Background:Several studies have been attempting to ascertain the risks of Sleep Apnea Syndrome (SAS) and its morbidity and mortality.Objective:The main objective was to verify whether SAS increases the risk of death; the secondary objective was to evaluate its morbidity in relation to cardiovascular disease and the number of days hospitalized.Methods:A systematic review and a meta-analysis were performed of the published literature. The research focused on studies comparing the number of deaths in patients with untreated SAS and in patients with non-SAS.Results:The meta-analysis was based on 13 articles, corresponding to a total of 13394 participants divided into two groups (non-SAS = 6631; SAS = 6763). The meta-analysis revealed a clear association of SAS with the occurrence of fatal events, where the presence of SAS corresponded to a 61% higher risk of total mortality (OR=1.61; CI: 1.43 - 1.81; p < 0.00001), while the risk of death from cardiac causes was 2.52 times higher in these patients (OR = 2.52; IC: 1.80 - 3.52; p < 0.00001). Similar results were obtained for mortality from other causes (OR = 1.68; CI: 1.08 - 2.61; p = 0.02). Resembling results were obtained in the remaining outcomes: non-fatal cardiovascular events were higher in the SAS group (OR = 2.46; IC: 1.80 - 3.36; p < 0.00001), the average number of days hospitalized was also higher in the SAS group (IV = 18.09; IC: 13.34 - 22.84; p < 0.00001).Conclusion:The results show that untreated SAS significantly increases the risk of death, cardiovascular events and the average number of days hospitalized.

AbstractBackground:The prevalence and clinical outcomes of heart failure with preserved left ventricular ejection fraction after acute myocardial infarction have not been well elucidated.Objective:To analyze the prevalence of heart failure with preserved left ventricular ejection fraction in acute myocardial infarction and its association with mortality.Methods:Patients with acute myocardial infarction (n = 1,474) were prospectively included. Patients without heart failure (Killip score = 1), with heart failure with preserved left ventricular ejection fraction (Killip score > 1 and left ventricle ejection fraction ≥ 50%), and with systolic dysfunction (Killip score > 1 and left ventricle ejection fraction < 50%) on admission were compared. The association between systolic dysfunction with preserved left ventricular ejection fraction and in-hospital mortality was tested in adjusted models.Results:Among the patients included, 1,256 (85.2%) were admitted without heart failure (72% men, 67 ± 15 years), 78 (5.3%) with heart failure with preserved left ventricular ejection fraction (59% men, 76 ± 14 years), and 140 (9.5%) with systolic dysfunction (69% men, 76 ± 14 years), with mortality rates of 4.3%, 17.9%, and 27.1%, respectively (p < 0.001). Logistic regression (adjusted for sex, age, troponin, diabetes, and body mass index) demonstrated that heart failure with preserved left ventricular ejection fraction (OR 2.91; 95% CI 1.35211;6.27; p = 0.006) and systolic dysfunction (OR 5.38; 95% CI 3.10 to 9.32; p < 0.001) were associated with in-hospital mortality.Conclusion:One-third of patients with acute myocardial infarction admitted with heart failure had preserved left ventricular ejection fraction. Although this subgroup exhibited more favorable outcomes than those with systolic dysfunction, this condition presented a three-fold higher risk of death than the group without heart failure. Patients with acute myocardial infarction and heart failure with preserved left ventricular ejection fraction encounter elevated short-term risk and require special attention and monitoring during hospitalization.

