 °C. At 34 °C, however, infection was completely inhibited. The duration of leaf wetness required for infection ranged from two hours at 30 °C to 32 h at 16 °C. The relationship of temperature (T) and leaf wetness (W) to disease severity (Y) was represented by the model equation Y = 0.00145[((T-13)1.78)((34.01-T )1.09)] * 25/[1+14 exp(-0.137W)], R² = 0.73 and P < 0.0001. Currently, this information is being used to manage the disease and to validate a forecast system for ALS.

The etiology and epidemiology of Pythium root rot in hydroponically-grown crops are reviewed with emphasis on knowledge and concepts considered important for managing the disease in commercial greenhouses. Pythium root rot continually threatens the productivity of numerous kinds of crops in hydroponic systems around the world including cucumber, tomato, sweet pepper, spinach, lettuce, nasturtium, arugula, rose, and chrysanthemum. Principal causal agents include Pythium aphanidermatum, Pythium dissotocum, members of Pythium group F, and Pythium ultimum var. ultimum. Perspectives are given of sources of initial inoculum of Pythium spp. in hydroponic systems, of infection and colonization of roots by the pathogens, symptom development and inoculum production in host roots, and inoculum dispersal in nutrient solutions. Recent findings that a specific elicitor produced by P. aphanidermatum may trigger necrosis (browning) of the roots and the transition from biotrophic to necrotrophic infection are considered. Effects on root rot epidemics of host factors (disease susceptibility, phenological growth stage, root exudates and phenolic substances), the root environment (rooting media, concentrations of dissolved oxygen and phenolic substances in the nutrient solution, microbial communities and temperature) and human interferences (cropping practices and control measures) are reviewed. Recent findings on predisposition of roots to Pythium attack by environmental stress factors are highlighted. The commonly minor impact on epidemics of measures to disinfest nutrient solution as it recirculates outside the crop is contrasted with the impact of treatments that suppress Pythium in the roots and root zone of the crop. New discoveries that infection of roots by P. aphanidermatum markedly slows the increase in leaf area and whole-plant carbon gain without significant effect on the efficiency of photosynthesis per unit area of leaf are noted. The platform of knowledge and understanding of the etiology and epidemiology of root rot, and its effects on the physiology of the whole plant, are discussed in relation to new research directions and development of better practices to manage the disease in hydroponic crops. Focus is on methods and technologies for tracking Pythium and root rot, and on developing, integrating, and optimizing treatments to suppress the pathogen in the root zone and progress of root rot.

OBJETIVO: Relatar as características demográficas, clínicas, diagnóstica e terapêutica de pacientes com abscesso esplênico (AE) causado por Salmonella. MÉTODO: Análise retrospectiva de dados de pacientes atendidos no Serviço de Cirurgia Geral e Aparelho Digestivo do Hospital Universitário Gaffrée-Guinle no período de janeiro de 2001 a dezembro de 2005, a estes se incluiu um caso tratado em outro hospital em época anterior. RESULTADOS: Dentre 4823 pacientes recentemente atendidos, dois apresentaram AE causado por Salmonella enteritidis, enquanto o caso mais antigo o agente responsável foi a Salmonella typhi. Todos eram homens, com idade média de 45 anos. Em nenhum deles foi identificada condição predisponente à formação do abscesso; os exames de imagem foram capazes de diagnosticar o AE. Todos foram tratados por esplenectomia e antibioticoterapia e evoluíram para cura. CONCLUSÕES: A Salmonella, apesar de infrequente, pode ser o agente causal do AE. Caracteristicamente os abscessos eram grandes e apresentavam material necrótico em seu interior. Nesta condição, a esplenectomia associada a antibioticoterapia mostrou-se eficaz no tratamento.

Foram caracterizadas antigenicamente amostras de Salmonella isoladas de aves (portadoras e doentes) provenientes de diversas regiões do país durante o período de 1962 a 1991. Nas 2123 culturas analisadas foram reconhecidos 90 sorovares, distribuídos em 14 sorogrupos com predominância dos grupos O:9 (40,0%), O:4 (33,3%), O:7 (10,6%) e O: 3,10 (6,7%). A maior diversidade de sorovares foi reconhecida no sorogrupo O:7 com 22 tipos distintos, secundado por O:4, O:3,10 e O:9, constituídos de 19, 15 e 10 sorotipos, respectivamente. No computo geral, foi determinada a média de 10,8 sorovares isolados por ano. Os sorovares classificados como muito frequentes nos três decênios, representando 65 a 67%, dos isolamentos, foram S. Gallinarum, S. Pullorum, S. Typhimurium, S. Heidelberg, S. Enteritidis e S. Infantis. Considerações de natureza bacteriológica e epidemiológica foram discutidas em relação a alguns dos sorotipos prevalentes.

