Dengue is a mosquito-borne viral infection that in recent decades has become a major international public health concern. Epidemic dengue fever reemerged in Brazil in 1981. Since 1990 more than one dengue virus serotype has been circulating in this tropical country and increasing rates of dengue hemorrhagic fever and dengue shock syndrome have been detected every year. Some evidence supports the association between the introduction of a new serotype and/or genotype in a region and the appearance of dengue hemorrhagic fever. In order to study the evolutionary relationships and possible detection of the introduction of new dengue virus genotypes in Brazil in the last years, we analyzed partial nucleotide sequences of 52 Brazilian samples of both dengue type 1 and dengue type 2 isolated from 1988 to 2001 from highly endemic regions. A 240-nucleotide-long sequence from the envelope/nonstructural protein 1 gene junction was used for phylogenetic analysis. After comparing the nucleotide sequences originally obtained in this study to those previously studied by others, and analyzing the phylogenetic trees, we conclude that, after the initial introduction of the currently circulating dengue-1 and dengue-2 genotypes in Brazil, there has been no evidence of introduction of new genotypes since 1988. The increasing number of dengue hemorrhagic fever cases seen in Brazil in the last years is probably associated with secondary infections or with the introduction of new serotypes but not with the introduction of new genotypes.

Whole blood samples (N = 295) were obtained from different locations in Amazonas and Sucre States, in Venezuela. Malaria was diagnosed by microscopy, OptiMAL™ and polymerase chain reaction (PCR), with Plasmodium vivax, P. falciparum, and P. malariae being detected when possible. We identified 93 infections, 66 of which were caused by P. vivax, 26 by P. falciparum, and 1 was a mixed infection. No infection caused by P. malariae was detected. The sensitivity and specificity of each diagnostic method were high: 95.7 and 97.9% for microscopy, 87.0 and 97.9% for OptiMAL, and 98.0 and 100% for PCR, respectively. Most samples (72.2%) showed more than 5000 parasites/µL blood. The sensitivity of the diagnosis by microscopy and OptiMAL decreased with lower parasitemia. All samples showing disagreement among the methods were reevaluated, but the first result was used for the calculations. Parasites were detected in the 6 false-negative samples by microscopy after the second examination. The mixed infection was only detected by PCR, while the other methods diagnosed it as P. falciparum (microscopy) or P. vivax (OptiMAL) infection. Most of the false results obtained with the OptiMAL strip were related to the P. falciparum-specific band, including 3 species misdiagnoses, which could be related to the test itself or to genetic variation of the Venezuelan strains. The use of the microscopic method for malaria detection is recommended for its low cost but is very difficult to implement in large scale, population-based studies; thus, we report here more efficient methods suitable for this purpose.

Angiotensin II (Ang II) plays a crucial role in the pathogenesis of renal diseases. The objective of the present study was to investigate the possible inflammatory effect of Ang II on glomerular endothelial cells and the underlying mechanism. We isolated and characterized primary cultures of rat glomerular endothelial cells (GECs) and observed that Ang II induced the synthesis of monocyte chemoattractant protein-1 (MCP-1) in GECs as demonstrated by Western blot. Ang II stimulation, at concentrations ranging from 0.1 to 10 µm, of rat GECs induced a rapid increase in the generation of reactive oxygen species as indicated by laser fluoroscopy. The level of p47phox protein, an NAD(P)H oxidase subunit, was also increased by Ang II treatment. These effects of Ang II on GECs were all reduced by diphenyleneiodonium (1.0 µm), an NAD(P)H oxidase inhibitor. Ang II stimulation also promoted the activation of nuclear factor-kappa B (NF-κB). Telmisartan (1.0 µm), an AT1 receptor blocker, blocked all the effects of Ang II on rat GECs. These data suggest that the inhibition of NAD(P)H oxidase-dependent NF-κB signaling reduces the increase in MCP-1 production by GECs induced by Ang II. This may provide a mechanistic basis for the benefits of selective AT1 blockade in dealing with chronic renal disease.

Oxidative low-density lipoprotein (Ox-LDL) is a key risk factor for the development of atherosclerosis, and it can stimulate the expression of a variety of inflammatory signals. As a new and highly sensitive inflammation index, OX40L may be a key to understanding the mechanisms that regulate interactions between cells within the vessel wall and inflammatory mediators during the development of atherosclerosis. To investigate whether Ox-LDL regulates OX40L expression through an oxidized LDL-1 receptor (LOX-1)-mediated mechanism, we investigated the effect of different concentrations of Ox-LDL (50, 100, 150 µg/mL) on endothelial cell proliferation and apoptosis. Stimulation with Ox-LDL increased OX40L protein 1.44-fold and mRNA 4.0-fold in endothelial cells, and these effects were inhibited by blocking LOX-1. These results indicate that LOX-1 plays an important role in the chronic inflammatory process in blood vessel walls. Inhibiting LOX-1 may reduce blood vessel inflammation and provide a therapeutic option to limit atherosclerosis progression.

