Este trabalho visou a comparar o potencial energético de resíduos produzidos no beneficiamento de grãos de café (Coffea canephora var. Conilon) e no processo de fresamento da madeira, sugerindo seu uso em substituição ao da lenha de eucalipto, no processo de secagem de grãos de café. O uso destes resíduos agrícola e florestal pode contribuir para a redução de problemas ambientais relacionados com a contaminação do solo, ar e água, devidos a seu descarte inadequado, e para reduzir os custos de produção e, ou, beneficiamento do café. Os subprodutos da destilação seca e o carvão vegetal dessas matérias-primas foram quantificados e comparados. De acordo com os resultados, pôde-se verificar que os resíduos de casca de café proporcionaram uma boa produção de carvão vegetal, visto que seu rendimento gravimétrico em carvão foi estatisticamente superior ao da lenha. Considerando a produtividade e qualidade do carvão vegetal, o melhor resultado foi obtido pela carbonização da casca de café nas temperaturas de 350 ºC a 550 ºC e dos resíduos da fresa de madeira a 550 °C, principalmente, graças aos rendimentos médios em carbono fixo, que, nesses casos, superaram aqueles apresentados pelos carvões derivados de lenha do eucalipto. Em se tratando do poder calorífico superior, verificou-se que os resíduos da fresa de madeira e os resíduos de casca de café poderão ser utilizados para secagem de grãos de café, visto que apresentaram valores de poder calorífico superior (PCS) muito próximo aos de lenha de eucalipto. Graças aos bons rendimentos gravimétricos e rendimentos em carbono fixo, os carvões vegetais derivados dos resíduos produzidos no beneficiamento de grãos de café e no processo de fresamento da madeira apresentaram potencial considerável para serem utilizados como insumo energético.

Right ventricular endomyocardial biopsies were studied in 30 patients, 15 with myocardiopathy from chronic Chagas'disease and 15 with idiopathic congestive myocardiopathy; five other myocardial samples were taken at necropsies of patients with chronic Chagas' disease. The authors tried to establish by means of direct immunofluorescence techniques whether there were immunoglobulins G, A and M, fibrinogen and C3 complement deposition in the myocardium; only one of these 30 patients exhibited a positive reaction to IgG, it was a patient with idiopathic congestive myocardiopathy. All fragments from patients with Chagas' disease showed no response to any of the fluorescent conjugates. These findings do not support the idea that anti-myoeardial antibodies have pathogenic importance in the evolution of dilated or chagasic myocardiopathies.

This report describes two patients with chronic Chagas' Heart Disease who developed clinical and laboratorial signs of myocardial infarction. Both patients presented sudden oppressive chest pain, without precipitating factor. In the first case, the highest MB-CK value was 65 IU, 22 hours after the beginning of the pain. On the second case, it was 77 IU at 18 hours after the beginning of the pain. In both cases ECG changes suggesting non-transmural infarction were present. The 99mTc PYP myocardial scintigram of the first case was positive. Coronary angiograms performed on the 18th and 9th day, respectively, after the acute infarction did not display obstructive lesions. Possible mechanisms causing myocardial infarction with normal coronary arteries in Chagas' Disease may include: embolic event's, particularly when there is associated congestive heart failure; coronary thrombosis and coronary spasms.

From January 1988 to January 1989 all the heart transplant and bone marrow recipients at the Instituto do Coração of the Hospital das Clínicas of the University of São Paulo Medical School were studied for the incidence and morbidity associated with herpesviruses infections after transplantation. Five bone marrow and 5 heart transplant recipients were followed for a mean of 4.2 months post-transplantation. All the patients were seropositive for cytomegalovirus (CMV) before admission and 80% experienced one or more recurrences during the observation period. Of the 12 episodes of CMV infection, that were identified in this study, 83% were accompanied by clinical or laboratory abnormalities. However, there was only one case of severe disease. The overall incidence of infection for herpes simplex (HSV) was 50%. Although most of HSV reactivations were oral or genital, one case of HSV hepatitis occurred. One of the 6 episodes of HSV infections that were treated with acyclovir showed an unsatisfactory response and was successfully managed with ganciclovir. All the individuals had anti-varicella zoster virus antibodies, but none of them developed infection. The study emphasizes the importance of active diagnostic surveillance of herpesvirus infections in transplant patients. Both CMV and HSV reactivations showed high incidence and important morbidity and thus, deserve prophylactic therapy.

