OBJETIVO: avaliação da hemorragia feto-materna (HFM) nas pacientes que receberiam profilaxia da aloimunização Rh com emprego de imunoglobulina anti-D (300 mig), pós-aborto precoce. MÉTODO: foram admitidas no estudo pacientes do grupo sanguíneo Rh negativo, com parceiro Rh positivo ou ignorado, com quadro de aborto até 12 semanas de gestação internadas para curetagem uterina. Uma amostra de 5 ml de sangue venoso destas pacientes foi obtida após o procedimento, na qual realizamos o teste qualitativo de roseta para detectar quais casos necessitariam determinação quantitativa do volume de sangue fetal transferido para circulação materna, que foi então apurado pelo teste de Kleihauer-Betke (K-B). RESULTADOS: das 26 pacientes avaliadas, em uma o teste de roseta foi positivo, e o teste de K-B apontou HFM de 1,5 ml. CONCLUSÕES: a dose de imunoglobulina anti-D nos casos de abortamento até a 12ª semana de gestação deveria ser substancialmente reduzida, parecendo-nos oportuna a disponibilização no mercado nacional de apresentação com 50 mig, que representaria além da economia, maior racionalidade.

Aloimunização é a formação de anticorpos quando há a exposição do indivíduo a antígenos não próprios, como ocorre, por exemplo, na transfusão de sangue incompatível e nas gestantes, cujos fetos expressam em suas células sanguíneas antígenos exclusivamente de origem paterna. Este artigo se restringe à aloimunização contra antígenos eritrocitários em pacientes obstétricas. Quase todos os anticorpos antieritrocitários podem ser enquadrados em um dos 29 sistemas de grupos sanguíneos já reconhecidos, sendo os mais comumente implicados na doença hemolítica perinatal o anti-D, anti-c e anti-Kell, seguidos por anti-C, anti-E, anti e, anti-Fyª e anti-Jkª. A pesquisa de anticorpos irregulares permite o diagnóstico de indivíduos aloimunizados e modernas técnicas genéticas têm melhor caracterizado estas pacientes para a profilaxia e segmento pré-natal. O tradicional acompanhamento das gestações de risco para a doença hemolítica perinatal, com a espectrofotometria do líquido amniótico e a transfusão intraperitoneal, vem rapidamente sendo substituído pela doplervelocimetria na artéria cerebral média e a transfusão intravascular guiada por ultrassonografia em tempo real. É possível também citar como avanços melhorias nos materiais e na qualidade do sangue transfundido, que, em conjunto, têm elevado a sobrevivência de fetos acometidos. Indubitavelmente, a correta aplicação da profilaxia com uso do anti-D é exitosa com potencial para reduzir os casos de aloimunização.

MeOH extract from the leaves of Plectranthus barbatus Andrews (Lamiaceae), showed in vitro anti-trypanosomal activity. The bioassay-guided fractionation resulted in the isolation of a gallic acid derivative, identified as 1,2,3,4,6-penta-O-galloyl-β-D-glucose (PGG), after thorough NMR and MS spectral analysis. Finally, this compound was tested against trypomastigote forms of T. cruzi and displayed an EC50 value of 67 µM, at least 6.6-fold more effective than the standard drug benznidazole. This is the first occurrence of PGG in the Plectranthus genus and the first anti-parasitic activity described for PGG in the literature.

Anti-cancer DNA vaccines have attracted growing interest as a simple and non-invasive method for both the treatment and prevention of tumors induced by human papillomaviruses. Nonetheless, the low immunogenicity of parenterally administered vaccines, particularly regarding the activation of cytotoxic CD8+ T cell responses, suggests that further improvements in both vaccine composition and administration routes are still required. In the present study, we report the immune responses and anti-tumor effects of a DNA vaccine (pgD-E7E6E5) expressing three proteins (E7, E6, and E5) of the human papillomavirus type 16 genetically fused to the glycoprotein D of the human herpes simplex virus type 1, which was administered to mice by the intradermal (id) route using a gene gun. A single id dose of pgD-E7E6E5 (2 µg/dose) induced a strong activation of E7-specific interferon-γ (INF-γ)-producing CD8+ T cells and full prophylactic anti-tumor effects in the vaccinated mice. Three vaccine doses inhibited tumor growth in 70% of the mice with established tumors. In addition, a single vaccine dose consisting of the co-administration of pgD-E7E6E5 and the vector encoding interleukin-12 or granulocyte-macrophage colony-stimulating factor further enhanced the therapeutic anti-tumor effects and conferred protection to 60 and 50% of the vaccinated mice, respectively. In conclusion, id administration of pgD-E7E6E5 significantly enhanced the immunogenicity and anti-tumor effects of the DNA vaccine, representing a promising administration route for future clinical trials.