In the current literature, there is evidence that psychological factors can affect the incidence and progression of some cancers. Interleukin 6 (IL-6) is known to be elevated in individuals experiencing chronic stress and is also involved in oncogenesis and cancer progression. However, the precise mechanism of IL-6 induction by the stress-related hormone norepinephrine (NE) is not clear, and, furthermore, there are no reports about the effect of NE on IL-6 expression in gastric epithelial cells. In this study, we examined the effect of NE on IL-6 expression in immortalized human gastric epithelial cells (GES-1 cells). Using real-time PCR and enzyme-linked immunoassay, we demonstrated that NE can induce IL-6 mRNA and protein expression in GES-1 cells. The induction is through the β-adrenergic receptor-cAMP-protein kinase A pathway and mainly at the transcriptional level. Progressive 5′-deletions and site-directed mutagenesis of the parental construct show that, although activating-protein-1 (AP-1), cAMP-responsive element binding protein (CREB), CCAAT-enhancer binding protein-β (C/EBP-β), and nuclear factor κ-light-chain-enhancer of activated B cells (NF-κB) binding sites are all required in the basal transcription of IL-6, only AP-1 and CREB binding sites in the IL-6 promoter are required in NE-induced IL-6 expression. The results suggest that chronic stress may increase IL-6 secretion of human gastric epithelial cells, at least in part, by the stress-associated hormone norepinephrine, and provides basic data on stress and gastric cancer progression.

Relata-se surto de malária por Plasmodium vivax entre usuários de drogas injetáveis, detectado na cidade de Bauru, Estado de São Paulo, Brasil. Até julho de 1989 12 casos haviam sido confirmados pelo exame de gota espessa, e 20 outros contactantes suspeitos estavam sendo investigados. Todos os casos relataram uso freqüente de cocaína injetável, compartilhando seringas e agulhas, e negaram viagens por áreas endêmicas de malária.

No início de 1993 foi detectado um foco de malária no Parque Estadual da Serra do Mar, área de Mata Altântica no Estado de São Paulo (Brasil). Quando da realização da pesquisa entomológica ocorreu a infecção malárica por Plasmodium vivax em três capturadores da equipe de campo. A transmissão ocorreu em área que vem apresentando baixos níveis de incidência de malária, e os capturadores tiveram um período de exposição extremamente reduzido, sendo de apenas três horas em um dos casos.

The importance of hydroelectric dams beside the human interchange in the maintenance of malarious foci and the occurrence of the Anopheles genus on the Binational Itaipu Reservoir were the main points of this retrospective study. Data were collected from existing registrations at National, State and Municipal Health Departments and literature systematic overview, from January 1984 to December 2003. The occurrence of some outbreak of malaria, mainly by Plasmodium vivax, and the prevalence of species of the Anopheles genus different from Anopheles darlingi in the region are discussed. The malaria in the left bank of Paraná River is a focal problem, which must be approached locally through health, educational and social actions to prevent the continuity of outbreaks in the area. Concomitantly, it is necessary to plan and apply effective surveillance measures in the influence area of the Itaipu Reservoir.

Malaria is an unusual complication after hematopoietic stem cell transplantation in non-endemic countries. However, transplant candidates, recipients and donors living in endemic regions frequently report previous episodes of malaria. This fact could represent an important risk for immunosuppressed recipients that could develop severe malaria cases. We report a case of hematopoietic stem cell transplant (HSCT) in which the donor had a history of previous malaria, and close monitoring was performed before and after procedure by parasitological and molecular tests. The donor presented Plasmodium vivax in thick blood smears one month after transplant and was treated according to Brazilian Health Ministry guidelines. The polymerase chain reaction (PCR) was able to detect malaria infection in the donor one week earlier than thick blood film. Even without positive results, the recipient was pre-emptively treated with chloroquine in order to prevent the disease. We highlight the importance of monitoring recipients and donors in transplant procedures with the aim of reducing the risk of malaria transmission.

Anopheles (Kerteszia) cruzii has been implicated as the primary vector of human and simian malarias out of the Brazilian Amazon and specifically in the Atlantic Forest regions. The presence of asymptomatic human cases, parasite-positive wild monkeys and the similarity between the parasites infecting them support the discussion whether these infections can be considered as a zoonosis. Although many aspects of the biology of An. cruzii have already been addressed, studies conducted during outbreaks of malaria transmission, aiming at the analysis of blood feeding and infectivity, are missing in the Atlantic Forest. This study was conducted in the location of Palestina, Juquitiba, where annually the majority of autochthonous human cases are notified in the Atlantic Forest of the state of São Paulo. Peridomiciliary sites were selected for collection of mosquitoes in a perimeter of up to 100 m around the residences of human malaria cases. The mosquitoes were analyzed with the purpose of molecular identification of blood-meal sources and to examine the prevalence of Plasmodium. A total of 13,441 females of An. (Ker.) cruzii were collected. The minimum infection rate was calculated at 0.03% and 0.01%, respectively, for P. vivax and P. malariae and only human blood was detected in the blood-fed mosquitoes analyzed. This data reinforce the hypothesis that asymptomatic human carriers are the main source of anopheline infection in the peridomiciliary area, making the probability of zoonotic transmission less likely to happen.