OBJETIVO: Avaliar as formas de apresentação da amilóidose cardíaca em hospital terciário. MÉTODOS: Nos últimos 15 anos, foram identificados 8 pacientes com amilóidose, sendo 5 mulheres , com idades entre 23 e 83 (média 62) anos. Após anamnese e exame físico foram submetidos a eletrocardiograma (ECG), ecocardiograma (ECO), estudo com pirofosfato de tecnécio e biópsia endomiocárdica, cujos resultados permitiram caracterizar suas formas clínicas. RESULTADOS: Sete pacientes apresentavam dispnéia aos esforços, 6 quadro de insuficiência cardíaca congestiva (ICC), 1 síncopes. O ECG identificou bloqueio atrioventricular total (BAVT) em 4 casos e área eletricamente inativa ântero-septal em outros 4. O ECO mostrou diâmetros normais em todos e fração de ejeção discretamente reduzida em 6. Hipertrofia do septo e parede posterior em todos, sendo em 7 com aspecto sugestivo de doença de depósito (aspecto granuloso). Os dados clínicos caracterizam dois grupos, um com BAVT e outro com cardiomiopatia restritiva. A evolução foi diferente com melhora clínica após o implante de marcapasso no primeiro grupo e má evolução no grupo com cardiomiopatia restritiva que evoluiu de maneira refratária, 3 falecendo em menos de 6 meses após diagnóstico. CONCLUSÃO: A presença de aumento da espessura das paredes ao ECO, discreta disfunção sistólica e aspecto de doença de depósito identificaram a quase totalidade dos casos. Cardiomiopatia restritiva e distúrbio de condução foram as formas de apresentação, sendo o prognóstico muito reservado nos pacientes com forma restritiva, evoluindo para ICC refratária.

OBJETIVO: Avaliar a evolução da hipertensão arterial (HA), e suas consequências, em pacientes submetidos a transplante cardíaco (TC) em uso de ciclosporina (CL). MÉTODOS: Em 65 pacientes submetidos a TC ortotópico, avaliamos a pressão arterial, creatinina sérica e níveis sangüíneos de CL nos períodos pré-operatório (15 dias antes do TC), pós-operatório precoce (15 e 30 dias) e tardio (6, 12, 24, 48 e 60 meses); em 20 pacientes analisamos índice cardíaco e resistência vascular no pré, 15 e 30 dias, 6 e 12 meses após TC; em 33 pacientes, estudamos estrutura e função ventricular ao ecocardiograma, 24±13 meses após TC. RESULTADOS: Após 30 dias, a HA estava presente em 58,5% dos pacientes (50% leve), enquanto na evolução tardia, a incidência da HA aumentou significativamente para 93% após um ano (85% moderada a grave). A creatinina sérica aumentou progressivamente do pré-TC (1,43±0,5mg/dl) até após um ano (1,83± 0,9 mg/dl), mantendo leve incremento até os 60 meses (2,4± 0,8mg/dl). Não houve correlação entre a HA, creatinina sérica e níveis de CL. O índice cardíaco aumentou na fase precoce, enquanto a resistência periférica diminuiu no início e aumentou significativamente aos 12 meses. Ao ecocardiograma, 54% dos pacientes apresentavam hipertrofia de ventrículo esquerdo com função normal. Dos 31 pacientes que faleceram durante a evolução, dois tiveram a causa mortis diretamente relacionada a HA. CONCLUSÃO: A HA em pacientes submetidos a TC em uso de CL ocorre precocemente, aumenta em prevalência e gravidade com o tempo e é mediada por aumento da resistência periférica, não se correlacionando com a nefrotoxicidade e com os níveis sangüíneos da CL, podendo agravar a insuficiência renal ou comprometer a longevidade do transplante, induzindo hipertrofia ventricular.

Mulher de 60 anos, com angina progressiva e revascularização do miocárdio, há oito anos, com ponte de veia safena para coronária direita e anastomose de artéria mamaria esquerda para artéria descendente anterior. Submetida a implante de stent Gianturco-Roubin II em terço proximal da ponte de veia safena para artéria coronária direita, com resultado insatisfatório pela persistência de lesão residual, provavelmente, decorrente de prolapso para dentro da luz de material aterosclerótico através dos coils. Foi implantado outro stent (Palmaz-Schatz biliar) dentro do stent GRII com sucesso e ótimo resultado angiográfico. Um 2º stent Palmaz-Schatz biliar foi implantado em lesão distal no corpo da ponte, ultrapassando os dois stents, anteriormente implantados, com sucesso. Em algumas situações, implante de stent dentro de outro stent é recurso útil para otimização de resultado angiográfico do implante de um stent.

