This paper consists in the evaluation of the exposure rate to ionizing radiation to which professionals working in surgical procedures which require radiological examinations are subjected. Were initially performed real-time readings of exposure rate within four distinct operating rooms during the execution of four surgical procedures that made use of fluoroscopy equipment (including three orthopedic surgeries, one in the shoulder, one in the arm, another for deployment of metal pin in the leg region, and a fourth for vascular procedure); in these surgeries were used ionization chamber detector and an electrometer. In order to check the values achieved, was made a re-evaluation of the distribution of the rate of exposure to radiation, from the surgical procedures, now with thermoluminescent dosimeters (TLDs). For this, thirty TLDs were distributed in the operating rooms, arranged in points of interest as occupation by professionals. The TLDs were prepared for thirty consecutive days, after which they were removed and replaced with new dosimeters not exposed yet. The dosimeters were subjected to reading of the rate of exposure; this procedure was repeated for four months without interruption. The quantification of the results sought primarily to convert the rate of exposure for equivalent dose rate, both in measurements with ionization chamber as in measurements with TLDs, in order to highlight the presence of the biological effect of ionizing radiation for comparisons within scientific context. Then, the results were plotted to establish the relationship between the values of equivalent dose and the distance to the central axis of the x -ray source, confirming the inverse square law for distance. Finally, the values were associated with the maximum limit recommended by the legislation for occupationally exposed individuals. The frontal sinus fractures are derived from accidents of great intensity. Are usually associated with fractures of the midface, including the naso-orbital fractures and zygomatic ethmoid. Several treatment modalities have been proposed. In this case it was used to access bicoronal reconstruction of the anterior frontal sinus, and restore contour fronto-naso-orbital-ethmoid with satisfactory results.

Melanoma subungueal in situ tratado com cirurgia funcionalHamilton Ometto StolfI, Hélio Amante MiotI, Nilton de Ávila ReisIIDepartamento de Dermatologia e Radioterapia, Universidade Estadual Paulista (Unesp)INTRODUÇÃOO melanoma subungueal representa aproximadamente 2% a 3% dos melanomas cutâneos em pacientes caucasianos1 e 20% em pacientes negros2 ou asiáticos.3A exposição solar, tida com principal fator de risco para o melanoma cutâneo,4 parece desempenhar papel secundário no desenvolvimento da variante subungueal,5 uma vez que a radiação ultravioleta dificilmente penetra no leito ungueal. Além disso, nevos subungueais como lesões precursoras são extremamente raros.6Os polegares e háluces são os mais acometidos, sendo o polegar responsável por 56% dos casos entre todos os dedos e o hálux por 86% dos dedos dos pés.7A confirmação do diagnóstico é feita a partir do exame anatomopatológico da lesão, geralmente localizada na matriz ou leito ungueal. A verificação histológica do melanoma subungueal é frequentemente postergada por conta do atraso no diagnóstico clínico,8-10 gerando piora do prognóstico.O prognóstico geralmente é ruim: as taxas de sobrevida real de cinco anos variam entre 16% a 20%, podendo atingir até 80% se consideradas taxas de sobrevida estimada.2,3,11-18RELATO DE CASOPaciente de 44 anos, do sexo feminino, professora, com queixa de aparecimento de mancha na unha do hálux direito há cinco anos. Relata aumento progressivo lento na largura da faixa. Ao exame dermatológico, apresentava faixa de melanoníquia extensa, irregular, bordas mal definidas e estria de pigmentação mais acentuada em uma das margens do leito ungueal do hálux direito (Figura 1). Exame dermatoscópico (aparelho Dermalite II Pro, aumento de 10 vezes) confirmou faixas irregulares e cores variadas de hiperpigmentação.Foi realizada a avulsão parcial da lâmina ungueal e incisão na prega ungueal lateral para visualização do local da origem da pigmentação no leito ungueal,

