The clinical course of leprosy is often interrupted by reactions, which are acute inflammatory episodes that can be classified as type I or type II. Type II reactions can present as cutaneous lesions that resemble erythema multiforme (EM). EM is classically associated with drug allergies or pre-existing viral infections. However, the differential diagnostic criteria of the diverse causative agents remain controversial. The aim of this study was to determine both the clinical relevance and the morphological and immunohistochemical characteristics of the EM-like lesions during the course of type II leprosy reactions. Twenty-seven skin biopsies were taken from typical EM-like lesions of multibacillary patients and reviewed; their histological features were correlated to their clinical aspects. Then, a computer-assisted morphometric analysis was performed to measure the extent of angiogenesis during these acute episodes. The histopathological and immunohistochemical analysis of the EM lesions revealed that they shared the same features that have been previously described for ENL, including immunopositivity in the identical cell-mediated immune markers. Our results point to leprosy as the cause of the EM-like lesions in our patients. Therefore, leprosy should be considered in the differential diagnosis of EM.

Contact surveillance is an important strategy to ensure effective early diagnosis and control of leprosy; passive detection may not be as efficient because it is directly tied to the ready availability of heath care services and health education campaigns. The aim of this study was to reinforce that contact surveillance is the most effective strategy for the control of leprosy. The analysed data were obtained from a cohort of contacts and cases diagnosed through a national referral service for leprosy. We analysed data from patients diagnosed between 1987-2010 at the Souza Araújo Ambulatory in Rio de Janeiro. Epidemiological characteristics of leprosy cases diagnosed through contact surveillance and characteristics of passively detected index cases were compared using a conditional logistic regression model. Cases diagnosed by contact surveillance were found earlier in the progression of the disease, resulting in less severe clinical presentations, lower levels of initial and final disability grades, lower initial and final bacterial indices and a lower prevalence of disease reaction. In this respect, contact surveillance proved to be an effective tertiary prevention strategy, indicating that active surveillance is especially important in areas of high endemicity, such as Brazil.

Leptospirosis in humans usually involves hypokalaemia and hypomagnesaemia and the putative mechanism underlying such ionic imbalances may be related to nitric oxide (NO) production. We previously demonstrated the correlation between serum levels of NO and the severity of renal disease in patients with severe leptospirosis. Methylene blue inhibits soluble guanylyl cyclase (downstream of the action of any NO synthase isoforms) and was recently reported to have beneficial effects on clinical and experimental sepsis. We investigated the occurrence of serum ionic changes in experimental leptospirosis at various time points (4, 8, 16 and 28 days) in a hamster model. We also determined the effect of methylene blue treatment when administered as an adjuvant therapy, combined with late initiation of standard antibiotic (ampicillin) treatment. Hypokalaemia was not reproduced in this model: all of the groups developed increased levels of serum potassium (K). Furthermore, hypermagnesaemia, rather than magnesium (Mg) depletion, was observed in this hamster model of acute infection. These findings may be associated with an accelerated progression to acute renal failure. Adjuvant treatment with methylene blue had no effect on survival or serum Mg and K levels during acute-phase leptospirosis in hamsters.

It has been reported that patients with progressive tuberculosis (TB) express abundant amounts of the antimicrobial peptides (AMPs) cathelicidin (LL-37) and human neutrophil peptide-1 (HNP-1) in circulating cells, whereas latent TB infected donors showed no differences when compared with purified protein derivative (PPD) and QuantiFERON®-TB Gold (QFT)-healthy individuals. The aim of this study was to determine whether LL-37 and HNP-1 production correlates with higher tuberculin skin test (TST) and QFT values in TB household contacts. Twenty-six TB household contact individuals between 26-58 years old TST and QFT positive with at last two years of latent TB infection were recruited. AMPs production by polymorphonuclear cells was determined by flow cytometry and correlation between TST and QFT values was analysed. Our results showed that there is a positive correlation between levels of HNP-1 and LL-37 production with reactivity to TST and/or QFT levels. This preliminary study suggests the potential use of the expression levels of these peptides as biomarkers for progression in latent infected individuals.

