Safety in Numbers It’s been eight months since we implemented UHC Patient Safety Net® (“PSN”) at John Dempsey Hospital, and we are delighted with its success. As you know, PSN is a web-based reporting tool for reporting patient safety-related events. Frontline staff are doing a great job entering data on patient care events. Here’s how PSN works:

Patient Safety Net Update

It’s been three months since we’ve gone live with Patient Safety Net, and it has been a great success! Managers and front line staff have been trained on how to input, review, and submit event reports that formerly were detailed on the hard copy “RIR” forms. Utilization of the webbased PSN system has been better than expected. PSN has enabled us to greatly improve the way we document and react to patient safety related events.

Psychiatrists' and Psychiatric APRNs' Views of the Adolescent Mental Health Nurse's Role In the Inpatient Setting

Current perceptions about nurses’ roles and responsibilities are examined in this study, specifically relating to adolescent inpatient MHNs. Psychiatrists and psychiatric advanced practice registered nurses (APRNs), who work with MHNs and have also published scholarly psychiatric articles, were contacted to request their participation in an anonymous survey hosted by SurveyMonkey.com. This research was conducted to examine the stereotypes that exist against nurses within the health care profession itself, as compared to the pre-existing stereotypes displayed by the media’s view of nurses. Due to investigator time constraints, only six subjects participated in the study. Analysis of survey responses revealed four overarching themes. First, MHNs are a critical component of the health care team, emerging as rigorous, independent leaders, although still classified as female and sociable. Second, MHNs complete a wide range of daily activities, many of which go unnoticed by observers, often resulting in mixed feelings regarding whether MHNs are given the respect and recognition deserved. Third, MHNs treat each patient as a person with unique thoughts, feelings, and physical make-up. Fourth, MHNs act as a coordinator of care between various health professionals to provide the patient with a holistic approach to healing.

Patient Safety Alert : A new way to share patient safety information among departments and units

Communication : If there is one topic that comes up over and over again as we discuss ways to make John Dempsey Hospital the safest hospital, it is “communication.” In fact, several of the 2006 and 2007 National Patient Safety Goals are centered around improving the effectiveness of communication among caregivers. There are many ways of doing this, and we have implemented several already. These include handoffs, medication reconciliation, “SBAR,” etc. On page two, we will talk in more detail about hand-offs and the use of “SBAR.”

Connecticut Poison Control Center. 2002 and 2003 Annual Report and Poison Exposure Statistics

The Connecticut Poison Control Center (CPCC) at the University of Connecticut Health Center (UCHC) was established in 1957 under Connecticut General Statute 10a- 132. The CPCC’s main responsibility is to provide 24-hour emergency toxicology management consultations for victims of poisoning, and serve as a source for pharmacology and toxicology-related information. The center monitors the epidemiology of human poisoning and provides surveillance for environmental and occupational chemical exposures, drug abuse, and pharmaceutical interactions and adverse effects. The CPCC performs toxicological research, and provides formal toxicology instruction for allied health professionals, as well as professional and consumer poison prevention education. The CPCC is one of 63 nationwide centers certified by the American Association of Poison Control Centers (AAPCC), and the only poison center in the state of Connecticut. The AAPCC establishes standards of care for poisoning and administers the Toxic Exposure Surveillance System (TESS), a national database of poisoning statistics, to which the CPCC is a contributor.

Patients expect to be safe from harm inside the walls of a hospital. Increasing reports of medical errors and adverse events have brought these concerns to public attention. Although we have celebrated many scientific advances over the past several decades, many patients do not benefit because the healthcare infrastructure is inadequate to deliver care to all. Studies confirm opportunities to improve in areas such as inpatient vaccination for flu and outpatient screening for breast, cervical or colon cancer. (Institute of Medicine, (IOM), 2000, 2001, 2004). This document outlines the steps needed to further increase our focus on patient safety in John Dempsey Hospital through the development of a multi-disciplinary Collaborative Center for Clinical Care Improvement (CCCCI). The dimensions of safety and outcomes are briefly discussed to provide some perspective on the scope of these challenges (Strongwater, 2003).

Complementary and Alternative Medicine and Pain Management

Over the past several years, there has been a growing interest in CAM therapies at UCHC. There are doctors, nurses, researchers and students all actively learning about, researching and using CAM modalities. Among them is Dr. Mitch Kennedy, the first Naturopathic Physician to treat patients at the University of Connecticut Health Center.