JUSTIFICATIVA E OBJETIVOS: A dor pós-operatória continua sendo uma das principais complicações pós-operatórias e motivo de desconforto, principalmente em crianças. O objetivo deste estudo foi avaliar o uso de analgésicos desde o término da cirurgia até a alta da sala de recuperação pós-anestésica (SRPA), como medida terapêutica ou profilática, para crianças com menos de 1 ano de idade. MÉTODO: Utilizando o banco de dados do Departamento de Anestesiologia, foi realizada análise retrospectiva, envolvendo o período de janeiro de 2000 a abril de 2001, das anestesias de crianças menores que 1 ano de idade submetidas a procedimentos cirúrgicos diversos, avaliando aspectos relacionados à analgesia pós-operatória. RESULTADOS: No período do estudo, foram anestesiadas 402 crianças menores que 1 ano, sendo que 194 (48,2%) não receberam analgésicos e 208 (51,8%) receberam. Com relação ao uso ou não de analgésicos, foi observado o que se segue: Sem analgésicos: (1) Idade: até 1 mês, 68/99; entre 1 e 6 meses, 53/126; entre 6 meses e 1 ano, 73/177. (2) Peso: 6,7 ± 3,1 kg (3). Sexo: masculino, 106/240; feminino, 88/162. (4) Estado físico ASA: ASA I, 69/187; ASA II, 56/113; ASA III, 46/79; ASA IV, 23/23. (5) Anestesia peridural sacral: 3/4. (6) Tempo de anestesia: 106 ± 32 minutos. (7) Encaminhamento para unidade de terapia intensiva (UTI): 93/119. Uso de analgésicos: (1) Idade: até 1 mês, 31/99; entre 1 e 6 meses, 73/126; entre 6 meses e 1 ano, 104/177. (2) Peso: 9 ± 2,3 kg. (3) Sexo: masculino, 134/240; feminino, 74/162. (4) Estado físico ASA: ASA I, 118/187; ASA II, 57/113; ASA III, 33/79; ASA IV, 0/23. (5) Anestesia peridural sacral: 1/4. (6) Tempo de anestesia: 130 ± 38 minutos. (7) Encaminhamento para UTI: 26/119. Os fármacos empregados para promover analgesia foram: dipirona (60,6%), dipirona + tramadol (25,5%), dipirona + nalbufina (5,3%), tramadol (3,8%), nalbufina (3,8%), meperidina (0,5%) e fentanil (0,5%). CONCLUSÕES: Utilizar analgésicos em crianças desde o término da cirurgia até a alta da SRPA não foi habitual, principalmente nas crianças menores e mais graves e em procedimentos cirúrgicos mais rápidos. O uso de dipirona, isoladamente, ou a associação dipirona/tramadol, foram as drogas analgésicas mais freqüentemente empregadas.

Em decorrência da alteração presente nas rupturas diafragmáticas ser de natureza anatômica, o tratamento cirúrgico é o único indicado. As abordagens cirúrgicas mais utilizadas são a laparotomia pela linha média e a toracotomia intercostal, porém, a correção cirúrgica pode ser dificultada, principalmente quando houver perda de tecido, evolução crônica, ou quando a lesão no músculo diafragma for extensa, necessitando de procedimentos cirúrgicos mais apropriados, tal como a utilização de implantes biológicos ou sintéticos. Os vários tipos de implantes e enxertos testados para reparar os defeitos no músculo diafragma obtiveram bons resultados e baixas taxas de mortalidade, sugerindo uma nova alternativa no reparo de defeitos diafragmáticos

The equine anesthesia is proven to be a high risk procedure, for both patient and anesthetist, compared to other animal species. In these conditions, a good anesthetic protocol that can deliver sedation, making access and manipulation easier, analgesia fair enough to realize cirurgical procedures, and minimal physiologic changes would be ideal. Since there aren’t any drugs suporting all these qualities, the Neuroleptoanalgesic Associations show themselves as a valuable technique to provide anesthetic quality and safety, possibly diminishing our drug of choice’s doses, therefore offering less fisiological changes and easier monetary access in some cases. Objective: This study aimed at evaluating the difficulties reported by last-year dentistry course students regarding basic and surgical periodontal procedures. Methods: A proposed questionnaire with 24 questions, 14 related to basic periodontal procedures and 10 to surgical procedures, was applied to 45 undergraduate students with jobs involving these procedures, during the practical activities of the Integrated Clinic Discipline, a compulsory subject in the curriculum of the course, taught to last-year students. Students were asked about their own perceptions of the difficulties in implementing the various steps related to the treatments. The degree of individual difficulty was determined according to the Likert scale: 0 = none, 1 = mild, 2 = moderate, 3 = high, 4 = very high. Exploratory analysis of data was applied, and the difficulty level was also dichotomized into none to mild (scores 0 and 1) and moderate to very high (scores 2–4). Results: Regarding the basic periodontal treatment, bone level probing was the procedure with the highest number of student difficulties (57.5%). As for the difficulties reported by students regarding the stages of periodontal surgery, only the procedures related to anesthesia and suture removal had no report of doubts. Conclusion: Periodontal surgery was the procedure involving the most prevalent questions reported as moderate to very high, and the indication for the most suitable surgical technique was the stage with the highest percentage of questions (66.6%) among the undergraduates.

Objectives: The aim of this study was to evaluate the behavior of the polymer histomorphometrically castor during the healing process of defects of critical size calvarial preparations in rats. Materials and Methods: Twenty animals underwent a surgical procedure that was to be held in the calvaria of each animal a critical defect of 8 mm in diameter with a drill trephine. The rats were divided into two groups according to the following procedures: group C received no treatment and the bone defect site was filled with blood clot, group M, the bone defect was filled with castor oil polymer particles. The animals were sacrificed 180 days after the surgical procedures. After routine laboratory procedures the specimens were subjected to analysis histomorphometric. Results: In groups C the newly formed bone tissue was well developed, with adjacent areas of osteoid matrix rich in osteoblasts, and restricted to the vicinity of the edges of the defect. In animals of group M was observed newly formed lamellar bone tissue restricted to the vicinity of the defect edges and particles of polymer Castor distributed throughout the defect. There was a higher percentage of newly formed bone area was statistically significant in group C compared to animals in group M. Conclusions: Within the limits of this study can conclude that the castor oil polymer is biocompatible and had kept the area during the healing of critical size defects in surgically prepared rat calvariae.