Procedeu-se uma revisão dos 58 casos de malária induzida diagnosticados em São Paulo, de janeiro de 1983 ajulho de 1990, correspondendo a 0,3% do total de casos de malária confirmados. As variáveis sexo, idade, nível de parasitemia, sintomatologia, evolução e tempo médio de início dos sintomas e o diagnóstico foram analisadas entre os 25 casos infectados por transfusão e 28 por uso de agulhas contaminadas. Predominou a infecção por Plasmodium vivax (79,4%), destacando-se 5,6% de casos com infecção mista. A idade média dos infectados por uso de agulhas foi significativamente menor (p 0,10). Apesar dos casos induzidos representarem percentual pequeno em relação ao total de casos notificados ea transmissão através do sangue estar sob controle, a transmissão acidental por agulhas contaminadas devido ao uso de drogas injetáveis, constitui risco parao restabelecimento da malária em áreas do Estado de São Paulo.

We have compared Duffy blood group genotype distribution, as determined by polymerase chain reaction with allele-specific primers, in 68 Plasmodium vivax-infected patients and 59 non-vivax malaria controls from Rondônia, Brazil. Homozygosity for the allele Fy, which abolishes Duffy antigen expression on erythrocytes, was observed in 12% non-vivax controls but in no P. vivax patient. However, no significant association was found between Fy heterozygosity and protection against P. vivax. The Fy x allele, which has recently been associated with very weak erythrocyte expression of Duffy antigen, was not found in local P. vivax patients.

Este estudo objetiva conhecer o perfil epidemiológico da malária no Estado de Santa Catarina, analisando dados disponibilizados pela Fundação Nacional de Saúde, relativos ao período de 1996/2001. Das 4.707 lâminas examinadas, 5,5% evidenciaram-se positivas. As infecções por Plasmodium vivax foram 69%, por Plasmodium falciparum 25,6%, infecções mistas por ambos foram 5% e, somente 0,4% por Plasmodium malariae. Foi observado 67,4% casos importados e 32,6% casos autóctones. Nos últimos anos houve um aumento de casos importados. A maioria destes veio da região Amazônica brasileira e o restante de países africanos. Identificou-se os municípios de Joinville, Blumenau, São Francisco do Sul e Florianópolis com maior número de autoctonia no biênio 1996/97. Medidas de controle e vigilância fazem-se necessárias, no sentido de prevenir a reintrodução do plasmódio, favorecendo a autoctonia. Será útil o mapeamento das áreas de risco, já que é contínua a expectativa de sua reemergência em áreas hoje consideradas sob controle.

O estudo foi realizado na localidade de Residencial Paraíso, São Luis, Maranhão, de 1999 a 2001, com o objetivo de analisar os aspectos epidemiológicos da malária, tendo sido constatado que a localidade é receptiva e vulnerável. Houve 129 casos, com maior acometimento do sexo masculino e da faixa etária de 20 a 29 anos. O Plasmodium vivax foi o agente responsável pelas infecções, tendo como vetor o Anopheles aquasalis. Foram identificadas evidências de transmissão extradomiciliar.

O estudo foi desenvolvido com o objetivo de caracterizar um foco de transmissão de malária em Guarapiranga, município de São José de Ribamar, MA. A unidade de estudo foi o grupo familiar, aplicando-se questionário sobre identificação, situação econômica, hábitos dos moradores e características dos domicílios. Foram visitadas 54 famílias, totalizando 207 habitantes, sendo 113 do sexo masculino e 95, do feminino. 95 pessoas tinham de 5 a 24 anos. Agricultura e pesca são a base da economia local, com 64,8% das famílias auferindo renda mensal menor que um salário mínimo. A maioria das casas é construída de taipa, coberta de palha, tem piso de chão batido e não tem instalações sanitárias. Em relação à entrada de mosquitos, 62,2% são parcialmente protegidas. A água para consumo provém de cacimbas ou poços rasos em 78,8%, e o lixo é deixado a céu aberto em 53,7% dos domicílios. Foram registrados 106 casos de malária por Plasmodium vivax. O provável transmissor foi o Anopheles (N) aquasalis e o tratamento com cloroquina associado à primaquina teve bons resultados.
