BACKGROUND & AIMS Pegylated interferon is still the backbone of hepatitis C treatment and may cause thrombocytopenia, leading to dose reductions, early discontinuation, and eventually worse clinical outcome. We assessed associations between interferon-induced thrombocytopenia and bleeding complications, interferon dose reductions, early treatment discontinuation, as well as SVR and long-term clinical outcome. METHODS All consecutive patients with chronic HCV infection and biopsy-proven advanced hepatic fibrosis (Ishak 4-6) who initiated interferon-based therapy between 1990 and 2003 in 5 large hepatology units in Europe and Canada were included. RESULTS Overall, 859 treatments were administered to 546 patients. Baseline platelets (in 10(9)/L) were normal (⩾150) in 394 (46%) treatments; thrombocytopenia was moderate (75-149) in 324 (38%) and severe (<75) in 53 (6%) treatments. Thrombocytopenia-induced interferon dose reductions occurred in 3 (1%); 46 (16%), and 15 (30%) treatments respectively (p<0.001); interferon was discontinued due to thrombocytopenia in 1 (<1%), 8 (3%), and in 8 (16%) treatments respectively (p<0.001). In total, 104 bleeding events were reported during 53 treatments. Only two severe bleeding complications occurred. Multivariate analysis showed that cirrhosis and a platelet count below 50 were associated with on-treatment bleeding. Within thrombocytopenic patients, patients attaining SVR had a lower occurrence of liver failure (p<0.001), hepatocellular carcinoma (p<0.001), liver related death or liver transplantation (p<0.001), and all-cause mortality (p=0.001) compared to patients without SVR. CONCLUSIONS Even in thrombocytopenic patients with chronic HCV infection and advanced hepatic fibrosis, on-treatment bleedings are generally mild. SVR was associated with a marked reduction in cirrhosis-related morbidity and mortality, especially in patients with baseline thrombocytopenia.

Der Beitrag geht der Frage nach, welchen Beitrag so genannte Online-Wahlhilfen zu einer gut funktionierenden modernen Demokratie leisten und welche Rolle sie im Rahmen des Smart City-Konzepts einnehmen können. Dabei dient die Schweizer Online-Wahlhilfe smartvote als Fallstudie und die Gemeinderats- (Legislative) und Stadtratswahlen (Exekutive) in der Stadt Zürich vom 9. Februar 2014 als konkretes Anwendungsbeispiel. Neben der Funktionsweise wird auch erläutert, wie die Benutzung durch Parteien und Kandidierende, Medien sowie Wähler in der Praxis abläuft. Es wird auch diskutiert, was Online-Wahlhilfen leisten können. Schließlich wird darauf eingegangen, wo bei der Anwendung dieser Plattformen noch ungenutzte Potenziale stecken und welche Risiken mit ihrem Einsatz verbunden sind. Der Beitrag zeigt, wie smartvote und vergleichbare Projekte idealtypisch im Sinne einer Smart Democracy in Verknüpfung mit anderen Daten und Instrumenten die Qualität einer modernen Demokratie verbessern können.

AIMS The Absorb bioresorbable vascular scaffold (Absorb BVS) provides similar clinical outcomes compared with a durable polymer-based everolimus-eluting metallic stent (EES) in stable coronary artery disease patients. ST-elevation myocardial infarction (STEMI) lesions have been associated with delayed arterial healing and impaired stent-related outcomes. The purpose of the present study is to compare directly the arterial healing response, angiographic efficacy and clinical outcomes between the Absorb BVS and metallic EES. METHODS AND RESULTS A total of 191 patients with acute STEMI were randomly allocated to treatment with the Absorb BVS or a metallic EES 1:1. The primary endpoint is the neointimal healing (NIH) score, which is calculated based on a score taking into consideration the presence of uncovered and malapposed stent struts, intraluminal filling defects and excessive neointimal proliferation, as detected by optical frequency domain imaging (OFDI) six months after the index procedure. The study will provide 90% power to show non-inferiority of the Absorb BVS compared with the EES. CONCLUSIONS This will be the first randomised study investigating the arterial healing response following implantation of the Absorb BVS compared with the EES. The healing response assessed by a novel NIH score in conjunction with results on angiographic efficacy parameters and device-oriented events will elucidate disease-specific applications of bioresorbable scaffolds.