AbstractBackground:Guidelines recommend that in suspected stable coronary artery disease (CAD), a clinical (non-invasive) evaluation should be performed before coronary angiography.Objective:We assessed the efficacy of patient selection for coronary angiography in suspected stable CAD.Methods:We prospectively selected consecutive patients without known CAD, referred to a high-volume tertiary center. Demographic characteristics, risk factors, symptoms and non-invasive test results were correlated to the presence of obstructive CAD. We estimated the CAD probability based on available clinical data and the incremental diagnostic value of previous non-invasive tests.Results:A total of 830 patients were included; median age was 61 years, 49.3% were males, 81% had hypertension and 35.5% were diabetics. Non-invasive tests were performed in 64.8% of the patients. At coronary angiography, 23.8% of the patients had obstructive CAD. The independent predictors for obstructive CAD were: male gender (odds ratio [OR], 3.95; confidence interval [CI] 95%, 2.70 - 5.77), age (OR for 5 years increment, 1.15; CI 95%, 1.06 - 1.26), diabetes (OR, 2.01; CI 95%, 1.40 - 2.90), dyslipidemia (OR, 2.02; CI 95%, 1.32 - 3.07), typical angina (OR, 2.92; CI 95%, 1.77 - 4.83) and previous non-invasive test (OR 1.54; CI 95% 1.05 - 2.27).Conclusions:In this study, less than a quarter of the patients referred for coronary angiography with suspected CAD had the diagnosis confirmed. A better clinical and non-invasive assessment is necessary, to improve the efficacy of patient selection for coronary angiography.

AbstractBackground:Galectin-3, a β-galactoside binding lectin, has been described as a mediator of cardiac fibrosis in experimental studies and as a risk factor associated with cardiovascular events in subjects with heart failure. Previous studies have evaluated the genetic susceptibility to Chagas disease in humans, including the polymorphisms of cytokine genes, demonstrating correlations between the genetic polymorphism and cardiomyopathy development in the chronic phase. However, the relationship between the galectin-3 single nucleotide polymorphism (SNP) and phenotypic variations in Chagas disease has not been evaluated.Objective:The present study aimed to determine whether genetic polymorphisms of galectin-3 may predispose to the development of cardiac forms of Chagas disease.Methods:Fifty-five subjects with Chagas disease were enrolled in this observational study. Real-time polymerase chain reaction (PCR) was used for genotyping the variants rs4644 and rs4652 of the galectin-3 gene.Results:For the SNP rs4644, the relative risk for the cardiac form was not associated with the genotypes AA (OR = 0.79, p = 0.759), AC (OR = 4.38, p = 0.058), or CC (OR = 0.39, p = 0.127). Similarly, for the SNP rs4652, no association was found between the genotypes AA (OR = 0.64, p = 0.571), AC (OR = 2.85, p = 0.105), or CC (OR = 0.49, p = 0.227) and the cardiac form of the disease.Conclusion:Our results showed no association between the different genotypes for both SNPs of the galectin-3 gene and the cardiac form of Chagas disease. AbstractBackground:The recording of arrhythmic events (AE) in renal transplant candidates (RTCs) undergoing dialysis is limited by conventional electrocardiography. However, continuous cardiac rhythm monitoring seems to be more appropriate due to automatic detection of arrhythmia, but this method has not been used.Objective:We aimed to investigate the incidence and predictors of AE in RTCs using an implantable loop recorder (ILR).Methods:A prospective observational study conducted from June 2009 to January 2011 included 100 consecutive ambulatory RTCs who underwent ILR and were followed-up for at least 1 year. Multivariate logistic regression was applied to define predictors of AE.Results:During a mean follow-up of 424 ± 127 days, AE could be detected in 98% of patients, and 92% had more than one type of arrhythmia, with most considered potentially not serious. Sustained atrial tachycardia and atrial fibrillation occurred in 7% and 13% of patients, respectively, and bradyarrhythmia and non-sustained or sustained ventricular tachycardia (VT) occurred in 25% and 57%, respectively. There were 18 deaths, of which 7 were sudden cardiac events: 3 bradyarrhythmias, 1 ventricular fibrillation, 1 myocardial infarction, and 2 undetermined. The presence of a long QTc (odds ratio [OR] = 7.28; 95% confidence interval [CI], 2.01211;26.35; p = 0.002), and the duration of the PR interval (OR = 1.05; 95% CI, 1.02&#8211;1.08; p < 0.001) were independently associated with bradyarrhythmias. Left ventricular dilatation (LVD) was independently associated with non-sustained VT (OR = 2.83; 95% CI, 1.01211;7.96; p = 0.041).Conclusions:In medium-term follow-up of RTCs, ILR helped detect a high incidence of AE, most of which did not have clinical relevance. The PR interval and presence of long QTc were predictive of bradyarrhythmias, whereas LVD was predictive of non-sustained VT.

