OBIETTIVO: Le fistole retto-vaginali Crohn-relate hanno un impatto significativo sulla qualità della vita. Quando il canale anale è alterato da ulcerazioni e stenosi o in pazienti con difetti estesi del perineo, la chirurgia locale produce risultati insoddisfacenti. Lo scopo di questo studio è quello di valutare l'efficacia della trasposizione del muscolo gracile nelle fistole retto-vaginali Crohn-relate e determinare i suoi effetti sulla qualità della vita. MATERIALI E METODI: Da gennaio 2012 a ottobre 2014 sono state trattate 10 pazienti; sono state raccolte alcune variabili (età, BMI, il fumo, CDAI, setone perioperatorio, precedenti procedure, uso di immunomodulatori e steroidi). Tutte le pazienti sono state sottoposte ad ileostomia temporanea prima della graciloplastica. La percentuale di successo è stata misurata come numero di pazienti con fistola guarita dopo la chiusura della stomia. Sono stati utilizzati tre questionari prima della graciloplastica e 3 mesi dopo la chiusura della stomia al fine di valutare la qualità della vita (SF-36), l’ incontinenza fecale e la funzione sessuale. RISULTATI: La fistola retto-vaginale è stata chiusa in 9 pazienti su 10 dopo graciloplastica, con un follow-up medio di chiusura della stomia di 19 mesi (range 4 -34). È stata documentata una recidiva di RVF. Il tempo operatorio era 90-150 minuti (media, 120). La degenza postoperatoria era 7-16 giorni (media 10). Complicanze postoperatorie precoci includevano deiscenza delle suture perineali in 2 casi. Le complicanze a lungo termine includevano disestesia della cicatrice perineale. Nei dati post-operatori abbiamo riportato un miglioramento della qualità di vita, della funzione sessuale e della continenza fecale. CONCLUSIONI: La chiusura della fistola retto-vaginale utilizzando la trasposizione del muscolo gracile è associata a morbidità minima e un alto tasso di successo.

Cutaneous melanoma (CM) represents the third most common cancer in Italian women under 49 years old. In the last decades, many molecular studies confirmed that sex hormones have a part in melanogenesis and melanoma genesis. However, many controversies are present regarding the role of exogenous oestrogens intake and endogenous hormonal status in female melanoma. Our study's primary objective is to investigate the features of melanoma in women of fertile age and women in postmenopausal age. The secondary aim is to evaluate the expression of ERα and ERβ by immunohistochemical analysis in women who underwent ovarian stimulation for medically assisted procreation and in women in cancer therapy for breast cancer (BC) comparing to two control groups. The tertiary objective is to correlate ERα and ERβ with the Breslow thickness and other relevant histopathological, clinical and dermoscopic characteristics Results A total of 998 women were included in the study. Women in fertile age are significantly more prone to have CM on the trunk. Conversely, postmenopausal females are more likely to develop CM on acral locations. Breslow thickness and ulceration were statistically significant among postmenopausal women (P-value <0,001). In the group for women with a history of breast cancer (BC), we observed a significantly higher CM percentage with “non-brisk” TILs. Upon immunohistochemical analysis, most cases with inhibitor aromatase therapy displayed a strong cytoplasmatic ERα positivity. Upon the Pearson correlation analysis, no association was shown between nuclear ERβ and Breslow thickness. The meaning of cytoplasmatic ERα in melanoma is still debated. It could suggest a potentially significant role of oestrogen non-genomic pathway in these patients, or it can be a mechanism of ERs modulation in response to aromatase inhibitor therapy. Our work tried to enlighten some of the existing shadows on the role of ERs and hormonal factors in CM.

Almost 18-35% of cutaneous melanomas are located in the head and neck region (2-5% of them in the scalp) and according to multiple epidemiological studies they are supposed to have a worse prognosis with respect to those of other body areas. The aim of this phD project is to perform a global evaluation of head/neck region cutaneous melanomas with a distinct analysis of histological, dermoscopic and molecular features of the selected cases. The diffuse distribution of folliculotropism (≥3 HF/specimen), the presence of atypical melanocytes into the isthmus, and the perifollicular involvement were associated with melanoma recurrence. The dermoscopic parameter grey circles in lentigo maligna was correlated to the depth of folliculotropism, with a higher probability of an isthmic or bulge follicular extension of neoplastic melanocytes. The detection of grey circles, light/dark brown pseudonetwork and light brown structureless areas in lentigo maligna melanoma was associated with the distribution of folliculotropism (focal/diffuse). MiR-146a-5p expression was shown to be significantly greater in melanomas with a mitotic rate ≥1/mm2 as well as in ulcerated lesions compared to those without ulceration. No difference emerged evaluating regression when considering all melanomas subtypes, but an higher expression was noticed in the lentigo maligna melanoma group. Moreover, miR-146a-5p expression was lower in head/neck region cutaneous melanomas with Breslow thickness ≥0.8 mm. These results give further insights for the management of cutaneous melanomas with specific adverse prognostic elements. As the majority of head/neck region cutaneous melanomas are wild type of BRAF and NRAS mutations and not suitable for targeted therapies, the discovery of new molecular targets may provide relevant opportunities for their treatment.