BACKGROUND 2013 AHA/ACC guidelines on the treatment of cholesterol advised to tailor high-intensity statin after ACS, while previous ATP-III recommended titration of statin to reach low-density lipoprotein cholesterol (LDL-C) targets. We simulated the impact of this change of paradigm on the achievement of recommended targets. METHODS Among a prospective cohort study of consecutive patients hospitalized for ACS from 2009 to 2012 at four Swiss university hospitals, we analyzed 1602 patients who survived one year after recruitment. Targets based on the previous guidelines approach was defined as (1) achievement of LDL-C target < 1.8 mmol/l, (2) reduction of LDL-C ≥ 50% or (3) intensification of statin in patients who did not reach LDL-C targets. Targets based on the 2013 AHA/ACC guidelines approach was defined as the maximization of statin therapy at high-intensity in patients aged ≤75 years and moderate- or high-intensity statin in patients >75 years. RESULTS 1578 (99%) patients were prescribed statin at discharge, with 1120 (70%) at high-intensity. 1507 patients (94%) reported taking statin at one year, with 909 (57%) at high-intensity. Among 482 patients discharged with sub-maximal statin, intensification of statin was only observed in 109 patients (23%). 773 (47%) patients reached the previous LDL-C targets, while 1014 (63%) reached the 2013 AHA/ACC guidelines targetsone year after ACS (p value < 0.001). CONCLUSION The application of the new 2013 AHA/ACC guidelines criteria would substantially increase the proportion of patients achieving recommended lipid targets one year after ACS. OBJECTIVE To determine the success of medical management of presumptive thoracolumbar disk herniation in dogs and the variables associated with treatment outcome. STUDY DESIGN Retrospective case series. ANIMALS Dogs (n=223) with presumptive thoracolumbar disk herniation. METHODS Medical records from 2 clinics were used to identify affected dogs, and owners were mailed a questionnaire about success of therapy, recurrence of clinical signs, and quality of life (QOL) as interpreted by the owner. Signalment, duration and degree of neurologic dysfunction, and medication administration were determined from medical records. RESULTS Eighty-three percent of dogs (185/223) were ambulatory at initial evaluation. Successful treatment was reported for 54.7% of dogs, with 30.9% having recurrence of clinical signs and 14.4% classified as therapeutic failures. From bivariable logistic regression, glucocorticoid administration was negatively associated with success (P=.008; odds ratio [OR]=.48) and QOL scores (P=.004; OR=.48). The duration of cage rest was not significantly associated with success or QOL. Nonambulatory dogs were more likely to have lower QOL scores (P=.01; OR=2.34). CONCLUSIONS Medical management can lead to an acceptable outcome in many dogs with presumptive thoracolumbar disk herniation. Cage rest duration does not seem to affect outcome and glucocorticoids may negatively impact success and QOL. The conclusions in this report should be interpreted cautiously because of the retrospective data collection and the use of client self-administered questionnaire follow-up. CLINICAL RELEVANCE These results provide an insight into the success of medical management for presumptive thoracolumbar disk herniation in dogs and may allow for refinement of treatment protocols.

Bimetallic, oxalate-bridged compounds with bi- and trivalent transition metals comprise a class of layered materials which express a large variety in their molecular-based magnetic behavior. Because of this, the availability of the corresponding single-crystal structural data is essential to the successful interpretation of the experimental magnetic results. We report in this paper the crystal structure and magnetic properties of the ferromagnetic compound {[N(n-C3H7)4][MnIICrIII(C2O4)3]}n (1), the crystal structure of the antiferromagnetic compound {[N(n-C4H9)4][MnIIFeIII(C2O4)3]}n (2), and the results of a neutron diffraction study of a polycrystalline sample of the ferromagnetic compound {[P(C6D5)4][MnIICrIII(C2O4)3]}n (3). Crystal data:  1, rhombohedral, R3c, a = 9.363(3) Å, c = 49.207(27) Å, Z = 6; 2, hexagonal, P63, a = 9.482(2) Å, c = 17.827(8) Å, Z = 2. The structures consist of anionic, two-dimensional, honeycomb networks formed by the oxalate-bridged metal ions, interleaved by the templating cations. Single-crystal field dependent magnetization measurements as well as elastic neutron scattering experiments on the manganese(II)−chromium(III) samples show the existence of long-range ferromagnetic ordering behavior below Tc = 6 K. The magnetic structure corresponds to an alignment of the spins perpendicular to the network layers. In contrast, the manganese(II)−iron(III) compound expresses a two-dimensional antiferromagnetic ordering.

