Olfactory impairment has been reported in drug-induced parkinsonism (DIP), but the relationship between dopaminergic dysfunction and smell deficits in DIP patients has not been characterized. To this end, we studied 16 DIP patients and 13 patients affected by Parkinson's disease (PD) using the "Sniffin' Sticks" test and [(123)I] FP-CIT SPECT (single-photon emission computed tomography). DIP patients were divided based on normal (n = 9) and abnormal (n = 7) putamen dopamine transporter binding. Nineteen healthy age- and sex-matched subjects served as controls of smell function. Patients with DIP and pathological putamen uptake had abnormal olfactory function. In this group of patients, olfactory TDI scores (odor threshold, discrimination and identification) correlated significantly with putamen uptake values, as observed in PD patients. By contrast, DIP patients with normal putamen uptake showed odor functions-with the exception of the threshold subtest-similar to control subjects. In this group of patients, no significant correlation was observed between olfactory TDI scores and putamen uptake values. The results of our study suggest that the presence of smell deficits in DIP patients might be more associated with dopaminergic loss rather than with a drug-mediated dopamine receptor blockade. These preliminary results might have prognostic and therapeutic implications, as abnormalities in these individuals may be suggestive of an underlying PD-like neurodegenerative process.

On 1 July 2007 a new transplant law came into force in Switzerland. The principal item of this new law is the change from centre-oriented allocation to patient-oriented national allocation of organs. The aim of the present study is to assess the impact on cold ischaemia time (CIT) and transport requirements.

Accurate diagnosis of the causes of chest pain and dyspnea remain challenging. In this preliminary observational study with a 5-year follow-up, we attempted to find a simplified approach to selecting patients with chest pain needing immediate care based on the initial evaluation in ED. During a 24-month period were randomly selected 301 patients and a conditional inference tree (CIT) was used as the basis of the prognostic rule. Common diagnoses were musculoskeletal chest pain (27%), ACS (19%) and panic attack (12%). Using variables of ACS symptoms we estimated the likelihood of ACS based on a CIT to be high at 91% (32), low at 4% (198) and intermediate at 20.5-40% in (71) patients. Coronary catheterization was performed within 24 hours in 91% of the patients with ACS. A culprit lesion was found in 79%. Follow-up (median 4.2 years) information was available for 70% of the patients. Of the 164 patients without ACS who were followed up, 5 were treated with revascularization for stable angina pectoris, 2 were treated with revascularization for myocardial infarction, and 25 died. Although a simple triage decision tree could theoretically help to efficient select patients needing immediate care we need also to be vigilant for those presenting with atypical symptoms.

Following the idea that response inhibition processes play a central role in concealing information, the present study investigated the influence of a Go/No-go task as an interfering mental activity, performed parallel to the Concealed Information Test (CIT), on the detectability of concealed information. 40 undergraduate students participated in a mock-crime experiment and simultaneously performed a CIT and a Go/No-go task. Electrodermal activity (EDA), respiration line length (RLL), heart rate (HR) and finger pulse waveform length (FPWL) were registered. Reaction times were recorded as behavioral measures in the Go/No-go task as well as in the CIT. As a within-subject control condition, the CIT was also applied without an additional task. The parallel task did not influence the mean differences of the physiological measures of the mock-crime-related probe and the irrelevant items. This finding might possibly be due to the fact that the applied parallel task induced a tonic rather than a phasic mental activity, which did not influence differential responding to CIT items. No physiological evidence for an interaction between the parallel task and sub-processes of deception (e.g. inhibition) was found. Subjects' performance in the Go/No-go parallel task did not contribute to the detection of concealed information. Generalizability needs further investigations of different variations of the parallel task.

En 1920, Cocteau et Radiguet éditent une revue intitulée Le Coq, qui se présente comme un dépassement de Dada, et qui aboutit à la création d’une « ligue antimoderne ». Pourtant, Dada est plusieurs fois cité dans la revue, qui reprend aussi les caractéristiques typographiques de revues dada. Mais il apparaît que Le Coq n’a pas saisi la «propagande» dada, qui n’utilise les moyens de la publicité que pour s’en abstraire.