OBJETIVO: Os efeitos dos beta-bloqueadores na insuficiência cardíaca (IC) refratária não têm sido adequadamente estudados. Investigamos os efeitos do carvedilol (bloqueador b1,b2,a1) nos sintomas e na função ventricular de portadores de IC refratária. MÉTODOS: Foram estudados 21 pacientes, idade média de 56±10 anos, 9 em classe funcional (CF) IV, e 12 em CF III intermitente com IV. A dose inicial de carvedilol foi de 6,25mg e, se tolerada, aumentada progressivamente. A dose média final foi 42±11mg. Os pacientes foram submetidos a avaliações clínicas e eletrocardiográficas seriadas. Realizaram-se, antes e com 196±60 dias de evolução, ecocardiograma e ventriculografia radioisotópica. RESULTADOS: O medicamento foi tolerado em 16 (76%) pacientes. Um paciente está em fase de titulação em CF II. Com 196±60 dias de evolução observaram-se 8 pacientes em CF I e 7 em II; redução da freqüência cardíaca de 96±15 para 67±10bpm (p<0,0001); redução do diâmetro diastólico final de ventrículo esquerdo (VE) de 73±13 para 66±12mm (ecocardiograma) (p<0,009); e aumento da fração de ejeção de VE de 0,21±0,06 para 0,34±0,12 (p<0,0003). CONCLUSÃO: O carvedilol devido aos seus efeitos benéficos na função ventricular, remodelamento e CF é, se tolerado, uma potencial alternativa terapêutica no tratamento medicamentoso da IC refratária. Entretanto, estudos adicionais são necessários para definição do efeito a longo prazo neste específico subgrupo de pacientes.

OBJECTIVE - To identify, the anaerobic threshold and respiratory compensation point in patients with heart failure. METHODS - The study comprised 42 Men,divided according to the functional class (FC) as follows: group I (GI) - 15 patients in FC I; group II (GII) - 15 patients in FC II; and group III (GIII) - 12 patients in FC III. Patients underwent a treadmill cardiopulmonary exercise test, where the expired gases were analyzed. RESULTS - The values for the heart rate (in bpm) at the anaerobic threshold were the following: GI, 122±27; GII, 117±17; GIII, 114±22. At the respiratory compensation point, the heart rates (in bpm) were as follows: GI, 145±33; GII, 133±14; GIII 123±22. The values for the heart rates at the respiratory compensation point in GI and GIII showed statistical difference. The values of oxygen consumption (VO2) at the anaerobic threshold were the following (in ml/kg/min): GI, 13.6±3.25; GII, 10.77±1.89; GIII, 8.7±1.44 and, at the respiratory compensation point, they were as follows: GI, 19.1±2.2; GII, 14.22±2.63; GIII, 10.27±1.85. CONCLUSION - Patients with stable functional class I, II, and III heart failure reached the anaerobic threshold and the respiratory compensation point at different levels of oxygen consumption and heart rate. The role played by these thresholds in physical activity for this group of patients needs to be better clarified.

It has been reported that growth hormone may benefit selected patients with congestive heart failure. A 63-year-old man with refractory congestive heart failure waiting for heart transplantation, depending on intravenous drugs (dobutamine) and presenting with progressive worsening of the clinical status and cachexia, despite standard treatment, received growth hormone replacement (8 units per day) for optimization of congestive heart failure management. Increase in both serum growth hormone levels (from 0.3 to 0.8 mg/l) and serum IGF-1 levels (from 130 to 300ng/ml) was noted, in association with clinical status improvement, better optimization of heart failure treatment and discontinuation of dobutamine infusion. Left ventricular ejection fraction (by MUGA) increased from 13 % to 18 % and to 28 % later, in association with reduction of pulmonary pressures and increase in exercise capacity (rise in peak VO2 to 13.4 and to 16.2ml/kg/min later). The patient was "de-listed" for heart transplantation. Growth hormone may benefit selected patients with refractory heart failure.