Lehrpersonen benötigen für professionelles pädagogisches Handeln berufliche Kompetenzen. Aktuell wird in der Diskussion um Kompetenzen (Weinert, 2002, S. 27-28) von Lehrpersonen vermehrt auf das Modell professioneller Handlungskompetenz nach Baumert und Kunter (2006, S. 482) Bezug genommen, dies allerdings meist ohne spezifische Berücksichtigung der zu unterrichtenden Fächer bzw. der Fachdisziplin (Frey, 2006, S. 42). Das gilt insbesondere für bislang wenig untersuchte Kompetenzen von Lehrpersonen des Fachs Sport. Diese lassen sich u.a. im Rahmen von subjektiven Theorien (Groeben, Wahl, Schlee & Scheele, 1988) über den eigenen Fachunterricht erfassen, da handlungsleitende Kognitionen für die Unterrichtsgestaltung darstellen. Die vorliegende Studie untersucht vor dem Hintergrund des Modells professioneller Handlungskompetenz die subjektiven Theorien von Lehrpersonen über Kompetenzen und Bereitschaften von Sport unterrichtenden Lehrpersonen der Vorschul- und Primarschulstufe. In einem Mixed-Methods-Design wurden im Rahmen einer zweistufigen Delphi-Befragung Interviews mit Expertenlehrpersonen sowie Fachdidaktikerinnen und -didaktikern durchgeführt, mittels qualitativer Inhaltsanalyse ausgewertet und daraus Kompetenzen und Bereitschaften von Sport unterrichtenden Lehrpersonen abgeleitet. Die Kompetenzen und Bereitschaften wurden anschliessend in einer quantitativen Fragebogenuntersuchung von Lehrpersonen (N = 313) bezüglich ihrer Wichtigkeit eingeschätzt und mittels explorativer Faktorenanalyse auf ihre Struktur untersucht. Auf Basis subjektiver Theorien von Lehrpersonen werden mittels Delphi-Befragung 58 Kompetenzen und Bereitschaften entwickelt, welche für Sport unterrichtende Lehrpersonen der Vorschul- und Primarschulstufe zentral sind. Diese sind fachspezifisch konnotiert und lassen sich den Aspekten des Modells professioneller Handlungskompetenz (Baumert & Kunter, 2006) zuordnen. Die Kompetenzen und Bereitschaften können aufgrund der Einschätzungen zur Wichtigkeit faktorenanalytisch auf 41 Kompetenzen und Bereitschaften reduziert und analog zu Baumert und Kunter (2006) eingeteilt werden in acht fachspezifische Kompetenzfacetten bezüglich des Professionswissens (Wissen über die didaktisch-methodische Gestaltung des Sportunterrichts, Wissen über den Nutzen von Bewegung, Wissen über die Planung des Sportunterrichts anhand transparenter Ziele und Kriterien, Entwicklungspsychologisches Wissen, sportwissenschaftliches Wissen, Wissen über den Umgang mit Unfällen, Wissen über den Einsatz vielfältiger Bewegungsaufgaben, Wissen über die Organisation der Klasse) und in zwei Bereitschaftsfacetten (Sportives Selbstverständnis, Intentionale Individuumsförderung). Alle Kompetenz- und Bereitschaftsfacetten werden im Mittel als äusserst wichtig, wichtig oder eher wichtig beurteilt. Die Ergebnisse stellen zentrale Kompetenzen und Bereitschaften von Sport unterrichtenden Lehrpersonen der Vorschul- und Primarschulstufe auf Basis von subjektiven Theorien dar. Sie lassen sich in das Modell professioneller Handlungskompetenz einordnen, weisen aber eine hohe fachspezifische Akzentuierung auf. Recent evidence suggests that individual differences in physical activity (PA) may be associated with individual differences in white matter microstructure and with grey matter volume of the hippocampus. Therefore, this study investigated the association between PA and white matter microstructure of pathways connecting to the hippocampus. A total of 33 young, healthy adults underwent magnetic resonance imaging (MRI). High angular resolution diffusion-weighted imaging and multi-component relaxometry MRI scans (multi-component driven equilibrium pulse observation of T1 and T2) were acquired for each participant. Activity levels (AL) of participants were calculated from 72-h actigraphy recordings. Tractography using the damped Richardson Lucy algorithm was used to reconstruct the fornix and bilateral parahippocampal cinguli (PHC). The mean fractional anisotropy (FA) and the myelin water fraction (MWF), a putative marker of myelination, were determined for each pathway. A positive correlation between both AL and FA and between AL and MWF were hypothesized for the three pathways. There was a selective positive correlation between AL and MWF in the right PHC (r = 0.482, p = 0.007). Thus, our results provide initial in vivo evidence for an association between myelination of the right PHC and PA in young healthy adults. Our results suggest that MWF may not only be more specific, but also more sensitive than FA to detect white matter microstructural alterations. If PA was to induce structural plasticity of the right PHC this may contribute to reverse structural alterations of the right PHC in neuropsychiatric disorder with hippocampal pathologies.