An analysis of the dietary content of haematophagous insects can provide important information about the transmission networks of certain zoonoses. The present study evaluated the potential of polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) analysis of the mitochondrial cytochrome B (cytb) gene to differentiate between vertebrate species that were identified as possible sources of sandfly meals. The complete cytb gene sequences of 11 vertebrate species available in the National Center for Biotechnology Information database were digested with Aci I, Alu I, Hae III and Rsa I restriction enzymes in silico using Restriction Mapper software. The cytb gene fragment (358 bp) was amplified from tissue samples of vertebrate species and the dietary contents of sandflies and digested with restriction enzymes. Vertebrate species presented a restriction fragment profile that differed from that of other species, with the exception of Canis familiaris and Cerdocyon thous. The 358 bp fragment was identified in 76 sandflies. Of these, 10 were evaluated using the restriction enzymes and the food sources were predicted for four: Homo sapiens (1), Bos taurus (1) and Equus caballus (2). Thus, the PCR-RFLP technique could be a potential method for identifying the food sources of arthropods. However, some points must be clarified regarding the applicability of the method, such as the extent of DNA degradation through intestinal digestion, the potential for multiple sources of blood meals and the need for greater knowledge regarding intraspecific variations in mtDNA.

In 2004, Aedes (Stegomyia) albopictus (Skuse, 1894) was observed for the first time in Catalonia, northeastern Spain. A decade later, it has spread throughout the eastern Mediterranean region of the country and the Balearic Islands. Framed within a national surveillance project, we present the results of monitoring in 2013 in the autonomous communities of the mainland Levante. The current study reveals a remarkable increase in the spread of the invasive mosquito in relation to results from 2012; the species was present and well-established in 48 municipalities, most of which were along the Mediterranean coastline from the Valencian Community to the Region of Murcia.

The diagnosis of single-lesion paucibacillary leprosy remains a challenge. Reviews by expert dermatopathologists and quantitative polymerase chain reaction (qPCR) results obtained from 66 single-plaque biopsy samples were compared. Histological findings were graded as high (HP), medium (MP) or low (LP) probability of leprosy or other dermatopathy (OD). Mycobacterium leprae-specific genes were detected using qPCR. The biopsies of 47 out of 57 clinically diagnosed patients who received multidrug therapy were classified as HP/MP, eight of which were qPCR negative. In the LP/OD (n = 19), two out of eight untreated patients showed positive qPCR results. In the absence of typical histopathological features, qPCR may be utilised to aid in final patient diagnosis, thus reducing overtreatment and delay in diagnosis.

RESUMO No atual contexto de transformações no campo da assistência à saúde mental no Brasil, a Universidade tem a responsabilidade de redirecionar a qualificação profissional da força de trabalho em saúde. Este artigo versa sobre a experiência de um Programa de Integração Docente-Assistencial envolvendo a Secretaria de Estado de Saúde e a Universidade de São Paulo. Particulariza a experiência de estudantes de enfermagem cursando a disciplina de enfermagem psiquiátrica. A análise da avaliação dos alunos do programa teórico-prático desenvolvido na unidade, ancorado nos pressupostos da Reabilitação Psicossocial indica, ao mesmo tempo, que o modelo de atenção interfere diretamente na qualificação das práticas e, simultaneamente, desenvolve nos alunos atitudes e conhecimentos que os qualificam para desempenhar de forma coerente suas funções.

Buscou-se reconstruir a história do ensino de enfermagem no Brasil desde a criação da Escola de Enfermagem Anna Nery, em 1922, através de seus programas de ensino, de 1923, 1926 e 1949, e dos currículos mínimos para o ensino de enfermagem no país, de 1962, 1972 e 1994. Apesar da enfermagem moderna no Brasil ter sido instituída para formar enfermeiras para atuar em saúde pública, desde o início a formação foi centrada no espaço hospitalar e voltada para o estudo sistemático das doenças, sem priorizar as questões vinculadas à saúde pública. Mesmo o currículo mínimo de 1994, construído coletivamente a partir de uma proposta contra-hegemônica, preservou a subdivisão em especialidades médicas, própria do modelo flexneriano. Verifica-se que continua presente o modelo biomédico, individualizado e hospitalocêntrico que marcou o ensino de enfermagem desde as suas origens no Brasil, e que a formação não está voltada para as necessidades de saúde da população, sendo portanto antagônica aos pressupostos da Saúde Coletiva.