Realizou-se inquérito sorológico e epidemiológico para cisticercose em indivíduos de cinco municípios da região Norte do Estado do Paraná, Brasil. De 2.180 indivíduos investigados através da reação de imunofluorescência indireta, 69 (3,2%) apresentaram títulos significativos de anticorpos anti-Cysticercus cellulosae. Os percentuais de indivíduos com títulos significativos encontrados em Sarandi (6,6%) e Marialva (4,7%) não diferem estatisticamente (Z=1.319, P=0,0936), mas diferem dos percentuais encontrados em Mandaguaçu, Paiçandu e Maringá (P<0,01). Destes indivíduos, 47,9% estavam na faixa etária de 21 a 49 anos e 79,4% eram do sexo feminino. Foi comum o relato de queixas como "dores de cabeça" (70,6%), "tonturas" (57,4%) e "convulsões" (7,4%), além de história de teníase (22,1%) e hábitos de ingestão de carne crua bovina (41,2%) ou suína (27,9%) e carne com "canjiquinha" (25,0%).

The determination of anti-HBs as a screening test before vaccination has been advisable in order to encounter immune individuals that don't need to receive vaccine protection. A case-report is presented and three other cases are reviewed from the literature. Anti-HBs was positive in these health-care personnels that developped typical acute B hepatitis. Different subtyping involving the d/y determinants were found in the first case, but false-positive anti-HBs even with high titres, determined by RIA, were found in the other cases. Concomitant determination of anti-HBc or absence of screening tests seem to be more reasonable policies until a low-cost and risk-free vaccine is produced.

In order to define an accurate assay for anti-adenovirus antibody detection, a recently developed ELISA was compared with IFA and CF. On 58 sera, the ELISA was more sensitive than both CF and IFA, which showed relative sensitivities of 63% and 94%, respectively. It was not possible to determine the exact specificity of the tests because of the lack of a gold standard. Furthermore, the ELISA was used to define the prevalence of adenovirus antibodies in 116 infants between 1 and 24 months old (mean 7.28). The data showed that maternal antibodies waned by the age of 5 to 6 months and that more than 80% of the children had been infected by adenoviruses by the age of 10 months.

Foi empregado o teste imunoenzimático com componentes antigênicos de Cysticercus cellulosae quimicamente ligados a suporte inerte constituído por discos de tecido-resina, ELISA-d, com a finalidade de investigar a entidade neurocisticercose (NC) em líquidos cefalorraquianos (LCR) de pacientes com meningites de etiologia indeterminada. Foram ensaiados 277 LCR de 128 crianças e 149 adultos. A densidade óptica média (DO) obtida para os 22 LCR de pacientes nos quais foi afastada a possibilidade diagnóstica de meningite foi de 0,03. Os 44 LCR de pacientes com meningites determinadas por diversos agentes etiológicos, não cisticercose, apresentaram DO de 0,05. O limiar de reatividade do teste ELISA-d calculado a partir desses dois grupos (controle) foi de 0,13 (DO + 3SD). No grupo de 13 LCR de pacientes com NC comprovada em episódio meningítico por essa causa, foi observada DO de 0,41 (0,10 a 0,91) no teste ELISA-d. Dos 198 LCR de meningites por agente etiológico não identificado pelos métodos usualmente empregados, 23 (11,6%) apresentaram DO acima de 0,13, sugerindo que a possível causa da meningite tenha sido por cisticercose, uma vez que o teste ELISA-d tem apresentado elevadas sensibilidade, especificidade e reprodutibilidade. Em cinco dos 23 LCR a alteração no exame quimiocitológico era às custas do aumento do número de células predominantemente linfomononucleares, em 13 o predomínio era de polimorfonucleares e nos cinco restantes ambos os tipos de células estavam em número aumentado.

Le test ELISA-TÉTANOS (Biosys, France) a été utilisé pour le titrage des anti-corps tétaniques (sensibilité = 0,0025 UI/ml) en sérums humains de donneurs de sang, 566 hommes et 108 femmes, âges de 18 à 58 ans, moyenne de 29 ans, provenant de São Paulo, SP, Brésil. L'OMS, acceptant seulement la séroneutralisation sur souris (NT), la méthode de référence, pour les études sur la protection contre le tétanos, préconise le titre de 0,01 UI/ml comme minimum protecteur. BOURLEAUD & HUET ont proposé la limite de 0,06 UI/ml quand s'emploie le test ELISA, en attendant à une certaine discordance inévitable entre les méthodes. Parmi les 674 sérums étudiés, 178 (26,41%) n'ont pas présenté d'anticorps (< 0,0025 UI/ml); 413 (61,28%) ont présenté des résultats égaux ou supérieurs à 0,01 UI/ml et en 232 (34,42%) les titres ont été égaux ou supérieurs à 0,06 UI/ml. Le pourcentage d'individus protégés a été inversement proportionnel à 1'âge: environ 50% dans le groupe le plus jeune (hommes de 18 à 35 ans et femmes de 18 à 23 ans) contre environ 10% dans le groupe de plus de 42 ans ont présenté des titres sûrement protecteurs (> 0,06 UI/ml).