Abstract Background: Acute coronary syndrome (ACS) is one of the main causes of morbidity and mortality in the modern world. A sedentary lifestyle, present in 85% of the Brazilian population, is considered a risk factor for the development of coronary artery disease. However, the correlation of a sedentary lifestyle with cardiovascular events (CVE) during hospitalization for ACS is not well established. Objective: To evaluate the association between physical activity level, assessed with the International Physical Activity Questionnaire (IPAQ), with in-hospital prognosis in patients with ACS. Methods: Observational, cross-sectional, and analytical study with 215 subjects with a diagnosis of ACS consecutively admitted to a referral hospital for cardiac patients between July 2009 and February 2011. All volunteers answered the short version of the IPAQ and were observed for the occurrence of CVE during hospitalization with a standardized assessment conducted by the researcher and corroborated by data from medical records. Results: The patients were admitted with diagnoses of unstable angina (34.4%), acute myocardial infarction (AMI) without ST elevation (41.4%), and AMI with ST elevation (24.2%). According to the level of physical activity, the patients were classified as non-active (56.3%) and active (43.7%). A CVE occurred in 35.3% of the cohort. The occurrence of in-hospital complications was associated with the length of hospital stay (odds ratio [OR] = 1.15) and physical inactivity (OR = 2.54), and was independent of age, systolic blood pressure, and prior congestive heart failure. Conclusion: A physically active lifestyle reduces the risk of CVE during hospitalization in patients with ACS.

Abstract Background: Blood pressure is directly related to body mass index, and individuals with increased waist circumference have higher risk of developing hypertension, insulin resistance, and other metabolic changes, since adolescence. Objective: to evaluate the correlation of blood pressure with insulin resistance, waist circumference and body mass index in adolescents. Methods: Cross-section study on a representative sample of adolescent students. One group of adolescents with altered blood pressure detected by casual blood pressure and/or home blood pressure monitoring (blood pressure > 90th percentile) and one group of normotensive adolescents were studied. Body mass index, waist circumference were measured, and fasting glucose and plasma insulin levels were determined, using the HOMA-IR index to identify insulin resistance. Results: A total of 162 adolescents (35 with normal blood pressure and 127 with altered blood pressure) were studied; 61% (n = 99) of them were boys and the mean age was 14.9 ± 1.62 years. Thirty-eight (23.5%) adolescents had altered HOMA-IR. The group with altered blood pressure had higher values of waist circumference, body mass index and HOMA-IR (p<0.05). Waist circumference was higher among boys in both groups (p<0.05) and girls with altered blood pressure had higher HOMA-IR than boys (p<0.05). There was a significant moderate correlation between body mass index and HOMA-IR in the group with altered blood pressure (ρ = 0.394; p < 0.001), and such correlation was stronger than in the normotensive group. There was also a significant moderate correlation between waist circumference and HOMA-IR in both groups (ρ = 0.345; p < 0.05). Logistic regression showed that HOMA-IR was as predictor of altered blood pressure (odds ratio - OR = 2.0; p = 0.001). Conclusion: There was a significant association of insulin resistance with blood pressure and the impact of insulin resistance on blood pressure since childhood. The correlation and association between markers of cardiovascular diseases was more pronounced in adolescents with altered blood pressure, suggesting that primary prevention strategies for cardiovascular risk factors should be early implemented in childhood and adolescence.