PURPOSE:To determine the indication for and incidence and evolution of temporary and permanent pacemaker implantation in cardiac transplant recipients. METHODS: A retrospective review of 114 patients who underwent orthotopic heart transplantation InCor (Heart Institute USP BR) between March 1985 and May 1993. We studied the incidence of and indication for temporary pacing, the relationship between pacing and rejection, the need for pemanent pacing and the clinical follow-up. RESULTS: Fourteen of 114 (12%)heart transplant recipients required temporary pacing and 4 of 114 (3.5%) patients required permanent pacing. The indication for temporary pacing was sinus node dysfunction in 11 patients (78.5%) and atrioventricular (AV) block in 3 patients (21.4%). The indication for permanent pacemaker implantation was sinus node dysfunction in 3 patients (75%) and atrioventricular (AV) block in 1 patient (25%). We observed rejection in 3 patients (21.4%) who required temporary pacing and in 2 patients (50%) who required permanent pacing. The previous use of amiodarone was observed in 10 patients (71.4%) with temporary pacing. Seven of the 14 patients (50%) died during follow-up. CONCLUSION: Sinus node dysfunction was the principal indication for temporary and permanent pacemaker implantation in cardiac transplant recipients. The need for pacing was related to worse prognosis after cardiac transplantation.

OBJECTIVE:To evaluate the use of methotrexate for the treatment of recurrent rejection in heart transplant recipients. METHODS: We studied 6 patients submitted to heart transplantation that showed rejection grade > or = 3A (ISHLT) in two consecutives endomyocardial biopsy specimens. The dose was 11.26±3.75mg/week. The evaluated data were: ventricular function, endomyocardial biopsy, white cell count and number of rejection episodes before and after methotrexate administration. RESULTS: There was a reduction in the number of rejection episodes (5.17±1.47 before methotrexate; 2.33±1.75 after 6 months and 3.17±2.99 after 12 months of treatment, p=0.0193). The ventricular function was normal with ejection fraction of 76.5±4.80 before and 75.6±4.59 after methotrexate (p=0.4859). One patient did not finish the treatment because he showed signs of rejection associated with severe pericardial effusion. Five patients had a reduction in the white cell count (8,108±23.72 before and 5650±1350 after methotrexate, p=0.0961). One pulmonary infection with complete resolution after antibiotic treatment was observed. CONCLUSION: Methotrexate in low doses is an effective adjunct therapy in the treatment of recurrent rejection after heart transplantation.

OBJECTIVE: To study the influence of immune and nonimmune risk factors on the development of allograft vasculopathy after cardiac transplantation. METHODS: We studied 39 patients with a mean age of 46±12 years. The following variables were analyzed: weight (kg), body mass index (kg/m²), donor's age and sex, rejection episodes in the first and second years after transplantation, systolic and diastolic blood pressures (mmHg), total cholesterol and fractions (mg/dL), triglycerides (mg/dL), diabetes, and cytomegalovirus infection. The presence of allograft vasculopathy was established through coronary angiography. RESULTS: Allograft vasculopathy was observed in 15 (38%) patients. No statistically significant difference was observed between the two groups in regard to hypertension, cytomegalovirus infection, diabetes, donor's sex and age, rejection episodes in the first and second years after transplantation, and cholesterol levels. We observed a tendency toward higher levels of triglycerides in the group with disease. Univariate and multivariate analyses showed statistically significant differences between the two groups when we analyzed the body mass index (24.53±4.3 versus 28.11±4.6; p=0.019). CONCLUSION: Body mass index was an important marker of allograft vasculopathy in the population studied.

OBJECTIVE: To determine the predictive values of noninvasive tests for the detection of allograft vascular disease. METHODS: We studied 39 patients with mean ages of 48±13 years and a follow-up period of 86±13 months. The diagnosis of allograft vascular disease was made by cine-coronary arteriography, and it was considered as positive if lesions existed that caused > or = 50% obstruction of the lumen. Patients underwent 24h Holter monitoring, thallium scintigraphy, a treadmill stress test, and dobutamine stress echocardiography. Sensitivity, specificity, and positive and negative predictive values were determined in percentages for each method, as compared with the cine-coronary arteriography results. RESULTS: Allograft vascular disease was found in 15 (38%) patients. The Holter test showed 15.4% sensitivity, 95.5% specificity. For the treadmill stress test, sensitivity was 10%, specificity was 100%. When thallium scintigraphy was used, sensitivity was 40%, specificity 95.8%. On echocardiography with dobutamine, we found a 63.6% sensitivity, 91.3% specificity. When the dobutamine echocardiogram was associated with scintigraphy, sensitivity was 71.4%, specificity was 87%. CONCLUSION: In this group of patients, the combination of two noninvasive methods (dobutamine echocardiography and thallium scintigraphy) may be a good alternative for the detection of allograft vascular disease in asymptomatic patients with normal ventricular function.
