Objectives: To describe the use of antenatal corticosteroid and clinical evolution of preterm babies. Methods: An observational prospective cohort study was carried out. All 463 pregnant women and their 514 newborn babies with gestational age ranging from 23 to 34 weeks, born at the Brazilian Neonatal Research Network units, were evaluated from August 1 to December 31, 2001. The data were obtained through maternal interview, analysis of medical records, and follow-up of the newborn infants. Data analysis was performed with the use of chi-square, t Student, Mann-Whitney, and ANOVA tests and multiple logistic regression, with level of significance set at 5%. Results: Treatment was directly associated with the number of prenatal visits, with maternal hypertension and with the antenatal use of tocolytic agents. Babies from treated pregnant women presented better Apgar scores at the 1st and 5th minute, reduced need for intervention in the delivery room and lower SNAPPE II. They were born with higher birth weight, longer gestational age and needed less surfactant use, ventilation, and oxygenation time. After multiple logistic regression, the use of antenatal corticosteroid independently improved birth conditions, decreased ventilation time, being related to increased occurrence of neonatal sepsis. Conclusions: The use of corticosteroid was associated with better prenatal care and birth conditions, better preterm evolution but higher risk of infection. A deficiência de glicose-6-fosfato desidrogenase em neonatos pode ser a responsável pela icterícia neonatal. Este comentário científico é decorrente do relato sobre o tema publicado neste fascículo e que preocupa diversos autores de outros países em relação às complicações em neonatos de hiperbilirrubinemia, existindo inclusive proposições de alguns autores em incluir o teste para identificar a deficiência de glicose-6-fosfato desidrogenase nos recém-nascidos.

A doença hemolítica perinatal (DHPN) ainda é um problema clínico. Nenhum teste isolado prediz, com segurança, a gravidade do quadro hemolítico. O objetivo do presente estudo foi determinar as subclasses de anticorpos IgG1 e IgG3 por citometria de fluxo no soro de 42 gestantes isoimunizadas e correlacionar os dados obtidos com a gravidade da DHPN. A distribuição dos fetos ou neonatos segundo a gravidade do quadro hemolítico evidenciou 13 casos com doença leve, 16 casos com doença moderada e 13 com doença grave. As subclasses foram detectadas em 33/42 (79%) amostras. A subclasse IgG1, isoladamente, foi evidenciada em 14/33 (42,4%) casos. Na relação entre gravidade da doença e subclasses de IgG, observou-se que IgG1 isolada foi encontrada em todos os grupos, e os valores da mediana de intensidade de fluorescência (MIF) foram significativamente mais altos nas formas mais graves da DHPN (p<0,01). Contrariamente, os valores da MIF para IgG3 se apresentaram mais homogêneos em todas as categorias (p=0,11). A presença de IgG3 parece, portanto, estar mais associada à hemólise leve. A associação das subclasses IgG1 e IgG3 está relacionada à situação clínica mais grave, o que se deve, possivelmente, à presença de IgG1 associada. Apesar dos altos valores para IgG1 e a associação de IgG1 com IgG3 indicarem maior gravidade da DHPN, sugere-se que outras variáveis sejam analisadas conjuntamente, uma vez que os relatos existentes na literatura, até o momento, não dão suporte para seu uso como instrumento exclusivo de avaliação de gravidade e prognóstico da doença.

As hemoglobinopatias são as doenças genéticas mais freqüentes na população humana. Cerca de 12 a 15% da população é portadora de uma ou mais formas de hemoglobinas anormais, resultando em um grande problema de saúde pública. O diagnóstico neonatal possibilita o tratamento e o aconselhamento genético precoce, incluindo a conscientização dos portadores sobre o risco do nascimento de homozigotos. O objetivo deste trabalho é apresentar os resultados da investigação de hemoglobinopatias em recém-nascidos do Hospital de Base de São José do Rio Preto. O estudo foi realizado em 913 amostras de sangue de cordão umbilical, que foram submetidas a testes eletroforéticos, bioquímicos e citológicos, específicos para análise de hemoglobinas. Foram identificadas 100 (10,95%) amostras com hemoglobinas anormais, das quais 40 (4,38%) com Hb Bart's, sugerindo alfa talassemia, 34 (3,72%) com Hb S, 23 (2,52%) com beta talassemia, duas (0,22%) com Hb C e uma (0,11%) amostra apresentou Hb rápida. A frequência elevada de alterações encontradas evidencia a necessidade da triagem neonatal de hemoglobinopatias que pode resultar em amplos benefícios para os portadores destas patologias e seus familiares.