Mme Michèle Crogiez traite dans sa communication de la querelle du théâtre à Genève entre Jean-Jacques Rousseau et D’Alembert à l’occasion de la publication de l’article "Genève" de l’ "Encyclopédie". La "Lettre à D’Alembert sur les spectacles" (1758) rédigée à Montmorency par Rousseau n’a rien d’une relation de voyage. Rousseau a quitté adolescent sa ville natale, et D’Alembert ne l’a connaît que très imparfaitement par de courts séjours … et par Voltaire. C’est donc d’un lieu largement imaginaire que parlent les deux écrivains. Paradoxalement, Rousseau qui est connu alors par des "discours" scandaleux l’est aussi par sa participation à la querelle des musiques italienne et française et par son activité d’auteur dramatique ("Le Devin de village"). Le contempteur du théâtre à Genève est lui-même homme de théâtre. Si le but de l’article "Genève" est aussi de faire connaître la République helvétique aux Français, le théâtre fait partie de la dimension politique d’une cité et justifie le développement de l’article sur ce qui n’existe pas à Genève, une vie théâtrale officielle. Dans sa "Lettre" (forme habituelle du débat ouvert au XVIIIe siècle), Rousseau emploie de façon récurrente la notion de "patrie" et de "patriotique" pour désigner la spécificité genevoise. Pour Rousseau, les hommes modernes sont corrompus, mais de manière diversifiée. Si les spectacles peuvent être autorisés à Paris (corruption forte et sans espoir de retour), ils doivent être bannis dans les pays où la corruption n’a pas atteint ce degré et où subsiste quelque chose de la primitive honnêteté. Derrière cette querelle, il y a évidemment Voltaire qui tente, des Délices à Carouge, d’introduire à Genève des spectacles théâtraux privés, complices de l’esprit corrupteur (voir ci-dessous sa lettre au marquis Capacelli). Et D’Alembert est son truchement. Chez Rousseau, Genève est une ville d’utopie pour un voyage dans sa mémoire affective, un lieu intime et menacé.

Admiratrice de Tronchin, la duchesse d'Enville se rend plusieurs fois à Genève pour de longs séjours. Elle y fréquente toute la bonne société. Et manifestement, elle y apprend à s'intéresser à la politique, à cause des troubles de la cité, qu'elle observe en personne en 1765-1766 et, par l'intermédiaire de ses amis genevois, au début des années 1780. Cette expérience lui a-t-elle servi au début de la révolution française, à l'époque où son salon sert de lieu de rivalité aux Jacobins ?

BACKGROUND Catecholamine-O-methyl-tranferase (COMT) initiates dopamine degradation. Its activity is mainly determined by a single nucleotide polymorphism in the COMT gene (Val158Met, rs4680) separating high (Val/Val, COMT(HH)), intermediate (Val/Met, COMT(HL)) and low metabolizers (Met/Met, COMT(LL)). We investigated dopaminergic denervation in the striatum in PD patients according to COMT rs4680 genotype. METHODS Patients with idiopathic PD were assessed for motor severity (UPDRS-III rating scale in OFF-state), dopaminergic denervation using [123I]-FP-CIT SPECT imaging, and genotyped for the COMT rs4680 enzyme. [123I]-FP-CIT binding potential (BP) for each voxel was defined by the ratio of tracer-binding in the region of interest (striatum, caudate nucleus and putamen) to that in a region of non-specific activity. Genotyping was performed using TaqMan(®) SNP genotyping assay. We used a regression model to evaluate the effect of COMT genotype on the BP in the striatum and its sub-regions. RESULTS Genotype distribution was: 11 (27.5%) COMT(HH), 26 (65%) COMT(HL) and 3 (7.5%) COMT(LL). There were no significant differences in disease severity, treatments, or motor scores between genotypes. When adjusted to clinical severity, gender and age, low and intermediate metabolizers showed significantly higher rates of striatal denervation (COMT(HL+LL) BP = 1.32 ± 0.04) than high metabolizers (COMT(HH), BP = 1.6 ± 0.08; F(1.34) = 9.0, p = 0.005). Striatal sub-regions showed similar results. BP and UPDRS-III motor scores (r = 0.44, p = 0.04) (p < 0.001) were highly correlated. There was a gender effect, but no gender-genotype interaction. CONCLUSIONS Striatal denervation differs according to COMT-Val158Met polymorphism. COMT activity may play a role as a compensatory mechanism in PD motor symptoms.