A participação paterna na gravidez de adolescentes ainda é pouco discutida, embora a ausência do companheiro seja apontada como um dos problemas sociais dela decorrente. Esta investigação, de cunho qualitativo, teve por objetivo descrever como se dá a participação do pai do bebê durante a gestação, nacimento e criação do filho nos primeiros meses de vida, na perspectiva de adolescentes que passaram pela experiência de engravidar e ter um filho. Na sistematização dos dados, os mesmos foram agrupados em duas categorias: vivenciando a gravidez e valorizando a presença e participação do companheiro/pai.

Considerando a freqüente queixa dos idosos frente ao seu desempenho mnemônico, este estudo tem por objetivo investigar o efeito da estimulação da memória sobre o desempenho no Mini-Exame do Estado Mental (MEM), e observar correlação sócio-demográfica com o desempenho cognitivo. O MEM foi aplicado em 46 idosos integrantes de uma oficina de memória, sem diagnóstico médico de demência, antes e após a execução das atividades da mesma. Observou-se correlação e significância estatística entre os escores dos MEMs aplicados antes e após a oficina, porém não houve associação significante estatisticamente entre esse desempeho e as variáveis sócio-demográficas.

Trata-se da interdisciplinaridade como fenômeno e assunto emergente da ciência (re)partida em várias especialidades ou disciplinas do saber/conhecimento e da formação profissional, principalmente na área da saúde e da enfermagem. A abordagem é de natureza crítica e filosófica e o propósito principal visa a expor dificuldades e problemas com intuito de favorecer e ampliar a discussão. Conceitos gerais, mais correntes, são apresentados de permeio com idéias epistemológicas subjacentes às repercussões do trabalho em grupo e transcendentes à consciência para além da filosofia do sujeito. A autora advoga sua opinião e postura em face do tema com exemplos de sua prática acadêmica e experiências da Escola de Enfermagem Anna Nery/UFRJ.

Estudo histórico-social sobre a emergência das profissões de nutricionista e de assistente social, entre os anos 30 e meados do século 20. O trabalho trata das circunstâncias do surgimento dos cursos de nutrição e de serviço social no interior da Escola Anna Nery/UFRJ, e compara as funções desempenhadas por enfermeiras, nutricionistas e assistentes sociais à época. As fontes primárias de pesquisa encontram-se no Centro de Documentação da Escola de Enfermagem Anna Nery/UFRJ e incluem documentos escritos e depoimento oral. As fontes secundárias foram artigos, livros e teses. A análise de textos e documentos evidenciou que a Escola teve papel decisivo na emergência dessas novas profissões, que vieram contribuir para uma melhor organização e funcionamento dos serviços de saúde e para a prestação de uma assistência mais completa à clientela. Ao mesmo tempo, sua característica feminina veio, ainda, favorecer a inserção de mulheres no mercado de trabalho qualificado na área da saúde.

Idosos podem manifestar sintomas de estresse em decorrência das mudanças biopsicossociais do envelhecimento. O objetivo deste estudo foi identificar sintomas de estresse em idosos e o estilo de coping utilizado por eles, verificando a relação entre estas variáveis. Foram aplicados a Lista de Sintomas de Stress (LSS) e o Inventário de Coping de Jalowiec, em 41 idosos saudáveis. Os idosos apresentaram sintomas de estresse, com intensidade média de 42.8 pontos. Observou-se predomínio do coping focado no problema, porém não houve diferença significativa entre as médias dos escores do LSS entre os indivíduos que utilizaram coping focado no problema ou na emoção (p=0.737). Neste estudo, embora os idosos tendessem a eleger o coping focado no problema, a intensidade dos sintomas de estresse independeu do tipo de coping utilizado, evidenciando que tanto o coping focado no problema como o focado na emoção estão associados a níveis semelhantes de estresse.

The objective of this study was to identify vulnerability to tuberculosis (TB) related to knowledge about the disease among 76 nursing students and professionals. A quantitative descriptive study was conducted using a closed questionnaire for the collection of data regarding transmission, preventive and biosafety measures, diagnosis, and prejudice regarding the disease. The SAS software version 9.1.3 was used for data analysis, with the level of significance set at 5% (p < 0.05). Nursing students and professionals showed a vulnerability to TB related to knowledge about transmission, preventive and biosafety measures, and diagnosis of the disease. With respect to transmission, vulnerability was higher among nursing professionals. The results indicate the need for investment by healthcare institutions surrounding this topic in view of the important role of nursing in the establishment of strategies for prevention and control of the disease.