Detection of HBV-DNA by PCR was compared with other serological markers (HBsAg, HBeAg and anti-HBe) in a series of49 Chronic Hepatitis B patients, including 12 with a spontaneous clearance of HBsAg. None of these HBsAg negative cases were PCR positive, but 33/37 (89.2%) HBsAg positive cases were PCR positive (p < 0.0001). Among HBsAg positive samples, nine cases were HBeAg positive and anti-HBe negative, all of them PCR positive. Other 3 patients were HBeAg and anti-HBe positive and these cases were also found PCR positive. A third group included 21 patients anti-HBe positive and HBeAg negative: 19 of them were PCR positive and 2 were PCR negative. The last 4 cases were HBeAg and anti-HBe negative, two of them were PCR positive. The detection of anti-HBe viremic cases in the present series suggest that preC variants could occur in our country. In conclusion, the integrated phase o f chronic hepatitis B seems to be less frequent than it was assumed, when only HBeAg or dot blot hybridization techniques were used. The new term "low replication phase" might favorably replace the former "integrated phase".

Data concerning HCV infection in Central Brazil are rare. Upon testing 2,350 voluntary blood donors from this region, we found anti-HCV prevalence rates of 2.2% by a second generation ELISA and 1.4% after confirmation by a line immunoassay. Antibodies against core, NS4, and NS5 antigens of HCV were detected in 81.8%, 72.7%, and 57.5%, respectively, of the positive samples in the line immunoassay. HCV viremia was present in 76.6% of the anti-HCV-positive blood donors. A relation was observed between PCR positivity and serum reactivity in recognizing different HCV antigens in the line immunoassay. The majority of the positive donors had history of previous parenteral exposure. While the combination of ALT>50 IU/l and anti-HBc positivity do not appear to be good surrogate markers for HCV infection, the use of both ALT anti-HCV tests is indicated in the screening of Brazilian blood donors.

Leptospirosis is one of the causes of meningitis, although its importance is not well known. In the present study we contributed to this knowledge by demonstrating specific IgM class anti-leptospira antibodies by the immunoenzymatic method ELISA in 14.6% of cerebrospinal fluid (CSF) samples from 171 patients with meningitis considered to be of indeterminate etiology. The frequencies of positivity were similar in cases with predominance of polymorphonuclear or lymphomononuclear leucocytes in the CSF. Age distribution showed a predominance of the 5 to 15 year age range (72%), and sex distribution showed a predominance of males (68%). The authors discuss the contribution of this method to the etiologic elucidation of meningitis.

Considering the impact of cysticercosis on public health, especially the neurologic form of the disease, neurocysticercosis (NC), we studied the frequency of positivity of anti-Taenia solium cysticercus antibodies in serum samples from 1,863 inhabitants of Cássia dos Coqueiros, SP, a municipal district located 80 km from Ribeirão Preto, an area considered endemic for cysticercosis. The 1,863 samples were tested by enzyme linked immunosorbent assay (ELISA) using an antigenic extract from Taenia crassiceps vesicular fluid (Tcra). The reactive and inconclusive ELISA samples were tested by immunoblotting. Of the 459 samples submitted to immunoblotting, 40 were strongly immunoreactive to the immunodominant 18 and 14 kD peptides. Considering the use of immunoblotting as confirmatory due to its high specificity, the anti-cysticercus serum prevalence in this population was 2.1%.

Simple and rapid latex-based diagnostic tests have been used for detecting specific antigens or antibodies in several diseases. In this article, we present the preliminary results obtained with a latex agglutination test (LAT) for diagnosing neurocysticercosis by detection of antibodies in CSF. A total of 43 CSF samples were assayed by the LAT: 19 CSF samples from patients with neurocysticercosis and 24 CSF samples from patients with other neurologic disorders (neurosyphilis, n = 8; neurotoxoplasmosis, n = 3; viral meningitis, n = 4, chronic headache, n = 9). The LAT exhibited 89.5% sensitivity and 75% specificity. The use of LAT seems to be an additional approach for the screening of neurocysticercosis with advantage of simplicity and rapidity. Further studies could be performed using purified antigens and serum samples.

In a transversal study on a sample of 386 children and adolescents from an outpatient clinic for filariasis in Recife, Northeast Brazil, the frequency of anti-Toxocara antibodies and its relation to age, gender, number of peripheral eosinophils, Wuchereria bancrofti microfilariae and intestinal helminths was determined. The total anti-Toxocara IgG antibody frequency was 39.4%, by ELISA technique. The difference in frequency between males (40.1%) and females (37.6%) was not statistically significant. The 6 to 10-year-old subset presented the highest frequency of anti-Toxocara antibodies (60%), and within this age group there was a statistically significant male bias. There was also a significant association between the number of eosinophils and the presence of anti-Toxocara antibodies. Intestinal parasite frequency was 52.1%, but no association was found between this data and the presence of anti-Toxocara antibodies. In the present sample, 42.2% of the patients were Wuchereria bancrofti carriers, however, again this was not associated with the presence of anti-Toxocara antibodies. In conclusion, anti-Toxocara antibodies were highly prevalent in this sample. The present data show that there is no cross correlation between anti-Toxocara IgG antibody and the presence of intestinal helminths and filariasis.