Abstract Background: Arterial hypertension is a major public health problem and has increased considerably in young individuals in past years. Thus, identifying factors associated with this condition is important to guide intervention strategies in this population. Objective: To determine high blood pressure prevalence and its associated factors in adolescents. Methods: A random sample of 1,242 students enrolled in public schools of the city of Curitiba (PR) was selected. Self-administered questionnaires provided family history of hypertension, daily energy expenditure, smoking habit, daily fat intake, and socioeconomic status. Waist circumference was measured following standardized procedures, and blood pressure was measured with appropriate cuffs in 2 consecutive days to confirm high blood pressure. Relative frequency and confidence interval (95%CI) indicated high blood pressure prevalence. Bivariate and multivariate analyses assessed the association of risk factors with high blood pressure. Results: The high blood pressure prevalence was 18.2% (95%CI 15.2-21.6). Individuals whose both parents had hypertension [odds ratio (OR), 2.22; 95%CI 1.28-3.85] and those with high waist circumference (OR, 2.1; 95%CI 1.34-3.28) had higher chances to develop high blood pressure. Conclusion: Positive family history of hypertension and high waist circumference were associated with high blood pressure in adolescents. These factors are important to guide future interventions in this population.

The present work deals with the study of the effects of selfing and crossing in pures lines of okra inbred for five generations and the methods of breeding in this plant. This work is party of a large program of this Dept. to study heterosis in plants naturally self pollinated. The technic of selfing consists of tying with a string the floral bud before anthesis. To make controlled crosses, it is necessary to emasculate the flowers removing the anthers with small forceps, and to cover the flowers with a bag and wait for 1 or 2 days until the blooming. Also, the male parents are covered with paper bags prior to flowering. Finally, the pollen is brushed lightly over the stigma of the emasculated flowers and the females unit rebagged. The authors have tried without sucess the technic of soda fountain straw used for cotton. The treatments were: I) Fl of the cross pure-line x foreign variety (not improved by breeding). II) Fl of the cross pure-line x parental variety and III) pure-line 5 generations inbred. In order to compare the production of these three treatments, a randomized blocks with 4 replications was designed; since we had 6 families in each treatment, the total number was: 4 replications x 3 treatments x 6 families: = 72. Each familiy was planted in lines of 10 plants. Owing to the design devised, the present experiment corresponds to a split-plot. The analysis of variance of the number and the weight of the pods is given in tables 2 and 4, and shows the following: 1) The production expressed in both numbers and weights of the cross, - pure lines x foreign variety - was statistically smaller than the others treatments, i, e., the cross of pure-lines x parental variety and the pure-lines; 2) The production of the treatments pure-lines x parental variety and selfed purelines was the same. It was proved that the selfing do not produce harmful effects in okra, it was benefical, since after 5 inbred generations the production was the same when compared with Fl of the parental variety. Also, the methods of pure-lines are indicated to improve varieties of okra.

Com base nos parâmetros Vm e Km obtidos de ensaios de cinética de absorção verificou-se a influência do Al e do Mn e do grau de tolerância de cultivares de trigo a cada um desses elementos sobre a absorção de Ca. Utilizaram-se raízes destacadas de plantas com 7 dias de idade de 3 cultivares com tolerância diferencial ao Al (Sonora 63C - suscetível; Yecora - intermediária e IAS 63 - tolerante) e de 3 cultivares com tolerância diferencial ao Mn (CNTl - suscetível; Sonora 63C - intermediária e IAS 55 - tolerante). Foram empregadas 10 soluções experimentais cujas concentrações de CaCl variaram de 5xl0-6M até 2,56xl0-3M, marcadas com 45Ca. Nos ensaios efetuados na presença de Al ou Mn, usaram-se concentrações de 10-4M de AlCl3 ou MnCl2. Os resultados propiciaram as seguintes -conclusões: a) a absorção de Ca se realiza através de 2 mecanismos, um operativo na faixa 1 de concentração de Ca (5x10-6M - 8 x 10-5M) e outro na faixa 2 de concentração (1,6x10-4M - 2,56 x 10-3M); b) na faixa 1 de concentração de Ca a Al e o Mn inibem a absorção do Ca e na faixa 2 inibem ou estimulam a absorção dependendo do cultivar; c) na ausência de Al ou de Mn existem diferenças genéticas na absorção de Ca somente na faixa 2 de concentração de Ca e estas não estão relacionadas com o grau de tolerância ao Al e ao Mn; d) na presença de Al não existem diferenças genéticas na absorção do Ca na faixa 1 de concentração de Ca, porém existem relacionadas ao grau de tolerância ao Al na faixa 2; e) na presença de Mn existem diferenças genéticas na absorção de Ca em ambas as faixas, porém somente na faixa 1 relacionadas ao grau de tolerância ao Mn.