Iron deficiency is the commonest nutritional deficiency in the world. Although it affects adults, particularly women of reproductive age and during gestation, the most vulnerable group is children under two years of age. It affects mainly people living in developing countries, who have less access to balanced diets and health services and are exposed to precarious sanitary conditions. Iron deficiency has an impact on the immunity, adult work capacity and the cognitive development of children. Combating and preventing iron deficiency is one of the priorities in promoting public health. The strategies to achieve this end include the evaluation and correction of iron deficiency in pregnant women, the encouragement of breast feeding, oral iron supplementation in premature and newborn babies of low birth-weight, food enrichment and mobilization of the community.

The concern with infant mortality has been a priority in public policies, especially for Brazil to achieve the Millennium Development Goal number four: reducing child mortality by 75% by the year 2015. It is known that prematurity has an intimate relationship with mortality of children under one year and therefore it is necessary an effective intervention in risk factors linked to premature births. To evaluate the profile of mothers and newborn babies living in Botucatu-SP, in the period 2001 to 2009, focusing on prematurity. A quantitative study, retrospective, descriptive, epidemiological, from the Information System Newborn Alive. The prevalence of preterm births was 15.1%, and low birth weight 14.7%. Among infants, the Apgar score below 7 at 1 and at 5 minutes was 13.1% and 2.4% respectively. Considering the total of pregnant women studied, 20.3% were adolescents and 10.3% were aged 35 years or more and most (63.2%) had eight or more years of school approval. The profile of mothers of premature infants shows that 23.6% were teenagers, 14.1% had at least 35 years and 60.4% had at least primary education. With regard to premature deliveries, caesarean sections and 58.4% were between the total newborns, this type of delivery was achieved in 46.6% of cases. Prematurity was associated with an Apgar score below seven in the first and fifth minutes of life, presence of malformation and multiple pregnancy. Among maternal variables, prematurity was associated with extremes of maternal age, maternal education, history of stillbirth, primiparas, white and presence of the mother's partner. With regard to delivery was associated with cesarean section and outside the hospital. This study assessed maternal characteristics associated with prematurity and point out how relevant the pregnancies at the extremes of age and those that occur in women with low education level, indicating the interference... P>A case of a type I rectal atresia (also known as membranous rectal atresia) in a newborn donkey is reported. Clinical examination, survey radiographs and barium enema radiographs suggested rectal atresia. An exploratory laparotomy was performed; however, surgical correction of the defect was not possible due to the narrow pelvic cavity. Euthanasia was performed. At necropsy, it was possible to observe a fibrous cord connecting the small colon to a residual rectal ampulla, which opened to a normal anus. The radiographic and anatomical characteristics enabled the classification of the defect as type I rectal atresia, which is a rare congenital anomaly that is difficult to correct through surgery. This report contributes to the study of rectal atresia in newborn equids, alerts for the occurrence in donkeys and is, to our knowledge, the first to be reported in this species in Brazil.

Purpose: To investigate the dynamics of ocular eyelid movements in newborn infants and preschool-age children.Methods: Fifty newborn infants and 200 preschool children aged 4-6 years were examined. Images of each child, with his or her eyes in the primary eye position looking at an object placed at the child's height, were recorded with a digital videocamera for 3 mins. Complete and incomplete blink rates, opening, closing and complete blink times were calculated.Results: Newborn infants presented a lower number of incomplete movements than preschool children. The complete blink rate was lower in newborn infants (6.2 blinks/min) than in preschool children (8.0 blinks/minute). Eyelid closing, opening and compete blink times were longer in newborn infants than in preschool children at all observation times.Conclusions: Newborn infants had a different pattern of eyelid movement compared with preschool children. Specific characteristics that are found in this group of children particularly, such as immaturity of the neural system and more resistant tear film, may explain these findings in part.