Foram caracterizadas antigenicamente amostras de Salmonella isoladas de matérias-primas e de ração para aves em 1976 e durante doze anos consecutivos (1979-1991). As 2293 culturas analisadas provieram de sete regiões distintas do país e possibilitaram o reconhecimento de 151 sorovares, classificados bioquimicamente nas subespécies I (99,6%) IIIa (0,33%) e IV (0,04%), respectivamente. Os sorovares identificados se distribuiram por 17 sorogrupos, com predominância de O:7 (30,4%), O:4 (24,5%), O:3,10 (19,1%), O:13 (7,8%), O:1,3,19 (4,9%) e O:18 (3,7), que representam 90% dos grupos sorológicos caracterizados e constituídos de 103 (68,2%) sorotipos. Dentre os dez sorovares mais frequentemente reconhecidos citam-se S. Montevideo, S. Senftenberg, S. Havana, S. Mbandaka, S. Tennessee, S. Infantis, S. Agona, S. Anatum, S. Cerro e S. Bredeney. Alguns aspectos de caráter epidemiológico foram discutidos, envolvendo particularmente, determinados sorotipos e inclusive confrontando-se os resultados obtidos com aqueles oriundos de investigação conexa em aves.

Um total de 150 carcaças de frango congeladas, de quatro marcas comerciais, foram analisadas para pesquisa de Salmonella. Foram examinadas 43 carcaças de cada uma das marcas A, B, D e 21 da marca C. Observou-se um percentual de 32,0% de contaminação. Foram identificados 11 sorotipos: S. Agona, S. Anatum, S. Enteritidis, S. Hadar, S. Havana, S. Mbandaka, S. Montevideo, S. Ouakam, S. Poona, S. Schwarzengrund e S. I 4, 5, 12: -. O antibiograma das cepas mostrou 100% de resistência à ampicilina, 75,0% à cefalotina, 52,1% à cefoxitina, 22,9% à tobramicina, 6,2% à polimixina B e à tetraciclina, 4,2% à gentamicina e 2,1% à netilmicina, ao aztreonam e à amicacina. Todas as cepas apresentaram sensibilidade total ao cloranfenicol e ao sulfazotrim.

Nas décadas de 60 e 70, houve um extraordinário incremento da exportação de produtos cárneos de eqüídeos dos países da América do Sul para a Europa e Japão. Este acontecimento favoreceu o aumento de risco da veiculação de Salmonella através desses produtos, para as populações humana e animal, consumidoras. Assim, num estabelecimento industrial e exportador de carne de eqüídeos localizado no nordeste do Brasil (Pernambuco), foram analisados bacteriologicamente, 19.238 fragmentos de músculos mais externos, que revelaram 666 exames positivos referentes a 433 animais (eqüinos e asininos) e resultando no isolamento de 745 cepas de Salmonella. Na amostragem foram caracterizados do ponto de vista antigênico 98 sorovares, predominantemente classificados na subespécie I (98,9%) e tendo como os mais freqüentes S. Anatum, S. Carrau, S. Saintpaul, S. Agona e S. Typhimurium. Pelas análises efetuadas admite-se que as causas primordiais da presença de Salmonella nas carnes, provavelmente decorreu do contato com os excretas dos animais abatidos, bem como pela possível contaminação ambiental resultante, tendo em vista a ausência de portadores humanos, pesquisados numa parcela do pessoal.

Foram colhidas amostras de fezes de lote de suínos em 10 granjas terminadoras do Estado do Rio Grande do Sul para pesquisa de Salmonella sp. Duas granjas consideradas negativas (N1 e N2) e uma positiva (P1) nesta primeira etapa foram escolhidas para colheita de amostras de fezes individuais de 25 animais escolhidos aleatoriamente. No abatedouro foram coletados swab retal, conteúdo intestinal e linfonodos mesentéricos dos mesmos animais amostrados na granja. Após a introdução de novos animais nas granjas N1 e P1, outros 25 animais foram amostrados em cada granja, da mesma forma descrita acima. Três granjas tiveram amostras de fezes de lote positivas, sendo que em duas foi constatado um alto nível de contaminação. Foram encontrados 8 sorotipos de Salmonella (Agona, Bredeney, Lexington, London, Mbandaka, Panama, Schwartzengrund, Salmonella sp), sendo os sorotipos Agona e Bredeney os mais encontrados. Na colheita individual realizada, todas as granjas amostradas foram positivas. Em 6,4% das amostras de fezes colhidas na granja, 5,3% das amostras de conteúdo intestinal e 5,6% dos linfonodos mesentéricos foi possível isolar Salmonella. O antibiograma das linhagens de Salmonella isoladas demonstrou 97,8% de resistência à sulfonamida, 82,6% à estreptomicina, 36,9% à tetraciclina e 15,2% à sulfazotrim.

Salmonella serovars isolated from swine are of particular interest not only because of the pathogenic potential for this animal species, but also due to its relevance with regard to public health. On basis of the profile of resistance to antimicrobials, 13 Salmonella strains were selected which belonged to the serovars Muenster (7), Derby (4), Typhimurium (1), and Braenderup (1). They were isolated from healthy swine as well as from the abattoir environment in the state of Rio de Janeiro. All strains of Salmonella were subjected to bacterial conjugation, and the E. coli K12 Nal r Lac+ F standard strain was used as receptor, with the purpose to verify the ability to transfer the resistance marks. Gene transfer phenomenon was detected in seven strains, and except SalmonellaTyphimurium which transconjugated to Sm, Tc and Su, the remaining ones were characterized by transferring mark Su only. By plasmidial analysis of strains used and their respective transconjugants, 63 Kb plasmid was found, which was probably related to S. Typhimurium resistance.