Plantas de aspargo (Aspargus officinalis cv. Mary Washington) foram cultivadas em silica e irrigadas por percolação com diferentes soluções nutritivas em ordem de: 1) Obter o quadro sintomatológico das carências em N, P, K, Ca, Mg, S, B. 2) Aquilatar o efeito das deficiências sobre o crescimento das plantas. 3) Obter níveis analíticos para macronutrientes e de boro em plantas bem nutridas e desnutridas. Os autores constataram que a identificação visual das carências é difícil e pouco precisa. A omissão dos macronutrientes e de boro afetam sensivelmente o desenvolvimento das plantas. Determinaram que a concentração dos ele mentos nos cládodios de plantas bem nutridas apresentam N% - 2,20 - 2,93; P%-0,10 - 0,20; K% 2,95 - 3,11; Ca% 0,67 -0,98; Mg% 0,76 - 1,35; S 0,16 - 0,18 ; B ppm 81,6 - 108,3- Os níveis de deficiência dos elementos detectados nos clãdodios apresentam: N% 1,22 - 1,36 ; P% 0,07 - 0,09; K% 0,39 - 0,75; Ca% 0,04 - 0,18; Mg% 0,05 - 0,15; S% 0,08 - 0,10 e B ppm 12,3 - 12,6.

Procurou-se avaliar o comportamento agronômico do Suporte Solido Capilar para Fertilizantes Fluídos (SC), que é um produto fertilizante experimental desenvolvido pela ULTRAFÉRTIL com comportamento potencial de Fertilizante de Solubilização Lenta. Procurou-se avaliar seu efeito sobre as plantas e sobre os solos além de compará-lo com fertilizantes convencionais, através de um ensaio de efeito residual. O SC não se comportou como Fertilizante de Solubilizaçao Lenta, igualando-se aos outros tratamentos. O K quando aplicado via SC é melhor aproveitado pelas plantas e a presença do SC no solo provoca variações nos teores de P, Ca e S - SO4-2 no solo.

Eimeria vitellini n. sp. is described from the faeces of the Rhamphastos v. vitellinus. Oocysts broadly ellipsoidal to oval (egg-shaped), 22,6 x 18.3 (20.0-25.0 x 16.3-22.5) micronm, shapeindex (length/width) 1.2 (1.1-1.1). Oocyst wall a single colourless layer about 0.5 micronm thick, becoming thinner at one ectremity, at which point the oocyst usually ruptures. No oocyst residuum, but 1 or 2 small polr bodies of about 1.0-1.5 x 0.5-1.0 micronm. Sporocysts ellongated ellipsoid (pearshaped), 14.3 x 7.5 (13.8-15.0 x 6.9-7.5) micronm, shape-index (1.9 (1.8-2.0), with a thin colourless wall bearing a very delicate Stieda body: a conspicuous sub-Stieda body is present. Sporozoites with anterior and posterior regractile bodies and strongly recurved around a bulky, compact sporocyst residuum composed of relatively fine granules and spherules.
